1.Factors affecting implementation of weight management services in primary medical and healthcare institutions based on the consolidated framework for implementation research

Objective:
To explore the influencing factors for implementation of weight management services in primary medical and healthcare institutions, so as to provide references for implementing sustainable services of weight management.
Methods:
From May to June 2025, Pinghu City, Zhejiang Province was selected as the survey site. Personnel responsible for weight management in primary medical and healthcare institutions were selected as the survey subjects using a combined method of purposive sampling and snowball sampling. Based on the five core domains of the consolidated framework for implementation research (CFIR), a semi-structured interview outline for weight management services in primary medical and healthcare institutions was designed. Original data was collected through face-to-face semi-structured interviews. Interview data was organized and analyzed using framework analysis. Factors affecting weight management services were quantitatively analyzed by referencing CFIR's structural rating criteria.
Results:
A total of 21 participants completed interviews, covering positions in nutrition, endocrinology, traditional Chinese medicine, general practice, maternal health, and public health. There were 9 males and 12 females. Fifteen participants (71.43%) were aged 35 years and above, 18 (85.71%) held a bachelor's degree or higher, and 15 (71.43%) were frontline medical staff. Fifteen factors affecting weight management services were identified across five domains: innovation, outer setting, inner setting, individuals, and implementation process. Six barrier factors were identified: difficulties in policy implementation, time-consuming interventions, limited incentive measures, lack of professional skills, unclear weight-loss plans and goal setting, and imperfect follow-up and evaluation mechanisms. Three neutral factors were identified: the development and refinement of policies and regulations, the implementation of weight management training, and the optimization of the referral process within integrated healthcare systems (medical alliances / communities). Six facilitating factors were identified: the relatively significant advantages of lifestyle interventions, collaboration and coordination across multiple departments, cooperative communication among different units within the institution, the inherent convenience of primary care settings, a strong sense of professional responsibility, and the establishment of multidisciplinary teams.
Conclusions
The delivery of weight management services in primary medical and healthcare institutions is influenced by a wide array of factors across multiple domains. It requires policy support, multi-department coordination, a practice-oriented training system, optimized team resource allocation, incentives, and improved professional skills of medical staff to jointly promote long-term implementation.

3.Evaluation of left atrial strain and left atrioventricular global strain in patients with cardiovascular immune-related adverse events related to immune checkpoint inhibitors

Objective:To evaluate the clinical utility of left atrial strain parameters and left atrioventricular global longitudinal strain(LAVGLS)in detecting cardiovascular immune-related adverse events(CV-irAEs)among non-small cell lung cancer patients receiving immune checkpoint inhibitors(ICIs).Methods:A total of 68 patients with non-small cell lung cancer were prospectively enrolled in Tianjin Medical University Cancer Institute and Hospital from October 2023 to October 2024. All patients were treated with ICIs for 6 cycles. Electrocardiogram,cardiac serological markers and echocardiography were examined before medication(T0 stage),4 cycles after medication(T1 stage)and 6 cycles after medication(T2 stage),respectively. According to the guidelines of the American Society of Clinical Oncology,all patients were divided into the CV-irAEs group( n=14)and the No-CV-irAEs group( n=54). AFI software and 4D Auto LAQ software were used to calculate LVGLS,left atrial reservoir longitudinal strain(LASr),LAVGLS and a series of left atrial parameters. Cox proportional hazards regression model was applied to find the risk factors for the occurrence of CV-irAEs. ROC curve was applied to analyze the diagnostic efficiency of these parameters for CV-irAEs. Results:Fourteen patients(20.6%)developed CV-irAEs after T2 stage. After ICIs treatment,LVGLS,LASr and LAVGLS decreased in both groups,LVGLS,LASr and LAVGLS decreased more significantly in the CV-irAEs group than those in the No-CV-irAEs group( P=0.038,0.047,0.005). Left ventricular ejection fraction(LVEF)decreased in the CV-irAEs group at the same time( P=0.003). Cox multivariate analysis showed that ΔLAVGLS(the difference between stage T0 and stage T2)was a risk factor for CV-irAEs( HR:1.395, P=0.019). ROC curve analysis showed the area under the curve of LVGLS,LASr,LAVGLS,ΔLVGLS,ΔLASr,ΔLAVGLS,and LVEF at the T2 stage for diagnosis of CV-irAEs were 0.68,0.67,0.75,0.79,0.73,0.82,and 0.72,respectively. Conclusions:Decline of LAVGLS is a risk factor for CV-irAEs in patients with non-small cell lung cancer receiving ICIs and can be used for early detection of CV-irAEs. LASr has potential diagnostic value for CV-irAEs,but it is less valuable than LVGLS and LAVGLS.

