1.The role of shed syndecan-4 in temporomandibular joint osteoarthritis in rats

Objective:
To investigate the mechanism of shed syndecan-4 (sSDC4) in temporomandibular joint osteoarthritis (TMJOA) in rats, aiming to provide experimental evidence for its prevention and treatment.
Methods:
This study was approved by the Institutional Animal Ethics Committee. Twelve 6-week-old female Sprague Dawley (SD) rats were randomly divided into two groups. They received a single intra-articular injection into the bilateral superior cavity of temporomandibular joint, which consisted of either 50 μL of 4 mg/mL monosodium iodoacetate (TMJOA model group) or 50 μL of phosphate-buffered saline (PBS, control group). After 4 weeks, the mandibular condylar cartilage was harvested for hematoxylin & eosin (H&E) staining, Safranin O-fast green (SO) staining, and type II collagen (Col-Ⅱ) immunohistochemical staining to assess the degree of cartilage degeneration. The synovium of the temporomandibular joint was collected for immunohistochemical staining to detect the expression levels of interleukin-6 (IL-6) and tumor necrosis factor-α (TNF-α) to evaluate the degree of synovial inflammation. Synovial fluid from the temporomandibular joint cavity was collected to measure sSDC4 levels by enzyme-linked immunosorbent assay (ELISA). In addition, 12 6-week-old female SD rats were randomly divided into a His-SDC4 group and a control group, receiving injections into the bilateral superior cavity of temporomandibular joint of either 100 ng/mL (50 μL) of His-SDC4 protein or 50 μL of PBS once every 3 days for a total of 28 days. The same experimental procedures were performed for H&E staining, SO staining, and immunohistochemical staining (Col-Ⅱ IL-6, TNF-α) to observe condylar cartilage degeneration and detect synovial inflammation. Rat synovial fibroblasts and condylar chondrocytes were cultured in vitro and randomly divided into a His-SDC4-stimulated (10 ng/mL) group and control group. Perform CCK-8 cytotoxicity assays and observe cellular morphology under optical microscopy, the mRNA expression levels of IL-6 and TNF-α were detected by real-time quantitative polymerase chain reaction (RT-qPCR), and the levels of IL-6 and TNF-α in cell culture supernatants were measured by ELISA.
Results:
Compared with the control group, the TMJOA group showed decreased condylar cartilage thickness, percentage of SO-positive area, and percentage of Col-Ⅱ-positive area (all P<0.001); an increased synovitis score (P<0.001) and increased percentages of IL-6- and TNF-α-positive cells in the synovium (all P<0.001); and a significant increase in sSDC4 levels in the synovial fluid (P=0.011). Following intra-articular injection of His-SDC4, condylar cartilage thickness, percentage of SO-positive area, and percentage of Col-Ⅱ-positive area all decreased (all P<0.001); the synovitis score increased (P=0.006), and the percentages of IL-6- and TNF-α-positive cells in the synovium increased (all P<0.001). In vitro experiments showed that His-SDC4 stimulation significantly upregulated the expression levels of IL-6 and TNF-α in both synovial fibroblasts and condylar chondrocytes (all P<0.01), and the levels of these two cytokines in the culture supernatants also significantly increased (all P<0.01).
Conclusion
During TMJOA progression, the level of sSDC4 in the synovial fluid is significantly elevated, which can directly stimulate synovial fibroblasts and condylar chondrocytes to secrete more pro-inflammatory cytokines, forming a vicious cycle that accelerates TMJOA progression.
2.Analysis of pharmaceutical clinic service in our hospital over the past five years

OBJECTIVE To summarize the current situation of pharmaceutical clinic service in our hospital over the past five years, and explore sustainable development strategies for service models of pharmaceutical clinics. METHODS A retrospective analysis was conducted on the consultation records of patients who registered and established files at the pharmaceutical clinic in our hospital from January 2019 to December 2023. Statistical analysis was performed on patients’ general information, medication- related problems, and types of pharmaceutical services provided by pharmacists. RESULTS A total of 963 consultation records were included, among which females aged 20-39 years accounted for the highest proportion (66.04%); obstetrics and gynecology- related consultations accounted for the largest number of cases. Additionally, 80 patients attended follow-up visits at our hospital’s pharmaceutical clinic. A total of 1 029 medication-related issues were resolved, including 538 cases of drug consultations (52.28%), 453 medication recommendations (44.02%), 22 medication restructuring(2.14%), and 16 medication education (1.55%); the most common types of medication-related problems identified were adverse drug events(70.07%). CONCLUSIONS Although the pharmaceutical clinic has achieved recognition from clinicians and patients, challenges such as low awareness among healthcare providers and the public persist. Future efforts should focus on strengthening information technology construction, enhancing pharmacist training, and establishing various forms of outpatient pharmaceutical service models.
3.Impact of case manager-led stratified out-of-hospital health management on health behaviour ability in the patients with chronic disease

