1.Establishment and evaluation of a prognostic model for ovarian cancer based on PD-1 and B lymphocyte infiltration

Objective:To construct a prognostic model for ovarian cancer based on programmed cell death receptor-1(PD-1)and B lymphocyte infiltration.Methods:A total of 102 patients with ovarian cancer admitted to the First Affiliated Hospital of Gannan Medical College from February 2019 to February 2022 were selected.PD-1 expression and CD20+B cell infiltration degree in cancer tissues were detected,relationship between them and pathological characteristics were analyzed,and correlation between PD-1 expres-sion with B lymphocyte infiltration degree and survival rate of patients were analyzed.Independent influencing factors of poor prognosis of ovarian cancer patients were analyzed,prediction model was built and validated according to independent factors.Results:PD-1 expression and CD20+B cell infiltration degree were closely related to tumor size,FIGO stage,tumor differentiation and lymph node metastasis(P<0.05).PD-1 expression was negatively correlated with B-lymphocyte infiltration degree(P<0.05).Patients with high PD-1 expression and low B-lymphocyte infiltration had higher mortality(P<0.05).FIGO stage,lymph node metastasis,PD-1 expres-sion and CD20+B cell infiltration were independent factors affecting poor prognosis of ovarian cancer patients.Prediction model was constructed by independent influencing factors,which had good differentiation and accuracy.Conclusion:PD-1 expression is closely related to CD20+B cell infiltration degree.There is a close relationship between PD-1 and CD20+B cell infiltration with clinico-pathological characteristics and prognosis of ovarian cancer patients,which are independent influencing factors for poor prognosis of ovarian cancer patients.
2.Transition of body mass index and metabolic syndrome in patients with major depressive disorder

Objective To evaluate the transition rules of normal body mass index(BMI),overweight and metabolic syndrome(MetS)in patients with major depressive disorder(MDD).Methods Patients with MDD who had multiple admission records between Jan 2016 and Nov 2021 in Beijing Anding Hospital,Capital Medical University were included.Based on the overweight and metabolic syndrome status assessed at each admission,the patients were categorized into three states:normal BMI,overweight and metabolic syndrome.A multi-state Markov model was used to analyze the transition intensity and transition frequency between three states and the influence of covariates on transitions.Results A total of 892 records of 398 subjects were included,with a median age of 56 years old and 31.4% males.The median follow-up period was 40 months.The multi-state model showed that there were 494 transitions between the three states,of which 5.1% moved from normal BMI to overweight and 5.5% moved from overweight to MetS.The intensity of transition was the highest from overweight to MetS,9.52 times greater than overweight to normal BMI.After 48.53 months,MDD patients with normal BMI began to transition to MetS.For overweight MDD patients,the transition to MetS started after 8.77 months.MDD patients with normal BMI or overweight had 31.4% and 50.4% probabilities of developing Mets after 36 months.For MDD patients comorbid with MetS,the probability of staying at MetS was 51.2% after 36 months.Multivariate analysis showed that being unmarried was a risk factor against developing overweight in normal BMI MDD patients,while a higher level of education was a protective factor against developing MetS in overweight MDD patients.Conclusion MDD patients exhibited a higher intensity and risk of developing MetS,and it is not easy to reverse MetS,suggesting that BMI management and MetS intervention should be strengthened in MDD patients.
3.Tryptanthrin inhibits the malignant growth of glioma cells by regulating the MAPK/ERK signaling pathway

Purpose To explore whether tryptanthrin(TRYP)can inhibit the malignant behavioral ability of glio-ma cells,and to elucidate the specific mechanism of its action.Methods MTT assay was performed to detect the effect of TRYP on the proliferation of glioma cells;Transwell assay was performed to detect the effect of TRYP on the migration and invasion of glioma cells;AnnexinV-FITC/PI apoptosis assay was performed to detect the effect of TRYP on the apoptosis of glioma cells;PI/RNase cell cycle assay was performed to detect the effect of TRYP on the cell cycle distribution of glioma cells;Western blot assay was performed to detect the effect of TRYP on the protein expressions of p-ERK and c-Myc in glioma cells.The effect of TRYP on the proliferation of glioma cells in vivo was verified by con-structing a subcutaneous transplantation tumor model in nude mice,and the effect of TRYP on the apoptotic ability of cells in the transplantation tumor was detected by TUNEL assay.Immunohistochemistry was used to detect the effect of TRYP on the expression of Ki67,BRAF,c-Myc,and p-ERK proteins in transplanted tumor tissues.Results MTT assay showed that TRYP could effectively inhibit the proliferation of glioma cells(P<0.001).Transwell assay showed that TRYP could inhibit the invasion and migration of glioma cells(P<0.001).AnnexinV-FITC/PI cell apoptosis as-say showed that TRYP could promote the apoptosis of glioma cells(P<0.001).The results of PI/RNase cell cycle as-say showed that TRYP was able to promote the G2 phase block of glioma cells(P<0.001).Western blot results showed that the expression levels of c-Myc and p-ERK proteins in the glioma cells were significantly reduced after TR-YP treatment(P<0.001).The results of subcutaneous transplantation tumor model in nude mice showed that TRYP could effectively inhibit the growth rate(P<0.01)and weight(P<0.05)of transplanted tumor.TUNEL assay showed that TRYP could promote the apoptosis of tumor cells in transplanted tumor(P<0.001).Immunohistochemis-try results showed that TRYP could effectively inhibit the protein expression of Ki67(P<0.01),BRAF,c-Myc,and p-ERK(P<0.001).Conclusion TRYP can inhibit the proliferation,invasion,and migration ability of glioma cells,promote apoptosis of glioma cells,and block the cell cycle of glioma cells.TRYP may inhibit the malignant pro-gression of glioma cells by suppressing the protein expression of BRAF,c-Myc and p-ERK1/2 in the MAPK/ERK sig-naling pathway.

