1.Correlation between serum total bile acid level and cognitive function in patients with stable schizophrenia and its predictive value for cognitive impairment

BackgroundPersistent cognitive impairment is prevalent among patients with stable schizophrenia. While serum total bile acid (TBA) level in acute-phase patients are known to be associated with cognitive dysfunction, the relationship between serum TBA and multi-dimensional cognitive functions in stable phase patients remains unclear. ObjectiveTo investigate the correlation between serum TBA level and cognitive function in patients with stable schizophrenia, and to evaluate its predictive value for cognitive impairment, thereby providing a serological biomarker for the timely identification and objective assessment of cognitive dysfunction. MethodsA cross-sectional study was conducted on 137 inpatients with stable schizophrenia at The Fourth People's Hospital of Yancheng from March to December 2024. All participants met the diagnostic criteria of the Diagnostic and Statistical Manual of Mental Disorders, fifth edition (DSM-5). Cognitive function was evaluated using the Chinese Brief Cognitive Test (C-BCT), patients were categorized into four groups: normal cognition (n=28), mild impairment (n=28), moderate impairment (n=47), and severe impairment (n=34). Fasting venous blood samples were collected, and serum TBA level was quantified using an enzymatic cycle assay. Spearman correlation analysis was ultilized to determine the relationship between serum TBA level, overall cognitive function, and specific cognitive domains. Binary Logistic regression model was used (adjusting for covariates such as age, gender, and disease duration) to analyze the impact of serum TBA level on overall and individual cognitive functions. The predictive value of serum TBA level for overall cognitive impairment was evaluated using receiver operating characteristic (ROC) curve. ResultsSerum TBA levels differed significantly among the four groups (H=18.677, P<0.01). Specifically, serum TBA levels in both the moderate and severe cognitive impairment groups were significantly higher than those in the normal cognitive group (adjusted P<0.01). Serum TBA level was positively correlated with the severity grading of overall cognitive impairment (rs=0.354, P<0.05), and negatively correlated with T-scores on the trail making test (rs=-0.328, P<0.05), continuous performance test (rs=-0.247, P<0.05), digit span (rs=-0.265, P<0.05), and symbol coding (rs=-0.221, P<0.05). Binary Logistic regression analysis identified serum TBA level as an independent risk factor for overall cognitive impairment (OR=1.322, 95% CI: 1.021 - 1.713, P=0.034), with a particularly robust predictive ability for impaired information processing speed (OR=1.325, 95% CI: 1.057 - 1.661, P=0.015). The area under ROC curve (AUC) for serum TBA level in predicting overall cognitive impairment was 0.738, with a sensitivity of 60.61% and a specificity of 78.64%. ConclusionIn patients with stable schizophrenia, elevated serum TBA levels are associated with worse overall cognitive function, as well as deficits in information processing speed, attention, working memory, and executive function. Serum TBA serves as an independent risk factor and exhibits moderate predictive value for overall cognitive impairmen,particularly in the domain of information processing speed. 2.Factors in self-management disorder of arteriosclerotic obliterans of lower extremity in aged patient:a qualitative study based on planned behaviour theory

Objective To explore the factors of self-management disorders in aged patients with the arteriosclerosis obliterans(ASO)of lower extremities according to planned behaviour theory so as to provide guidance for the development of programs in self-management intervention.Methods Using the description nature research method,13 ASO patients aged 60 and above in our hospital were selected for a semi-structured interviews based on the theory of planned behaviour,between November to December 2023.NVivo12 software and content analysis were used to analyse the data acquired from interviews and to refine the theme.Results Factors in 3 themes and 7 sub-themes were extracted from the aged ASO patients with self-management disorders,including self-management behaviour negative attitude(negative emotions about the disease,lack of awareness of the importance of self-management),poor subjective norm of self-management behaviour(insufficient family support and social support),low perceived behavioural control ability of self-management behaviour(lack of self-management knowledge and self-care ability,facing role conflict,disease coexistence).Conclusion There are barriers to the self-management among the aged ASO patients.Interventions should be developed and implemented to improve the self-management ability of the aged ASO patients.

4.Soothing moisturizing repair cream in promoting wound repair of acne depressed scar after laser operation: a clinical analysis

