OBJECTIVE:
To preliminarily investigate the effects and mechanism of action of Yishen Yangsui Formula (, YSYSF)on the recovery of neurological function in rats with degenerative cervical myelopathy.
METHODS:
Fifty adult SD female rats were randomly divided into control group, sham group, model group, YSYSF group and positive drug group by using randomized numerical table method. In the model group, YSYSF group and positive drug group, polyvinyl alcohol acrylamide interpenetrating network hydrogel(water-absorbent swelling material) was used to construct a rat spinal cord chronic compression model. The sham group was implanted with the water-absorbent swelling material and then removed without causing spinal cord compression. The control group, the sham group and the model group were given equal amounts of saline by gavage, the group of YSYSF was given Chinese herbal medicine soup by gavage 9.1 g·kg-1 once a day, and the positive drug group was given tetrahexylsalicylglucoside sodium monosialate ganglioside by intraperitoneal injection 4.2 mg·kg-1 once a day. The motor function of the rats was assessed by the BBB method after 1, 3, 7, and 14 d of drug administration. The spinal cord tissues were taken from rats executed 14 d after drug administration, and the morphological changes of the spinal cord compression site were observed by HE staining, and the expression levels of Caspase-1, GSDMD, NLRP3, PYCARD, IL-1β, and IL-18 were detected in the area of spinal cord injury by Western blot method.
RESULTS:
The BBB scores of the control group and the sham group were normal at all time points after modeling, which were higher than the BBB scores of the model group, the YSYSF, and the positive drug group (P<0.05). From the 3rd day after gavage, at all time points, the BBB scores of rats in the YSYSF group and the positive drug group were higher than those of rats in the model group (P<0.05). The staining pattern of HE spinal cord tissue was normal in the control group and the sham group, and the HE spinal cord in the model group was severely damaged with a large number of neuron deaths, whereas the damage to the spinal cord and neuron cells was reduced in the YSYSF group and the positive drug group. The expression levels of caspase-1, GSDMD, NLRP3, PYCARD, IL-1β and IL-18 in the spinal cord of the model group were significantly higher than those of the sham group (P<0.0001), and the expression levels of caspase-1, GSDMD, NLRP3, PYCARD, IL-1β, and IL-18 in the YSYSF group and the drug group were significantly lower than those in the model group (P<0.05).
CONCLUSION
YSYSF can improve the motor function of rats with degenerative cervical spinal cord disease, alleviate the pathological changes, and promote the recovery of spinal cord neurological function. The specific mechanism may be related to the inhibition of the activation of inflammatory vesicles NLRP3 and PYCARD, the reduction of the release of inflammatory factors IL-1β and IL-18, the reduction of the expression of caspase-1 and GSDMD, the reduction of cellular death, and the inhibition of inflammatory response.

A method for determination and targeted screening of diflorasone components in health products using ultra performance liquid chromatography-quadrupole time of flight mass spectrometry(UPLC-Q-TOF/MS)was established.Four representative diflorasone and esters(diflorasone, diflorasone diacetate,diflorasone-17-propionate,and diflorasone-21-propionate)were selected to optimize the pretreatment conditions, and 10 mL of extraction solvent dosage,15 min of extraction time and 5 g of salting-out agent as the optimal conditions were selected by response surface methodology. The results showed that the four analytes exhibited good linearity within the concentration range of 2.0?100 μg/L with the chromatographic peak area, and the correlation coefficients(R2)were all greater than 0.9990,while the results of recovery and relative standard deviation could satisfy the requirements of determination. The common characteristic ions of diflorasone and esters werem/z121 andm/z335,and their specific structures were obtained by analyzing the cleavage pathway based on the optimized determination conditions.A targeted screening method for other esters of diflorasone based on characteristic ions guidance strategy was established. This method had many advantages such as high efficiency, high sensitivity and good reproducibility, and could be used for targeted screening of diflorasone and esters in health products. The developed characteristic ion guided strategy could be employed to construct mass spectral databases for various glucocorticoids, enabling comprehensive targeted screening across a broad range of compounds.

