Objective:To analyze the relationship between serum neutrophil elastase (NE) level and assisted pregnancy outcome in infertility patients with polycystic ovary syndrome (PCOS).Methods:A total of 137 infertility patients with PCOS who planned to undergo in vitro fertilization-embryo transfer (IVF-ET) treatment in Xianyang Central Hospital from January 2020 to January 2023 were prospectively selected. Serum NE level was measured before IVF-ET transplantation, and the pregnancy-assisted outcome of infertility patients with PCOS were analyzed. The relationship between serum NE level and assisted pregnancy outcome in PCOS infertility patients was analyzed. Results:The proportion of age ≥ 35 years old and serum NE levels in the failure group were higher than those in the success group: 50.00% (41/82) vs. 30.91%(17/55), (73.64 ± 6.74) mg/L vs. (60.71 ± 5.99) mg/L, while serum anti-Mullerian hormone (AMH) level were lower than those in the success group: (3.61 ± 1.17) μg/L vs. (4.89 ± 1.25) μg/L. The thickness of the endometrium was thicker than that in the success group: (10.63 ± 1.78) mm vs. (7.88 ± 1.28) mm, with statistically significant differences ( P<0.05). Multivariate Logistic regression analysis showed that thick endometrial thickness, high serum NE level and low serum AMH level were associated with assisted pregnancy failure in PCOS infertility patients ( P<0.05). A receiver operating characteristic (ROC) curve was drawn, and the results showed that the AUC of endometrial thickness, serum NE and AMH levels for predicting IVF-ET assisted pregnancy failure in PCOS infertility patients was greater than 0.7, which had certain predictive value. Serum NE had the best predictive effect, and when serum NE was 66.60 mg/L, ideal sensitivity and specificity of 85.40% and 87.30% could be obtained. Conclusions:The level of NE is related to the outcome of assisted pregnancy in PCOS infertility patients, and an increase in serum NE level can increase the risk of assisted pregnancy failure in PCOS infertility patients.

Objective:To explore the surgical technique and clinical effect of pressure boost in repairing soft tissue defects of limbs with thinned anterolateral thigh perforator flap (ALTP) .Methods:From January, 2015 to December, 2018, 18 cases with soft tissue defects of limbs with various damages of blood vessels and nerves with explosure of tendon and bone. There were 13 males and 5 females aged between 18 to 56 (averaged of 36.3) years, which were 6 defects in shank, 4 in foot and ankle, 5 in forearm, and 3 in hand. The soft tissue defect area was 7 cm ×12 cm to 13 cm ×30 cm. Thinned ALTP was used to repair the wound surface. The perforating vessels of the distal flap were anastomosed with one branch of the internal vessel pedicle flap to increase the pressure hence the blood supply of the distal region. The donor sites were sutured directly or covered by skin graft. Followed-up was conducted by 1-2 monthly clinic visits and telephone or on-line review to check the flap survival and recovery of functions. Results:All flaps survived without arterial or venous crisis. One flap had partial necrosis at the distal end, and healed after dressing change. One case had a swelling flap due to a congestion beneath the flap. The wound achieved primary healing after removal of sutures, ligation of subcutaneous vessels and drainage of hematoma. All patients were followed-up for 6 to 18 (average, 9.5) months. All flaps had good appearance and texture. After rehabilitation treatment, most of the joint activity had been recovered: extension and flexion of wrists joints ranged 60°-80°, 70°-80° for metacarpophalangeal joints and 40°-60° for ankle joints. One patient underwent ankle joint dorsiflexion function reconstruction and flap thinning at 6 months after operation due to the defects of most of the extensor tendon. Conclusion:During the use of free ALTP to repair soft tissue defect of limbs, application of the technique of pressure boost is able to increase blood supply to the distal region of flap. It helps to reduce the incidence of infection and necrosis at the edge of the flap.

AIM: To investigate the effects of uric acid sodium salt (UANa) as adjuvant on humoral and cellular immune response in BALB/c mice. METHODS: BALB/c mice were immunized with trichosanthin (TCS) as antigen together with UANa suspension as adjuvant. The antibody titers of IgG were detected by enzyme-linked immunosorbent assay. Dendritic cells (DC) were induced in vitro, the phenotypes of DC were analyzed by flow cytometry and the effect of UANa on DC maturity was evaluated. A delayed-type hypersensitivity (DTH) model was used to analyze the effect of UANa on cellular immune responses in vivo. The in vitro proliferation of lymphocytes was determined by ConA stimulation. RESULTS: Freunds adjuvant greatly enhanced the antibody response of mice to TCS, while UANa adjuvant failed to promote the antibody response but significantly reduced the antibody response as compared to TCS only. No effect of UANa on the expression of CD11c and CD83 in DC was observed by flow cytometry analysis. However, UANa significantly enhanced the expression of MHC II molecule. In the DTH model, UANa enhanced the degree of allergen-induced ear swelling and promoted the ability of lymphocyte proliferation in vitro. CONCLUSION: UANa suspension as adjuvant significantly enhances the cellular immune response but inhibits the humoral immune response to a certain degree, suggesting that UANa has potential application in the vaccine research.