Objective:To investigate the effectiveness and safety of soothing moisturizing repair cream on acne depressed scar exfoliative fractional laser wound repair.Methods:From October 2018 to June 2020, the Department of Dermatology, Qingdao Haici Hospital Affiliated to Qingdao University took 33 patients with acne depressed scars as the research object, including 8 males and 25 females, aged from 20 to 36 years (29.6±8.6) years. The left and right face comparison method was adopted. After laser operation, the trial side was given a soothing moisturizing repair cream, and the control side was given a placebo. By collecting the patient's facial pictures and objective skin data before and after the laser operation, 1 h, 1 d, 3 d, 7 d, 21 d, and combined with the researcher's semi-subjective evaluation and patient's subjective evaluation the wound skin reaction and wound healing were observed.Results:At 1, 3, 7, 21 days after laser operation, the skin water content of the test side was higher than that of the control side ( P<0.05), and the skin water loss was lower than the control side ( P<0.05); at 3, 7, 21 days, the skin pigment of the test side was lower than the control side ( P<0.05); at 3, 7 d, the test side skin erythema index was lower than the control side ( P<0.05); at 1, 3, 7 d, the test side wound skin erythema, edema, dryness and tightness, etc. were better than the control side ( P<0.05). The duration of pain, crusting time, scab removal time, and complete healing time of the wound on the test side were shorter than those on the control side ( P<0.05). The patient's satisfaction with the moisturization and comfort of the nursing products on the trial side was better than that on the control side ( P<0.05). Conclusions:There is no adverse reaction to the soothing moisturizing repair cream after laser surgery, which can better inhibit skin inflammation, reduce post-inflammatory pigmentation, promote skin healing, and help repair the wound after laser surgery.

6.Comparative study on the difference of nurse specialist training field between US and China

Results The nurse specialist training in America has a mature system meanwhile it is not yet mature in China. Conclusions Reference to the mature development experience of America, combining the actual situation of China, suitable strategy should be developed for the Chinese nurse specialist training.
7.Brain-derived neurotrophic factor transgenic neural stem cells can alleviate toxicity of β-amyloid to cultured neurons

Objective To explore the resistance of neural stem cells (NSCs) in β-amyloid (Aβ) toxicity after being transfected with brain-derived neurotrophic factor (BDNF) and its mechenism.Methods NSCs in the C57 mice were transfected with BDNF gene by using lipofectamine technique; the BDNF gene expression in transfected NSCs and their differentiated cells were detected by immune-fluorescent staining and ELISA.Rat neurons were cultured in vitro and divided into 3 groups:neurons group,neurons co-cultured with NSCs group,neurons co-cultured with BDNF transfected NSCs group; two-parallel group design was adopted.Three d after the group culturing,cells from each parallel group were added Aβ40 protein.Neuron survival of each group was recorded 24 and 48 h after adding Aβ40 by ELISA.The Aβ,tau,and phosphorylating tau (ptau) levels were detected by Western blotting.Results BDNF-transfected NSCs could continuously express BDNF gene products during the first 1.5 weeks.Both transfected NSCs and their progenies expressed BDNF gene products.Using neurons group as reference,the mean survival rates of neurons plus Aβ40 group,neurons co-cultured with NSCs plus Aβ40 group,neurons co-cultured with BDNFtransfected NSCs plus Aβ40 group were 51.86%±0.03%,73.68%±0.04% and 85.34%±0.01% at 24 h after adding Aβ40,and 38.76 %±0.01%,59.65 %±0.04%,75.61%± 0.07% at 48 h after adding Aβ40; the survival rate in the neurons plus Aβ40 group and neurons co-cultured with NSCs plus Aβ40 group was significantly lower than the other groups (P<0.05).High level of Aβ protein was noted in the neurons plus Aβ40 group,neurons co-cultured with NSCs plus Aβ40 group and neurons co-cultured with BDNF transfected NSCs plus Aβ40 group,without significant differences between each two groups (P>0.05).The tau protein level showed no significant differences between each two groups (P>0.05),while the p-tau protein level was significantly different (that in the neurons co-cultured with NSCs plus Aβ40 group and neurons co-cultured with BDNF transfected NSCs plus Aβ40 group was significantly higher than that in the other groups,P<0.05).Conclusion BDNF-transfected NSCs express BDNF gene products during the early time,which alleviate the toxicity of Aβ to NSCs and reduce the p-tau protein level,leading to increased survival rate of neurons.
8.Expression of potassium ion channel mRNA in myocardial tissue of mice with low selenium

Objective To examine the expression of potassium channel mRNA in myocardial tissue of mice with low selenium.Methods Forty C57BL/6 mice were randomly divided into four groups according to body weight(18-22 g),10 mice in each group,half male and half female.The low-selenium treatment groups were fed with low-selenium diet(selenium content was 0.004 mg/kg) for 4,12 and 24 weeks,respectively,and the control group was fed with normal diet(selenium content was 0.256 mg/kg).The mice were killed by cutting neck method,hearts were taken out and RNA was extracted by Trizol method.The expressions of potassium ion channel genes (KCNA4,KCND2,KCND3,KCNE1,KCNE2,KCNJ2,KCNJ12 and KCNQ1) at the mRNA level in heart were determined using real-time polymerase chain reaction.Results In low-selenium 4 weeks group,the mRNA expressions of KCNA4 gene(25.3 ± 0.09) and KCND2 gene(4.85 ± 0.05) were higher than that of the control group (1.00 ± 0.00,1.00 ± 0.00,all P < 0.05); in low-selenium 24 weeks group,the mRNA expression of KCNJ12 gene (22.7 ± 0.10) was higher than that of the control group(1.00 ± 0.00,P < 0.05); the mRNA expressions of KCND3,KCNE1,KCNE2,KCNJ2,KCNQ1 genes were compared with the corresponding control groups,the differences were not statistically significant(all P > 0.05).Conclusions Low selenium can affect the mRNA expression of mouse cardiac potassium ion channel genes.
9.Relationship between expression of selenocysteine synthase and endothelial cell line ECV-304 injury induced by hydrogen peroxide

