2.Effect of dexmedetomidine on neural injury in rats with kainic acid-induced epilepsy and its mechanism

Objective To investigate the effect of dexmedetomidine(Dex)on neural injury in rats with kainic acid(KA)-induced epilepsy and its possible mechanism.Methods Rats were ran-domly divided into Sham group,model group,Dex low-dose(Dex-L)group,Dex high-dose(Dex-H)group,and Dex-H+recombinant high mobility group box 1(rHMGB1)group using a random number table method,with 10 rats in each group.Except for the Sham group,epilepsy modeling was per-formed in the other groups.The seizure conditions of rats in each group were observed.The Morris water maze test was used to assess the spatial learning and memory abilities(latency,time spent in the target quadrant,and the number of platform crossings)of the rats.Enzyme-linked immunosorbent assay(ELISA)was employed to detect the levels of tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),interleukin-18(IL-18),and γ-aminobutyric acid(GABA)in the hippocampal tissue of the rats.Nissl staining was used to observe the morphological changes in the hippocampal tissue.The TUNEL method was applied to detect the apoptosis rate of hippocampal neurons.Western blotting was used to measure the expression levels of proteins related to the high mobility group box 1(HMGB1)/receptor for advanced glycation end products(RAGE)/nuclear factor κB(NF-κB)pathway.Results Compared with the Sham group,the model group had higher seizure scores and hippocampal neuronal apoptosis rates,elevated levels of TNF-α,IL-18,IL-6,and cleaved caspase-3,HMGB1,RAGE,and phos-phorylated(p)-NF-κB/NF-κB proteins in the hippocampal tissue,prolonged seizure duration and latency,decreased hippocampal neuronal numbers and platform crossings,shortened time spent in the target quadrant,and reduced GABA levels,with statistically significant differences(P<0.05).Compared with the model group,the Dex-L and Dex-H groups had lower seizure scores and hipp-ocampal neuronal apoptosis rates,reduced levels of TNF-α,IL-18,IL-6,and cleaved-caspase-3,HMGB1,RAGE,and p-NF-κB/NF-κB proteins in the hippocampal tissue,shortened seizure dura-tion and latency,increased hippocampal neuronal numbers and platform crossings,prolonged time spent in the target quadrant,and elevated GABA levels,with statistically significant differences(P<0.05).The rHMGB1 could weaken the ameliorative effect of Dex on neural injury in KA-in-duced epileptic rats.Conclusion Dex can alleviate inflammatory responses,oxidative stress,and neuronal apoptosisin KA-induced epileptic rats,and improve neurological function,possibly by in-hibiting the HMGB1/RAGE/NF-κB pathway.
3.Research on the application of evaluation and warning system based on Shewhart control chart in medical device related pressure injury

Objective:To explore the effectiveness of the Shewhart control chart-based assessment and early warning system in prevention of medical device-related pressure injury(MDRPI).Methods:152 critically ill patients admitted to Hebei Central Hospital from January 2020 to December 2021 were selected and divided into a control group and an observation group based on different methods of assessing MDRPI risk,with 76 cases in each group.The control group adopted the Braden scale to assess the risk of MDRPI.The observation group adopted a safety early warning system based on Shewhart control charts to assess the risk of MDRPI in patients.Nursing measures were undertaken according to MDRPI risk grade in both groups.The occurrence of adverse events of MDRPI,nursing safety quality and nursing comprehensive quality were compared between the two groups.Results:The incidence rate of head,neck and face adverse events of MDRPI and the total incidence of adverse events of MDRPI of the patients in the observation group were lower than those in the control group(x2=4.802,5.758,P<0.05).The safety quality and comprehensive quality of nursing of 20 nurses in the observation group were higher than those in the control group(t=6.654,7.172,P<0.05).Conclusion:The application of assessment and early warning system based on Shewhart control chart in clinical nursing management can effectively reduce the incidence of MDRPI adverse events and improve the quality of nursing safety and comprehensive nursing.
4.Cranial ultrasound in perioperative period of acute severe traumatic brain injury

