Metabolic associated fatty liver disease (MAFLD) is fundamentally driven by an imbalance in hepatic fatty-acid flux: the influx of fatty acids exceeds the liver’s capacity for disposal, resulting in excessive hepatic lipid accumulation, predominantly in the form of triglycerides (TGs). The occurrence and progression of MAFLD depend on disordered regulation across multiple metabolic steps, including fatty-acid uptake, de novo lipogenesis (DNL), fatty-acid oxidation (FAO), and very low-density lipoprotein (VLDL) export. Forkhead box protein O1 (FOXO1) is a key transcriptional regulator within the hepatic network coordinating glucose and lipid metabolism. Under metabolic stress and insulin resistance (IR), FOXO1 expression is frequently increased, whereas its inhibitory phosphorylation is reduced. These changes enhance FOXO1 nuclear localization and transcriptional activity, thereby reprogramming the expression of genes related to metabolism in the liver. Because hepatic lipid deposition is the central pathological feature of MAFLD, the functional status of FOXO1 directly influences hepatic lipid homeostasis. Growing evidence suggests that FOXO1 can exert bidirectional, environment-dependent effects on hepatic lipid accumulation; however, the molecular basis for this functional switch remains incompletely understood. This review systematically summarizes the biological functions and regulatory mechanisms of FOXO1 and its roles in hepatic lipid metabolism, with a particular focus on its crosstalk with insulin signaling. FOXO1 expression is shaped by RNA modifications and epigenetic regulation mediated by non-coding RNAs. Its transcriptional output is precisely governed by post-translational modifications—such as phosphorylation and acetylation—as well as by coordinated nucleocytoplasmic shuttling. Notably, these regulatory patterns vary markedly across nutritional states, degrees of insulin resistance, and stages of disease. In the fed state, insulin/IGF-1 signaling activates the PI3K-AKT pathway, promoting the inhibitory phosphorylation of FOXO1 and facilitating additional modifications, including acetylation, methylation, and ubiquitination. Together, these events drive FOXO1 export from the nucleus and dampen its transcriptional activity, suppressing gluconeogenesis and constraining lipogenic programs. Conversely, during fasting or when insulin signaling is weakened, FOXO1 inhibition is relieved. FOXO1 accumulates in the nucleus, binds to DNA, and regulates the transcription of downstream target genes. Mechanistically, FOXO1 can aggravate hepatic lipid accumulation by activating genes involved in TG synthesis while repressing FAO-related pathways, thereby favoring storage over oxidation. However, under specific conditions, FOXO1 may also alleviate the hepatic lipid burden by promoting TG hydrolysis and enhancing VLDL secretion, thereby reducing the net hepatic lipid load. In addition, lipotoxic signals mediated by ceramides and diacylglycerols (Cer/DAG) activate atypical protein kinase C (aPKC), further exacerbating the disruption of the AKT-FOXO1 axis. This vicious cycle ultimately produces a metabolic paradox in which increased hepatic glucose output coexists with persistent, insulin-independent lipogenesis, accelerating MAFLD progression. Importantly, FOXO1 regulation is not uniform: during early metabolic overload, insulin-mediated suppression may remain effective, whereas in advanced insulin resistance, the loss of AKT control permits sustained FOXO1 activity. Such stage-dependent dynamics may help explain why FOXO1 can either promote steatosis or, in certain contexts, support programs that facilitate lipid turnover. Accordingly, interventions should be liver-specific and tuned to the disease stage, aiming to curb maladaptive FOXO1 signaling while preserving its capacity to promote triglyceride hydrolysis and VLDL secretion when advantageous. Overall, this review offers an important perspective on MAFLD pathogenesis, emphasizing FOXO1 as a potential therapeutic target and providing a theoretical basis for developing liver-specific, disease-course-dependent precision interventions.

Background:
Surgeons face challenges in selecting cost-effective and biologically active agents for rotator cuff healing, given the numerous commercial products available, such as polydeoxyribonucleotide (PDRN) and atelocollagen (ATC). However, the precise efficacy of PDRN and ATC in rotator cuff healing remains debatable, and there is currently a lack of studies directly comparing the effects of the 2 agents on repaired cuff tendons. Therefore, the purpose of this study was to compare the efficacy of PDRN and ATC on bone-to-tendon interface (BTI) healing using a chronic rotator cuff tear (RCT) model in rabbits.
Methods:
Forty-eight rabbits were randomly divided into 3 groups. To create chronic RCT models, transected tendons were left untreated for 6 weeks, and then were repaired in a transosseous manner with PDRN and ATC injection into the repair site according to group allocation (group A: saline, group B: PDRN, group C: ATC; n = 16 per group). Genetic and immunofluorescence analyses were performed at 4 weeks after surgery. Furthermore, genetic, histologic, and biomechanical analyses were performed at 12 weeks after surgery.
Results:
At 4 weeks after surgery, ATC-injected shoulders showed the highest mRNA expression levels of collagen type I alpha 1 and aggrecan compared to the other 2 groups (p < 0.001 and p = 0.002, respectively). Meanwhile, there was more preliminary fibrocartilaginous matrix formation in the ATC-injected group. At 12 weeks after surgery, ATC-injected shoulders demonstrated better collagen fiber continuity and orientation, denser collagen fibers, a more mature bone-to-tendon junction, and greater fibrocartilage layer formation compared to the other 2 groups (all p < 0.001). Furthermore, ATC-injected shoulders also demonstrated a significantly higher load-to-failure value (40.4 ± 4.5 N/kg) than the remaining groups (group A, 26.7 ± 3.0 N/kg; group B, 32.8 ± 4.2 N/kg; p < 0.001).
Conclusions
ATC demonstrated superior efficacy in promoting BTI healing following surgical repair in a chronic RCT model of rabbits.

