Background:
Adrenocortical hormones, particularly 11-oxygenated androgens, are pivotal in female reproductive health and fertility. Standardized detection kits and population-specific reference intervals are lacking in China, hindering related clinical applications.
Methods:
A HPLC-tandem mass spectrometry (HPLC-MS/MS) pipeline was developed, rigorously validated, and applied to simultaneously quantify corticosterone, cortisone, cortisol, 18-OH cortisol, androstenedione (A4), 11β-hydroxyandrostenedione (11-OH A4), dehydroepiandrosterone, and dehydroepiandrosterone sulfate in serum samples from 455 reproductive-aged women (18–45 yrs) in Guangxi, China. Age-dependent concentration trends were analyzed, and reference intervals stratified by age (2.5th to 97.5th percentiles) were established. Correlations with body-composition metrics, ethnicity, and the menstrual cycle were investigated.
Results:
The HPLC-MS/MS method demonstrated high precision (intra- and inter-assay CVs < 15%), accuracy, and sensitivity. All eight hormones exhibited significant age-related declines (P < 0.001 for seven hormones; P = 0.001 for 11-OH A4). Notably, 11-OH A4 levels were significantly lower in the 35–45-yr (3.05 nmol/L) and 25–34-yr (3.09 nmol/L) age groups than in the 18–24-yr (3.57 nmol/L) age group, whereas no significant difference was observed between the 35–45-yr and 25–34-yr age groups. Weak negative correlations were observed between the body mass index and corticosterone and cortisone levels, whereas ethnicity and the menstrual cycle showed no significant associations with hormone levels.
Conclusions
We developed an HPLC-MS/MS-based method for simultaneously quantifying eight adrenocortical hormones, including 11-OH A4, and defined age-specific reference intervals for reproductive-aged Chinese women. These findings advance the clinical utility of adrenocortical hormones in diagnosing and managing reproductive disorders.

Objective:To investigate the relationship between oxygen consumption (VO 2), carbon dioxide production (VCO 2), and Oxygen Consumption/lactate (VO 2/Lac) with risk of death in patients with severe ARDS. Methods:A retrospective cohort study method was used, and the study subjects were hospitalized for >5 days adult patients with severe ARDS in the central intensive care unit of Henan Provincial People's Hospital from 1 March 2020 to 30 June 2023. The following patients were excluded: IC test was not completed on the 4th day of ICU admission, IC test results were unreliable, mechanical ventilation duration had exceeded 48 h at the time of ICU transfer or admission, palliative care patients and pregnant and parturient women. Using indirect calorimetry to determine VO 2 and VCO 2 values on the 4th day of admission, reviewing medical records to obtain general condition, disease information, blood gas analysis (including lactate value), diagnostic and therapeutic measures, and following up deaths by telephone and time of death. The primary outcome measure was death at 90 days, and the secondary outcome measure was death at 28 days, length of stay in ICU, total length of stay, and total hospitalization cost. Cox regression analysis and linear regression analysis were used to investigate the relationship between VO 2, VCO 2, VO 2/Lac and primary and secondary outcome indexes. Results:A total of 216 patients were enrolled, 78 patients (36.1%) died and 138 patients (63.9%) survived at 90 days. After correction for confounders, the results of multifactorial Cox regression analysis suggested that compared with the Q4 group, HR (95% CI) for 90-day risk of death in the VO 2 Q1 and Q2 groups was 3.21 (1.38, 7.49) and 3.24 (1.42, 7.38), and HR (95% CI) for 90-day risk of death in the VCO 2 Q1, Q2 and Q3 groups was 5.88 (2.33, 14.84), 4.26 (1. 60, 11.34) and 3.54 (1.34, 9.35), respectively, and the HR (95% CI) for 90-day risk of death in the VO 2/Lac Q1, Q2 and Q3 groups were 8.72 (3.01, 25.25), 8.43 (2.91, 24.47) and 4.04 (1.34, 12.17) respectively. P-trends were all <0.05, indicating that VO 2, VCO 2 and VO 2/Lac were linearly and negatively associated with the risk of 90-day mortality. In addition, VO 2, VCO 2, and VO 2/Lac were negatively associated with 28-day risk of death and higher VO 2/Lac was negatively associated with length of ICU stay. Conclusions:VO 2, VCO 2 and VO 2/Lac were negatively associated with 90-day mortality risk and 28-day mortality risk in patients with severe ARDS and may be independent risk factors predicting mortality risk of such patients.

OBJECTIVE:
To explore a case of abnormal fetal development due to a rare paternal t(10;14)(p11.2;p11) translocation.
METHODS:
A fetus undergoing prenatal diagnosis at Northwest Women's and Children's Hospital on June 21,2024 was selected as the study subject. Clinical data were collected. Amniotic fluid sample of the fetus and peripheral venous blood samples of its parents were collected for chromosomal karyotyping and copy number variation (CNV) analysis. This study was approved by the Ethics Committee of the hospital (Ethics No.: 2024-132).
RESULTS:
Ultrasound scan at 23+4 gestational weeks revealed nasal bone dysplasia. Amniotic fluid analysis revealed that the fetus has a karyotype of 46,X?,der(14)t(10;14)(p11.2;p11)dpat, while its father had a 46,XY,t(10;14)(p11.2;p11) karyotype. No chromosomal abnormality was found in its mother. CNV analysis revealed that the fetus had a 30.46 Mb duplication in the 10p15.3-p11.23 region. Based on the guidelines from the American College of Medical Genetics and Genomics (ACMG), the duplication was classified as pathogenic.
CONCLUSION
By combining conventional cytogenetic methods with molecular techniques, the fetus was diagnosed with partial trisomy 10p syndrome caused by a rare paternal t(10;14)(p11.2;p11) translocation. Above finding holds significant clinical value for genetic counseling and prenatal diagnosis for the family.

