1.Status quo,driving factors and clinical intervention measures of intrinsic capacity in elderly patients with cerebrovascular disease

Objective To investigate the status quo,driving factors and clinical intervention measures of intrinsic capacity(IC)in elderly patients with cerebrovascular disease.Methods Clinical data of 206 elderly patients with cerebrovascular disease admitted to our hospital from January to December 2024 were retrospectively collected.According to the IC score,they were divided into a low IC group(123 cases,IC score<4)and a good IC group(83 cases,IC score≥4).The IC scores were compared between the two groups,and the driving factors affecting the IC were analyzed.Results The low IC group exhibited significantly lower IC score than the good IC group(1.94±0.49 vs 4.52±0.15,P<0.01).The age ≥80 years,dysfunctions,three or more chronic diseases,frailty,albumin ≤ 35 g/L,pre-albumin ≤280 mg/L,rehabilitation guidance and regular exercises were the influencing factors of IC in elderly patients with cerebrovascular disease(P<0.05,P<0.01).Conclusion The IC of elderly patients with cerebrovascular disease is associated with many factors,and corresponding clinical intervention measures should be implemented to promote the improvement of patient's IC.
2.Mediating effect of rehabilitation self-efficacy between outcome expectations and exercise adherence in knee arthroplasty patients

Objective:To explore the mediating role of rehabilitation self-efficacy between outcome expectations and exercise adherence in knee arthroplasty patients.Methods:From June 2023 to May 2024, a total of 205 knee arthroplasty patients from the Department of Orthopedics at the First Affiliated Hospital of Zhengzhou University were selected using convenience sampling. Data were collected using a general information questionnaire, the Hospital for Special Surgery Knee Replacement Expectations Survey (HSS-KRES), the Chinese version of the Self-Efficacy for Rehabilitation Outcome Scale (SER), and the Orthopedic Exercise Adherence Scale (OEAS). Pearson correlation analysis was used to explore the relationships among outcome expectations, rehabilitation self-efficacy, and exercise adherence. Structural equation modeling and mediation analysis were conducted using AMOS 21.0 software.Results:A total of 205 questionnaires were distributed, and 193 valid responses were collected, with an effective response rate of 94.15%. The mean scores for the HSS-KRES, SER, and OEAS were (60.24±5.12), (80.87±11.00), and (33.87±5.49), respectively. Correlation analysis showed that outcome expectations were negatively correlated with rehabilitation self-efficacy and exercise adherence, while rehabilitation self-efficacy was positively correlated with exercise adherence (all P<0.05). Mediation analysis revealed that rehabilitation self-efficacy partially mediated the relationship between outcome expectations and exercise adherence, with a mediation effect estimate of -0.261, accounting for 59.86% of the total effect. Conclusions:Preoperative outcome expectations in knee arthroplasty patients are relatively high, while rehabilitation self-efficacy and exercise adherence are at moderate or lower levels. Rehabilitation self-efficacy serves as a mediating variable between outcome expectations and exercise adherence. Clinical nurses can enhance patients' rehabilitation self-efficacy and manage outcome expectations rationally to improve exercise adherence.

5.Safety of interferon β-1a for treatment of COVID-19: a real-world study based on FAERS database

ObjectiveTo systematically evaluate the safety of interferon β-1a for treatment of corona virus disease 2019 (COVID-19), and to provide references for interferon β-1a's clinical application. MethodsThis study was conducted with the database from US Food and Drug Administration adverse event reporting system (FAERS) from January 1, 2015 to March 31, 2021. Information component (IC) and reporting odds ratio (ROR) methods were applied for signal mining. ResultsA total of 463 700 records of COVID-19 were selected for analysis, and 45 positive drug adverse event signals were detected. Headache (IC025=1.09, ROR025=2.28), pyrexia (IC025=0.51, ROR025=1.51) and multiple sclerosis relapse (IC025=3.67, ROR025=14.71) were positive adverse events with higher frequency. Autoimmune disorder (IC025=4.42, ROR025=24.03), streptococcal infection (IC025=4.12, ROR025=19.82), and multiple sclerosis relapse (IC025=3.67, ROR025=14.71) were positive adverse events. Acute lung injury, cardio-respiratory arrest and metabolic acidosis were associated with a higher proportion and frequency of death. ConclusionThere are certain safety issues with interferon β-1a in the treatment of COVID-19, and some adverse events with high frequency and high death rate deserve further attention by medical staffs.

Objective:To detect genomic aberrations and investigate the expression and clinical significance of TBX2,CHK2, and p53 in malignant peripheral nerve sheath tumor (MPNST) tissues. Methods:We collected 63 cases of MPNST tissue samples, which were re-moved by resection and were confirmed by pathology, from January 1991 to December 2011 in Department of Bone and Sofer Tissue Tumor, Tianjin Medical University Cancer Institute and Hospital. Twelve fresh tumor samples with qualified DNA quality were selected from the above 63 cases of tissue samples. Genome abnormalities of 12 MPNST tissues were detected by next-generation sequencing. The protein expression levels of TBX2, CHK2, and p53 in 63 MPNST tissue samples were assessed by immunohistochemistry staining. Results:One case of TBX2 gene mutation was observed out of the 12 MPNST tissue samples. In 63 MPNST tissue samples, the protein expression rates of TBX2, CHK2, and p53 were 60.3%(38/63), 47.6%(30/63), and 30.2%(19/63), respectively. TBX2 expression was sig-nificantly correlated with AJCC (American Joint Committee on Cancer, AJCC) stage, recurrence, and metastasis (P<0.05). TBX2 expres-sion was directly correlated with that of CHK2 (r=0.254, P=0.045), and CHK2 expression was directly correlated with that of p53 (r=0.343, P=0.006). In terms of the disease-free survival and overall survival time, patients with high expression levels of TBX2, CHK2, and p53 had significantly worse prognosis than patients with low expression levels of TBX2, CHK2, and p53(all P<0.05). TBX2, CHK2, and p53 were independent prognostic factors of MPNST. Conclusion:TBX2 and its associated proteins may play important roles in MPNST development and progression. Detecting TBX2 expression may provide the theoretical basis for estimating the prognosis of patients with MPNST.
8.Applying contact-mode argon plasma coagulation for retroflex colonoscopic treatment of ultra-lower rectal polyps

