1.Curcumin-loaded exosomes from hypoxia-treated mesenchymal stem cells alleviate microglial inflammatory response in a combined therapy approach

ObjectiveTo investigate the effects of hypoxia-treated mesenchymal stem cell (MSCs) exosomes (Exo) and their loading with curcumin on microglial inflammatory responses, and to explore the enhancing effect of hypoxia treatment on the function of MSCs Exo. MethodsThe supernatants of human umbilical cord (hUC)-MSCs cultured under normal and hypoxic conditions were collected, and Exo were isolated using ultracentrifugation. After identification by transmission electron microscopy and Western blot, curcumin was loaded using the co-incubation method. The lipopolysaccharide (LPS)-induced microglial inflammation model was treated with dimethyl sulfoxide (DMSO), curcumin, normoxia Exo, hypoxia Exo, normoxic Exo loaded with curcumin, and hypoxic Exo loaded with curcumin, respectively. The expression of the M1-type marker inducible nitric oxide synthase (iNOS) in BV2 cells was detected by immunofluorescence (IF). Western blot and enzyme-linked immunosorbent assay (ELISA) were used to measure the expression and secretion levels of tumor necrosis factor-α (TNF-α), interleukin-1β (IL-1β), and IL-6 in the cells and their culture supernatants. ResultsNormoxia Exo, hypoxia Exo, normoxic Exo loaded with curcumin, and hypoxic Exo loaded with curcumin exhibited a "saucer-like" shape with a diameter ranging from 30~150 nm, and the expression of exosomal markers CD9, CD81, and TSG101 were positive. After treating the BV2 cell inflammation model, IF results showed that, compared with the normoxia Exo group, treatment with hypoxic Exo significantly reduced the expression of iNOS. Moreover, when compared with the curcumin group and the normoxic Exo loaded with curcumin group, the expression level of iNOS significantly decreased after treatment with hypoxic Exo loaded with curcumin. The results of Western blot and ELISA indicated that, in comparison with the normoxia Exo group, treatment with hypoxic Exo significantly reduced the expression and secretion of the inflammatory cytokines TNF-α, IL-1β, and IL-6. Additionally, when compared with the curcumin group and the normoxic Exo loaded with curcumin group, both the expression and secretion of TNF-α, IL-1β, and IL-6 significantly decreased after treatment with hypoxic Exo loaded with curcumin. ConclusionHypoxia preconditioning can enhance the ability of hUC-MSCs-Exo in the inhibition of microglial polarization and inflammatory factors’ secretion. Additionally, using Hypoxia-MSCs-Exo as a drug-delivery carrier of curcumin can improve its solubility and stability, facilitating its absorption by cells and exerting the therapeutic effect of combination therapy.
2.Short-Term Efficacy and Long-Term Recurrence Rate of Traditional Chinese Medicine Versus Western Surgical Treatment for Mixed Hemorrhoids:A Multi-Center Retrospective Cohort Study Based on Real-World Data

ObjectiveTo observe the short-term and long-term efficacy of traditional Chinese medicine (TCM) surgical operations in treating mixed hemorrhoids. MethodsA multi-center retrospective cohort study was conducted, collecting clinical data from 17,831 mixed hemorrhoid surgery patients in 8 top-tier TCM hospitals in Jiangsu Province. Standardized and structured datasets were obtained through artificial intelligence models. Patients who underwent western surgical treatment were categorized into the western surgery group (11,646 cases), and those receiving TCM surgical operations were categorized into the TCM surgery group (6185 cases). Propensity score matching (1∶1 matching) was used to balance baseline data between groups. The primary outcome was the one-year recurrence rate, and secondary outcomes included the main symptoms (rectal bleeding, degree of prolapse) and secondary symptoms (anal distension, anal edema, wound secretion and exudation, anal stenosis, residual skin tags, perianal itching, and anal pain) measured on days 7, 28, and 60 after discharge. ResultsAfter matching, 2194 patients were included in each group. Symptom scores showed that at 28 days after discharge, the TCM surgical group had superior improvement in rectal bleeding [OR=5.786, 95%CI (3.092,10.827)], degree of prolapse [OR=4.510, 95%CI (1.649,12.333)], and anal edema [OR=3.188, 95%CI (1.295,7.845)] compared to the western surgical group. At 60 days post-discharge, the TCM group still showed advantages in improving rectal bleeding [OR=5.237, 95%CI (1.077,25.464)] and anal pain [OR=11.697, 95%CI (1.186,115.336)] (P<0.05). Long-term follow-up showed that the one-year recurrence rate in the TCM surgery group was 1.1% (8/726), while that in the western surgery group was 2.3% (10/444), with no statistically significant difference between the two groups (P>0.05). ConclusionBased on real-world data, TCM surgical treatment for mixed hemorrhoids shows significant short-term symptom improvement, particularly in terms of hemostasis, reducing swelling, and alleviating prolapse of anal masses.
3.Analysis on the Acupoint Selection Law of Acupuncture and Moxibustion for Pediatric Cerebral Palsy Based on Data Mining Techniques