Objective To observe the value of cranial ultrasound for perioperative patients with acute severe traumatic brain injury(sTBI).Methods Data of 55 sTBI patients who underwent craniotomy were retrospectively analyzed.The patients were divided into observation group(n=15)and control group(n=40)according to received perioperative cranial ultrasound or not.The general data and surgical data were compared between groups,and ultrasonic data of observation group were analyzed.Results The proportions of good prognosis 1 and 6 months after operation in observation group were both higher than those in control group,while the incidence of cerebral infarction in observation group was lower than that in control group(all P<0.05).No significant difference of general data nor other surgical data was found between groups(all P>0.05).Acute encephalocele occurred in 1 case in observation group during operation,and cranial ultrasound accurately showed the contralateral secondary epidural hematoma.Increased intracranial pressure in different degrees were found in all 15 cases(15/15,100%)in observation group after operation with transcranial color coded Doppler(TCCD)or transcranial Doppler(TCD),while cerebral vascular spasm was observed in 5 cases(5/15,33.33%),among them 4 cases(4/5,80.00%)were diagnosed cerebral infarction based on CT examination.Conclusion Cranial ultrasound could be used to evaluate changes of sTBI in perioperative period and guide adjusting treatment strategy in time,being valuable for reducing risk of postoperative cerebral infarction and improving prognosis.
5.Comparison of lateral veneer fixation versus tunnel fixation for reconstruction of the medial patellofemoral ligament in the treatment of recurrent patellar dislocation in adolescents

Objective:To compare the lateral veneer fixation versus the tunnel fixation for reconstruction of the medial patellofemoral ligament (MPFL) in the treatment of recurrent patellar dislocation in adolescents.Methods:A retrospective study was conducted to analyze the data of 50 adolescent patients with recurrent patellar dislocation who had been treated at Department of Pediatric Orthopedics, The Sixth Affiliated Hospital of Xinjiang Medical University from January 2020 to January 2023. There were 12 males and 38 females with an age of (15.0±0.9) years, and 23 cases on the left and 27 cases on the right. The number of preoperative dislocations was (2.5±0.9) times. According to the surgical methods, the patients were divided into 2 groups: a veneer group of 25 cases whose MPFL was reconstructed by the patellar lateral veneer fixation, and a tunnel group of 25 cases whose MPFL was reconstructed by the patellar tunnel fixation. The 2 groups were compared in terms of Kujala score, Lysholm score, lateral patellofemoral angle (LPFA), congruence angle (CA), patellar tilt angle (PTA), and lateral patellar displacement (LPT) at the last follow-up.Results:As the differences in the preoperative general data between the groups were insignificant, the 2 groups were comparable ( P>0.05). The 50 patients were followed up for (24.1±5.1) months after surgery. There were no complications like patellar fracture or limitation of joint flexion-extension in either group after surgery, and neither group experienced any further patellar dislocation at the last follow-up. The Kujala score, Lysholm score, LPFA, CA, PTA and LPT at the last follow-up were (87.8±4.3) points, (88.3±4.7) points, 9.5°±1.1°, 10.8°±1.4°, 9.6°±1.0°, and 9.2±1.3 mm in the veneer group, and (86.0±4.4) points, (87.7±4.2) points, 9.5°±1.1°, 11.0°±1.6°, 9.0°±1.8°, and (9.2±1.3) mm in the tunnel group, respectively, showing no statistically significant difference between the 2 groups ( P>0.05). However, there were significant improvements within each group in the comparisons of the above indexes between pre-surgery and the last follow-up ( P<0.05). Conclusion:In the treatment of recurrent patellar dislocation in adolescents, both lateral veneer fixation and tunnel fixation to reconstruct the MPFL can improve the alignment of the patellofemoral joint in adolescents, contributing to the improvement of knee joint function and patellar stability.
6.Establishment and evaluation of a dual fluorescent RT-LAMP assay for PEDV and TGEV detection

