1.Treatment of Asthma by Regulation of Intestinal Flora in Traditional Chinese Medicine Based on "Lung-Intestinal Coordination Therapy"

Bronchial asthma (asthma) is a heterogeneous disease characterized by airflow limitation, airway remodeling, and recurrent symptoms such as wheezing, shortness of breath, chest tightness, and cough. Its prevalence is gradually increasing, and a portion of patients are still poorly controlled, leading to a serious social and medical burden. Modern studies have proposed the concept of the "lung-gut axis", which is based on the crosstalk between microorganisms and their metabolites in the lungs and large intestine, and have indicated that microbial dysbiosis in these organs may affect the onset and progression of asthma. Traditional Chinese medicine (TCM) has found the phenomenon of lung-intestinal comorbidity and put forward the importance of "lung-intestinal coordination therapy" in the treatment of lung-related diseases. It has been found that intestinal flora and their metabolites can modulate immune responses through the lung-gut axis, demonstrating great potential for predicting asthma susceptibility, anticipating phenotypes, assessing asthma severity, and guiding treatment. TCM comopunds that embody lung-intestinal coordination therapy, including herbal formulas, single herbs, acupuncture, moxibustion, acupoint application, and spinal pinching therapy, has been shown to regulate intestinal flora, improve metabolism, regulate immunity, alleviate lung inflammation, reduce mucus secretion, inhibit airway remodeling, effectively alleviate symptoms, and delay lung function decline. Based on "lung-intestinal coordination therapy", this paper used intestinal flora as the entry point to summarize the underlying mechanisms of TCM in asthma treatment and highlighted the pivotal role of intestinal flora in asthma, providing a new idea for its clinical treatment through the intestinal flora .

Dyskinesia-hyperpyrexia syndrome(DHS)is an acute hyperpyrexia syndrome that is different from Parkinsonism-hyperpyrexia syndrome and serotonin syndrome in patients with advanced Parkinson's disease(PD),with the main symptoms of high fever,disturbance of consciousness,elevated creatine kinase,and dyskinesia. Syndrome of inappropriate secretion of antidiuretic hormone(SIADH)is a clinical syndrome caused by excessive secretion of antidiuretic hormone,which leads to the symptoms of dilutional hyponatremia,water retention,and increases in urine sodium and urine osmotic pressure. DHS has not received widespread attention in clinical practice,and there are rare reports of DHS comorbid with SIADH. This article reports the diagnosis and treatment processes of a case of DHS comorbid with SIADH and reviews the relevant literature,in order to guide the diagnosis and treatment of PD-related critical diseases.

ObjectiveTo evaluate the efficacy and safety of Xiaoji Hufei Formula in protecting children with close contact exposure to influenza, and to provide reference and evidence-based support for better clinical prevention and treatment of influenza in children. MethodsA multicenter, prospective, non-randomized, parallel, controlled trial was conducted from October 2021 to May 2022 in five hospitals, including Dongfang Hospital of Beijing University of Chinese Medicine. Confirmed influenza cases and influenza-like illness (ILI) cases were collected, and eligible children with close contact exposure to these cases were recruited in the outpatient clinics. According to whether the enrolled close contacts were willing to take Xiaoji Hufei formula for influenza prevention, they were assigned to the observation group (108 cases) or the control group (108 cases). Follow-up visits were conducted on days 7 and 14 after enrollment. The primary outcomes were the incidence of ILI and the rate of laboratory-confirmed influenza. Secondary outcomes included traditional Chinese medicine (TCM) symptom score scale for influenza, influenza-related emergency (outpatient) visit rate, influenza hospitalization rate, and time to onset after exposure to influenza cases. ResultsA total of 216 participants were enrolled, with 108 in the observation group and 108 in the control group. Primary outcomes: (1) Incidence of ILI: The incidence was 12.0% (13/108) in the observation group and 23.1% (25/108) in the control group, with the observation group showing a significantly lower incidence (χ2=4.6, P<0.05). (2) Influenza confirmation rate: 3.7% (4/108) in the observation group and 4.6% (5/108) in the control group, with no statistically significant difference. Secondary outcomes: (1) TCM symptom score scale: after onset, nasal congestion and runny nose scores differed significantly between the two groups (P<0.05), while other symptoms such as fever, sore throat, and cough showed no significant differences. (2) Influenza-related emergency (outpatient) visit rate: 84.6% (11 cases) in the observation group and 96.0% (24 cases) in the control group, with no significant difference. (3) Time to onset after exposure: The median onset time after exposure to index patients was 7 days in the observation group and 4 days in the control group, with a statistically significant difference (P<0.05). ConclusionIn previously healthy children exposed to infectious influenza cases under unprotected conditions, Xiaoji Hufei formula prophylaxis significantly reduced the incidence of ILI. Xiaoji Hufei Formula can be recommended as a specific preventive prescription for influenza in children.

