1.Survey and drug resistance analysis on pathogens in the intestines of healthy humans in Guangzhou from 2018 to 2023

Objective To investigate the carrying status and drug resistance analysis of pathogenic bacteria in the gut of healthy population in Huadu District,Guangzhou,and provide reference for epidemiological re-search.Methods A total of 337 459 anal swabs were directly isolated and cultured from healthy individuals who underwent physical examinations from 2018 to 2023.600 anal swabs from 2022 were randomly selected for multi pathogen nucleic acid testing,direct isolation and culture,and enrichment culture.Identification of bacteria was conducted by using VITEK 2 Compact and VITEK MS,serological testing was used for determi-nation of type,and drug susceptibility testing of pathogenic bacteria was conducted.Results A total of 128 pathogenic bacteria were isolated from rectal swabs,including 52 strains of Salmonella,71 strains of Aero-monas,3 strains of Vibrio parahaemolyticus,and 2 strains of Shigella flexneri,with a total detection rate of 3.79/10 000.The detection rate of Salmonella in direct isolation and culture of 600 rectal swabs was 0.50%,the detection rate of enrichment culture was 1.00%,and the detection rate of multi-pathogen nucleic acid de-tection was 1.17%.52 strains of Salmonella were divided into 27 serotypes,and Salmonella Ⅰ 4,5,12:i:—was the dominant serotype.Aeromonas sobria was the dominant serotype in Aeromonas.The sensitivity of Salmonella to piperacillin/tazobactam,imipenem,cefepime,levofloxacin,and aztreonam was up to 92.00%or more,however,the phenomenon of multidrug resistance was severe,with a multidrug resistance rate as high as 40.38%.The resistance rate of Aeromonas to ampicillin and ampicillin/sulbactam was up to 85.00%or more,while Aeromonas was relatively sensitive to other antibiotics.Conclusion There are many species and types of intestinal pathogenic bacteria carried by healthy individuals in Huadu District,Guangzhou,with Aero-monas and Salmonella ranking first and second,and levofloxacin treatment could be preferred.The use of bac-terial culture and multi pathogen nucleic acid testing methods could improve the detection rate of intestinal pathogenic bacteria,and attention should be paid to the monitoring of intestinal pathogenic bacteria among rel-evant practitioners.
2.Application of individualized selection of local skin flaps for primary repair of partial nasal alar defects

Objective:To explore the possibility of repairing partial nasal alar defects with individualized design of localized skin flaps.Methods:The clinical data of 38 patients with nasal alar region tumor from October 2015 to June 2019 in Dalian Municipal Central Hospital were retrospectively analyzed, including 5 cases with intradermal nevus, 8 cases with junction nevus, 21 cases with basal cell carcinoma, 3 cases with trichoepithelioma, and 1 case with nasal alar sulcus fistula combined with infection. Surgical treatment with local anaesthesia was applied, and intraoperative freezing pathology was used to confirm the diagnosis and determine the safe margin. There was no nasal alar cartilage infiltration in all patients. The defect areas after resection of nasal alar lesions ranged from 1.0 cm × 1.0 cm to 3.0 cm × 2.5 cm. Local skin flap was aesthetically designed in accordance with the location and size of the nasal alar defect to primarily repair the defect. Among them, 15 cases were repaired with pedicled nasolabial groove flap, 10 cases with modified rhomboid flap, 6 cases with rotatory nasolabial groove flap, 5 cases with V-Y push flap, and 2 cases with double lobe flap.Results:One case had blood transportation obstacle after operation caused by compression and bandaging, 1 case had postoperative infection. Healing of the two cases delayed after treatment, and other patients healed properly. All the flaps survived without facial deformity, and the cosmetic effect was good.Conclusions:The primary repair of the nasal alar defect needs to follow the aesthetic requirements of the nose and face, which varies with diseases and experience of doctors. Flap selection should be individualized to achieve both the purpose of repairing defects and beauty.
3.Effect of small interfering RNA silencing high mobility group box 1 expression on the biological characteristics of hep-2/v resistant cell line of human laryngeal squamous cell carcinoma

Objective:To study the effect of small interfering RNA (siRNA) silencing high mobility group box 1 (HMGB1) on the biological characteristics of hep-2/v resistant cells line of human laryngeal squamous cell carcinoma (HSCC).Methods:In Dalian Municipal Central Hospital, siRNA-HMGB1 was transfected into Hep-2/v cells. The expression of HMGB1 and MDR1 protein were detected by Western blot and cell immunofluorescence, cell proliferation activity was detected by CCK-8, and cell cycle was detected by flow cytometry.Results:Compared with that of the negative control group and the blank control group, the proliferation ability of the positive control group decreased by 20.18% at 24 h ( P<0.01) and 21.92% at 48 h ( P<0.05). The proportion of G 1 cells increased obviously. The expression of HMGB1 protein decreased 41.38% ( P<0.01), the expression of HMGB1 protein decreased 26.77% ( P<0.05). Conclusions:Reduction of HMGB1 expression in hep-2/v cells can inhibit cell proliferation and reduce drug resistance.
4.The Clinical Application of Eucalyptol, Limonene and Pinene Enteric Soft Capsules in the Treatment of the Prophylaxis of Tympanum Fluid in Secretory Otitis Media in Children

Objective To investigate the effects of the eucalyptol,limonene and pinene enteric soft capsules (mucoregulatory agents) in preventing from tympanum fluid in children.Methods A total of 341 children (497 ears) with acute otitis media were randomly divided into two groups:the experimental group and the control group.The experimental group was given symptomatic treatment and oral in-take of the eucalyptol,limonene and pinene enteric soft capsules,while the control group with acute otitis media received conventional symptomatic treatment.All the patients were followed-up 7 and 14 days after treatment.Clinical data were collected,including symptoms,and tympanogram changes.Results Seven days after the initial treatment,tympanum fluid occurred in 110 cases of 170 ears(65.09%,110/170) in the control group,and in 51 cases of 70 ears (29.65%,51/172) in the experimental group.The difference was statistically significant (P<0.01).After 14 days,41 cases (66 ears)of tympanum fluid occurred in the control group (24.26%,41/169),32 cases(36 ears) of otitis media (18.60%,32/172) occurred in the experimental group.The difference was statistically significant (P<0.01).In the control group,the tympanum fluid disappeared in 54 cases(60 ears) after regular treatment 72.0%,54/75),and reliefed in 15 cases(22 ears) with irregular treatment (42.86%,15/35).The difference was statistically significant (P<0.01).Conclusion Eucalyptol,limonene and pinene enteric soft capsules can effectively reduce the incidence of tympanum fluid in children with acute otitis media.Mucoregulatory agents may promote the eduction of effusion.
