1.Intervention Mechanism of Guizhi Fulingwan in Delaying Colitis-associated Colon Cancer via Modulating and Restoring MDSCs and Reshaping Immune Microenvironment

ObjectiveThis paper aims to investigate the efficacy and related actions of Guizhi Fulingwan in intervening in the mice with colitis-associated colon cancer (CAC) based on the immunosuppressive microenvironment associated with myeloid-derived suppressor cells (MDSCs). MethodsSixty male C57BL/6 mice were randomly assigned to a blank group, a model group, an aspirin group (0.04 g·kg-1), and low-, medium-, and high-dose Guizhi Fulingwan groups (4.87, 9.75, and 19.50 g·kg-1), with ten mice per group. The CAC mouse model was established via combined induction of azoxymethane (AOM)/dextran sulphate sodium (DSS). Drug intervention commenced in week five, with continuous intragastric administration for nine weeks. The food intake, body weight, fecal characteristics, and haematochezia were observed and recorded, and disease activity index (DAI) scores were calculated according to scoring criteria. Hematoxylin and eosin (HE) staining was used to observe the histopathological changes in the colon tissues of the mice. Immunohistochemistry was used to determine proliferating cell nuclear antigen-67 (Ki67) expression in the colon tissues, and enzyme-linked immunosorbent assay (ELISA) was used to detect the contents of interleukin-6 (IL-6), IL-1β, and tumor necrosis factor-α (TNF-α) in the serum of the mice. Flow cytometry was employed to determine the proportion levels of MDSCs, CD4+ T cells, and CD8+ T cells in the spleen tissues of the mice. The mRNA expressions of MDSC-associated effector molecules, including arginase 1 (Arg1) and inducible nitric oxide synthase (iNOS), were detected by real-time quantitative polymerase chain reaction (Real-time PCR). After that, an in vitro co-culture model of MDSCs and CD8+ T cells was established, and drug-containing serum of Guizhi Fulingwan was used for intervention. The Flow cytometry was employed to assess the effects of drug-containing serum of Guizhi Fulingwan with different concentrations on the levels of reactive oxygen species (ROS) and iNOS in MDSCs and the proliferation of CD8+ T cells. The levels of granzyme B (GZMB) and interferon-γ (IFN-γ) in cell supernatant were detected by ELISA. ResultsCompared with those in the control group, the mice in the model group exhibited significantly reduced body weight, elevated DAI scores, shortened colon length (P<0.01), increased number of tumors and Ki67 expression (P<0.01), and significantly elevated contents of IL-6, IL-1β, and TNF-α in the serum (P<0.01). Significant increases in the number of MDSCs were observed in mouse spleens, alongside marked reductions in the levels of CD4+ T and CD8+ T cells (P<0.01). Furthermore, the mRNA expressions of MDSC function-associated effector molecules Arg1 and iNOS were significantly upregulated (P<0.01). Compared with those in the model group, the mice in the middle-dose Guizhi Fulingwan group exhibited increased body weight and significantly decreased DAI scores (P<0.05, P<0.01). The mice in the middle- and high-dose Guizhi Fulingwan groups exhibited significantly improved colon shortening, significantly decreased number of tumors and Ki67 expression (P<0.05, P<0.01), and significantly decreased contents of IL-6, IL-1β, and TNF-α in the serum (P<0.05, P<0.01). Furthermore, administration of Guizhi Fulingwan markedly reduced MDSC infiltration in the spleen of the mice, with different degrees of increase in the levels of both CD4+ T and CD8+ T cells (P<0.05, P<0.01), alongside significant decreases in the mRNA expressions of Arg1 and iNOS (P<0.05, P<0.01). In vitro cell co-culture shows that administration of drug-containing serum of Guizhi Fulingwan significantly decreases the activity levels of ROS and iNOS in MDSCs and promotes the proliferation of CD8+ T cells and the secretion of GZMB and IFN-γ (P<0.05, P<0.01). ConclusionGuizhi Fulingwan can reduce pro-inflammatory cytokine secretion and inhibit tumor proliferation in the colon tissues of CAC mice. Its potential mechanism may involve reducing MDSC infiltration, enhancing effector T cells, particularly CD8+ T cell response, and improving the tumor immunosuppressive microenvironment.
2.Anti-inflammatory and antioxidant mechanism of berberine for sepsis rats

