1.Serum ferritin and triglyceride-glucose index interaction on metabolic dysfunction-associated fatty liver disease

Objective:To explore the relationship between serum ferritin(SF), triglyceride-glucose(TyG) index, and metabolic dysfunction-associated fatty liver disease(MAFLD), and to assess their interaction on MAFLD risk in the health checkup population.Methods:A cross-sectional analysis was conducted with 1 439 participants from the Health Management Centre of Nanjing Drum Tower Hospital in 2022. Data were collected through physical examination, laboratory tests, and abdominal imaging. Differences in metabolic indicators, SF, and TyG index were compared between MAFLD and non-MAFLD groups. Logistic regression analysis was used to assess the associations of SF and TyG index with MAFLD, and their on MAFLD interaction was evaluated. Results:Both SF and TyG index were significantly higher in the MAFLD group between than those in the non-MAFLD group( P<0.05). After adjusting for sex, age, history of hypertension, history of diabetes, body mass index, waist circumference, haemoglobin, alanine aminotransferase, aspartate aminotransferase, and uric acid, the SF and TyG index were positively associated with the risk of MAFLD[ OR(95% CI), SF: 1.00(1.00-1.00); TyG index: 2.98(2.19-4.06). The additive interaction analysis showed that the risk of MAFLD was significantly higher in the G4 group(SF≥135.4 ng/mL, TyG index≥8.52) compared to the G1 group(SF<135.4 ng/mL, TyG index<8.52) [ OR 4.43(95% CI 2.70-7.25)]. Conclusions:Elevated SF and TyG index were independently associated with increased risk of MAFLD, with a significant synergistic interaction between the two.

3.Design and Application of an IVD Reagent Management Information Systems Based on Unique Code

The management of in vitro diagnostic(IVD)reagents in hospitals often faces issues such as the lack of a unified coding system,unclear consumption patterns,and unknown cost-to-income ratios.It is necessary to employ information systems to achieve comprehensive,detailed,and traceable management of IVD reagents.An information management system for IVD reagents based on unique coding is introduced,which integrates admission,acceptance,and consumption processes through unique codes.The system calculates the income per experimental item based on the consumption of IVD reagents and the charge for each experimental item.The system enhances the efficiency of the IVD reagent supply chain management and promotes detailed oversight of IVD reagent usage.
4.Association between nocturnal sleep duration combined with snoring exposure and SGA,LGA in the first trimester of pregnancy based on birth cohort

Objective :
To investigate the correlation between nocturnal sleep duration combined with snoring in the first trimester of pregnancyand small for gestational age(SGA) ,large for gestational age(LGA) .
Methods :
Multi- variate Logistic regression model was used to analyze the association between nocturnal sleep duration ,snoring, their combined effects and SGA,LGA.
Results :
Compared to nocturnal sleep duration 7 to 9 h in the first trimester of pregnancy,sleep duration<7 h was positively correlated with SGA in male newborn( OR = 4. 22,95% CI : 1. 69 - 10. 52) ; After stratified by snoring,the sleep duration of snoring women<7 h was positively correlated with SGA ( OR = 5. 68,95% CI : 1. 02-31. 51) ,and the sleep duration of non-snoring women<7 h was positively correlated with LGA ( OR = 2. 10,95% CI : 1. 16 -3. 81) .
Conclusion
Sleep duration<7 h in the first trimester of preg- nancy is a risk factor for SGA and LGA,and snoring may enhance the association between sleep duration<7 h in the first trimester of pregnancy and SGA.Pregnant women should keep adequate nocturnal sleep duration to reduce the risk of abnormal neonatal weight.
5.Literature case analysis of perioperative euglycemic diabetic ketoacidosis induced by sodium-glucose cotransporter 2 inhibitors

Objective:To explore the clinical characteristics of perioperative euglycemic diabetic ketoacidosis (euDKA) induced by sodium-glucose cotransporter 2 (SGLT2) inhibitors.Methods:The case reports of perioperative euDKA caused by SGLT2 inhibitors published up to June 30, 2019 were collected by searching the relevant databases and the following information of patients including demographic characteristics, types of diabetes, use of SGLT2 inhibitors, onset time and clinical manifestation of euDKA, the blood glucose and pH, serum bicarbonate and anion gap, β-hydroxybutyric acid and ketone body concentration in urine when diagnosing euDKA, predisposing factors of euDKA, as well as the treatments and outcomes were collected. The clinical characteristics of perioperative euDKA induced by SGLT2 inhibitors were analyzed descriptively.Results:A total of 27 patients (from 20 articles) were collected, including 13 males and 14 females with an age of (58±12) years; 26 patients were with type 2 diabetes mellitus and 1 with type 1 diabetes mellitus. Of them, 15 cases were treated with canagliflozin, 6 cases were treated with dapagliflozin, and 6 cases were treated with empagliflozin; the onset time of euDKA was 10 hours to 10 days after operation and within 3 days after operation in 21 cases (77.8%); 24 cases had similar symptoms as ketoacidosis and 3 cases had no obvious symptoms; the blood glucose was (9.5±2.2) mmol/L when diagnosing euDKA and the other laboratory test results were similar to those of ketoacidosis. The main factors inducing euDKA were operation and low carbohydrate diet. After the occurrence of euDKA, all patients received insulin and rehydration therapy, and then 26 cases (96.3%) got better and 1 (3.7%) died.Conclusions:The perioperative euDKA mainly occurred within 3 days after operation. The main inducing factors of euDKA were operation and low carbohydrate diet. After insulin and rehydration therapy, most patients had a good prognosis.

7.Correlation of serum vitamin D with serum lipid in middle-aged and elderly people

Objective To investigate the correlation of serum 25-hydroxy-vitamin D (25-OH-D) with lipid profiles in middle-aged and elderly patients.Methods A total of 284 patients aged over 40 years and admitted in our hospital from May 2016 to May 2017 were selected and divided into four groups according to 25-OH-D levels as the group of normal level which was not less than 30 ng/ml,the group of slight deficiency which was between 20 and 29 ng/ml,the group of moderate deficiency which was between 10 and 19 ng/ml,and the group of severely deficiency which was under 10 ng/ml.Fasting blood draw were taken to detect liver and renal functions,lipids,calcium,phosphorus and 25-OH-D levels.The level of lipids between groups was compared by one-way ANOVA,and the association between lipid profiles and 25-OH-D levels was assessed by Pearson correlation and multiple linear regression.Results The levels of triglyceride (TG),low density lipoprotein cholesterin (LDL-C) and total cholesterol/high density lipoprotein cholesterol (TC/HDL-C) were significantly higher and the HDL-C levels were significantly lower in the groups of severely deficiency,moderate deficiency and slight deficiency than in the normal group (P=0.035,P=0.015,P=0.013).The levels of serum 25-OH-D were significantly higher in HDL-C ≥ 1.04 mmol/L group than in H DL-C< 1.04 mmol/L group (Female P =0.007,male P=0.034).The correlation analysis showed that serum 25-OH-D level was positively associated with HDL-C (female r =0.309,P=0.002,male r =0.241,P=0.002).The multiple linear regression analysis showed that 25-OH-D level was positively correlated with HDL-C,after adjusting age,BMI,liver and kidney function,serum calcium and serum phosphorus (femaleβ =0.259,P =0.047,maleβ =0.217,P =0.049).Conclusion The incidence of vitamin D deficiency is high in middle-aged and elderly patients,and the level of serum 25-OH-D is positively correlated with HDL-C.
