Objective To evaluate the diagnostic value of adenosine load-resting gated myocardial perfusion imaging for three-vessel disease in coronary artery disease(CAD)patients using coronary angiography as the gold standard.Methods A retrospective study was conducted,including 318 patients diagnosed with CAD who underwent coronary angiography at Yanan Hospital Affiliated to Kunming Medical University from January 2021 to December 2022.Based on the results of coronary angiography,the 318 CAD patients were divided into a three-vessel disease group(n=166)and a non-three-vessel disease group(single and double vessel disease group,n=152).All the subjects underwent adenosine stress-resting GMPI within two weeks.Adenosine stress-resting GMPI myocardial perfusion parameters(SSS,SRS,SDS),cardiac function parameters(LVEF,LVEDV,LVSV)and left ventricular mechanical contraction synchronization parameters(PSD,PHB)were collected.The diagnostic value of adenosine stress-resting gated myocardial perfusion imaging for three-vessel disease in CAD was explored.Results Among the perfusion parameters,SSS had the highest AUC of 0.781,while sLVEF had the highest AUC of 0.748 among cardiac function parameters,and sPHB had the highest AUC of 0.724 among synchrony parameters.The AUCs of combined parameters were all higher than those of perfusion parameters,cardiac function parameters,and synchrony parameters(P＜0.05).The changes in Δ LVESV and Δ LVEF between the three-vessel disease group and the non-three vessel disease group showed statistical significance(P＜0.05).Conclusion The perfusion,cardiac function and synchronization parameters of adenosine stress-resting gated myocardial perfusion imaging have high diagnostic value for three-vessel coronary heart disease,and the combined detection of the three parameters provides even greater diagnostic value for three-vessel coronary heart disease.

Hepatic steatosis can be observed in chronic liver diseases of different etiologies. The main predisposing factors for hepatic steatosis include chronic viral hepatitis， cholestatic liver disease， alcoholic liver disease， and nonalcoholic fatty liver disease. Simple fatty liver disease is the initial manifestation of hepatic steatosis， followed by steatohepatitis， liver fibrosis， liver cirrhosis， and even hepatocellular carcinoma. With the development of medical imaging technology， magnetic resonance imaging-proton density fat fraction （MRI-PDFF） has been widely used in the diagnosis of fatty liver disease （FLD） in clinical practice. MRI-PDFF is gradually becoming the gold standard for the noninvasive diagnosis of FLD due to its high accuracy and good repeatability. This article reviews the clinical application of MRI-PDFF in liver fat quantification and related research advances.

BACKGROUND:Studies have exhibited that inhibiting apoptosis caused by endoplasmic reticulum stress can save part of nerve function.Epigallocatechin-3-gallate can inhibit endoplasmic reticulum stress,but it has poor bioavailability and is difficult to penetrate the blood-brain barrier.In combination with exosomes targeting spinal cord repair and high-potency drug loading,theoretically,the combination of the two can play a greater role in spinal cord protection. OBJECTIVE:To investigate the effects of epigallocatechin-3-gallate combined with bone marrow mesenchymal stem cell exosomes on endoplasmic reticulum stress and neurological function in rats with spinal cord ischemia/reperfusion injury. METHODS:Fifty SD male rats were randomly divided into sham surgery group,model group,epigallocatechin-3-gallate group,exosome group,and combined treatment group,with 10 rats in each group.The spinal cord ischemia/reperfusion injury model was made in the other four groups except for the sham surgery group.Local injection of physiological saline,exosomes,epigallocatechin-3-gallate,epigallocatechin-3-gallate + bone marrow mesenchymal stem cell exosomes was performed 2 hours after surgery through a caudal vein.Neurological function scores were performed on 7,14 and 28 days after spinal cord injury.14 days after spinal cord injury,hematoxylin-eosin staining,Nissl staining,and immunofluorescence staining of endoplasmic reticulum stress markers such as ATF6 and GADD153 were performed in the spinal cord tissues. RESULTS AND CONCLUSION:(1)Compared with the sham surgery group,neurological function scores of the model group,exosome group,epigallocatechin-3-gallate group and combined treatment group all decreased to different degrees.The neurological function score of combined treatment group was better than that of the epigallocatechin-3-gallate group,exosome group and model group 14 days after surgery(P<0.05).The neurological function score of the combined treatment group was better than that of the model group and epigallocatechin-3-gallate group 28 days after surgery(P<0.05).(2)Hematoxylin-eosin staining and Nissl staining displayed that the number of neurons in the model group decreased,with a large number of cavity necrosis and scar hyperplasia in the spinal cord injury area.The number of neurons and peripheral cavity necrosis improved to varying degrees in the epigallocatechin-3-gallate group,exosome group,and combined treatment group,with the most significant improvement in the combined treatment group.(3)The expression of endoplasmic reticulum stress-related proteins ATF6 and GADD153:14 days postoperatively,the expression of GADD153 in the combined treatment group was lower than that in the model group and epigallocatechin-3-gallate group(P<0.05),and the expression of ATF6 in the combined treatment group was lower than that in the model group,exosome group,and epigallocatechin-3-gallate group(P<0.05).(4)These findings confirm that epigallocatechin-3-gallate combined with bone marrow mesenchymal stem cell exosome can enhance the neurological function in rats with spinal cord ischemia/reperfusionn injury,which may be associated with the inhibition of the expression of endoplasmic reticulum stress-related proteins ATF6 and GADD153.