The paper reports a 32-year-old female acute myeloid leukemia patient who developed graft-versus-host disease after paternal hematopoietic stem cell transplantation, which subsequently led to renal thrombotic microangiopathy. She subsequently required a kidney transplant from the same donor 5 years later due to renal failure. Considering that both the bone marrow and kidney were from the same donor and the recovery of renal function was favorable, immunosuppressive therapy was discontinued after a short course of anti-rejection treatment, with maintained stable kidney function. This case suggests that under the condition of high chimerism, allogeneic hematopoietic stem cell transplantation and kidney transplantation from the same donor can achieve immune tolerance, potentially improving solid organ transplantation success rate. The findings provide a novel therapeutic approach for solid organ transplantation following allogeneic hematopoietic stem cell transplantation.

Paraplegia caused by spinal cord injury is a serious neurological complication, for which surgery is currently the main treatment method. Due to different surgical approaches, patients are usually expected to maintain a passive prone position for a long time or switch between the supine and prone positions. Affected by multiple factors such as neurogenic sensory disorders, pathological changes in muscle tone and operative duration, the risk of intraoperative acquired pressure injury (IAPI) is significantly increased. Current clinical prevention strategies for IAPI in these patients predominantly focus on localized pressure relief during positioning, lacking systematic, standardized comprehensive prevention protocols or evidence-based guidelines. To address it, Department of Nursing, Orthopedics Branch, China International Exchange and Promotive Association for Medical and Health Care, Spinal Trauma Professional Committee, Orthopedics Branch, Chinese Medical Doctor Association, Nursing Group of Spine and Spinal Cord Professional Committee of Chinese Association of Rehabilitation Medicine organized experts in relevant fields to formulate Guideline for the prevention of intraoperative acquired pressure injury in paraplegic patients with spinal cord injury ( version 2025), based on evidence-based medical evidence and latest research results and clinical practice at home and abroad. Eleven recommendations were put forward from the aspects of preoperative risk assessment, intraoperative prevention strategies, postoperative handover and monitoring, and supportive mechanisms for IAPI prevention, aiming to standardize the prevention measures and management strategies of IAPI in paraplegic patients with spinal cord injury and accelerate the recovery of patients and improve the therapeutic effect.

Thoracolumbar spine fracture often leads to severe pain, functional impairments, and neurological deficits, for which open reduction and internal fixation can effectively restore the spinal structural stability. Open decompression and reduction with internal fixation can help relieve spinal cord compression and improve spinal function in cases of concomitant cord injury. Although spinal stability can be restored through surgery, patients often face chronic pain and functional impairments postoperatively. A postoperative rehabilitation program is critical in optimizing therapeutic outcomes, reducing complications, and minimizing the risk of secondary injuries. However, current rehabilitation methods, such as physical therapy, functional training, and pain management, are confronted with problems in clinical practice, including significant variation in efficacy, poor patient adherence, and prolonged rehabilitation period. There is an urgent need for a unified rehabilitation strategy to address these problems. To this end, the Spinal Trauma Group of the Orthopedic Physicians Branch of the Chinese Medical Association and the Spine Health Professional Committee of the Chinese Human Health Technology Promotion Association organized experts from relevant fields to formulate Evidence-based guidelines for rehabilitation treatment after internal fixation of thoracolumbar spine fracture in adults ( version 2025) by integrating evidences from clinical researches and advanced rehabilitation concepts at home and abroad. A total number of 14 recommendations concerning the rehabilitation treatment with multimodal analgesia, psychological intervention, deep vein thrombosis prevention, core muscle and extremity exercise, appropriate use of braces, early weight-bearing, device-aided rehabilitation exercise, neuroregulatory therapy, rehabilitation team were put forward, aiming to standardize the post-operative rehabilitation process following internal fixation, promote the functional recovery, and enhance patients′ quality of life.