5.Mechanism Study of Coptisine in Treating Ulcerative Colitis in Mice Based on Non-Targeted Metabolo-mics Technology

OBJECTIVE To investigate the effects of coptisine on endogenous metabolites in a dextran sulfate sodium(DSS)-in-duced ulcerative colitis(UC)mouse model,and to explore its potential mechanisms of action employing non-targeted metabolomics technology.METHODS SPF-grade male C57BL/6 mice were randomly divided into a control group,a model group,a sulfasalazine group(100 mg·kg-1),and low and high dose groups of coptisine groups(25,50 mg·kg-1).To induce ulcerative colitis(UC),all groups except the control group had free access to a 2.5%DSS solution for 7 days.At the same time,they also received daily intragastric ad-ministration of their corresponding treatments until the 10th day.Body weight changes,stool characteristics,and bloody stool occur-rence were recorded daily,and the disease activity index(DAI)was calculated.After the experiment,colon tissues were collected for pathological examination.Through UPLC-Q-TOF-MS/MS,non-targeted metabolomic analysis was performed to identify differential metabolites,and metabolic pathway enrichment analysis was conducted using the KEGG database.RESULTS Compared to the model group,coptisine significantly ameliorated weight loss,DAI scores,and pathological damage in colon tissues of UC mice(P<0.05,P<0.01).Metabolomic analysis identified 56 differential metabolites,mainly involved in purine metabolism,tryptophan metabo-lism,niacin and nicotinamide metabolism,glutathione metabolism,and the biosynthesis of phenylalanine,tyrosine,and tryptophan.Coptisine intervention significantly reversed the abnormal expression of these metabolites.CONCLUSION Coptisine can markedly improve metabolic disorders in DSS-induced UC mice by modulating multiple key metabolic pathways,thereby exerting a therapeutic effect.

7.Lactylation for hepatocyte proliferation and liver regeneration after hepatectomy

Aim To investigate the changes in hepatic lactylation during liver regeneration and its impact on regeneration.Methods A partial hepatectomy(PHx)mouse model was used to study liver regenera-tion.Sodium oxamate was administered intraperitoneal-ly to inhibit lactate dehydrogenase,and blood and liver tissues were collected at different time points post-sur-gery.The histopathological status was observed using HE staining.Proliferating cell nuclear antigen(PC-NA)levels were detected by immunohistochemistry.Lactylation was assessed using immunofluorescence.Liver LDH enzyme activity,lactate levels and serum alanine aminotransferase(ALT)and aspartate trans-aminase(AST)levels were measured using assay kits.Results After PHx,the liver volume of mice gradual-ly increased,returning to preoperative size on day 7.PCNA levels peaked at 48 hours post-surgery.Liver tissue lactate levels increased to approximately 1.5 times the preoperative level at 12 hours post-surgery and remained elevated until day 7.The lactylation lev-el in hepatocytes peaked at 24 hours post-surgery,gradually declined after 48 hours,and returned to pre-operative levels on day 7.Compared to the PHx group,the sodium oxamate(750 mg·kg-1)+PHx group showed significantly reduced lactylation levels in hepa-tocytes and a smaller liver regeneration volume on day 7.Conclusion Lactylation regulates hepatocyte pro-liferation and promotes liver regeneration after PHx.
8.Preliminary Clinical Study of Contrast-free Endovascular Aortic Repair(FLARE Technique)

