Objectives:
. The prognostic value of the pretreatment albumin-to-alkaline phosphatase ratio (AAPR) in head and neck cancer (HNC) remains uncertain. This meta-analysis aimed to evaluate the predictive role of AAPR for survival outcomes in patients with HNC.
Methods:
. A comprehensive search of the Cochrane Library, PubMed, and Embase databases was conducted to identify relevant studies published up to July 30, 2024. We included studies on AAPR and survival outcomes in HNC patients.
Results:
. Eight studies comprising 1,737 HNC patients were analyzed using random-effects models. Lower AAPR values were significantly correlated with worse overall survival (hazard ratio [HR], 2.08), progression-free survival (HR, 2.00), and disease-free survival (HR, 2.18). Sensitivity analyses confirmed the robustness of these results, with no significant publication bias detected.
Conclusion
. Our findings suggest that pretreatment AAPR could serve as a valuable and cost-effective prognostic indicator in HNC, potentially aiding clinicians in risk stratification and treatment decision-making. However, additional validation studies are warranted to confirm its clinical applicability.

Objective To investigate the relationship between primary pulmonary mucinous adenocarcinoma (PPMA) mass type and pneumonia type and their difference in malignant degree, and to analyze the role of clinical manifestations and CT features in the diagnosis of this disease. Methods The clinical data of PPMA patients admitted in the First Affiliated Hospital of Xiamen University from May 2011 to March 2022 were retrospectively analyzed. According to CT features, they were divided into a mass type group and a pneumonia type group. The clinical manifestations, CT features and the degree of malignancy between the two groups were analyzed and compared. Results A total of 57 PPMA patients were enrolled. There were 17 males and 40 females, with an average age of (53.82±10.65) years, and 28 (49%) patients had reversed hato-like sign. There were 42 patients in the mass type group and 15 patients in the pneumonia type group. PPMA often occurs in both lower lungs, with clinical manifestations mainly of coughing and expectorating white mucoid sputum. There were statistical differences between the two groups in the maximum diameter of tumor (P<0.001), boundary condition (P<0.001) and pleural indentation sign (P=0.019). There was no statistical difference between the two groups in Ki-67 index (P>0.05). Conclusion There is no statistical difference in the degree of malignancy between the two types of PPMA. Considering their clinical manifestations and differences in imaging features, it is supported that the pneumonia type is just a progression of the mass type. CT can present various manifestations, among which the reversed hato-like sign is expected to become an important imaging feature. Combined with a high proportion of solid components, pleural indentation sign, and vacuole sign, reversed hato-like sign can play a significant role in the diagnosis of PPMA.

Objective To analyze the virus subtypes, molecular evolutional and molecular transmission network features of the first confirmed mpox case in Huai’an City, Jiangsu Province, so as to provide insights into understanding of the transmission and evolution dynamics of mpox virus and formulation of the mpox control strategy in the city. Methods Genomic DNA was extracted from swabs of the first confirmed mpox case’s skin lesions in Huai’an City, and the amplicon sequencing library was constructed using the hypersensitive mpox virus whole-genome capture kit. High-throughput sequencing was performed using the GridION X5 nanopore sequencer on the Nanopore sequencing platform, and single nucleotide polymorphism (SNP) analysis of mpox virus genome sequences was performed following sequence assembly. In addition, phylogenetic analysis, genetic genealogy and molecular traceability analysis were performed. Results The virus whole genome sequence of the first confirmed mpox case was successfully obtained by high-throughput sequencing, with a full length of 197 182 bp, and was named hMpxV/China/JS-HA01/2023, which belonged to the clade IIb (West African clade) lineage B.1.3. Compared with the mpox virus reference sequence MPXV-M5312_HM12_Rivers-001 (GenBank accession number: NC_063383), the genome sequence of the Huai’an virus isolate carried 86 SNPs, including 40 SNPs in the coding region as non-synonymous mutations and 73 SNPs as nucleotide mutations caused by APOBEC3 (APOBEC3). Of the 97 mpox virus gene sequences, 79 sequences were included in the molecular network (81.44%), and the threshold of the genetic distance accessed to the network was 0.35/105. There were two large molecular transmission clusters and one scattered cluster in the molecular transmission network of the mpox virus, andthehMpxV/China/JS-HA01/2023 sequence was located in the large cluster. The 97 gene sequences formed 92 haplotypes, including three shared haplotypes Hap_4, Hap_6 and Hap_38, and an exclusive haplotype Hap_1 of hMpxV/China/JS-HA01/2023 generated from mutation of the exclusive haplotype Hap_43, while the exclusive haplotype Hap_43 was generated from mutation of the shared haplotype Hap_38. Conclusions The whole genome sequence of the mpox virus isolated from the first confirmed mpox case in Huai’an City has been successfully obtained, and the molecular evolutionary and molecular transmission network characteristics of the virus have been preliminarily understood.