Objective:To explore the utility of adenosine stress-rest gated myocardial perfusion imaging (G-MPI) in evaluation of left ventricular (LV) myocardial perfusion, heart function, and synchronization in patients with multivessel coronary artery disease after coronary artery bypass grafting (CABG).Methods:Fifty-five patients (42 males, 13 females, age (59.3±9.0) years) with multivessel coronary artery disease who underwent CABG surgery in Yan′an Hospital Affiliated to Kunming Medical University between January 2021 and June 2023 were retrospectively collected. All of them underwent G-MPI (one-day method) one week before and six months after CABG. Paired t-test or Wilcoxon signed rank test was used to compare LV myocardial perfusion parameters including summed stress score (SSS), summed rest score (SRS), and summed different score (SDS) before and after CABG, as well as cardiac function parameters (LV ejection fraction (EF), LV end-diastolic volume (EDV), LV end-systolic volume (ESV), stroke volume (SV), peak filling rate (PFR)), and LV mechanical contraction synchronization parameters such as phase histogram bandwidth (PHB) and phase standard deviation (PSD). Differences of the changes of parameters after CABG compared to baseline (Δ) between adenosine stress imaging and rest imaging were also compared by Wilcoxon signed rank test. The relationships between rest LVEF, rest PFR and myocardial perfusion parameters were analyzed by Spearman rank correlation analysis. Results:After CABG, the SSS, SRS, and SDS were significantly lower than those before surgery (6(3, 12) vs 16(9, 23), 1(0, 9) vs 4(1, 15), 3(1, 5) vs 8(6, 12); z values: 6.37, 4.84, 6.24, all P<0.001); postoperative rest/stress LVEF(60%(49%, 67%), 58%(48%, 68%)) and PFR (3.67(3.12, 4.28), 3.23(2.77, 4.43) EDV/s) significantly increased compared with preoperative values (51%(33%, 62%), 53%(27%, 61%); 2.76(2.08, 3.52), 2.83(1.86, 3.62) EDV/s; z values: 3.23-5.58, all P<0.01); postoperative rest/stress LVEDV and LVESV were significantly lower than those before surgery ( t values: 6.40, 5.27; z values: 5.33, 5.40; all P<0.001); rest/stress PHB (45°(35°, 70°), 53°(44°, 113°)) and PSD (14.7°(10.9°, 27.3°), 20.0°(14.6°, 33.8°)) were significantly lower than those before surgery (60°(45°, 131°), 83°(58°, 198°), 20.4°(16.0°, 49.1°), 27.2°(19.6°, 60.4°); z values: 4.19-4.81, all P<0.001). The ΔSSS was greater than the ΔSRS, and the ΔPFR was greater in rest imaging than that in stress imaging ( z values: 6.24, 2.77, both P<0.05). Rest LVEF, PFR were negatively correlated with SSS and SRS ( rs values: from -0.741 to -0.431, all P<0.05). Conclusion:The LV myocardial perfusion, function, and mechanical contraction synchronicity information obtained from adenosine stress-rest G-MPI can be used to evaluate the recovery after CABG, which may help to better identify patients who may have adverse cardiac events.