To develop a rapid differential detection method for porcine epidemic diarrhea virus(PEDV)and transmissible gastroenteritis virus(PEDV),M gene sequences of PEDV and TGEV were compared,the inner and outer primer pairs and probes were designed according to the highly conserved region.PEDV-Probe was labeled with FAM at5'end and BHQ1 at 3'end.TGEV-Probe was labeled with CY5.5 at the 5'end and BHQ2 at the 3'end.After optimizing the reaction condi-tions and system,a dual fluorescent RT-LAMP assay for PEDV and TGEV rapid identification was established.The amplification could be completed within 60 min in a 63 ℃ water bath and then stopped at 85 ℃ for 10 min.Then the tubes were placed in a multicolor imaging system,and the re-sults could be observed under 520 nm and 690 nm dual channels.There was no cross-reaction when other common swine viral pathogens were detected by this method.The sensitivity of the assay was evaluated with a 10-fold diluted recombinant plasmid as templates.The lowest detection limit was 102 copies/μL recombinant plasmid,which was 10 times more sensitive than the conventional RT-PCR method.Seventy-two PEDV-positive samples,49 TGEV-positive samples,and 40 PEDV and TGEV co-infected samples were detected from 175 anal swab samples of diarrheic piglets by the established method,which were all higher than the detection rates of the conventional RT-PCR method.The dual fluorescent RT-LAMP method established in this study can be used to amplify the target gene in an ordinary water bath without gel electrophoresis,which provides technical sup-port for rapid and convenient differential diagnosis of PED and TGE and simultaneous detection of PEDV and TGEV co-infection.
7.Effects of TGF-β1 on the migration of oral cancer-associated fibroblasts in two and three dimensional co-culture models

Objective:
To observe the effect of transforming growth factor-β1 (TGF-β1) on the migration of oral carcinoma associated fibroblasts (CAFs) with two-dimensional culture model and three-dimensional model.
Methods :
Under two-dimensional culture conditions, CAFs stimulated by TGF-β1 with the addition of 10 ng/mL medium were used as the experimental group, and untreated CAFs were used as the control group. The migration of CAFs with the stimulation of TGF-β1 was measured by cell scratch assay and transwell assay. CAFs positive for green fluorescent protein (GFP) were cultured by retrovirus transfection. Human tongue squamous cell carcinoma cells SCC25, GFP(+) CAFs and CAFs with three-dimensional cell co-culture models were established. The three-dimensional model cultured under the stimulation of TGF-β1 with 10 ng/mL medium was used as the experimental group, and the three-dimensional model without TGF-β1 was used as the control group. The migration of CAFs with the stimulation of TGF-β1 was also measured by the three-dimensional models.
Results:
It was verified that 10 ng/mL TGF-β1 promoted the migration of CAFs in the two-dimensional culture model. The three-dimensional co-culture models of SCC25, GFP(+) CAFs and CAFs were successfully established. The migration of SCC25 and CAFs was detected in the three-dimensional model. However, 10 ng/mL TGF-β1 had little effect on their migration.
Conclusion
The effect of TGF-β1 in vitro on the migration of oral CAFs was associated with different culture models in two and three dimensions.
8.Effect of Hydrogen on ischemia/reperfusion-induced cardiocyte apoptosis in diabetic rats