Objective:To evaluate the clinical equivalence between a domestic calcipotriol/betamethasone dipropionate ointment and the originator product in the treatment of stable plaque psoriasis. Methods:A multicenter, randomized, double-blind, three-arm, parallel-group, active- and placebo-controlled study was conducted, and 449 patients aged 18 - 65 years with stable plaque psoriasis were enrolled from 25 hospitals (such as the First Affiliated Hospital of China Medical University). Eligible patients had a baseline physician's global assessment (PGA) score of ≥ 3 points, baseline body surface area (BSA) involvement of 5% - 30%, and a target lesion psoriasis area and severity index (TL-PASI) for plaque elevation of ≥ 3 points. Participants were randomly assigned in a 2:2:1 ratio to the test group ( n = 179), reference group ( n = 180), and placebo group ( n = 90), and applied the domestic calcipotriol/betamethasone dipropionate ointment, originator product, and ointment base respectively, once daily in the evening for 4 weeks. Efficacy and safety were assessed at weeks 1, 2, and 4. The primary efficacy endpoints were the treatment success rates and clinical success rates in each group at week 4. The per-protocol set (PPS) was used for the primary efficacy analysis, and the intention-to-treat (ITT) set for supplementary efficacy analysis. Equivalence between the test and reference preparations was tested using the Cochran-Mantel-Haenszel method adjusted for randomization strata. Superiority of the test and reference preparations over the placebo was also tested. Measurement data were compared among the 3 groups using analysis of variance or non-parametric tests, while treatment success rates, clinical success rates, and incidence rates of adverse reactions were compared using the chi-square test. Results:The ITT, PPS, and safety sets included 447, 420, and 448 patients, respectively. In the ITT set, patients were aged 43.6 ± 12.8 years, including 320 (71.6%) males and 127 (28.4%) females, and the disease duration was 11.21 ± 9.05 years; 316 (70.7%) had a PGA score of 3 points and 131 (29.3%) had a PGA score of 4 - 5 points. No significant differences in the baseline characteristics (including age, sex, disease duration and disease severity) were observed among the 3 groups (all P > 0.05). Based on the PPS analysis, the treatment success rates were 57.9% (99/171) in the test group, 50.3% (86/171) in the reference group, and 7.7% (6/78) in the placebo group, and the clinical success rates were 57.9% (99/171), 50.3% (86/171), and 10.3% (8/78), respectively; both the test and reference groups were superior to the placebo group in both treatment and clinical success rates (all P < 0.001) ; the rate differences for treatment success (90% confidence interval [ CI]: -1.3% - 16.4%) and clinical success (90% CI: -1.3% - 16.3%) between the test and reference groups were entirely within the pre-defined equivalence margin (-20% - 20%). Subgroup analyses by baseline PGA scores: for patients with a baseline PGA score of 3 points, the treatment success rates in the test, reference, and placebo groups were 60.8% (73/120), 52.1% (62/119), and 11.1% (6/54), respectively, and the corresponding clinical success rates were 61.7% (74/120), 53.8% (64/119), and 13% (7/54), respectively; the test and reference groups did not differ significantly in treatment or clinical success rates (both P > 0.05), but both showed higher success rates than the placebo group (all P < 0.001) ; the results of statistical comparisons among the 3 groups in patients with a baseline PGA score of 4 - 5 points were consistent with those observed in patients with a baseline PGA score of 3 points. The percentage reductions in PGA and TL-PASI scores from baseline to weeks 1, 2, and 4 showed significant differences among the 3 groups, which were significantly higher in the test and reference groups than in the placebo group (all P < 0.001), but did not differ between the test and reference groups (all P > 0.05). The primary adverse reactions were local skin reactions, such as pruritus, pain, and erythema. The incidence rates of adverse reactions were 8.9% (16/179) in the test group, 7.3% (13/179) in the reference group, and 7.8% (7/90) in the placebo group, with no significant difference among the 3 groups ( P > 0.05) . Conclusions:The domestic calcipotriol/betamethasone dipropionate ointment demonstrated clinical equivalence to the originator product in the treatment of stable plaque psoriasis, and the two agents exhibited comparable efficacy for patients with varying degrees of disease severity, and were comparable in the speed and degree of clinical improvement, with similar favorable safety profiles.