Objective:To screen the risk factors of acute kidney injury (AKI) in patients with multiple trauma, construct a prediction model accordingly, and evaluate its predictive value. Methods:A retrospective cohort study was performed to analyze the clinical data of 560 multiple trauma patients who were admitted to while Affiliated Hospital of Jiangsu University from January 2017 to June 2023, including 424 males and 136 females, aged 18-91 years [(55.5±15.0)years]. The patients were randomly divided into a training set ( n=392) and validation set ( n=168) with a ratio of 7∶3. Of all, 77 patients were combined with AKI in the training set, while 33 patients combined with AKI in the validation set. The AKI group and non-AKI group in the training set were compared in terms of gender, age, hypertension, diabetes, cause of injury, abbreviated injury scale (AIS) score of head and neck injury, AIS score of maxillofacial injury, AIS score of chest injury, AIS score of abdominal injury, AIS score of extremities and pelvic injury, AIS score of body surface injury, systolic blood pressure, diastolic blood pressure, heart rate, respiratory rate, body temperature, red blood cell and plasma transfusion volume within 24 hours following admission, emergency surgery, mechanical ventilation, vasoactive drug therapy, Glasgow coma score (GCS) on admission, revised trauma score (RTS) on admission, acute physiology and chronic health assessment II (APACHE II) on admission, injury severity score (ISS) on admission, and laboratory test results on admission including white blood cell count, neutrophil count, lymphocyte count, C-reactive protein, hemoglobin, platelet count, activated partial thromboplastin time (APTT), prothrombin (PT), fibrinogen (FIB), thrombin time (TT), international normalized ratio (INR), D-dimer, blood lactate, base excess, total bilirubin, alanine aminotransferase (ALT), aspartate aminotransferase (AST), albumin, globulin, urea nitrogen, serum creatinine, blood glucose, potassium, sodium and chloronium. In the training set, univariate analysis and Lasso regression analysis were used to screen the risk factors of AKI in patients with multiple trauma, which were then included into multivariate logistic regression analysis to identify the independent risk factors. A nomogram prediction model was constructed using the R software based on the above independent risk factors. Hosmer-Lemeshow (H-L) goodness-of-fit test was performed to evaluate the fitting degree of the prediction model in the training set and the validation set, and the receiver operating characteristic (ROC) curve, calibration curve and clinical decision curve (DCA) were plotted in the training set and the validation set to evaluate the predictive performance of the prediction model. Results:There were statistically significant differences in AIS score of abdominal injury, heart rate, body temperature, red blood cell and plasma transfusion volume within 24 hours following admission, emergency surgery, mechanical ventilation, vasoactive drug therapy, GCS on admission, RTS on admission, APACHE II on admission, ISS on admission as well as hemoglobin, platelet count, APTT, PT, FIB, TT, INR, blood lactate, base excess, AST, albumin, globulin, urea nitrogen, serum creatinine, blood glucose and sodium on admission between the AKI group and the non-AKI group ( P<0.05 or 0.01). The characteristic variables screened by Lasso regression analysis included AIS score of abdominal injury, red blood cell transfusion volume within 24 hours following admission, mechanical ventilation, vasoactive drugs therapy, blood lactate on admission, blood creatinine on admission, AST on admission, and blood sodium on admission. Multivariate logistic regression analysis showed that red blood cell transfusion volume within 24 hour following admission ( OR=1.09, 95% CI 1.01, 1.18), mechanical ventilation ( OR=2.49, 95% CI 1.06, 5.85), vasoactive drug therapy ( OR=2.04, 95% CI 1.03, 4.03), blood lactate on admission ( OR=1.10, 95% CI 1.01, 1.21) and serum creatinine on admission ( OR=1.02, 95% CI 1.01, 1.03) were independent risk factors for AKI in patients with multiple trauma ( P<0.05). The regression equation was constructed: Logit[ P/(1- P)]=0.086 2×"red blood cell transfusion volume within 24 hour following admission"+0.912 7×"mechanical ventilation"+0.713 2×"vasoactive drug therapy"+0.098 9×"blood lactate on admission"+0.019 2×"serum creatinine on admission" -4.822 3. H-L goodness-of-fit test showed χ2 value of 9.50 in the training set ( P>0.05) and 6.43 in the validation set ( P>0.05). The results of the ROC curve indicated that the area under the curve (AUC) was 0.84 (95% CI 0.78, 0.89) in the training set and 0.80 (95% CI 0.72, 0.88) in the validation set. The calibration curves showed good agreement with the actual curves, with the predicted probability consistent with the actual probability in both training set and validation set. DCA analysis showed that the threshold probability ranged from 2% to 70% with the net benefit rate of the prediction model greater than 0 in the training set, while the threshold probability ranged from 3% to 69% with the net benefit rate of the prediction model greater than 0 in the validation set. Conclusions:Red blood cell transfusion volume within 24 hours following admission, mechanical ventilation, vasoactive drug therapy, lactate and serum creatinine on admission are independent risk factors for AKI in patients with multiple trauma. The nomogram prediction model based on the above 5 predictive variables of AKI in patients with multiple trauma shows good predictive efficacy and clinical application value.
