Objective:To investigate the effect of interaction between body mass index (BMI) and gender on the survival of advanced gastric cancer after immunotherapy.Methods:The retrospective cohort study was conducted. The clinicopathological data of 317 patients with advanced gastric cancer who were admitted to the Nanfang Hospital, Southern Medical University from November 2019 to October 2023 were collected. There were 205 males and 112 females, aged 56 (range, 21-79)years. All 317 patients were divided into three groups based on BMI of patients, including 58 cases with BMI <18.5 kg/m2 were classified as the low body mass group, 183 cases with BMI 18.5-24.0 kg/m2 were classified as the normal body mass group, and 76 cases with BMI >24.0 kg/m2 were classified as the overweight or obese group. Patients included in the study were treated with a programmed death-ligand 1 (PD-L1) based immunotherapy regimen for 3 cycles based on their specific conditions, and further decision was made whether to undergo radical surgery or continue comprehensive treatment after evaluating the efficacy. Observation indicators: (1) clinicopathological characteristics of patients; (2) follow-up and mortality status; (3) analysis of factors affecting survival of patients with advanced gastric cancer after immunotherapy. Comparison of measurement data with normal distribution among groups was conducted using the ANOVA. Comparison of measurement data with skewed distribution among groups was conducted using the Kruskal-Wallis H test. Comparison of count data among groups was conducted using the chi-square test. The Cox proportional hazard model was used for univariate and multivariate analyses. Nonlinear trend was analyzed using the restricted cubic spline (RCS) curve, and trend and correction graphs were created using the rcssci package (v1.0). Results:(1) Clinicopathological characteristics of patients. There was no significant difference in gender, age, Borrmann classification, Lauren classification, combined positive score of PD-L1, expression of human epidermal growth factor receptor 2, Epstein-Barr virus infection, carcino-embryonic antigen, CA19-9, CA72-4, alpha-fetoprotein, conversion surgery among the 3 groups of patients ( P>0.05), and there was a significant difference in mismatch repair combined with micro-satellite stability among the 3 groups of patients ( P<0.05). (2) Follow-up and mortality status. Of the 317 patients, 316 cases completed follow-up and 1 case in the overweight or obese group was lost to follow-up. The follow-up time of the 316 cases was 13.8(range, 0.9-48.2)months. During the follow-up, the number of death in the low body mass group, normal body mass group and overweight or obese group were 27, 70 and 31, respectively. (3) Analysis of factors affecting survival of patients with advanced gastric cancer after immunotherapy. Results of multivariate analysis showed that gender and BMI were independent factors affecting survival of patients with advanced gastric cancer after immunotherapy ( hazard ratio=0.066, 0.922, 95% confidence interval as 0.005-0.846, 0.855-0.994, P<0.05). Results of further analysis showed that the interaction between BMI and gender was an independent factor affecting survival of patients with advanced gastric cancer after immuno-therapy ( hazard ratio=1.152, 95% confidence interval as 1.024-1.296, P<0.05). Results of Cox regre-ssion analysis based on different gender showed that took patients of the normal body mass group as a reference, the male patients of the low body mass group had a significantly increased risk of death, showing a significant statistically difference ( hazard ratio=1.809, 95% confidence interval as 1.037-3.155, P<0.05). Results of RCS curve analysis showed that there was a non-linear correlation between BMI and survival of patients with advanced gastric cancer after immunotherapy ( P<0.05). Results of corrected RCS curve analysis using the rcssci package showed that there was a U-shaped relationship between BMI and survival of patients with advanced gastric cancer after immuno-therapy ( P<0.05), with the optimal cut-off value of BMI as 22.2 kg/m 2. Results of RCS curve analysis based on different gender showed that there was a U-shaped relationship between BMI and survival of male patients with advanced gastric cancer after immunotherapy ( P<0.05), with the optimal cut-off value of BMI as 22.7 kg/m 2. Conclusions:Gender, BMI and the interaction between BMI and gender are independent factors affecting survival of patients with advanced gastric cancer after immuno-therapy. A 29-year-old female patient received cervical injection of oxytocin injection 10 U twice daily because of postpartum subinvolution of uterus. On the 4th day of treatment,the patient received a transrectal administration of carboprost methylate suppositories 0. 5 mg due to poor efficacy of oxytocin injection. About 20 minutes after rectal medication,the patient developed diarrhea with watery stools 4 times within 70 minutes. About 2 hours after rectal medication,the patient developed persistent abdominal colic and gradually aggravated. Intramuscular injections of metoclopramide 10 mg and pethidine 100 mg were given. However,her abdominal pain did not improve obviously and abdominal ultrasound examination showed no abnormality. Her diarrhea and abdominal pain were considered to be related to carboprost methylate. Symptomatic supportive treatments with spasmolysis,antidiarrheal,potassium supplementation, fluid replacement were given continuously. Five days later,diarrhea and abdominal pain disappeared.

