Rolling circle amplification(RCA)is an efficient isothermal enzymatic reaction using a circular DNA single strand as a template and catalyzed by DNA polymerases.It is simple,fast,and efficient,and can rapidly produce ultra-long single-stranded DNA with a large number of tandem repeats in a short time.Due to the codability of DNA,template DNA sequences can be designed according to needs to result in a wide range of functional products,displaying a unique advantage in the fields of biological detection and drug delivery.This review has summarized the basic principle and classification of RCA technology,focused on the applications of RCA technology in tumor detection and drug delivery in recent years,and finally discussed the application prospects and potential challenges of this technology.

ObjectiveTo explore the role and molecular mechanism of Weifuchun （WFC） in inhibiting inflammation and alleviating gastric precancerous lesions （GPL）. MethodHuman gastric mucosal epithelial cells （GES-1） were stimulated with N-methyl-N′-nitro-N-nitrosoguanidine （MNNG） for the modeling of GPL （MC cells）， with Caspase-3 inhibition by Z-DEVD-FMK. MC cells were divided into control （20% blank serum）， WFC （15% and 20% WFC-containing serum）， and caspase-3 inhibitor groups. The cell counting kit-8 （CCK-8） was used to examine the viability of GES-1 cells or MC cells. The Transwell assay and 5-acetylidene-2′-deoxyuridine （EdU） staining were employed to examine cell invasion and proliferation， respectively. Flow cytometry was employed to determine the level of reactive oxygen species. Real-time PCR was conducted to determine the mRNA levels of interleukin （IL）-1， IL-6， and tumor necrosis factor （TNF）-α. Gene Expression Omnibus （GEO） was used to analyze the role of pyroptosis in gastric cancer progression. Western blotting was employed to determine the protein levels of nuclear factor-κB （NF-κB） p65， gasdermin E （GSDME）， and Caspase-3. Immunofluorescence staining was employed to detect the NF-κB p65 protein level and nuclear translocation. Hematoxylin-eosin staining was carried out to observe the pathological changes in the gastric mucosa before and after WFC treatment in the patients. ResultCompared with the control group， MC cells presented enhanced proliferation and invasion energy （P<0.01）. Compared with the blank serum group， WFC-containing serum inhibited the proliferation and invasion of MC cells （P<0.01）， down-regulated the mRNA levels of IL-1， IL-6， and TNF-α， and lowered the level of reactive oxygen species （P<0.05， P<0.01）. The transcriptome data at different stages of gastric cancer showed that pyroptosis was involved in gastric cancer progression， and the GSDME level was significantly higher in GPL patients than in the normal group. Compared with the blank serum， WFC-containing serum lowered the level of NF-κB and inhibited the nuclear translocation of NF-κB （P<0.05）， and it inhibited pyroptosis by suppressing the cleavage of Caspase-3 on GSDME （P<0.05， P<0.01）. The analysis of patient specimens further demonstrated that WFC treatment down-regulated the NF-κB level and GSDME cleavage （P<0.01）， inhibited pyroptosis， and alleviated gastric mucosal inflammation and intestinal epithelial metaplasia. ConclusionPyroptosis is involved in the progression of gastric cancer， and WFC inhibits pyroptosis via the NF-κB/GSDME pathway， thereby alleviating gastric mucosal inflammation in GPL.

Objective To explore the effect of Dong's extraordinary point acupuncture combined with medium frequency pulse electrotherapy on lumbar disc herniation.Methods Eighty patients with lumbar disc herniation who were admitted to Xiangshan Hospital of Traditional Chinese Medicine from January 2021 to October 2023 were selected and randomly divided into study group and control group,with 40 cases in each group.Twelve patients were lost during treatment,and eventually 68 patients were enrolled in this study,including 34 in the study group and 34 in the control group.The control group was treated with conventional acupuncture combined with medium frequency pulse electrotherapy,and the study group was treated with Dong's extraordinary point acupuncture with medium frequency pulse electrotherapy,both for 2 weeks,3 times a week.Visual Analogue Scale(VAS)score,Japanese Orthopaedic Society(JOA)score and Oswestry Disability Index(ODI)were compared between the two groups before and after treatment.Results VAS score and ODI were significantly decreased and JOA score was significantly increased after treatment in both groups(P<0.001).VAS score and ODI in the study group were lower than those in the control group after treatment,while JOA score in the study group was higher than that in the control group(P<0.001).Conclusion Dong's extraordinary point acupuncture combined with medium frequency pulse electrotherapy is superior to conventional acupuncture combined with medium frequency pulse electrotherapy in treatment of lumbar disc herniation.It can faster relieve clinical symptoms and restore lumbar disc function,and is worthy of clinical application with satisfactory safety.