Objectives:To evaluate the safety and feasibility of endovascular aortic repair without contrast agent under branch artery guidewire marking(FLARE technique).Methods:The clinical data of 7 patients who underwent contrast-free endovascular aortic repair with branch artery guidewire marking in Fuwai Hospital from 2024 to 2025 were retrospectively analyzed.The criteria for patient selection included renal insufficiency,history of contrast agent allergy,high risk of high-pressure angiography due to extensive calcification of the aortic arch,and patients'strong personal wishes,all patients merited with anatomically friendly and anchored area criteria.The patients were evaluated by preoperative computed tomography or color Doppler ultrasound,and the occlusive stent anchor point was located by branch artery guidewire marking combined with bone marking during surgery.The primary endpoints were early stage of postoperative renal function changes(comparison of preoperative and postoperative serum creatinine)and surgical technique success rate,and the secondary endpoints included the incidence of internal leakage,re-intervention rates,and incidence of aneurysm and kidney-related adverse events during follow-up.Results:Among the seven patients who underwent endovascular aortic repair without contrast using a branch artery guidewire,four were male,with an average age of(72.0±5.9)years.Six of these patients had infrarenal abdominal aortic aneurysms,two of them with bilateral renal artery severe stenosis and renal insufficiency underwent renal artery stenting combined with endovascular aortic repair,one patient had isolated chronic renal insufficiency,one had a history of iodine contrast skin allergy,and the remaining two cases wished this surgery option.The seventh patient had a penetrating ulcer in the aortic arch and descending aorta,along with extensive thrombosis and calcification in the ascending aorta,aortic arch and descending aorta.All the patients achieved surgical technique success.No iodine contrast agent was used during the procedure for endovascular aortic repair.In patients with chronic renal insufficiency and renal artery stenosis before surgery,serum creatinine levels were significantly improved after surgery.All patients did not need hemodialysis,there was no allergic reaction,and no graft-related or perioperative complications.The average follow-up was(5.8±3.0)months,all patients recovered well without re-intervention or complications.The creatinine levels did not fluctuate significantly after surgery.Conclusions:Branch artery guidewire marked contrast-free aortic endovascular repair may be a safe and feasible treatment option in selected patients,especially in patients with contraindications to contrast agents.
9.Effects of Supplemented Wendan Decoction on glycolipid metabolism and PI3K/Akt/FOXO1 signalling pathway in 3T3-L1 adipocytes

AIM To investigate the impact of varying dosages of Supplemented Wendan Decoction on the PI3K/Akt/FOXO1 glycolipid metabolic pathway in 3T3-L1 adipocytes.METHODS The CCK-8 assay was used to determine the concentration of Supplemented Wendan Decoction-medicated serum.The mature adipocytes differentiated from 3T3-L1 preadipocytes after induction were further divided into the blank control group,the model group,the rosiglitazone group(10 mg/L),and the Supplemented Wendan Decoction groups(5%,10%,and 20%),followed by the sample collections after 48 hours of treatment.Oil red O staining quantified lipid accumulation in 3T3-L1 adipocytes;extracellular glucose levels were measured using glucose oxidase(GOD)assay;RT-qPCR analyzed mRNA expressions of IRS-1,PI3K,Akt,GLUT4,IL-6,TNF-α and IL-1β;Western blot assessed protein expressions of INSR,IRS-1,PI3K-p85,Akt,FOXO1 and GLUT4.RESULTS No significant changes in cell viability(P>0.05)were observed in 3T3-L1 preadipocytes exposed to serum containing supplemented Wendan Decoction at different concentrations for 24,48,or 72 hours.The 3T3-L1 preadipocytes held the capacity to differentiate into mature adipocytes within a 14-day induction period.Compared to the model group,all supplemented Wendan Decoction groups exhibited reduced lipid accumulation in adipocytes and downregulated mRNA expression of IRS-1,IL-6,TNF-α and IL-1β(P<0.01);the low-dose group demonstrated increased mRNA expressions of PI3K and GLUT4(P<0.05,P<0.01),alongside elevated protein expressions of INSR,IRS-1,PI3K-p85,Akt and GLUT4(P<0.05,P<0.01);the medium-dose group showed enhanced GLUT4 mRNA expression,and upregulated protein expressions of INSR and FOXO1(P<0.01).After 24 hours intervention,the high-dose Supplemented Wendan Decoction group exhibited increased glucose consumption in adipocytes(P<0.01),and elevated protein expression of INSR,Akt and FOXO1(P<0.05,P<0.01).CONCLUSION Supplemented Wendan Decoction reduces lipid accumulation in adipocytes,regulates glucose and lipid metabolism,and promotes metabolic homeostasis through PI3K/Akt/FOXO1 signaling pathway.
10.Two cases of microsatellite stable locally advanced gastric cancer achieving pathological complete response by laparoscopic total gastrectomy after immunotherapy combined with neoadjuvant chemotherapy

Objective:To evaluate immunotherapy combined with neoadjuvant chemotherapy before radical total gastrectomy in microsatellite stable locally advanced gastric cancer in two cases.Methods:Two male patients with clinical stage cT3N 1M0 and microsatellite-stable locally advanced gastric cancer were treated with neoadjuvant chemotherapy with PD-1 inhibitor (Nivolumab) combined with SOX (Oxaliplatin+S-1) for 4 cycles before surgery. Standard laparoscopic assisted total gastrectomy with D 2 lymphadenectomy was performed on Feb 2023 and Oct 2023 respectively after the neoadjuvant treatment. Pathological tumor regression grade(TRG) was observed to assess the degree of tumor regression, and follow-up was conducted to monitor tumor markers and abdominal enhanced CT to detect recurrence. Results:Two patients achieved pathological complete response(TRG0). They were followed up until May 2024 and no recurrence was observed.Conclusion:Preoperative combination of chemotherapy and immunotherapy may provide survival benefit for microsatellite stable locally advanced gastric cancer patients.