Objective To investigate the anti-inflammatory and antioxidant effects of berberine for sepsis rats based on kelch-like ECH-associated protein 1(Keap1)/nuclear factor erythroid 2-relat-ed factor 2(Nrf2)/antioxidant response element(ARE)signaling pathway.Methods Adult health-y rats were randomly divided into normal group(20 rats)and sepsis group(80 rats).The sepsis model in rats of the sepsis group was established by cecal ligation and puncture.According to differ-ent doses of berberine administered by gavage,the sepsis group was further divided into model group(0 mg/kg,without berberine administration),low-dose group(25 mg/kg),medium-dose group(50 mg/kg)and high-dose group(100 mg/kg),with 20 rats in each group.Both the model group and the normal group were given an equal volume of pure water by gavage.Ten rats were randomly selected from each group for a survival test,and they were continuously observed for 5 days to com-pare the survival rates among the groups.Lung,kidney,and liver tissues of rats in each group were collected for hematoxylin-eosin(HE)staining,enzyme-linked immunosorbent assay(ELISA),Western blot,and real-time fluorescence quantitative polymerase chain reaction(qRT-PCR)detection.The pathological tissue changes,serum inflammatory cytokine levels,serum biochemical indicator levels,and the relative expression levels of pathway-related proteins and their mRNAs in each group were observed and compared.Results The survival rates of rats in the medium-dose and high-dose groupsat various time points were all higher than those in the model group,and the differences were statistically significant(P<0.05).Compared with the normal group,the model group showed in-creased levels of serum alanine aminotransferase(ALT),aspartate aminotransferase(AST),serum creatinine(SCr),blood urea nitrogen(BUN),interleukin-6(IL-6),interleukin-1 β(IL-β)and tumor necrosis factor-α(TNF-α).There were obvious pathological injuries in lung,liver,and kid-ney tissues.The relative expression levels of Nrf2 and heme oxygenase-1(HO-1)proteins and their mRNAs in lung tissues were decreased,while the relative expression levels of Keap1 mRNA and Keap1 protein were increased,and the differences were statistically significant(P<0.05).Com-pared with the model group,the serum SCr and BUN levels in the medium-dose group,and the ser-um ALT,AST,SCr and BUN levels in the high-dose group were all decreased,with statistically sig-nificant differences(P<0.05).Compared with the model group,the pathological injuries in lung,liver,and kidney tissues of rats in the low-dose,medium-dose,and high-dose groups were all alle-viated.The relative expression levels of Nrf2 mRNA and HO-1 mRNA as well as Nrf2 and HO-1 proteins in the lung tissues showed a dose-dependent increase,while the relative expression levels of Keap1 mRNA and Keap1 protein exhibited a dose-dependent decrease.The serum levels of IL-6 and TNF-α in the low-dose group,as well as the serum levels of IL-6,IL-1 β and TNF-α in the medium-dose and high-dose groups were all reduced(P<0.05).Conclusion Berberine can alleviate the inflammatory response and activate the antioxidant response in sepsis rats by regulating the Keap1/Nrf2/ARE signaling pathway.
3.Efficacy and safety of high-frequency repetitive transcranial magnetic stimulation in postherpetic neuralgia