Objective To explore the relationship between the expression of M2 pyruvate kinase(PKM2)and E-cad-herin(E-cadherin)in tumor tissues and the clinicopathology and prognosis of breast cancer patients.Methods 447 patients with breast cancer admitted from February 2020 to February 2022 were selected as the objects of this study.Breast cancer tissues and adjacent tissues of patients were taken,and patients were divided into good prognosis group(261 cases)and poor prognosis group(186 cases)according to the prognosis of patients.Immunohistochemistry was used to determine the expression of PKM2 and E-cadherin in the tissues of the patients,and the patients were followed up for 6 months.The patients were divided into groups according to whether the patients had recurrence and metastasis,and the relationship between the expression of PKM2 and E-cadherin in the breast cancer tissues of the patients and the clinicopathology was explored.The expression of PKM2 and E-cad-herin protein between breast cancer tissue and adjacent tissue,and between good prognosis group and poor prognosis group were compared,and the correlation between PKM2 and E-cadherin protein expression and poor prognosis of patients was explored.Results There were no differences in the expression of PKM2 and E-cadherin in breast cancer patients with different ages,tumor sites and tumor sizes(P＞0.05).The positive rate of PKM2 protein expression was higher and the positive rate of E-cadherin protein expression was lower in postmenopausal,middle and highly differentiated,stage I to II and no lymph node metastasis(P＜0.05).The positive rate of PKM2 in patients with poor prognosis was 94.62%,higher than 81.23%in patients with good prognosis;The positive rate of E-cadherin in patients with poor prognosis was 30.65%,which was lower than 46.36%in patients with good prognosis(P＜0.05).By Spearman correlation coefficient analysis,poor prognosis was positively correlated with the positive rate of PKM2 in breast cancer tissues(r=0.195,P＜0.001),and negatively correlated with the positive rate of E-cad-herin protein(r=-0.158,P=0.001).Conclusion PKM2 and E-cadherin protein expression in tumor tissues of breast canc-er patients has some link with the clinicopathology of patients,and there is some correlation with patient prognosis.

Objective To discuss the correlation between family history of malignant neoplasms (MN-FH) and the clinicopathological features of patients with non-small cell lung cancer (NSCLC). Methods The clinicopathological data of 326 patients with NSCLC in Yangzhou University Affiliated Hospital from June 2016 to June 2018 were analyzed retrospectively. The patients were divided into two groups based on with or without MN-FH, and the clinicopathological features of the two groups were analyzed by χ 2 test. Results Of the 326 patients with NSCLC, 41 (12.6％) were in the MN-FH group and 285 (87.4％) in the MN-FH group. There were no significant differences in sex, age, smoking, location of tumors and histological classification between patients with or without MN-FH (χ 2 values were 0.031, 0.769, 0.546, 0.117, and 0.945, all P > 0.05), but in patients with MN-FH, the proportion of tumor diameter < 5 cm [65.9％ (27/41) vs. 42.5％(121/285), χ 2 = 14.173, P < 0.05], undifferentiation and low differentiation [70.7％ (29/41) vs. 53.7％(153/285), χ 2 = 4.224, P < 0.05], TNM stage Ⅲ+Ⅳ [65.9％ (27/41) vs. 46.7％ (133/285), χ 2 = 5.280, P <0.05], lymph node metastasis [78.0％ (32/41) vs. 60.0％ (171/285), χ 2 = 4.970, P < 0.05], distant metastasis [75.6％ (31/41) vs. 53.3％ (152/285), χ 2 = 7.224, P < 0.05], high degree of malignancy [70.7％ (29/41) vs. 51.6％ (147/285), χ 2 = 5.293, P < 0.05] and combination of other tumors [29.3％ (12/41) vs. 7.7％ (22/285),χ 2 = 17.817, P < 0.05] were significantly higher than those in patients without MN-FH. Conclusions NSCLC patients with MN-FH have a higher degree of malignancy. For people with MN-FH, physical examination is very important.