Objective To observe the clinical efficacy of Liuweianxiao capsule combined with omeprazole to the treatment of functional dyspepsia.Methods 111 patients with functional dyspepsia from June 2014 to June 2016 were randomly divided into observation group 55 cases and control group 56 cases.The control group received Omeprazole treatment,observation group were given Liuweianxiao capsule combined with omeprazole.Recorded and analyzed motilin levels,EGG frequency changes,antral contraction amplitude,symptom score,the clinical efficacy and adverse reactions during treatment.Results ①After treatment,Motilin and EGG frequency levels were(491.59±25.01)mg/L、(2.92±0.34)cpm,better than the control group(416.73±23.14)mg/L、(1.89±0.23)cpm,and the differences were statistically significant(P<0.05).② After treatment,antral contraction amplitude levels were(1.60±0.09)％,better than the control group(1.34±0.07)％,and the differences were statistically significant(P<0.05).③After treatment,symptom score levels were(10.56±1.74)point,better than the control group(14.68±2.13)point,and the differences were statistically significant(P<0.05).④The total effective rate of observation group after treatment was 90.91％,significantly higher than the control group 73.21％,and the differences were statistically significant(P<0.05).⑤ The adverse reactions of observation group during treatment was 7.27％,and compared to the control group 10.71％,the difference was not significant.Conclusion Liuweianxiao capsule combined with Omeprazole can effectively remission related symptoms,and it is a safe and effective treatment.

Objective To investigate status and risk factors of drug resistance of smear-positive pulmonary tuber-culosis (TB)patients in Hunan Province,and provide reference for the prevention and control of drug-resistant TB. Methods 1 935 Mycobacterium tuberculosis (MT)complex strains identified by 20 TB prevention and control insti-tutes in Hunan Province between 2012 and 2014 were collected and performed drug susceptibility testing,and influ-encing factors associated with drug resistance of TB were analyzed statistically.Results Of 1 935 MT complex strains,1 207 (62.38%)were sensitive to 6 kinds of antituberculosis drugs,728 were drug-resistant strains,overall drug resistance rate was 37.62%;467 (24.13%)were multidrug-resistant (MDR)strains,64 of which were exten-sively drug-resistant (XDR)strains,XDR rate was 3.31 %,resistance rates from high to low were as follows:isoniazid(INH)29.32%,rifampicin(RFP)25.84%,streptomycin(SM)20.73%,thambutol(EMB)9.00%,ofloxa-cin(OFX)7.83%,and kanamycin(KM)2.21 %.Multivariate logistic regression analysis showed that patients hav-ing a history of treatment,aged 20-39 and 40-60 years old were risk factors for drug resistance and MDR of pul-monary TB.Among patients who failed in retreatment,OR (95% CI )of resistance to INH,RFP,SM,EMB, OFX,KM,and MDR were 13.5(9.9-18.4),21 .2(15.2-29.5),5.3(3.9-7.2),11 .9(7.6-18.7),7.6(4.6-12.6),7.9(3.6-17.5),and 25.0(17.7-35.1 )respectively;among patients who had recurrence,OR(95% CI ) of resistance to INH,RFP,SM,EMB,OFX,and MDR were 7.4(5.5 -10.0),10.3 (7.4 -14.2),3.5 (2.5 -4.8),7.3(4.5 -11 .9),4.1 (2.5 -6.8),and 12.2(8.7 -17.1 )respectively;among patients who failed in initial treatment,OR (95% CI )of resistance to INH,RFP,SM,EMB,and MDR were 7.6 (4.7 - 12.3 ),9.8 (5.9 -16.0),4.1(2.5-6.8),12.1(6.5-22.7),and 11 .4(6.9-18.9)respectively.Among patients aged 20-39 years old,OR (95% CI )of resistance to INH,RFP,SM,and MDR were 2.5 (1 .8 -3.4),3.6(2.5 -5.2),2.9(2.0-4.1),and 4.1(2.8 -6.1 )respectively;among patients aged 40 -60 years old,the OR (95% CI )of resistance to INH,RFP,SM,and MDR were 2.2(1 .6-3.0),3.1(2.2-4.4),2.3(1 .6-3.2),and 3.3(2.3 -4.7)respectively. Conclusion Drug resistance of smear-positive pulmonary TB patients is serious in Hunan Province,patients receiv-ing anti-tuberculosis treatment and aged between 20-60 years old have high risk for drug resistance and MDR.