Objective To analyze the acupoint selection law of acupuncture and moxibustion for pediatric cerebral palsy based on data mining techniques.Methods Clinical research literature about acupuncture and moxibustion for the treatment of pediatric cerebral palsy was retrieved from CNKI,Wanfang Data,VIP,CBM,PubMed,Web of Science,Embase and Cochrane Library from the establishment of the databases to April 1,2024.Excel 2021 was used to establish a database of prescription acupoints.The frequency of use of acupoints,attributed to the meridians,parts and specific acupoints were under statistically analysis,and SPSS Modeler 18.0 and SPSS Statistics 27.0 were used to conduct the association rules analysis,factor analysis and clustering analysis of acupoints.Results A total of 579 articles were included,579 prescriptions were extracted,containing 245 acupoints with the frequency of 6 422 times.The high-frequency acupoints were Zusanli,Baihui,Hegu,Sanyinjiao and Quchi,etc.The commonly used meridians were Governor Vessel,gall bladder meridian,stomach meridian,large intestine meridian and bladder meridian;mostly used acupoints were in the lower limbs,head and neck,upper limbs,waist and back.The association rules analysis showed that the top combinations were"Hegu-Quchi","Zusanli-Quchi","Zusanli-Quchi-Hegu","Hegu-Quchi-Zusanli";clustering analysis showed that 6 valid clusters were extracted;factor analysis extracted 7 valid common factors.Conclusion Acupuncture and moxibustion treatment for pediatric cerebral palsy is based on the principle of strengthening the brain,improving intelligence and tonifying the five zang organs,reflecting the principle of matching acupoints far and near.The core combination of acupoints is"Zusanli-Quchi-Hegu".

5.Systematic review and Meta-analysis of factors influencing taste alterations in cancer patients undergoing chemotherapy

Objective To systematically evaluate the factors affecting taste alteration in patients undergoing cancer chemotherapy.Methods PubMed,Cochrane Library,Web of Science,CINAHL,Embase,CBM,CNKI,WanFang Data and VIP were searched from the establishment until August 1,2024.The retrieved literature was independently screened,evaluated and the data were extracted by 2 researchers,and statistical analysis was performed using Stata 17.0 software.Results A total of 18 studies were included,involving 4 686 patients.The incidence of taste changes is 73.46%.Totally 9 influencing factors were extracted through quantitative analysis,including oral mucositis(OR=1.98),dry mouth(OR=1.82),nausea(OR=3.05),loss of appetite(OR=2.41),use of triple antiemetic drugs(OR=2.45),gynecological cancers(OR=0.67),lung cancer(OR=0.57),paclitaxel types of chemotherapy d rugs(OR=2.86),and smoking(β=9.38).Conclusion The alteration of taste in cancer chemotherapy patients is in-fluenced by multiple factors.Nurses should regularly and dynamically assess changes in taste and implement individualized and refined nursing interventions in clinical practice to prevent or delay the development of taste alterations,thereby improving patients' quality of life and treatment adherence.
6.BK virus nephropathy after allogeneic hematopoietic stem cell transplantation: a case report and literature review

A 20-year-old male patient with T-lymphoblastic lymphoma/leukemia received 9/10 human leukocyte antigen-compatible unrelated peripheral blood stem cell transplantation. He was transplanted with 5.91×10 8 mononuclear cells/kg and 2.88×10 6 CD34 + cells/kg, and neutrophil engraftment was obtained at +11 days and platelet engraftment at +9 days. After transplantation, he presented with repeatedly increased serum creatinine levels, BK virus (BKV) -associated hemorrhagic cystitis, and BKV viremia. BK virus nephropathy was diagnosed based on renal biopsy and metagenomic next-generation sequencing. After adjusting the immunosuppressant, intravenous immunoglobulin, and donor lymphocyte infusion treatment, the patient’s renal function deteriorated progressively, and he eventually died of multiple organ failure at +289 days.
7.Evaluation of host nucleic acid removal and pathogen enrichment methods in animal tissue samples