Objective:To evaluate the efficacy and safety of high-frequency repetitive transcranial magnetic stimulation (hrTMS) in postherpetic neuralgia (PHN).Methods:A prospective randomized controlled trial was performed; 63 PHN patients treated in Department of Neurology and Department of Dermatology and Venereology of Anqing Municipal Hospital from June 2024 to March 2025 were enrolled; they were randomly assigned to 2 groups: an hrTMS group ( n=32) received hrTMS (frequency: 10 Hz; total pulses: 2,400; intensity: 90% of resting motor threshold) to the contralateral primary motor cortex (M1), and a sham stimulation group ( n=31) received sham stimulation using a sham figure-of-eight coil generating no actual magnetic field. Scores of short-form McGill pain questionnaires (pain rating index [PRI] total score, PRI sensory subscore, PRI affective subscore, visual analogue scale [VAS] score, present pain intensity [PPI] score) and N100 amplitude were collected before treatment and at 1, 2, 3, and 4 weeks after treatment. Adverse events during treatment were recorded. Results:At 2 weeks after treatment, significant difference was observed between the hrTMS group and sham stimulation group in PRI total score, VAS score, and N100 amplitude ( P<0.05). At 3 and 4 weeks after treatment, significant differences were found between the two groups in PRI total score, PRI sensory subscore, PRI affective subscore, VAS score, PPI score and N100 amplitude (3 weeks after treatment: 8.59±1.13 vs. 9.61±1.20, 5.34±0.79 vs. 5.90±0.94, 3.25±0.57 vs. 3.71±0.46, 5.78±0.66 vs. 6.42±0.92, 2.16±0.37 vs. 2.55±0.51, and [2.53±0.51] μV vs. [2.13±0.34] μV; 4 weeks after treatment: 7.53±0.92 vs. 9.68±1.35, 4.94±0.62 vs. 6.00±1.07, 2.59±0.56 vs. 3.68±0.60, 5.06±0.67 vs. 6.23±1.06, 1.97±0.17 vs. 2.52±0.51, and [2.81±0.40] μV vs. [2.16±0.52] μV, P<0.05). In the hrTMS group, PRI total score, PRI sensory subscore, PRI affective subscore, VAS score, PPI score and N100 amplitude were significantly different at 2, 3, and 4 weeks after treatment compared with those before treatment ( P<0.05). Incidence of adverse events (headache, dizziness or tinnitus) did not differ significantly between the two groups ( P>0.05). Conclusion:The hrTMS applied to the M1 region in PHN patients is effective by obviously reducing pain intensity and improving negative emotional states, with favorable safety profile.

6.Urgent recommendation and practice of prevention and control of novel coronavirus disease 2019 (COVID-19) in intensive care units in West China Hospital of Sichuan University during the epidemics

Objective To provide recommendations for the management of intensive care unit patients without novel coronavirus disease 2019 (COVID-19). Methods We set up a focus group urgently and identified five key clinical issues through discussion. Total 23 databases or websites including PubMed, National Guideline Clearing-House, Chinese Center for Disease Control and Prevention and so on were searched from construction of the library until February 28, 2020. After group discussion and collecting information, we used GRADE system to classify the evidence and give recommendations. Then we apply the recommendations to manage pediatric intensive care unit in the department of critical care medicine in our hospital. Results We searched 13 321 articles and finally identified 21 liteteratures. We discussed twice, and five recommendations were proposed: (1) Patients should wear medical surgical masks; (2) Family members are not allowed to visit the ward and video visitation are used; (3) It doesn’t need to increase the frequency of environmental disinfection; (4) We should provide proper health education about the disease to non-medical staff (workers, cleaners); (5) Medical staff do not need wear protective clothing. We used these recommendations in intensive care unit management for 35 days and there was no novel coronavirus infection in patients, medical staff or non-medical staff. Conclusion The use of evidence-based medicine for emergency recommendation is helpful for the scientific and efficient management of wards, and is also suitable for the management of general intensive care units in emergent public health events.
7.Application of cluster nursing for children with severe pneumonia in ICU

Objective To investigate effect of cluster nursing for children with severe pneumonia in ICU.Methods A total of 88 patients with severe pneumonia were randomly divided into two groups,the control group received routine care,and the experimental group conducted cluster nursing.Blood gas analysis results after intervention,adjuvant effect of nursing,related clinical indicators,VAP,mortality and nursing satisfaction were compared.Results The experimental group had higher levels of p (O2),SaO2 and pH,lower p (CO2) level,ventilation time,ICU time and length of hospital stay and incidence of VAP than the control group (P < 0.01).The experimental group had higher total efficiency and nursing satisfaction,and lower mortality than the control group (P < 0.05).Conclusion Cluster nursing for children with severe pneumonia in ICU has significant efficacy,and can significantly improve nursing satisfaction.
8.Influence of water colloid dressing on the skin of tracheal intubation in less than 6 months newborns with congenital heart disease

Objective To discuss the influence of water colloid dressing on the skin of tracheal intubation in less than 6 months newborns with congenital heart disease.Methods A total of 80 newborns with congenital heart disease accepted therapy in our hospital were selected and were divided into control group and experimental group,with 40 cases in each group.3 M elastic tape was used to fix tracheal intubation in control group while water colloid dressing was used to protect the cheek and then 3 M elastic tape was applied to fix catheter in experimental group.The skin condition of the cheek was compared after intubation.Results The skin condition of experimental group was significantly better than control group (P < 0.05).Conclusion Water colloid dressing could protect the skin and decrease the rate of avulsion injury of skin and pressure sores.