Objective To explore the effect of stratified out-of-hospital health management led by case health manager on the health behaviour ability of the patients with chronic diseases.Methods A convenience sampling method was employed to select 481 patients with chronic diseases who underwent physical examinations at outpatient department of a Tier-IIIA hospital from April 2022 to April 2023.A health management team led by case health managers conducted questionnaire survey to investigate the individual characteristics of the patients,established personal record based on the physical examination,and implemented stratified out-of-hospital health management for the patients.The intervention lasted for 12 months.The health behaviour ability and chronic disease self-management efficacy of the patients before and after the intervention were compared with.Results After the case manager-led stratified out-of-hospital health management,the health behaviour ability of patients with chronic diseases was stronger than that before the implementation,the self-management efficacy of chronic diseases was better than that before the implementation,and the patients'satisfaction was higher than that before the implementation(all P<0.001).Conclusion Stratified out-of-hospital health management led by a case health manager can improve the health behaviour ability and self-management efficacy of the patient with chronic diseases,thereby improve the patients'satisfaction.
4.Influencing factors of cholecystolithiasis among Tibetan residents living at extreme altitude

Objective To analyze the influencing factors of cholecystolithiasis among Tibetan residents living at extreme altitude.Methods From Aug.10 to 17,2023,Nima County People's Hospital in Nagqu,in collaboration with Tibetan-aid doctors,organized physical examinations for Tibetan residents in Jiwa Township.The physical examination data were collected,and the general information and dietary habits of the examinees were recorded.Based on the results of abdominal ultrasound examination,the 448 enrolled participants were assigned to a cholecystolithiasis group(106 cases)and a non-cholecystolithiasis group(342 cases).Additionally,multivariable logistic regression analysis was conducted to investigate the factors influencing the occurrence of cholecystolithiasis among Tibetan residents.Results There were significant differences in age,weight,body mass index,systolic blood pressure,diastolic blood pressure,finger pulse oxygen,and educational level between the 2 groups(all P<0.05).The results of multivariate logistic regression analysis showed that age(odds ratio[OR]=1.062,95%confidence interval[CI]1.020-1.106),weight(OR=1.120,95%CI 1.067-1.176),smoking(OR=4.751,95%CI 1.627-13.869),cholesterol(OR=1.429,95%CI 1.129-1.810),amount of butter tea drink(OR=2.123,95%CI 1.656-2.721),alanine transaminase(OR=1.045,95%CI 1.028-1.062),low educational level(OR=6.994,95%CI 2.615-18.702),fatty liver(OR=4.409,95%CI 1.499-12.968)and meat-based diet(OR=3.725,95%CI 1.255-11.055)were correlated with cholecystolithiasis.Conclusion The incidence of cholecystolithiasis among Tibetan residents living at extreme altitude may be related to various factors mentioned above,which provides clues for the prevention,early diagnosis,and treatment of local cholecystolithiasis.
5.Effect of regulating intestinal acupuncture combined with fire-dragon cupping in the treatment of slow transit constipation after stroke in the elderly

Objective To observe the therapeutic effect of regulating intestinal acupuncture combined with fire-dragon cupping in the treatment of slow transit constipation(STC)after stroke in the elderly.Methods A total of 132 patients with STC after stroke who were diagnosed and treated in Hangzhou Hospital of Traditional Chinese Medicine from January 2023 to June 2024 were selected and divided into control group and observation group according to the random number table method,with 66 cases in each group.Patients in control group received defecation training,while patients in observation group were treated with regulating intestinal acupuncture combined with fire-dragon cupping on the basis of control group.Both groups of patients were treated continuously for 2 weeks.The therapeutic effects,Cleveland clinic constipation score(CCS),Bristol stool form scale(BSFS)scores and serum intestinal neurotransmitter levels of two groups of patients were compared,and the recurrence rate was statistically analyzed.Results The clinical effective rate of patients in observation group was significantly higher than that in control group(x2=5.893,P=0.015).After 2 weeks of treatment,the CCS,BSFS score and serum vasoactive intestinal peptide(VIP)level of two groups of patients were significantly lower than those before treatment in same group,and 5-hydroxytryptamine(5-HT)level was significantly higher than that before treatment in same group(P<0.05).The CCS,BSFS score and serum VIP level of patients in observation group were significantly lower than those in control group,and 5-HT level was significantly higher than that in control group(P<0.05).The recurrence rate of patients in observation group was significantly lower than that in control group(x2=3.986,P<0.05).Conclusion The treatment of regulating intestinal acupuncture combined with fire-dragon cupping can effectively alleviate the constipation degree of STC after stroke in the elderly,and its effectiveness may be related to the regulation of intestinal neurotransmitters.
6.Analysis of epidemiological and viral genetic characteristics of human coronavirus OC43 in Hangzhou area from 2018 to 2023