BACKGROUNDThe Xpert MTB/RIF showed high sensitivity and specificity in previous studies carried out in different epidemiological and geographical settings and patient populations in high-burden tuberculosis (TB) countries. However, there were little data obtained by validation or demonstration study of the assay in China. In this study, the performance of Xpert MTB/RIF was investigated in two county-level laboratories in Hunan Province, China.

METHODSConsecutive patients with suspected pulmonary tuberculosis (PTB) and suspicion for multidrug-resistant tuberculosis (MDR-TB) were enrolled. For each patient suspected to have PTB, three sputum specimens (one spot sputum, one night sputum, and one morning sputum) were collected and each sputum was tested with smear microscopy, Löwenstein-Jensen (LJ) culture, and Xpert MTB/RIF test. For comparison across subgroups and testing methods, 95% confidence intervals were calculated. All analyses were done with SPSS 16.0, and P < 0.05 was regarded as significant.

RESULTSFor case detection, the sensitivity of Xpert MTB/RIF was 100% for smear- and culture-positive TB and 88.6% for smear-negative and culture-positive TB; the overall sensitivity was 94.5% for all culture-positive patients. The specificity was 99.8%. The sensitivity of Xpert MTB/RIF assay was 22.0% in clinical TB patients and the specificity reached 100.0% in the group of patients who are infected with nontuberculous mycobacteria. For the detection of rifampin resistance, the sensitivity of MTB/RIF RIF-resistance detection was 92.9%, and the specificity was 98.7%. Of the 26 Xpert MTB/RIF-positive and RIF-resistant patients confirmed by LJ proportion tests, 20 (76.9%) patients were infected by MDR-TB.

CONCLUSIONSThe Xpert MTB/RIF assay is a highly sensitive and specific method for diagnosis of TB and RIF resistance, which will enable it to have the potential to be used in county-level laboratories and lead to the reduction of the infectious pool and improvements in TB control in China. Further evaluations in county-level laboratories for implementing the assay are still required.

Objective: To investigate the effects of FGFR1OP and p57/Kip2 proteins on the genesis and progression of gliomas and their clinical significance. Methods: The expression of FGFR1OP and p57/Kip2 in 54 glioma specimens was detected by SP immunohistochemical technique. The relationship between the ex-pression levels of those proteins and various clinical pathologic factors was evaluated. Results: The expres-sion of FGFR1OP and p57/Kip2 was found in 66.7% and 44.4% gliomas, respectively. The OD value of FG-FR1OP was 0.131±0.010 in high grade gliomas, and 0.118±0.010 in low grade ones, with a statistical signifi-cance (t=-5.497, P=0.000), showing that higher expression of FGFR1OP was significantly associated with glo-ma cell differentiation. The OD value of p57/Kip2 was 0.156±0.008 in high grade gliomas, and 0.165±0.006 in low grade ones, with a statistical significance (t=0.296, P=0.014), showing that lower p57/Kip2 expression was correlated with high grade gliomas. FGFR1OP was negatively correlated with p57/Kip2 in gliomas (r=-0.732, P<0.01). Conclusion: Increased expression of FGFR1OP and/or decreased expression of p57/Kip2 may play an important role in the genesis and progression of gliomas and may indicate a poor prognosis.
