Objective:To investigate the efficacy of a modified intermittent horizontal mattress suture technique in arterial anastomosis with intimal dissection during free flap transplantation.Methods:A retrospective analysis was conducted on patients with limb wounds who underwent free flap transplantation with the modified intermittent horizontal mattress suture technique at the First People’s Hospital of Pingyuan County from September 2016 to November 2023. After debridement, free flap transplantation was performed. Under microscopic examination, recipient arteries with intimal dissection were identified, and proximal trimming failed to resolve the delamination. The modified intermittent horizontal mattress suture technique was applied to anastomose the dissected recipient arteries to donor arteries, while veins were conventionally horizontal mattress sutured. Postoperative flap viability, complications, and patient satisfaction were evaluated.Results:Fourteen patients were enrolled, including 8 males and 6 females, aged (51±14) years old (range: 18-71 years). Injuries involved the hand (5 cases) and foot (9 cases), with wound sizes ranging from 7 cm×8 cm to 22 cm×18 cm. Flaps included anterolateral thigh perforator flaps (11 cases) and groin flaps (3 cases), sized 8 cm×9 cm to 24 cm×20 cm. Seven patients had intimal dissection of the dorsalis pedis artery, with anastomoses performed to the descending branch of the lateral circumflex femoral artery (LCFA) in 5 cases, and to the superficial circumflex iliac artery in 2 cases. Intimal dissection of the medial plantar artery was identified in 2 cases, both of which were anastomosed to the descending branch of LCFA. And 5 cases had intimal dissection of the radial artery, with anastomoses performed to the LCFA in 3 cases, to the transverse branch of LCFA in one case, and to the superficial circumflex iliac artery in one case. The patients were followed up for (11.6±7.6) months (range: 3-30 months) after surgery. Eleven flaps survived completely, with partial flap margin necrosis in one case, in another case necrosis of one-third of the flap because of arterial insufficiency, both cases healed with dressing changes. The third patient resumed smoking and subsequently fell from the bed, resulting in necrosis of two-thirds of the flap five days post-discharge (12 days postoperatively). After debridement of the necrotic part of the flap, a split-thickness skin graft was applied for repair. Thirteen flaps exhibited no ulceration, with satisfactory texture and appearance, these patients expressed satisfaction. One patient with skin graft infection achieved scar healing.Conclusion:The modified intermittent horizontal mattress suture technique ensures reliable anastomosis of arteries with intimal dissection in free flap surgery, improving success rates and yielding favorable outcomes with high patient satisfaction.

Objective To explore the effects of tanshinoneⅡA on the injury of dopaminergic neurons of Parkinson’s disease (PD)mouse model induced by 1-methy-4-phenyl-1,2,3,6-tetrahydropyridine (MPTP),and to clarify the possible mechanism of its protective effects on dopaminergic neurons.Methods 60 C57BL/6N mice were randomly divided into control group,PD model group and tanshinone ⅡA group(n=20).The mice in PD model group and tanshinone ⅡA group were treated with MPTP to establish PD models. The bebavior of the mice in various groups was observed.The number of tyrosine hydroxylase (TH),cd11b,p47-phox,and inducibleni tricoxidesynthase (iNOS)-positive cells,and the protein expression levels of TH,cd11b,p47-phox and iNOS in the substantia nigra (SN)of the midbrain of the mice in various groups were detected with immunohistochemistry, Western blotting and double-labeling immunofluorescence assay.Results Compared with control group,the mice in PD model group exhibited typical symptoms of PD, and the number of TH-positive cells and the protein expression level of TH in the substantia nigra of the mice in PD model group were reduced by about 45% and 50%;the number of cd11b,p47-phox,iNOS-positive cells and the protein expression levels of cd11b,p47-phox,iNOS were markedly increased (P<0.01 ). Compared with PD model group, the symptoms of PD of the mice in tanshinone ⅡA group were alleviated,the number of TH-positive cells and the expression level of TH protein in the SN were increased(P<0.01),and the number of cd11b,p47-phox and iNOS-positive cells and the TH protein expression levels were obviously decreased(P<0.01).Conclusion Tanshinone ⅡA could mitigate the loss of dopaminergic neurons in the PD mouse model induced by MPTP. The mechanism of neuprotective effect may be related to the inhibition of microglial activation,NADPH oxidase and iNOS expressions.

Objective To investigate the effect of Shuyusan on the behavior, content of tumor necrosisfactor-α(TNF-α) in cortex of cerebral and neuropeptide expression in hippocampus of rats with chronic mild unpredictable stress (CMUS) depression. Methods Fifty mice were randomly divided into 5 groups:normal group, model group, fluoxetine group, and high-, low-dose group of Shuyusan. Except normal group, the other groups were given CMUS. Tail suspend test (TST) and sucrose preference test (SPT) were used to estimate animals’ behavior. The contents of TNF-αwas determined by enzyme linked immunosorbent assay (ELISA). The immunocytochemical technique was used to evaluate hippocampus neuropeptide immunoreactive expression. Results The fluoxetine group and high dose of Shuyusan group showed significant increase in the sucrose consumption and decrease in the immobility time in TST, there was significant difference compared with CMUS model group (P<0.05). The TNF-αlevel was significantly lower in fluoxetine group and high dose of Shuyusan group, there difference was significant compared with model group (P<0.05). The neuropeptide immunoreactive neurons in hippocampus area was decreased by a marked number in fluoxetine group and high dose of Shuyusan group, the average optical density decreased significantly, compared with the model group, there were significant differences (P<0.05). Conclusion Shuyusan has anti-depression likely effect, with dose-effect relationship.