Objective To study the effect of Hydrogen on ischemia/reperfusion(I/R)-induced cardiocyte apoptosis and apoptosis related proteins expression in diabetic rats .Methods Diabetes mellitus was induced by intraperitoneal injection of streptozotocin (STZ) ,then feed 4 weeks before build ischemia/reperfusion model .60 SD male rats ,weight during 300-350 g ,were randomly divid-ed to six groups :non-diabetic sham-operated group (NS) ,non-diabetic I/R group(NI/R) ,non-diabetic hydrogen treated group (NH) ,diabetic sham-operated group(DS) ,diabetic I/R group(DI/R)and diabetic hydrogen treated group(DH) ,each group has 10 rats .The cardiac muscle I/R model was made by 30 min occlusion of the left anterior descending coronary artery and 2 h reperfu-sion .The rats in Hydrogen group were treated with 5 mL/kg hydrogen by intraperitoneal administration at the beginning of reper-fusion .The apoptosis index (AI) was calculated by TUNEL .The positive expressions of Bcl-2 ,Bax ,Caspase-3 in cardiomyocytes were respectively detected by immunohistochemistry .Results The apoptotic rates of cardiomyocytes and the positive expressions of Bcl-2 ,Bax ,Caspase-3 were significantly increased(P<0 .01) ,Compared with I/R group ,the apoptotic rates of cardiomyocytes in hydrogen treatment group were obviously decreased(P<0 .01) ,and the positive expressions of Bcl-2 were increased(P<0 .01) ,at the same time ,the positive expressions of Bax ,Caspase-3 were decreased(P<0 .01) .Conclusion Hydrogen inject by intraperitoneal method on myocardial ischemia-reperfusion injury of diabetic rats has a protective effect ;Its mechanism may be related to its inhibi-ting myocardial apoptosis by advanced the Bcl-2 protein expression and reduced the Bax 、Caspase-3 protein expression .
9.Role of PI3K/Akt signaling pathway in hydrogen-induced inhibition of cell apoptosis during myocardial ischemia/reperfusion in rats

Objective To evaluate the role of phosphoinositide 3 kinase/protein kinase B (PI3K/Akt) signaling pathway in hydrogen-induced inhibition of cell apoptosis during myocardial ischemia/reperfusion (I/R) in rats.Methods Forty healthy male Sprague-Dawley rats,weighing 300-350 g,were randomly allocated into 4 groups (n =10 each) using a random number table:sham operation group (S group),I/R group,hydrogen group (group H),and hydrogen + LY294002 group (group HL).Myocardial I/R was induced by occlusion of the anterior descending branch of left coronary artery for 30 min followed by 120 min reperfusion.In H and HL groups,99.6 % hydrogen 5 ml/kg was injected intraperitoneally immediately after beginning of reperfusion,and in addition LY294002 (0.3 mg/kg) was injected through the caudal vein before hydrogen injection in group HL.Arterial blood samples were collected at the end of 120 min reperfusion for determination of serum creatine kinase isoenzyme-MB (CK-MB) and lactate dehydrogenase (LDH) activities.The rats were then sacrificed.Myocardial apoptosis was detected by TUNEL and apoptosis index (AI) was calculated.The expression of Bcl-2,Bax and caspase-3 was detected by immuno-histochemistry.Bcl-2/Bax ratio was calculated.Results The serum CK-MB and LDH activities and AI were significantly increased,the expression of myocardial Bcl-2,Bax and caspase-3 was upregulated,and the ratio of Bcl-2/Bax was decreased in group I/R as compared with group S.Compared with group I/R,the serum CK-MB and LDH activities and AI were significantly decreased,the expression of myocardial Bcl2 was up-regulated,while the expression of myocardial Bax and caspase-3 was down-regulated,and the ratio of Bcl-2/Bax was increased in group H,and no significant changes were found in the parameters mentioned above in group HL.The serum CK-MB and LDH activities and AI were significantly increased,the expression of myocardial Bcl-2 was down-regulated,while the expression of myocardial Bax and caspase-3 was up-regulated,and Bcl-2/Bax ratio was decreased in group HL as compared with group H.Conclusion Hydrogen can activate the PI3K/Akt signaling pathway,and further up-regulates Bcl-2 expression and down-regulates Bax and Caspase-3 expression,thus inhibiting cell apoptosis during myocardial I/R in rats.
