1.Influencing factors for atrial fibrillation in elderly patients with heart failure and construction of a nomogram model

Objective To elucidate the independent risk factors for AF in elderly patients with HF,develop a nomogram prediction model,and assess its predictive value for AF in elderly HF pa-tients.Methods A retrospective study was conducted on 299 patients of first HF hospitalization in Hengshui People's Hospital from June 2019 to December 2023,and based on the presence or absence of AF,they were divided into an AF group(133 cases)and a non-AF group(166 cases).The patients admitted from June 2019 to December 2022 were assigned into a modeling cohort(206 cases),while those from January 2023 to December 2023 into a validation cohort(93 cases).The general data were collected.Multivariate logistic regression analysis was performed on the modeling cohort to identify the independent predictors of AF in elderly HF patients,and a nomo-gram prediction model was constructed.ROC curve,calibration curve and clinical decision curve analyses were applied to evaluate the discrimination,calibration and clinical practicability of the prediction model.Results Multivariate logistic regression analysis revealed that the etiology of HF(CHD vs HP,OR=0.610,95%CI:0.301-1.250,P=0.178;CMD vs HP,OR=0.213,95%CI:0.052-0.883,P=0.033),LAD(OR=1.081,95%CI:1.020-1.152,P=0.015),HF classifi-cation(HFmrEF vs HFrEF,OR=5.897,95%CI:2.448-14.201,P=0.000;HFpEF vs HFrEF,OR=7.211,95%CI:2.522-20.640,P=0.001),pre-albumin(PALB)(OR=0.438,95%CI:0.217-0.901,P=0.030),UAC(OR=2.186,95%CI:1.075-4.430,P=0.025),and direct biliru-bin(DBi)(OR=4.531,95%CI:2.052-9.990,P=0.000)were independent risk factors for AF in the elderly HF patients.ROC curve analysis showed the AUC value of the prediction model based on these factors in the modeling cohort was 0.831,and the AUC value in the validation cohort was 0.840.Decision curve analysis indicated that the model possessed clinical practicability within a probability threshold range of 10%-82%for the modeling cohort and 12%-100%for the vali-dation cohort.Conclusion Our nomogram prediction model based on multivariate logistic regres-sion analysis exhibits good predictive value for the occurrence of AF in elderly HF patients,and can facilitate clinical decision-making on diagnosis and treatment.
2.Correlation Analysis between Serum miR-128-3p,miR-126 Levels and Gensini Score in Patients with Coronary Artery Disease

Objective To analyzing the correlation between serum microRNA(miRNA,miR)-128-3p,miRNA(miR)-126 expression and Gensini score in patients with coronary artery disease.Methods A retrospective study was conducted on 60 patients with coronary artery disease who were treated in the Department of Cardiology,Harrison International Peace Hospital from June 2020 to 2022.They were selected as the observation group,and another 60 patients with non-coronary artery disease who were treated in the hospital during the same period were selected as the control group.Serum miR-128-3p,miR-126 and Gensini scores were measured and compared between the two groups,and compared the serum miR-128-3p,miR-126 and Gensini scores of patients with single,double,and multiple vessel lesions in the observation group.According to the stenosis rate of coronary arteries,they were divided into I degree,II degree,III degree and IV degree groups.The serum miR-128-3p,miR-126 and Gensini scores were compared among different stenosis grading groups.According to the Gensini score,patients were divided into mild and severe lesion groups.According to the Gensini score,patients were divided into mild and severe lesion groups.The serum miR-128-3p and miR-126 levels were compared between the mild and severe lesion groups.Pearson analysis determined the correlation between serum miR-128-3p and miR-126 levels and Gensini score.Receiver operating characteristic curve(ROC)was plotted,and the area under curve(AUC)was calculated to analyze the predictive efficacy of the combined detection of serum miR-128-3p and miR-126 levels for coronary artery disease.Logistic regression analysis of influencing factors of coronary artery disease.Results Compared with the control group,the observation group showed an increase in serum miR-128-3p(4.28±0.52 vs 2.61±0.36)and Gensini score(31.29±5.62 score vs 6.16±1.04 score),observation group showed a decrease in serum miR-126,with statistically significant differences(t=21.678,34.058,11.002,all P<0.05).Compared with the double vessel disease and single vessel disease groups,the serum miR-128-3p and Gensini score in the multi vessel disease group increased(t=4.945,7.171;6.795,11.686),the serum miR-126 decreased(t=3.104,5.033),and the differences were statistically significant(all P<0.05).Compared with the I~II group,the serum miR-128-3p and Gensini score in the III~IV group increased significantly(t=5.590,12.961),the serum miR-126 decreased significantly(t=6.021),with statistical significance differences(all P<0.05).Compared with the mild lesion group,the serum miR-128-3p level increased in the severe lesion group(t=4.056),the serum miR-126 level decreased(t=4.806),and the differences were statistically significant differences(all P<0.05).There was a positive correlation between serum miR-128-3p and Gensini score(r=0.404,P<0.05),and a negative correlation between serum miR-126 and Gensini score(r=-0.393,P<0.05).The AUC of combined detection of serum miR-128-3p and miR-126 was higher than that of single detection,and the differences were statistically significant(Z=2.768,2.152,all P<0.05).The differences in smoking,triglycerides(TG),uric acid(UA),fibrinogen(FIB)between the observation group and the control group were statistically significant(t/χ2=4.231~28.732,all P<0.05).Conclusion Patients with coronary artery disease have high expression of serum miR-128-3p and low expression of serum miR-126,which is correlated with Gensini score.The combined detection of serum miR-128-3p and miR-126 can improve the predictive efficiency of coronary artery disease,and the occurrence of coronary artery disease is related to multiple factors,which should be given clinical attention.