Objective To study the expression of ultra-conserved gene sequence uc.454 in non-small cell lung cancer (NSCLC) tissues and its clinical significance.Methods Real-time fluorescence quantitative polymerase chain reaction (qRT-PCR) was used to detect the expression level of uc.454 mRNA in 130 cases of NSCLC and adjacent normal tissues from Affiliated Hospital of Yangzhou University between January 2014 and December 2017,and its association with clinicopathological factors and prognosis of NSCLC patients was analyzed.Results The expression level of uc.454 mRNA in NSCLC tissues was lower than that in adjacent normal tissues (2.98 ±0.96 vs.13.95± 2.23,t =98.441,P ＜ 0.05),and its expression level was associated with tumor infiltration depth (x2 =7.823,P =0.040),TNM stage (x2 =20.719,P ＜ 0.01) and lymph node metastasis (x2 =24.142,P ＜ 0.01).The survival time of patients with low expression of uc.454 mRNA was shorter than that of patients with normal or high expression (26.9 months vs.43.7 months),and the difference was statistically significant (P ＜ 0.05).Conclusion The low expression of ultra-conserved gene sequence uc.454 mRNA is related to the occurrence,development and prognosis of NSCLC.

Objective To analyze the characteristics and regularity of the dosage of single-flavored traditional Chinese medicine decoction pieces, and to provide reference for determining the reasonable dosage range. Methods The dose distribution of single-flavored drugs with a frequency of more than 10％ of the total prescriptions from January to June2018 was investigated, and compare with the 2015 edition of the Pharmacopoeia of the People's Republic of China (hereinafter referred to as "Chinese Pharmacopoeia") and other recommended doses. Results There were 32 kinds of single-flavored traditional Chinese medicine decoction pieces, which accounted for more than 10％ of the total prescription. The top 5 dose distributions were 10 g, 15 g, 9 g, 30 g and 6 g, respectively, accounting for nearly 90％.These 32 pieces were not used in large-dose applications in our outpatient prescriptions, and there were some ultralow-dose applications; there were 21 pieces with more than 90％ of the doses in the scope of the Chinese Pharmacopoeia. In the clinical use of traditional Chinese medicine decoction pieces, there were cases of out of the scope of the Chinese Pharmacopoeia, but were not prominent. Conclusion The reasonable dosage ranges of commonly used traditional Chinese medicines needs further study; the clinical use of traditional Chinese medicine decoction pieces should consider both safety and dose-effect relationship.

Objective To investigate the effect of radial artery calcification (RAC) on survival of arteriovenous fistula (AVF) and the patients in end?stage renal disease. Methods Adult ESRD patients undergoing AVF surgery between January 2013 and January 2016 at the Eighth Affiliated Hospital of Sun Yat?sen University were enrolled in this study. The clinical and biochemical data were collected. Segment of radial artery were obtained from the operation of AVF. RAC at the site of anastomotic were observed by alizarin red S and hematoxylin and eosin staining. According to RAC, the patients were divided into calcification group and non?calcification group. Kaplan?Meier analysis was performed to analyze the survival rates of the two groups, and Cox proportional hazards regression——model was used to estimate the risk factors of AVF dysfunction and all?cause mortality in ESRD patients. Results Among 180 cases of ESRD patients, 38 cases (21.1％) were developed RAC at the site of anastomotic in different degrees. Compared with the non?calcification groups, the calcification groups had a longer dialysis vintage, a higher proportion of diabetes and higher level of HbAlc (all P﹤0.05). Binary logistic regression analysis showed that dialysis vintage>5 years and diabetics were two independent risk factors of RAC at the site of anastomotic. Kaplan?Meier survival analysis demonstrated that there were no statistical differences between two groups in AVF survival (χ2=0.009, P=0.926). Calcification group had higher all?cause mortality than non?calcification groups (χ2=9.809, P=0.002). Multivariate Cox regression analysis demonstrated that homocysteine was independent risk factor for AVF dysfunction (HR=1.027, 95％CI: 1.003-1.051, P=0.027). Age was independent risk factor for all?cause mortality (HR=1.078, 95％CI: 1.035-1.122, P=0.000). Conclusions Dialysis vintage>5 years and diabetes were two independent risk factors of RAC at the site of anastomotic in ESRD patients. RAC at the site of anastomotic had no effect on AVF survival, but increased all?cause mortality.
