1.Meta-analysis of the effect of early rehabilitation activities on diaphragm function in patients with mechanical ventilation

Objective To systematically evaluate the effect of early rehabilitation activities on diaphragm function in patients with mechanical ventilation.Methods Computerized system search databases included PubMed,Embase,Web of Science,Cochrane Library,China Biology Medicine disc,China National Knowledge Infrastructure,Wanfang Data Knowledge Service Platform,and VIP on the effects of early rehabilitation activities on diaphragm function in patients with mechanical ventilation.The search period was from the creation of the database to March 2025.Two researchers independently performed literature screening,literature quality evaluation,and data extraction.Results 9 papers,including 902 patients,were finally included.Meta-analysis showed that early rehabilitation activities improved diaphragm inspiratory thickness[standard mean difference(SMD)=0.67,95%CI:0.39-0.95,P<0.000 01],diaphragm expiratory thickness(SMD=0.39,95%CI:0.16-0.63,P=0.0001),diaphragm thickness fraction(SMD=0.76,95%CI:0.58-0.94,P<0.000 01)and diaphragm excursion(SMD=0.49,95%CI:0.27-0.72,P<0.000 01),and reduced the incidence of patient failure to withdraw from the machine(relative risk=0.45,95%CI:0.31-0.65,P<0.0001).Conclusion Early rehabilitation activities can improve diaphragm function in patients with mechanical ventilation,which has good clinical value and is worth standardized promotion and application in intensive care unit;The intervention strategy should be further refined in the future to improve the prevention and treatment strategy of diaphragm dysfunction.
2.Progress in proteomics of schizophrenia based on peripheral blood

Schizophrenia is a debilitating mental disorder with complex etiology. Early identification of high-risk individuals and early intervention for early-stage patients can help improve prognosis, but effective identification methods are lacking. In recent years, research on proteomic techniques based on peripheral blood has achieved certain advancements in exploring the etiology of schizophrenia, as well as in early recognition and diagnostic modeling. This paper provides a review of the research methodologies of peripheral blood-based proteomics and the candidate biomarkers identified through these method.
3.A case of chorea-acanthocytosis misdiagnosed as tardive dyskinesia

The patient was a 25-year-old male who initially presented with emotional issues and later developed involuntary movements following the use of antipsychotic and antidepressant medications. He was initially misdiagnosed with Tardive Dyskinesia, a condition commonly associated with psychotropic drugs. However, due to the severity of his involuntary movements, the ineffectiveness of treatment, and a notable family history, genetic testing was performed. The test indicated a mutation in the VPS13A gene of the patient, and provided evidence for a final diagnosis of Chorea Acanthocytosis. This case report aims to enhance the recognition of movement disorders among psychiatrists and facilitate earlier identification of neurological diseases whose primary manifestation is involuntary movement.
4.GLP-1RA induces time-dependent depression-like behaviors through neurotransmitter imbalance

Objective:This study aimed to investigate the effects of glucagon-like peptide-1 receptor agonists(GLP-1RAs) on the emotional behaviors in wild-type mice and explore the underlying mechanisms.Methods:The study consisted of two parts.(1) Drug effect evaluation: C57BL/6 mice were randomly assigned to saline, semaglutide, or liraglutide groups. After 4 weeks of continuous subcutaneous administration, anxiety-like behaviors were evaluated by the open field tests and elevated plus maze, and depression-like behaviors were assessed by the forced swimming test and tail suspension test. Neurotransmitter levels, including NE, DA, 5-HT, GABA, and glutamate, were measured in target brain regions using LC-MS/MS. Hypothalamic activation subregions were identified by c-fos immunohistochemistry.(2) Time-course analysis: Mice received short-term administration(2 weeks), long-term administration(4 weeks), or drug withdrawal(assessed 4 weeks post-withdrawal). Behavioral performance, hypothalamic neurotransmitter levels, and plasma adrenocorticotropic hormone and corticosterone were measured.Results:Compared with the saline group, both GLP-1RAs significantly prolonged immobility time in the forced swimming and tail suspension tests. Multiple brain regions exhibited neurotransmitter abnormalities, including marked reductions in hypothalamic norepinephrine, dopamine, and γ-aminobutyric acid levels. Both GLP-1RAs significantly increased c-fos positive cell counts in the suprachiasmatic nucleus and arcuate nucleus of the hypothalamus. Time-course analysis revealed that short-term administration did not induce depressive phenotypes, whereas long-term administration led to depression-like behaviors, which recovered after drug withdrawal. Plasma adrenocorticotropic hormone and corticosterone levels were elevated even after short-term treatment and remained high after drug withdrawal, while hypothalamic neurotransmitter abnormalities normalized upon withdrawal.Conclusion:GLP-1RAs induce depression-like behaviors in a time-dependent manner, with neurotransmitter imbalance and persistent hyperactivation of the HPA axis likely contributing to the pathophysiological mechanism.