Objective To investigate the effect of Shenlongtang decoction on learning and memory ability and expression of Bcl-2 and Bax on rats hippocampus with cerebral ischemia model. Methods The cerebral ischemia animal model was established by two-vessel occlusion. Fifty rats were randomly divided into control group, model group, pisacetam group, Shenlongtang groups (low and high dosage). After modeling, rats were administrated with corresponding drugs for 28 days, the learning and memory ability was tested with Morris water maze, the number of positive cells and the integral optical density of the immunostaining on Bcl-2 and Bax protein expression in the hippocampus was detected. Results With Morris water maze test, the latency time increased significantly and times of searching submerged platform decreased significantly in the model group, the difference was significant compared with control group (P<0.01). Compared with model group, the latency time of Shenlongtang high dosage group decreased significantly and times of searching submerged platform increased significantly (P<0.05). Compared with model group, the expression of positive cells number of Bax protein expression in the hippocampus area was significantly increased (P<0.05), the Bcl-2 protein expression in the hippocampus region was significantly decreased (P<0.01). Conclusion Shenlongtang could significantly improve the learning and memory ability of cerebral ischemia rats, the mechanism may be related to regulating the expression of Bcl-2 and Bax.

Objective To investigate the effect of Shenlongtang decoction (SLT) on the learning and memory ability and serum level of neuropeptide Y (NPY) and expression of NPY in hippocampus of rats with cerebral ischemia/reperfusion. Methods 50 rats were randomly divided into 5 groups: control group with sham operation; model group with cerebral ischemia/reperfusion; pisacetam group as positive control;and SLT groups (low and high dosages). The rats' model was established with two-vessel occlusion. After modeling, the rats were administrating with SLT or pisacetam for 28 d. Learning and memory ability was tested with Morris water maze, the level of NPY was detected with radioimmunoassay, and the number of positive cells of NPY in hippocampus was detected. Results Compared with the control group, the latency time increased significantly and frequence of searching submerged platform decreased significantly in the model group (P<0.01); and compared with the model group, the latency time decreased significantly and frequence of searching submerged platform increased significantly in SLT high dosage group (P<0.05). Compared with the control group, the serum levels of NPY increased significantly in the model group, and the number of positive cell of NPY in hippocampus area significantly increased (P<0.01); and compared with the model group, the serum levels of NPY decreased significantly, and the number of positive cell of NPY in hippocampus area significantly decreased in the SLT high dosage group and pisacetam group (P<0.05). Conclusion SLT could improve the learning and memory function.The mechanism may be related to regulate the serum level and expression of NPY in hippocampus area of rats with cerebral ischemia/reperfusion.

Objective To investigate the effect of Shuyuningxin decoction on the expression of Bcl-2 and Bax in hippocampus of chronicstress depression rats. Methods 60 SD rats were randomly divided into control group, model group, fluoxetine group, high dose ofShuyuningxin decoction group, middle dose of Shuyuningxin decoction group and low dose of Shuyuningxin decoction group. All thegroups were given the chronic unpredicted mild stress for 21 d to induce depression models except the control group. Then they were administratedtheir drugs for 21 d. The level of Bcl-2 and Bax in hippocampus were investigated with immunohistochemistry technique. ResultsCompared with the control group, the level of Bax increased and Bcl-2 decreased in the model group (P<0.05). Compared with modelgroup, the level of Bax decreased and Bcl-2 increased in the middle dose of Shuyuningxin decoction group, high dose of Shuyuningxin decoctiongroup and the fluoxetine group (P<0.05). Conclusion The anti-depression effect of Shuyuningxin decoction may be associated withthe expression of Bax and Bcl-2.

Objective The therapeutic efficacy comparison of the ulcerative colitis(UC) disease between the enema treatment combined by Chinese and Western Medicine and purely regular western medicine oraltaking,and infusion treatment.Methods There were 30 cases of treatment group,and we used selfplanned herb medicine and western to conduct enema treatment altemative.And there were 30 cases of control group which adopted purely western medicine took orally,intramuscular,and intravenous driping treatment. Results The total effective rate treatment group was 98%,and control group was 68%.The difference of two sets had statistical significance(P＜0.05).The recurrence rate:treatment enema group Was 10%,and control group was 40%(P＜0.05);The adverse reaction enema group didn't show conspicuous adverse reaction,and in the control group there were headache,nausea,full-moon-shaped face etc to different extent. Conclusions Using the interchanging of Chinese medicine and Western medicine to conduct enema treatment towards ulcerative colitis conservatively,the curative effect of clinical observation is conspicuous,and there is rarely adverse reaction.