5.Clinical observation of Budigafol combined with Staphylococcus and Neisseria tablets in the maintenance treatment of patients with stable moderate to severe chronic obstructive pulmonary disease

OBJECTIVE To investigate the clinical effect of Budigafol combined with Staphylococcus and neisseria tablets in maintenance treatment of patients with stable moderate to severe chronic obstructive pulmonary disease (COPD). METHODS A total of 122 patients with stable moderate to severe COPD who were admitted to the infections diseases ward of the department of respiratory and critical care medicine in our hospital from October 1, 2021 to January 31, 2023 were divided into control group (n=61) and observation group (n=61) according to the random number table method. Patients in the control group were treated with Budigafol inhalation aerosol alone (2 presses each time, twice a day), and patients in the observation group were treated with Staphylococcus and neisseria tablets (1.2 mg each time, 3 times a day) on the basis of the control group. The treatment course of both groups was 3 months. The quality of life, exercise tolerance, lung function, inflammatory indexes, immune function, as well as the number of acute attacks of COPD and the occurrence of adverse reactions were compared between the two groups before and after treatment. RESULTS Control group and observation group shed 12 and 9 patients, respectively. After treatment, the 6 minute walking distance of the two groups of patients was significantly prolonged compared with that before treatment (P<0.05). The COPD assessment test questionnaire score, St George’s respiratory questionnaire score, the ratio of residual volume to total lung capacity, the fractional exhaled nitric oxide, the serum levels of C-reactive protein and interleukin-6 were significantly E-mail:13956685295@163.com decreased compared with those before treatment (P<0.05). The forced expiratory volume in one second (FEV1), the ratio E-mail:13966580920@163.com of FEV1 to forced vital capacity, the percentage of the measured value of carbon monoxide diffusion capacity to the predicted value, and the percentage of the measured value of FEV1 to the predicted value were significantly increased compared with those before treatment (P<0.05). The improvement of the above indicators in the observation group was significantly better than that in the control group (P<0.05). Compared with before treatment, there was no statistical significance in the immune function indexes of the control group or the serum levels of immunoglobulin A (IgA) and IgM in the observation group after treatment (P>0.05). The percentages of CD3+ , CD4+ , B lymphocytes and natural killer cells, the levels of CD4+/CD8+ and IgG were significantly increased in the observation group (P< 0.05), the percentage of CD8+ cells was significantly decreased (P<0.05), and the improvement degree of the observation group was significantly better than that of the control group (P<0.05). There was no significant difference in the number of acute exacerbations of COPD during the follow-up period and the incidence of adverse events during treatment between the two groups (P>0.05). CONCLUSIONS Budigafol combined with Staphylococcus and neisseria tablets can effectively improve the immune function of patients with stable moderate to severe COPD, further reduce the level of inflammation, and improve their exercise tolerance and their quality of life with good safety.
6.Effects of hyperbaric oxygen combined with warming needle moxibustion on physical function, joint range of motion, and inflammatory factors in patients with ankylosing spondylitis