Objective:To investigate the regulatory effect of long non-coding RNA (lncRNA) FTX on gastric cancer cell proliferation through miR-22-3p/NOD-like receptor protein 3 (NLRP3) inflammasome pathway.Methods:The gastric cancer cell line NCI-N87 were divided into blank control group, si-FTX-NC group, si-FTX group, si-FTX+miR-22-3p inhibitor-NC group and si-FTX+miR-22-3p inhibitor group. Quantitative real-time fluorescent PCR was performed to analyze the expression levels of lncRNA FTX and miR-22-3p, clone formation assay was performed to analyze the proliferation ability of NCI-N87 cells, western blotting was performed to analyze the expressions of NLRP3 inflammasome pathway proteins, and dual-luciferase reporter assay was performed to analyze the targeting relationship between lncRNA FTX and miR-22-3p.Results:The relative expressions of lncRNA FTX in the blank control group, si-FTX-NC group, si-FTX group, si-FTX+miR-22-3p inhibitor-NC group and si-FTX+miR-22-3p inhibitor group were 1.03±0.09, 1.01±0.15, 0.42±0.08, 0.45±0.06 and 0.46±0.13 respectively, with a statistically significant difference ( F=52.19, P<0.001). The relative expressions of miR-22-3p were 1.04±0.12, 0.97±0.08, 2.26±0.15, 2.23±0.13 and 1.15±0.11 respectively, with a statistically significant difference ( F=178.53, P<0.001). Compared with the blank control group and si-FTX-NC group, the relative expressions of lncRNA FTX in the si-FTX group, si-FTX+miR-22-3p inhibitor-NC group and si-FTX+miR-22-3p inhibitor group decreased (all P<0.001). Compared with the blank control group, si-FTX-NC group and si-FTX+miR-22-3p inhibitor group, the relative expressions of miR-22-3p in the si-FTX group and si-FTX+miR-22-3p inhibitor-NC group increased (all P<0.001). The clones of the five groups were 115.50±7.25, 112.33±8.46, 54.83±5.17, 56.17±6.32 and 85.67±9.43, with a statistically significant difference ( F=91.67, P<0.001). The levels of NLRP3 protein in the five groups were 1.84±0.17, 1.86±0.12, 0.95±0.09, 0.97±0.11 and 1.28±0.19, with a statistically significant difference ( F=60.62, P<0.001). Compared with the blank control group and si-FTX-NC group, the number of clones and the level of NLRP3 protein of the si-FTX group, si-FTX+miR-22-3p inhibitor-NC group and si-FTX+miR-22-3p inhibitor group decreased (all P<0.05). Compared with the si-FTX+miR-22-3p inhibitor group, the number of clones and the level of NLRP3 protein in the si-FTX group and si-FTX+miR-22-3p inhibitor-NC group decreased (all P<0.05). The dual-luciferase reporter assay found that miR-22-3p was the target gene of lncRNA FTX. Conclusion:Silencing the expression of lncRNA FTX can inhibit the proliferation of gastric cancer cells, and the mechanism may be related to the regulation of lncRNA FTX on the miR-22-3p/NLRP3 inflammasome pathway.

Objective: To explore the causes of vascular crisis caused by replantation of severed finger, and summarize the experience of prevention and treatment. Methods: From April 2015 to February 2017, 108 patients with wounded finger treated in the First People's Hospital of Yongkang were divided into two groups according to the occurrence of postoperative vascular crisis situation: the control group (56 cases, vascular risk), the observation group (52 cases, vascular crisis happened). The clinical data of all patients were collected, the various data were compared, and the causes of vascular crisis were analyzed.The observation group was given low molecular dextran treatment, the changes of blood coagulation function indicators before and after treatment were observed. Results: The age, disconnection plane, ischemia time had statistically significant differences between the two groups(all P<0.05). After treatment, the four coagulation function index in the observation group [fiber protease: (3.32±0.29)g/L, activated partial thrombin time: (27.56±2.08)s, thrombin time: (12.13±1.62)s, prothrombin time: (11.47±1.29)s] were improved obviously (t=4.86, 8.63, 6.04, 4.70, all P<0.05). Conclusion The age, disconnection plane and ischemic time are the factors that cause vascular crisis.In addition, postoperative pain, patients' mood, postoperative posture and other factors may also lead to vascular crisis.Preoperative assessment of the condition should be done well, postoperative monitoring should be strengthened, and prophylactic medication can be used to prevent vascular crisis.

Objective To lay foundation for the establishment of national standards of electroacupuncture devices by exploring the problems existing in their clinical application.Methods An analysis was made of the models, stimulus waveforms and complex waveform characteristics, stimulus frequencies, current intensities and pulse widths of electroacupuncture devices reported in clinical literature. The actual output frequencies of three kinds of electroacupuncture devices were measured using RM6240C multi-channel physiological signal acquisition and processing system to observe the accuracy of their actual output parameters and summarize the problems existing in electroacupuncture devices. Results The reports on electroacupuncture device parameters showed that of the five indicators analyzed statistically, the frequency of described waveforms was the highest (96.2%); the frequencies of described models (88.8%), stimulus frequencies (61.7) and current intensities (11.2%) were next; the frequency of described pulse widths was the lowest (1.8%). The electroacupuncture device most commonly used clinically was G6805 series, but the subtypes were confused because of many manufacturers. The waveform was described more frequently but complex waveform characteristics were less. The difference of frequencies of clinical application of electroacupuncture was larger. They were from 0.8-400 Hz but described less accurately. The output frequency was indicated by the range in most of the literature. The output adjusters of the three kinds of electroacupuncture devices did not correspond to their actual output frequencies. There was a larger difference between the actual range of output frequencies and the adjustable frequency range indicated in the instruction.Conclusions Electroacupuncture devices have the problems of lacking a display unit of actual output parameter values, the adjusting parameters of the knob adjustable controller being not precise enough and the actual values of output frequencies not reaching the rated range in the instruction.