Chinese Journal of Otorhinolaryngology Head and Neck Surgery 2025;60(10):1259-1266
Objective:To investigate the clinical features, imaging characteristics, surgical strategies, and therapeutic outcomes of otitis media complicated with inflammatory response of local meninges. Methods:A retrospective analysis was conducted on the clinical data of 8 patients with chronic suppurative otitis media complicated with inflammatory response of local meninges, treated by the Department of Otolaryngology, Head and Neck Surgery, PLA General Hospital from 2019 to 2023. Appropriate surgical strategies were selected based on the patient′s clinical manifestations, imaging characteristics, extent of lesions, and facial nerve function. Follow-up was performed postoperatively to assess the therapeutic outcomes. Results:Among the eight patients, there were six males and two females, with an average age of (55.9±12.6) years old. The primary clinical manifestations included otorrhea, hearing loss, facial paralysis, earache, headache, and fever. All patients had a history of chronic suppurative otitis media and tympanic membrane perforation, with varying degrees and types of hearing loss. Seven patients presented with peripheral facial palsy preoperatively, with the House-Brackmann (H-B) grading scale as follows: 4 cases (4/7) in grade Ⅳ, 1 case (1/7) in grade Ⅴ, and 2 cases (2/7) in grade Ⅵ. The mean duration of otorrhea and/or hearing loss was (24.68±12.18) years, while, the average duration of severe headache, aggravated otorrhea and facial paralysis was (2.73±3.92) months. Preoperative high-resolution CT scan of the temporal bone revealed soft tissue shadow in the middle ear and mastoid process, with partial defects in the mastoid cortex. Cranial MRI T1WI showed high signal in the meninges on the affected side, with contrast-enhanced MRI indicating localized meningeal thickening. Four patients (4/8) had diabetes mellitus, and 2 patients (2/8) had a history of middle ear/mastoid surgery. All patients underwent surgical treatment, including thorough removal of lesions, adequate drainage, and facial nerve decompression. Tympanoplasty and hearing reconstruction were performed when conditions permitted. Specifically, 5 patients underwent intact canal wall radical mastoidectomy with facial nerve decompression and tympanoplasty, 2 patients underwent canal wall down mastoidectomy with facial nerve exploration decompression, and 1 patient underwent modified radical mastoidectomy. Postoperatively, patients experienced significant relief and gradual disappearance of ear and head pain. The postoperative H-B grading scale of facial nerve function was as follows: 4 cases in grade Ⅰ (4/8, including 1 case without preoperatively facial palsy), 2 cases (2/8) in grade Ⅱ, and 2 cases (2/8) in grade Ⅲ. Postoperative cranial MRI showed a significant reduction in localized meningeal thickening on the affected side. Conclusions:Patients with long-term chronic suppurative otitis media and/or cholesteatoma who suddenly presented with headache, fever, aggravated otorrhea, and facial paralysis should be suspected of having inflammatory response of local meninges. High-resolution CT of temporal bone and cranial MRI provide crucial diagnostic information. Early surgical exploration and thorough lesion removal are effective treatment methods.
8.Treating Prediabetes from the Perspective of"Excessive Earth Stagnation and Fivefold Heat Scorching"
Journal of Zhejiang Chinese Medical University 2025;49(6):786-790
[Objective]To investigate the etiology and pathological mechanisms of prediabetes based on the theory of"excessive earth stagnation,"thereby expanding novel therapeutic approaches for prediabetes management.[Methods]Integrating classical Chinese medical literature with contemporary clinical insights, this study elucidates the correlation between Dunfu(thickened stagnation)and excessive earth stagnation and its pivotal role in prediabetes pathogenesis, and proposes the therapeutic strategies.[Results]The Dunfu mechanism documented in The Inner Canon of the Yellow Emperor demonstrates significant correlation with the"spleen-earth stagnation"pathological state in prediabetes, which primarily disrupts the ascending-descending dynamic of Qi in middle-Jiao and contributes to glucose metabolism dysregulation.Dunfu(thickened stagnation)acts as a quintessential manifestation of excessive earth stagnation. When Dunfu develops,damp-heat pathogens accumulate, causing retention of food essence and further aggravating earth stagnation, the retained food and essence transforming into dampness, heat and phlegm. These turbid pathogens obstruct the middle-Jiao,impairing spleen function and Qi-blood flow, eventually generating heat that scorches the Zang-Fu organs. This progression from Pidan to Xiaodan underscores Dunfu as the root pathology.Treatment should prioritize resolving stagnation, clearing heat and eliminating turbidity, with tailored Zang-Fu syndrome differentiation to restore Beihua(preparation and transformation).[Conclusion]"Excessive earth stagnation"is the core pathological mechanisms of prediabetes, leading to retention of food essence and further aggravation earth stagnation, and the treatment should prioritize resolving stagnation.Treating prediabetes based on"excessive earth stagnation"has clinical guidance significance.