Objective To observe the influence of selenocysteine synthase(SEPSECS) on injury of human umbilical vein endothelial cell line EVC-304 induced by hydrogen peroxide (H2O2). Methods Transfection was conducted to transfect EVC-304 which was maintained in vitro. The cells were divided into four groups: control group, SEPSECS over-expression group, empty vector group and SEPSECS silenced expression group, then Real-time PCR and Western blotting were performed to detected SEPSECS mRNA and protein expression , respectively. Flow cytometry(FCM) was performed to detect cell cycle. Different concentrations of H2O2, which including 0, 200, 400, 600, 800, 1 000 μmol/L, were used to treat EVC-304 . Then malonaldehyde (MDA), superoxide dismutase(SOD) secreted by the cells which were treated with H2O2 for 6 h, were checked by MDA or SOD kit. Results The SEPSECS mRNA expressions of control, SEPSECS silenced expression, empty vector and SEPSECS over-expression groups were 1.03 ± 0.24, 0.43 ± 0.11, 0.98 ± 0.27 and 1.61 ± 0.13, respectively. The protein expressions of control, SEPSECS silenced expression, empty vector and SEPSECS over-expression groups were 1.00 ± 0.26, 0.51 ± 0.10, 1.12 ± 0.38 and 1.51 ± 0.20, respectively. There was a significant difference between control and SEPSECS silenced expression groups (all P<0.05), at the same time , this phenomenon was also observed between empty vector and SEPSECS over-expression groups (all P<0.05). The level of MDA was increased along with the H2O2 concentration. Besides, cell cycle was also tested, however, significant difference was not observed(all P > 0.05). Meanwhile, MDA of SEPSECS silenced expression groups[(15.8 ± 0.5),(19.6 ± 1.5)μmol/L] were significantly higher than control groups[(12.4 ± 0.1),(17.1 ± 0.5)μmol/L, all P < 0.05], on the other hand, MDA of SEPSECS over-expression groups[(10.8 ± 0.4),(14.2 ± 1.1)μmol/L] were lower than empty vector groups [(12.7 ± 0.7),(16.2 ± 1.1)μmol/L, all P < 0.05], when the H2O2 concentration was 800 or 1 000μmol/L. The level of SOD was decreased along with the H2O2 concentration. SOD of SEPSECS silenced expression groups[(7.7 ± 0.4),(2.4 ± 0.3)μmol/L] were lower than control groups[(10.0 ± 1.0),(6.0 ± 0.6)μmol/L, all P < 0.05], on the contrary, SOD of SEPSECS over-expression groups[(11.3 ± 0.6),(12.7 ± 1.6)μmol/L] were higher than empty vector groups[(9.2 ± 0.6),(6.7 ± 0.2)μmol/L, all P < 0.05], when the H2O2 concentration was 800 or 1 000μmol/L. Conclusion Expression of SEPSECS has a significant protective role on damaged EVC-304 which was induced by H2O2.
10.Correlation analysis of self-care ability and life quality in patients with chronic hepatitis B

Objective To investigate the self-care ability and life quality in patients with chronic hepatitis B, and analyze the correlation between them .Methods One hundred outpatients with chronic hepatitis B were surveyed by the exercise of self-care agency scale ( ESCA ) and the short form-36 health survey ( SF-36), and the correlation between self-care ability and life quality was analyzed .All data were processed by the statistical software SPSS 16.0.Results ESCA totel score for patients with chronic hepatitis was (97.53 ± 18.75 ) , in the medium level .The scores of different dimensions in SF-36 including physiological function , physical role, body pain, general health were respectively (79.8 ±17.5), (65.3 ±32.1), (74.6 ±19.9), (55.7 ±18.6), and were lower than those in the adult norm , and the differences were statistically significant (t=7.08, 4.02, 3.59, 5.19, respectively;P<0.01).SF-36 other dimension scores were lower than that of the adult norm,the difference was statistically significant (P<0.01).The Personal correlation analysis showed that there was a positive correlation between the scores of ESCA and SF-36 in patients with chronic hepatitis B ( r=0.262-0.528,P<0.05).Conclusions The self-care ability and life quality in patients with chronic hepatitis B are at low level , and improving their self-care ability is helpful to improve their health condition and keep the better life quality .