Objective:To analyze the epidemiological characteristics and genetic evolution of human coronavirus OC43(HCoV-OC43)in Hangzhou from 2018 to 2023,and provide molecular epidemiological basis for the prevention and control of HCoV-OC43 infection.Methods:A total of 11 904 samples of cases with acute respiratory tract infection were collected from two sentinel hospitals in Hangzhou. The collection period was from January 2018 to December 2023. HCoV-OC43 was detected by real-time fluorescent quantitative RT-PCR. The positive samples were amplified by PCR using specific primers,and the amplified products were sequenced using third-generation nanopore technology for the whole genome. The genetic evolution features were analyzed.Results:A total of 265 HCoV-OC43 positive samples were detected,with a positive detection rate of 2.23%(265/11 904). There was no significant difference in detection rate between males and females(χ 2=0.042, P>0.05);the detection rate varied significantly among different age groups(χ 2=60.104, P<0.01),with higher detection rates observed in children under five years old and individuals over 65 years old. The infection peaks in 2018-2019 and 2023 occurred in spring and summer,while the infection peak in 2020-2022 was in summer and autumn. A total of 85 complete HCoV-OC43 whole genome sequences were obtained through high-throughput sequencing. The evolutionary analysis indicated that the K genotype was predominant in Hangzhou from 2018 to 2023. In 2019,both J and K genotypes co-existed. In 2023,a new genotype,temporarily named genotype L,was discovered,which originated from genotypes J and K. The K genotype had 22 specific amino acid variation sites,most of which were located in the important domain of S protein. The J genotype had seven specific amino acid variation sites,most of which were located in non-structural proteins. The new genotype L had 10 specific amino acid mutation sites,which were distributed across six proteins,and these mutation sites had the most extensive coverage. Conclusions:HCoV-OC43 is an important pathogen causing respiratory diseases in children and the elderly in the Hangzhou area,and there are multiple genotypes in the Hangzhou area,with a new recombinant genotype L discovered in 2023.
7.Latent profile analysis and influencing factors of self-management ability in patients with metabolic-associated fatty liver disease

Objective:To analyze the latent profile characteristics of self-management ability in patients with metabolic-associated fatty liver disease (MAFLD) and explore its influencing factors.Methods:This was a cross-sectional study. A total of 311 patients with metabolic-associated fatty liver disease (MAFLD) were selected from the First Affiliated Hospital of Sun Yat-sen University (including those receiving treatment and undergoing physical examinations) between August and October 2024. Data were collected using a general information questionnaire, the self-management Scale for Patients with Non-Alcoholic Fatty Liver Disease, and the Social Support Rating Scale. After excluding 51 patients due to incomplete questionnaire responses or logical inconsistencies, 260 patients were finally included in the analysis. Latent profile analysis was used to identify potential categories of patients′ self-management abilities. With the latent categories as the dependent variable and items with P<0.05 in univariate analysis as independent variables, a multivariate logistic regression analysis (with the "poor self-management group" as the reference group) was performed to explore the influencing factors. Results:Among the 260 participants included in the study, three potential categories of self-management behaviors were finally identified, namely the active self-management group with 106 cases (40.8%), the moderate self-management group with 118 cases (45.4%), and the passive self-management group with 36 cases (13.8%).Results of multivariate logistic regression analysis showed that, compared with the passive self-management group:Patients who resided in rural areas ( OR=0.130, 95% CI: 0.040-0.420), often stayed up late ( OR=0.200, 95% CI: 0.060-0.590), or had an average daily sleep duration of≤5 hours ( OR=0.160, 95% CI: 0.050-0.510) had a significantly lower probability of belonging to the "active self-management group";In contrast, patients with an education level of senior high school or above ( OR=7.530, 95% CI: 1.740-34.160) or a higher total score of social support ( OR=1.120, 95% CI: 1.030-1.210) had a significantly higher probability of being in the "active self-management group" (all P<0.05). Conclusion:There is heterogeneity in self-management ability among patients with metabolic associated fatty liver disease (MAFLD). Residential area, educational level, frequency of staying up late, average daily sleep duration, and social support are influencing factors of patients′ self-management ability.
8.Feasibility of small incision bile duct stone removal in primary hospitals in extremely high-altitude areas