Objective To evaluate the diagnostic value of procalcitonin in early ventilator-associated pneumonia. Methods CNKI database, Wanfang database and PubMed (2000-2014) were applied for document retrieval through keyword searching approach. Meta-analysis was used in all eligible studies. Results Thirteen studies were collected. The sample bank includes 1 137 cases , of which 599 cases were diagnosed as ventilator-associated pneumonia. Heterogeneity test showed that the homogeneity of the studies was good. The value of the weighted sensitivity was 59% (43% ~ 73%), the specificity was 89% (82% ~ 93%), the positive likelihood ratio was 3.69 (3.21 ~ 4.23), the negative likelihood ratio was 0.42 (0.32 ~ 0.55), and the SROC area under the curve (AUC) was 0.86. The value of the weighted sensitivity was 81%(73% ~ 87%), the specificity was 83%(73% ~ 90%). Conclusion Procalcitonin can be regarded as a valuable marker in early ventilator-associated pneumonia diagnosis , when the cut-off point of PCT is 2.0 μg/L.

Objective To investigate the relationship between inflammation and blood coagulation function in the patients with acute exacerbation of chronic cor pulmonale (AECCP) and discuss the potential mechanism and influence on the patients. Methods The present study was based on 30 healthy controls and 141 cases of AECCP in our hospital from January 2011 to June 2014.Levels of white blood cell (WBC), neutrophil (NEUT), high-sensitivity C-reactive protein (hs-CRP, Complement 3 (C3), prothrombin time (PT), activated partial thromboplastin time (APTT), fibrinogen (FIB) and thrombin time (TT) in the patients were determined. Results Compared with the healthy controls, the patients had higher levels of WBC, NEUT, hs-CRP, PT, APTT, FIB, TT (all P < 0.001) and lower level of C3 (P < 0.001). Significant positive correlations were found between the levels of WBC, NEUT and FIB (r = 0.196 and r = 0.199, both P < 0.05); hs-CRP and APTT, FIB(r = 0.234, P < 0.01 and r = 0.466, P < 0.001); C3 and FIB(r = 0.466, P < 0.001), and significant negative correlations were observed between the levels of C3 and PT, APTT, TT (r=-0.258, P<0.01;r=-0.279, P < 0.01 and r = -0.168, P < 0.05, respectively). Compared with the survival patients, the cases of death had higher levels of WBC and NEUT (both P < 0.01). The area under receiver operating characteristic curve of WBC and NEUT, predicting the prognosis, was 0.666 (95% CI 0.552, 0.780; P < 0.01) and 0.695 (95% CI 0.558, 0.801; P = 0.001) respectively. Conclusions Inflammation and blood coagulation function disorder usually coexist in the patients with AECCP, and are closely associated with the severity. Levels of both WBC and NEUT can be used as prognosis predictors for the patients.

Objective To investigate the infiltration and prognostic significance of tumor-infiltrating mast cells (TIMs) in nasopharyngeal carcinoma (NPC).Methods Immunohistochemistry for tryptase was performed on 154 NPC specimens.The median value of TIM density was used as a cutoff point to separate the patient cohort into two groups with either low or high TIM infiltration.The associations between TIM and clinicopathological factors were analyzed by Mann-Whitney U text.Survival curves were plotted according to the Kaplan-Meier method.Univariate and multivariate analyses were performed using the Log-rank test and the Cox proportional hazard models,respectively.P ＜ 0.05 was considered statistically significant.All statistical analyses were conducted using SPSS 13.0.Results TIM was mainly in the stroma of NPC and detected in all specimens.The median value of TIM density (25.60/high power field) was used as a cutoff point to separate the patient cohort into two groups with either low or high TIM infiltration.The density of TIM was positively correlated with N stage (Z =-2.193,P ＜ 0.05) and clinical stage (Z =-2.551,P ＜ 0.05).The 3-year overall survival (OS) and progression-free survival (PFS) of patients were 64.4％ and 55.7％ in the high TIM density group;78.3％ and 77.0％ in the low TIM density group.For survival evaluation,high density of TIM was associated with worse OS and PFS (P ＜ 0.05).Multivariate Cox regression model analysis showed TIM infiltration was an independent risk factor for both OS and PFS.Conclusions The density of TIM in NPC increased with tumor stage.High TIM infiltration was associated with poor overall survival and progression-free survival.