5.Rapid on-site detection of 4 kinds of amphetamines in saliva samples using pulsed direct current electrospray ionization mass spectrometry

Objective To develop a method of pulsed direct current electrospray ionization mass spectrometry(pulsed-dc-ESI-MS)for rapid detection of 4 kinds of amphetamines in saliva.Methods Saliva samples were adjusted to optimal pH using NaOH,followed by protein precipitation with anhydrous magnesium sulfate and liquid-liquid extraction with ethyl acetate.The supernatant was then analyzed via pulsed-dc-ESI-MS.Results The limits of detection were 0.01~0.5 ng/mL and the limits of quantitative were 1.0~5.0 ng/mL.In addition,in the concentration range,4 amphetamines have good linear relationships,the correlation coefficients(R2)were all greater than 0.999,the recoveries of 97.21%~104.70%and the relative standard deviations(RSDs)were less than 10%.Conclusion The method has the advantages of simple operation,fast detection,high sensitivity,selectivity and good stability.It can be used for on-site rapid detection of amphetamines in saliva.

9.Research progress in programmed cell death protein ligand-1 targeting peptide-based radionuclide-labeled molecular probes

Programmed cell death protein ligand-1(PD-L1)is an important immune checkpoint molecule that plays an important role in regulating the body's immune response.Several clinical studies have shown that the expression level of PD-L1 in tumors is closely related to the efficacy of immune checkpoint inhibitors.Due to the spatial and temporal heterogeneity of tumors,immunohistochemical methods commonly used in clinical practice cannot accurately and comprehensively reflect the ex-pression level of PD-L1 in patients.Given that nuclear-medicine molecular imaging technology can noninvasively,real-time,dy-namically and visually monitor the expression level of PD-L1 in vivo at the molecular level,this article mainly focuses on the re-search of peptide-based radiolabeled molecular probes targeting PD-L1,with the aim of providing guidance for the search of no-vel peptide molecular probes for immunoimaging as well as for the screening of immunotherapy-suitable patients and evaluation of therapeutic efficacy,and other clinical applications.
10.Survey of the cognition of interventional radiology and employment intention in the students majoring in imaging technology in medical school

Objective To discuss the cognition of interventional radiology(IR)and employment intention in the students majoring in imaging technology in medical school.Methods From June 7 to July 31 of 2023,a voluntary and anonymous online questionnaire survey concerning the cognition of IR and employment intention was conducted in the students majoring in medical imaging technology in Sichuan University,North Sichuan Medical College,Harbin Medical University,Hebei Medical University,Changzhi Medical College and Sichuan Vocational College of Health and Rehabilitation.The data obtained from the survey were analyzed by descriptive analysis and chi-square test using SPSS 24.0 statistical software.Results A total of 126 valid questionnaires were received.Eighty-seven students(69.05%),94 students(74.61%)and 81(64.28%)students had no knowledge or just had a little bit knowledge about the medical position and current status of IR in the fields of cardiovascular diseases,neurological diseases,and tumor diagnosis and treatment respectively.The IR course was obligatory course for the medical imaging technology specialty in all the schools participating in this survey,and 112 students(91.8%)stated that the school curriculum is the main way to acquire knowledge of IR,and the internet platforms,television science popularization and academic conferences were the other ways to acquire knowledge of IR.Four students(3.17%),17 students(13.49%),57 students(45.24%),39 students(30.95%)and 9 students(7.14%)participating in this survey expressed that they had no intention,no willingness,neutral attitude,high willingness and high intention respectively to engage in IR work in future.The difference in the degree of willingness to engage in IR specialty in future between students of different gender as well as between students with different schooling level was not statistically significant,and the exposure to radiation,work-related high stress,and long working time were the main influencing factors for students to consider IR as their professional career.Conclusion At present,both the cognition of IR and the employment intention are not high in the students majoring in imaging technology in medical school.Therefore,it is very important to improve their cognition of IR and willingness to engage in IR profession.