5.Effects of quercetin combined with iron death inhibitor on calcium oxalate-induced HK-2 cell damage

Objective To explore the effect of quercetin combined with iron death inhibitor Ferrostain-1 on oxalate-induced HK-2 cell injury.Methods HK-2 cells were randomly divided into control group(normal cultured cells),model group(0.5 mmol·L-1 calcium oxalate crystals),quercetin group(0.5 mmol·L-1 calcium oxalate crystals+100 μmol·L-1 quercetin),inhibitor group(0.5 mmol·L-1 calcium oxalate crystals+8 μmol·L-1 Ferrostain-1)and combination group(0.5 mmol·L-1calcium oxalate crystals+100 μmol·L-1quercetin+8 μmol·L-1 Ferrostain-1).Cell counting kit-8(CCK-8)assay was used to detect cell survival rate;Western blot was used to detect iron death related protein expression such as glutathione peroxidase 4(GPX4);flow cytometry and Tunel assay were used to detect cell apoptosis,and assay kit was used to detect cellular iron ions and antioxidant levels.Results The cell survival rates of control group,model group,quercetin group,inhibitor group and combination group were(100.00±2.55)%,(54.49±4.11)%,(64.26±6.30)%,(58.03±3.04)%and(79.37±4.29)%,respectively;GPX4 protein expression levels were 0.98±0.11,0.33±0.05,0.56±0.05,0.78±0.07 and 1.11±0.11,respectively;cell apoptosis rates were(4.15±0.28)%,(23.12±2.49)%,(17.28±1.07)%,(15.08±1.41)%and(8.95±0.75)%,respectively;Fe2+levels were(100.00±0.87)%,(162.55±14.70)%,(149.09±9.50)%,(144.95±11.12)%and(131.76±12.18)%,respectively;superoxide dismutase(SOD)levels were(58.67±3.46),(21.56±1.32),(33.60±2.03),(35.15±3.02)and(44.27±3.89)U·mL-1,respectively.The above indicators of the model group were compared with the control group,and the above indicators of the quercetin group,inhibitor group,and combination group were compared with the model group,the above indicators of the combination group were compared with the quercetin group,and inhibitor group,all they all showed statistical significance(all P<0.05).Conclusion Iron death inhibitors can enhance the inhibitory effect of quercetin in vitro on oxalate-induced renal tubular epithelial cell injury.
6.Effects of Astragalus extract regulating Nrf2/ARE signaling pathway on oxidative stress in diabetic nephropathy rats

Objective To investigate the effect of Astragalus extract on oxidative stress in diabetic nephropathy rats by regulating nuclear factor E2-related factor 2(Nrf2)/antioxidant response element(ARE)signaling pathway.Methods The diabetic rat model was constructed by high-fat diet combined with intraperitoneal injection of streptozotocin,and was randomly divided into model group,experimental-L group,experimental-H group,experimental-H+ML385 group,with 12 rats in each group,and 12 rats were selected as blank group.Rats in blank group and model group were intragastric with equal volume of normal saline;rats in experimental-L,-H groups were intragastric with 50 mg·kg-1 astragalus extract and 100 mg·kg-1 Astragalus extract,respectively;rats in experimental-H+ML385 group were intragastric with 100 mg·kg-1 Astragalus extract and intraperitoneally injected with 20 mg·kg-1 ML385 once a day.Eight weeks in a row.The content of oxidative stress-related indexes in rat renal tissues was detected,the expression level of reactive oxygen species was detected by dihydroethidine staining,and the protein expression level of quinone oxidoreductase 1(NQO1),heme oxygenase 1(HO-1)and nuclear Nrf2 in rat renal tissues was detected by Western blot.Results Superoxide dismutase in blank group,model group,experimental-H group and experimental-H+ML385 group were(163.89±20.28),(71.35±12.72),(132.11±19.29)and(73.04±13.28)U·mg-1,respectively;glutathione peroxidase were(12.82±1.57),(4.91±1.18),(8.54±1.09),(5.10±1.43)U·mg-1,respectively;reactive oxygen species were 0.02±0.01,0.09±0.01,0.05±0.01 and 0.08±0.01,respectively;nuclear Nrf2 values were 0.63±0.09,0.28±0.06,0.60±0.08,0.32±0.05,respectively;the NQO1 values were 0.58±0.11,0.27±0.07,0.63±0.12 and 0.31±0.08,respectively;the HO-1 values were 0.53±0.08,0.23±0.06,0.59±0.09 and 0.28±0.05,respectively.Compared with the model group,the above indexes in the experimental-H group were statistically significant(all P<0.05).The above indexes of the experimental-H+ML385 group were statistically significant compared with the experimental-H group(all P<0.05).Conclusion Astragalus extract can alleviate oxidative stress damage in diabetic nephropathy rats,and the mechanism may be achieved by regulating Nrf2/ARE signaling pathway.
7.Experimental study on treatment of severe limb ischemia with Ad-hVEGF-hHGF gene