Objective:To explore the effects of hyperbaric oxygen (HBO) combined with warming needle moxibustion on the improvement of physical function, joint range of motion, and inflammatory factors in patients with ankylosing spondylitis (AS).Methods:A total of 128 AS patients admitted to the Department of Traditional Chinese Medicine of Yantai Mountain Hospital from June 2017 to December 2018 were selected as the study objects and randomly divided into control group ( n=64) and observation group ( n=64). The control group was given conventional treatment combined with warming needle moxibustion, and the observation group was given HBO on the basis of the treatments in the control group. The physical function of the patients was assessed by using the Bath Ankylosing Spondylitis Functional Index (BASFI), and the visual analogue scale (VAS) score was included to jointly evaluate the lumbosacral back pain and joint pain. The Bath Ankylosing Spondylitis Metrology Index (BASMI) was used to assess joint range of motion, and the enzyme linked immunosorbent assay (ELISA) was used to detect the changes in inflammatory factor levels before and after treatment. Results:After treatment, the physical function and the severity of pain of the patients in both groups were significantly improved compared with those before treatment ( P<0.05), and the BASFI and VAS scores of the observation group were significantly lower than those of the control group ( P<0.05). Meanwhile, the test results of thoracic range of motion, Schober test, fingertips to floor distance, lumbar lateral flexion, extension of the lumbar spine, and occiput to wall distance were all significantly improved, and the improvements of the above tests in the observation group wee all greater than those in the control group, all with statistically significant differences ( P<0.05). The levels of inflammatory factors, such as erythrocyte sedimentation rate (ESR), C-reactive protein (CRP), and tumor necrosis factor-α (TNF-α) significantly decreased ( P<0.05), moreover, the levels of those factors in the observation group were all lower than those in the control group, all with statistically significant differences ( P<0.05). The total effective rate in the observation group (92.19%) was higher than that in the control group (78.13%), and the difference was statistically significant ( P<0.05). Conclusion:Hyperbaric oxygen combined with warming needle moxibustion can significantly improve the curative effect on AS patients by effectively reducing the levels of inflammatory factors, decreasing the pain, and improving the physical function and joint range of motion, which is worthy of clinical promotion and application.

Objective To explore the feasibility,safety and efficacy of customized porous tantalum acetabular patch made by three-dimensional (3D) printing technique in treating adult developmental dysplasia of the hip (DDH).Methods Eight adult patients with Crowe Ⅰ type DDH (2 men and 6 women,with a mean age of 43.75±7.81 years,range 33-58 years) who were treated with 3D printed customized porous tantalum acetabular patch hip reconstruction from January 2017 to September 2017 were included.The 3D printing technique was used for reconstructing and designing the optimal acetabular patch for the personalized hip joint of each patient.The acetabular patch was subjected to porous processing and finite element analysis until the biomechanical requirements were met.The 3D printing of porous tantalum acetabular patch and post-processing was subsequently performed.The acetabular patch was implanted through the anterior approach of the hip joint.The operative duration,intraoperative blood loss and complications were recorded.All the included patients were followed up at 1.5,3 and 6 months postoperatively.Pain was assessed using the visual analog scale (VAS),and the hip joint function was evaluated using the Harris score and gait analysis.The patients underwent anterior-posterior radiography and 3D computed tomography of the hip joint aiming to observe the position of the acetabular patch and osteoarthritis progression.Results The mean operative duration was 1.13±0.23 h,and the mean blood loss was 114.17±41.22 ml.All patients were followed up for 6 to 12 months,with an average of 8.2 months.The mean lateral central-edge angle and anterior central-edge angle ranged from 9.83°±5.34° preoperatively to 32.67°±2.53° postoperatively and from 3.83°± 2.79° preoperatively to 21.67°± 1.87° postoperatively,respectively.The rate of acetabular coverage increased from 57.33%±7.97% preoperatively to 87.33%±4.56% postoperatively.The VAS and Harris scores ranged from 2.92± 0.79 preoperatively to 0.83±0.72 postoperatively and from 69.67±4.62 preoperatively to 84.25±4.14 postoperatively with statistically significant difference,respectively.The results of gait analysis showed that the step speed,stride,the range of motion of hip and knee were better than that preoperatively.The images suggested a close contact between the tantalum acetabular patch and the iliac bone without loosening and progress of osteoarthritis in hip joint.Conclusion 3D printed customized porous tantalum acetabular patch could reconstruct the defect of acetabular,increase the coverage,and delay the progress of osteoarthritis of the hip joint.This method could reduce the difficulty of hip reconstruction,and the patients could obtain better joint function at the early stage.