Objective To explore the clinical values of applying contact-mode argon plasma coagulation (APC) for retroflex colonoscopic treatment of ultra-lower rectal polyps. Methods 46 wide outsole and applanate polyps smaller than 1.0 cm located at ultra-lower rectum in 17 cases of patients were treated by contact-mode APC under U-type retroflex colonoscopy after failure in regular colonoscopic treatment. Then observe the contacting rate of APC probe with polyps, success rate of curing polyps, rate of probe being adhered and blocked by the solidification structures, incidence of submucosal emphysemas, and incidence of colonoscope ambustion. Results Under U-type retroflex colonoscopy, the probe could contact with polyps in 17 patients. The polyps in every patient were cured by APC in the first time of colonoscopic treatment, whereas all 46 polyps were cured by 97 times of APC spurt. Mild adhesions occurred between the probe and solidification structures at 5 times (5.15 %) among 97 times of APC spurt, without injuries to the coagulation surface from which when the probe separated. The solidification structures blocked the probe only twice (2.06 %). When the solidification structures were cleared, efficiency of the probe restored. No sub-mucosal emphysemas and colonoscope ambustion happened. Conclusion Applying contact-mode APC for retroflex colonoscopic treatment of ultra-lower rectal polyps is safe and effective, it can prevent the damage of colonoscope from the argon knife.
9.Effect of Probucol in combination with Enalapril-folic acid tablets on endothelial function in elderly patients with H-type hypertension

Objective To investigate the effect of Probucol combined with Enalapril-folic acid tablets on endothelial function in elderly patients with H-type hypertension.Methods A total of 180 elderly patients with H type hypertension were randomly divided into three groups (n =60,each):Group A (conventional treatment),Group B (conventional treatment + a tablet containing 10 mg enalapril and 0.8 mg Folic acid/day) and Group C (conventional treatment + 500 mg Protocol twice daily + a tablet containing 10 mg Enalapril and 0.8 mg folic acid/day).Plasma hemocyanin (Hcy) and asymmetric dimethylarginine (ADMA) levels and serum nitric oxide (NO) and endothelial NO synthase (eNOS) levels were measured before treatment,and 1 and 6 months after treatment.Results In Group A,plasma Hcy and ADMA levels and serum NO and eNOS levels had no significant differences before treatment versus 1 and 6 months after treatment (P>0.05).Plasma Hcy and ADMA levels decreased and serum NO and eNOS levels increased in Group B 6 months after treatment,compared with those at pretreatment or 1 month after treatment,which were either lower or higher than those in Group A 6 months after treatment (all P<0.05).Plasma Hcy and ADMA levels decreased and serum NO and eNOS levels increased in Group C 6 months after treatment,compared with those at pretreatment or 1 month after treatment,which were either lower or higher than those in Groups A and B 6 months after treatment (all P<0.05).After adjustment for other risk factors,partial correlation analysis showed that plasma ADMA was positively correlated with plasma Hcy,while plasma ADMA and Hcy were negatively correlated with serum eNOS and NO (all P<0.05).Conclusions Probucol in combination with Enalapril-folic acid tablets may be beneficial in improving endothelial function and mitigating the development of atherosclerosis.
10.Signifcance of Detection of Serum Carbonic Anhydrase IX in the Diagnosis of Lung Cancer

Background and objective Carbonic anhydrase IX (CAIX) is a transmembrane protein involved in the metabolism of tumor cells. CAIX is expressed in only a few normal tissues but is overexpressed in various tumor types. Te aim of this study is to detect the serum CAIX level of patients with lung cancer, evaluate the significance of CAIX detection in the diagnosis of lung cancer, and analyze the serum CAIX level among diferent pathological types and TNM stages of lung can-cer. Methods Forty-seven patients with lung cancer and 31 healthy subjects were selected to participate in this study. Serum CAIX level was examined through enzyme-linked immunosorbent assay (ELISA). Te subjects were grouped according to histological type and TNM staging, and serum CAIX level was compared among the groups. With pathological diagnosis as the gold standard, receiver operating characteristic curve of the serum CAIX level was established for the diagnosis of lung cancer. Results Te CAIX serum level was significantly higher in patients with lung cancer than that in the healthy group (P<0.001). Te serum CAIX level in patients with squamous cell carcinoma and small cell carcinoma was also significantly higher than that in patients with adenocarcinoma. No statistically significant diferences were observed in the serum CAIX level between I+II and III+IV staging. Te AUC of serum CAIX level was 0.961. At a threshold level of 115.115 pg/mL, sensitivity and specificity were 95.7% and 90.3%, respectively. Conclusion Detection of the serum CAIX level through ELISA exhibits high sensitivity and specificity and is important for the diagnosis of lung cancer.