Objective To monitor the antifungal resistance of clinical isolates of Candida spp.in the East China region.Methods MALDI-TOF MS or molecular methods were used to re-identify the strains collected from January 2018 to December 2022.Antifungal susceptibility testing was performed using the broth microdilution method.The susceptibility test results were interpreted according to the breakpoints of 2022 Clinical and Laboratory Standards Institute(CLSI)documents M27 M44s-Ed3 and M57s-Ed4.Results A total of 3 026 strains of Candida were collected,65.33%of which were isolated from sterile body sites,mainly from blood(38.86%)and pleural effusion/ascites(10.21%).The predominant species of Candida were Candida albicans(44.51%),followed by Candida parapsilosis complex(19.46%),Candida tropicalis(13.98%),Candida glabrata(10.34%),and other Candida species(0.79%).Candida albicans showed overall high susceptibility rates to the 10 antifungal drugs tested(the lowest rate being 93.62%).Only 2.97%of the strains showed dose-dependent susceptibility(SDD)to fluconazole.Candida parapsilosis complex had a SDD rate of 2.61%and a resistance rate of 9.42%to fluconazole,and susceptibility rates above 90%to other drugs.Candida glabrata had a SDD rate of 92.01%and a resistance rate of 7.99%to fluconazole,resistance rates of 32.27%and 48.24%to posaconazole and voriconazole non-wild-type strains(NWT),respectively,and susceptibility rates above 90%to other drugs.Candida tropicalis had resistance rates of 29.55%and 26.24%to fluconazole and voriconazole,respectively,resistance rates of 76.60%and 21.99%to posaconazole and echinocandins non-wild-type strains(NWT),and a resistance rate of 2.36%to echinocandins.Conclusions The prevalence and species distribution of Candida spp.in the East China region are consistent with previous domestic and international reports.Candida glabrata exhibits certain degree of resistance to fluconazole,while Candida tropicalis demonstrates higher resistance to triazole drugs.Additionally,echinocandins resistance has emerged in Candida albicans,Candida glabrata,Candida tropicalis,and Candida parapsilosis.
10.Influencing factors of early postoperative quality of life in lung cancer patients based on symptom clusters and psychological stress

Objective:To observe the relationship between early quality of life symptom clusters and psychological stress and its influencing factors,so as to lay a foundation for clinical timely control of symptom clusters,improving of psychological stress state,and improving the early quality of life after lung cancer surgery.Methods:A total of 248 patients with lung cancer who received surgical treatment in our hospital from Oct 2021 to Oct 2023 were selected to evaluate their general condition,quality of life,symptom clusters,and psychological stress by general situation questionnaire,Functional Assessment of Cancer Therapy(FACT),Anderson Symptom Assessment Scale,and Symptom Self-Rating Scale(SCL-90).Pearson method was used to analyze the correlation between FACT score symptom clusters and SCL-90 score.On the basis of multivariable logistic regression analysis,cumulative ratio regression model was used to analyze the early quality of life after lung cancer surgery.Results:A total of 239 patients with lung cancer were enrolled.FACT-L score was(53.66±6.85),SCL-90 score was(109.24±11.97),symptom clusters score was(104.03±16.83),including 117 patients in low group,69 patients in middle group,and 53 patients in high group,48.95%,28.87%and 22.18%,respectively.Pearson analysis showed that FACT score was negatively correlated with SCL-90 score and symptom clusters(r value was-0.779 and-0.737,respectively,P<0.05).Multivariate cumulative ratio logistic regression analysis showed that seven factors including clustered airway management and sleep quality were protective factors for early quality of life after lung cancer surgery(P<0.05),and malnutrition,pain degree,operation duration,SCL-90 score and symptom clusters score were risk factors for early quality of life after lung cancer surgery(P<0.05).Conclusions:The influencing factors of early postoperative quality of life of lung cancer patients come from many aspects.Clinical prevention and control strategies can be formulated according to the influencing factors to improve the level of early postoperative quality of life of lung cancer patients.