This study was aimed at investigating the effectiveness of various host nucleic acid removal and non-specific amplifica-tion techniques in animal tissue samples,to increase the accuracy of pathogen identification in tissue samples.Simulated samples were prepared with a mixture of mouse lung tissue homogenates and Klebsiella pneumoniae fluids,and processed with six host nucleic acid removal kits and three non-specific amplification techniques.The effectiveness of each method in removing host DNA and enriching nucleic acids of pathogenic microorganisms was evaluated through real-time fluorescence quantitative PCR and high-throughput se-quencing.For host nucleic acid removal techniques,the method of selective cleavage and quantitative degradation of host DNA(Com-plete5 kit)effectively decreased the host nucleic acid content in tissue samples and increased the relative abundance of pathogen nucleic acids.In contrast,the magnetic bead method for host DNA removal(Next microbiome DNA enrichment Kit kit)was less effec-tive.At lower pathogen concentrations(77 CFU/mL),the Vazyme kit was more effective than the other kits in removing host nucleic acids.Non-specific amplification techniques(MALBAC whole genome amplification,MDA isothermal amplification,and random primer amplification)were not applicable to tissue samples and were not effective in increasing the relative abundance of pathogen nucleic acids.Selective lysis and quantitative degradation of host DNA were suitable for processing tissue samples with high host back-ground and low pathogenic microorganism levels,whereas non-specific amplification methods were not applicable to tissue samples for pre-processing of macro-genome high-throughput sequencing.
8.Defining fertility preservation based on Rodgers' evolutionary concept analysis method

Objective:To analyze and elaborate on the concept connotation of fertility preservation, so as to provide guidance for clinical nursing practice.Methods:Research on fertility preservation was retrieved from databases including China National Knowledge Infrastructure, Wanfang Data, VIP, SinoMed, PubMed, Cochrane Library, and CINAHL. The search period was from the establishment of the database to January 31, 2025. Literature was selected based on inclusion criteria and analyzed using Rodgers' evolutionary concept analysis method.Results:A total of 43 articles were included. The conceptual attributes of fertility preservation encompassed five aspects, including targeting individuals with reproductive needs, prioritizing prevention, covering multi-level medical interventions, relying on assisted reproductive technologies for preservation, and being oriented toward enhancing fertility capacity and quality. The implementation of fertility preservation was influenced by demographic factors, disease factors, medical resources, and cultural factors. The anticipated outcomes included not only delayed childbirth but also improved in patients' mental health, quality of life, and interpersonal relationships. However, this may also raise ethical concerns.Conclusions:Fertility preservation plays a vital role in maintaining birth rates and enhancing population quality. Defining the concept of fertility preservation helps healthcare providers understand its essence and provides a reference for developing scientifically grounded fertility preservation programs.
9.Innovative practice of inter-hospital transfer services for critically ill patients under the guidance of party building

The deep integration of party building and business operations is a crucial pathway to strengthen party building in public hospitals and promote high-quality hospital development under the new situation.This article takes the"Hongyan Van-guard Inter-hospital Transfer Service Task Force for Critically Ill Patients"of The Second Hospital of Chongqing Medical Universi-ty as the research object.Based on cross-branch collaboration practices,it systematically constructs a three-dimensional integra-tion model of"goal synergy-organizational reconstruction-capability closed-loop"and deeply analyzes the internal logic of party building leading business challenges.The research uses party building as the ideological guide and organizational reconstruction as an important measure to establish a standardized transfer process and a digital quality control system.Data shows that more than 150 cases of inter-hospital transfers of critically ill patients have been completed within a year,with a growth rate exceeding 40%.Patient satisfaction reached 98.7%,and the CMI value improved significantly.This provides an empirical model for public hospitals to solve the collaboration problems between cross-functional departments and clinical frontline departments,and achieve the"double helix"development of party building and business operations.
10.Influencing factors of non-alcoholic fatty liver disease in aircrews based on classification tree model

Objective To establish a classification tree model for non-alcoholic fatty liver disease(NAFLD)among aircrews,screen for influencing factors of NAFLD,so as to provide scientific basis for prevention and intervention decisions for NAFLD.Methods Aircrews who underwent recuperation at a sanatorium from January 2019 to December 2023 were selected as the research objects.Their annual physical examination data were collected and the NAFLD detection rate was calculated.Age,body mass index(BMI),blood pressure,waist circumference,blood routine,biochemistry indexes,and thyroid function were incorporated,and a NAFLD risk model was constructed using classification regression tree method.The predictive performance of the NAFLD classification tree model was evaluated through model misclassification matrix,risk statistics,and receiver operating characteristic curve.Results A total of 4088 aircrews were included in the study,and NAFLD was detected in 380 persons(380/4088,9.30%).The NAFLD model consisted of three layers,and five explanatory variables affecting the onset of NAFLD were extracted,including BMI,triglycerides(TG),high-density lipoprotein cholesterol(HDL-C),alanine aminotransferase(ALT),and total bilirubin(TBIL).BMI was located at the top of the classification tree and was the most important risk factor for NAFLD in aircrews.The area under the curve(AUC)of the model was 0.853.The predictive accuracy of NAFLD was 90.9%,indicating that the model has good accuracy and fitting effect.Conclusion In this study,the detection rate of NAFLD in aircrews was 9.30%.BMI,TG,HDL-C,ALT,and TBIL are risk factors for the onset of NAFLD.NAFLD is mainly related to weight gain and lipid metabolism disorders caused by unhealthy lifestyles.