Objective:To investigate the relationship between circular RNA (circRNA) circ-DCAF7 and the survival of prostate cancer patients, as well as the effects and potential mechanisms of circ-DCAF7 on proliferation and migration of prostate cancer cells in vitro. Methods:The expression and survival data of circ-DCAF7 gene were obtained from 151 prostate cancer patients using the PROGgeneV2 online platform. Patients were divided into circ-DCAF7 high and low expression groups based on the median expression level of circ-DCAF7 gene, and the difference in overall survival between the two groups was compared. Real-time fluorescence quantitative polymerase chain reaction (qPCR) was used to detect the expression of circ-DCAF7 at transcription level in human prostate cancer cell lines 22Rv1, VCaP, PC-3, DU-145, LNCaP, and human normal prostate epithelial cell line RWPE-1. The control plasmid (carrying irrelevant control sequences) and the plasmid carrying circ-DCAF7 sequences were transfected into VCaP cells using liposome reagents, respectively, and referred to as the control group and circ-DCAF7 group. The proliferation and migration abilities of two groups of VCaP cells were detected by plate clone formation assay and cell scratch assay, respectively; using CircInteractome online software, specific binding sites were predicted between circ-DCAF7 and miRNA-18a-5p (miR-18a-5p) sequences, and validated using dual luciferase reporter gene assay; qPCR was used to detect the expression of miR-18a-5p at transcription level in VCaP cells of control group and circ-DCAF7 group; Western blotting was used to detect the expression levels of AKT signaling pathway factors p-AKT, p-mTOR, HIF-1α, and c-myc proteins in two groups of VCaP cells. Results:Analysis using the PROGgeneV2 online tool showed that the overall survival of prostate cancer patients in the circ-DCAF7 high expression group was better than that in the circ-DCAF7 low expression group, and the difference was statistically significant ( P < 0.001). Compared with the normal prostate epithelial cell line RWPE-1, the relative expression of circ-DCAF7 at transcription level in all prostate cancer cell lines was lower, and the differences were statistically significant (all P < 0.05). Among them, the expression level of circ-DCAF7 in VCaP cells was the lowest ( P < 0.001). The relative expression of circ-DCAF7 at transcription level in VCaP cells in the circ-DCAF7 group was higher than that in the control group (9.88±1.58 vs. 1.04±0.39), and the difference was statistically significant ( t = 5.42, P = 0.002). The number of VCaP cell clone formation in the circ-DCAF7 group was less than that in the control group (35±12 vs. 116±11), and the difference was statistically significant ( t = 4.96, P = 0.003). The 25-hour scratch healing rate of VCaP cells in the circ-DCAF7 group was lower than that in the control group [(16±3)% vs. (52±6)%], and the difference was statistically significant ( t = 5.37, P = 0.002). The dual luciferase reporter gene assay showed that compared with VCaP cells co-transfected with miR-18a-5p unrelated control and carrying wild-type circ-DCAF7 sequence plasmid, the relative luciferase activity of VCaP cells co-transfected with miR-18a-5p and carrying wild-type circ-DCAF7 sequence plasmid was lower, and the difference was statistically significant ( t = 7.31, P < 0.001). The relative expression of miR-18a-5p at transcription level in VCaP cells in the circ-DCAF7 group was lower than that in the control group (0.99±0.15 vs. 7.55±1.12), and the difference was statistically significant ( t = 5.83, P = 0.001). Western blotting analysis showed that the expression levels of p-AKT, p-mTOR, HIF-1α, and c-myc proteins of the AKT signaling pathway in VCAP cells of the circ-DCAF7 group were lower than those in the control group. Conclusions:The low expression level of circRNA circ-DCAF7 may be associated with poor prognosis of prostate cancer patients; upregulation of circ-DCAF7 level in prostate cancer cells may inhibit cell proliferation and migration by regulating the miR-18a-5p/AKT signaling pathway.