Objective To explore the role and efficacy of VEGF and HGF gene adenovirus vector in promoting angiogenesis in ischemic tissue.Methods 84 Kunming mice were randomly divided into sham group,control group,VEGF group,HGF group and VEGF+HGF group,and the left lower limb ischemia model was established.The blood supply of ischemic tissue was observed by rheometer,and the expression levels of VEGF and HGF in each group were detected by Western Blot and ELISA.Immunohistochemical staining was used to detect angiogenesis(CD31,SMA)in ischemic tissues.Safety was assessed by side effects during treatment in mice.Results After the successful modeling,the blood flow velocity of the left lower limb in each group decreased significantly.On the 7th day after operation,the blood flow of the left lower limb in each group was significantly better than that on the 0th day after operation(P<0.05),and the blood flow of the left lower limb in Ad-VEGF-HGF group was significantly better than that in other groups(P<0.05).On the 28th day after operation,the blood flow of the left lower limb in Ad-VEGF-HGF group gradually stabilized,the blood flow in Ad-VEGF-HGF group was significantly better than that in other groups,and both VEGF group and HGF group were significantly better than the control group(P<0.05).On the 7th,14th,and 28th days following surgery,HGF and VEGF protein levels in the Ad-HGF,Ad-VEGF,and Ad-VEGF-HGF groups were substantially greater than those in the control group(P<0.05).The expression level in the Ad-VEGF-HGF group peaked on the 14th day(all P<0.001)and subsequently declined to preoperative levels on the 28th day after operation.Conclusion Ad-VEGF-HGF gene injection can effectively boost VEGF and HGF protein expression and rapidly reach the relative peak level,encour-aging angiogenesis after lower limb ischemia,increasing blood flow,and improving lower limb circulation.

Poststroke cognitive impairment (PSCI) is a common complication after ischemic stroke, which seriously affects the recovery of neurologic function and lowers the quality of daily life of patients. In a considerable portion of patients, the PSCI is reversible. This article reviews the influencing factors of cognitive impairment after ischemic stroke, including genetic predisposition, demographic factors, lifestyles, clinical manifestations, imaging findings and drug administration, etc. to provide references for prevention and intervention of PSCI.

Anti-tumor traditional Chinese medicine has a long history of clinic application, in which the star molecules have always been the hotspot of modern drug research, but they are limited by the solubility, stability, targeting, bioactivity or toxicity of the monomer components of traditional Chinese medicine anti-tumor star molecules and other pharmacokinetic problems, which hinders the traditional Chinese medicine anti-tumor star molecules for further clinical translation and application. Currently, the nanosystems prepared by supramolecular technologies such as molecular self-assembly and nanomaterial encapsulation have broader application prospects in improving the anti-tumor effect of active components of traditional Chinese medicine, which has attracted extensive attention from scholars at home and abroad. In this paper, we systematically review the research progress in preparation of supramolecular nano-systems from anti-tumor star molecule of traditional Chinese medicine, and summarize the two major categories and ten small classes of carrier-free and carrier-based supramolecular nanosystems and their research cases, and the future development direction is put forward. The purpose of this paper is to provide reference for the research and clinical transformation of using supramolecular technology to improve the clinical application of anti-tumor star molecule of traditional Chinese medicine.