Objective To explore the feasibility and precautions of small incision bile duct stone removal in primary hospitals in extremely high-altitude areas.Methods The experience of small incision biliary exploration and cholecystectomy under general anesthesia at primary hospitals during the author's medical aid to Xizang in the high-altitude areas of northern Xizang was summarized(from June 2022 to December 2022).Results A total of 11 cases of small incision common bile duct stone removal were completed.Abdominal drainage was performed in all patients,including 6 cases with T-shaped tubes and 5 cases with primary closure of the common bile duct.The patients recovered well after surgery and was discharged.Conclusion In extreme high-altitude areas,under the guidance of medical aid doctors,it is completely feasible for primary hospitals to carry out small incision bile duct stone removal by selecting appropriate cases,training surgical skills,and performing detailed preoperative preparation.
9.Incidence and influencing factors of diarrhea after minilaparotomy cholecystectomy among residents living in extremely high-altitude areas

Objective To investigate the diarrhea after minilaparotomy cholecystectomy among Tibetan residents in extremely high-altitude areas.Methods A retrospective analysis was conducted on the clinical data of 71 patients who underwent small incision cholecystectomy at the County People's Hospital during the period of medical aid(from June 2022 to December 2022).There were 38 patients(53.52%)with postoperative diarrhea and 33 patients(46.48%)without diarrhea.Results The duration of diarrhea was less than 1 week in 8 patients,ranged from 1 week to 3 months in 21 patients,and lasted for more than 3 months in 9 patients.There were significant differences between the two groups in terms of age(P<0.000 1),body mass index(P<0.000 1),proportions of patients with liver cirrhosis(P=0.012 9),history of acute cholecystitis(P=0.002 1),history of coronary heart disease(P=0.040 7),and presence of tenderness in the right upper abdomen during admission(P<0.000 1),white blood cell count(P=0.007 0),alanine aminotransferase(P=0.047 3),gallbladder diameter(P=0.003 4),gallbladder wall thickness(P=0.000 4),surgical duration(P=0.004 7),postoperative antibiotic use(P=0.000 6),postoperative hospital stay(P=0.000 2),and immediate resumption of butter tea(P<0.000 1).Age and immediate resumption of butter tea after surgery were independent risk factors for diarrhea after minilaparotomy cholecystectomy.Conclusion There is a higher incidence of diarrhea following small incision cholecystectomy in the residents of extremely high-altitude areas,which is associated with various factors such as the environment and lifestyle of the region.It is essential to enhance health education on gallstone and cultivate good personal health habits.Clinicians should conduct a detailed preoperative assessment.Patients should avoid drinking butter tea during perioperative period to avoid postoperative diarrhea.
10.Transcriptome analysis of radiation-induced brain injury in mice subjected to Flash whole-brain irradiation with electron beams

Objective:To reveal the differences in the transcriptome maps of brain tissues in mice subjected to Flash irradiation and conventional dose rate irradiation with electron beams and to explain the biological effect and mechanisms of Flash irradiation from multiple perspectives.Methods:Following the principle of grouping based on approximate body weights, 36 female C57BL/6J mice were divided into three groups, i. e., the control, conventional dose rate irradiation (CONV), and Flash irradiation (Flash) groups, with 12 mice in each group. Both the CONV and Flash groups received a single 15 Gy whole-brain irradiation with 9 MeV electron beams. At 3 d post-irradiation, the whole-brain tissue specimens were collected for hematoxylin-eosin (HE) staining to observe pathological changes. At 1, 3, and 10 weeks post-irradiation, the motion function, cognitive ability, depression level, and spatial memory capacity of the mice were assessed using ethology. At 1 and 10 weeks after behavioral experiments, brain tissue samples were collected and snap-frozen in liquid nitrogen for reference-based transcriptome sequencing. Accordingly, the differences in the transcriptome maps of radiation-induced brain injury between CONV and Flash groups were analyzed.Results:The HE staining-based pathological result revealed that compared to the CONV group, the Flash group exhibited reduced glial cell hyperplasia and inflammatory cell infiltration in brain tissues. Ethological research result at 1 week post-irradiation showed that the CONV group manifested a significantly decreased total traveled distance compared to the control and Flash groups ( t = 5.51, 2.38, P < 0.05) and a significantly increased immobility time compared to the control group ( t = 3.60, P < 0.05). Ethological research result at 3 weeks post-irradiation indicated that compared to the CONV group, the Flash group displayed significantly alleviated cognitive impairment ( t = 3.35, P < 0.05) and reduced depression levels ( t = 2.39, P < 0.05). Ethological research result at 10 weeks post-irradiation demonstrated that the CONV group showed the worst cognitive performance, significantly differing from the control group ( t = 4.53, P < 0.05). Transcriptome sequencing result revealed that besides immune-related pathways, the Flash group also exhibited multiple upregulated metabolic pathways and fibroblast growth factor (FGF)-related pathways compared to the CONV group. Conclusions:Compared to conventional dose rate irradiation, Flash irradiation can effectively alleviate radiation-induced brain injury in mice. This effect is associated with various metabolic pathways (including amino acid metabolism) and FGF-related pathways besides immune pathways.