ObjectiveTo compare the effects of the Rho-associated protein kinase (ROCK) inhibitors Fasudil and Y27632 on promoting angiogenesis in vivo and in vitro and on flap survival in mice. Methods Thirty-six SPF-grade adult male ICR mice were randomly divided into the Y27632 group, Fasudil group, and control group, with 12 mice in each group. From 3 d before surgery, the mice were intraperitoneally injected daily with Y27632 solution, Fasudil solution, and an equal volume of normal saline, respectively. Then, a peninsular flap model pedicled on the iliolumbar vessels on the back of the mice was established surgically. Laser speckle contrast imaging was used to evaluate flap blood perfusion immediately after surgery (day 0) and on day 5, and the pedicle artery diameter was measured and proximal and distal blood flow were calculated. On day 5 after surgery, the flap necrosis rate was calculated, and CD31-positive microvessel density in the distal part of the flap was detected by immunofluorescence. In addition, EdU cell proliferation, scratch, Transwell, and Matrigel tube formation assays were used to compare the effects of Fasudil and Y27632 on the proliferation, migration, invasion, and tube formation ability of human umbilical vein endothelial cells (HUVECs) in vitro, with DMSO treatment as the control. ResultsOn day 5 after surgery, the flap necrosis rate in the Fasudil group was (2.0±1.5)%, which was significantly lower than that in the control group and the Y27632 group (P<0.001), and the necrosis rate in the Y27632 group was also significantly lower than that in the control group (P<0.001). The pedicle iliolumbar artery diameter in the Fasudil group on day 0 and day 5 after surgery was (143.8±4.2) μm and (177.6±7.6) μm, respectively, both significantly larger than those in the control group and the Y27632 group (P<0.001). On day 5 after surgery, the proximal and distal blood flow in the Fasudil group were (453.0±12.5) PU and (79.2±4.8) PU, respectively, both significantly higher than those in the control group(P<0.001) and the Y27632 group (P<0.05). The microvessel density in the distal surviving tissue of the flap was (71.6±3.5) vessels/region, significantly higher than that in the control group (P<0.001) and also significantly higher than that in the Y27632 group (P<0.05). On day 0 after surgery, the pedicle iliolumbar artery diameter in the Y27632 group was smaller than that in the control group (P<0.05), while on day 5 after surgery, there was no statistically significant difference between the two groups (P>0.05). The proximal and distal blood flow on day 5 after surgery were also significantly higher than that in the control group (P<0.001), and the microvessel density was also significantly higher than that in the control group (P<0.05), but the distal blood flow showed no significant difference compared with the control group (P>0.05). In vitro experiments showed that the proliferation rate, migration rate, and number of invasive cells of HUVECs in the Fasudil and Y27632 groups were significantly increased compared with the control group (P<0.001), but there was no statistically significant difference between the two drug groups (P>0.05), whereas the tube formation ability of the Fasudil group was significantly better than that of the Y27632 group (P<0.05). ConclusionBoth Fasudil and Y27632 can effectively promote angiogenesis and improve flap survival rate, but the effect of Fasudil is more significant, which may be related to its sustained regulation of hemodynamics and vascular stability.

Objective :
To investigate the diagnostic value of plasma calprotectin in childern with refractory mycoplasma pneumoniae pneumonia(RMPP).
Methods :
A multicenter, prospective cohort study was conducted, enrolling 228 children with mycoplasma pneumoniae pneumonia(MPP). Among these, 177 cases were diagnosed with general mycoplasma pneumoniae pneumonia(GMPP), while the remaining 51 cases were RMPP. Plasma was collected at the time of admission of the children in both groups, and calprotectin levels were measured. A one-way difference analysis was performed on the blood test indexes of the children in the two groups, and the difference variables withP<0.05 between the two groups were included in a multifactorial logistic regression to analyze the risk factors for the progression of GMPP to RMPP. The differential diagnostic value of plasma calprotectin for GMPP and RMPP was analyzed by the receiver operating characteristic(ROC) curves.
Results :
Univariate analysis showed that plasma calprotectin levels were significantly higher in the RMPP group than those in the GMPP group, and the difference was statistically significant(P<0.05). Logistic regression analysis showed that plasma calprotectin was an independent risk factor for RMPP(OR=1.323,P<0.001), ROC curve analysis showed that plasma calprotectin had a higher diagnostic value for the differential diagnosis of GMPP and RMPP(AUC =0.839), and its combination with C-reactive protein and albumin could significantly improve the diagnostic efficiency.
Conclusion
Plasma calprotectin has good clinical value for the diagnosis of RMPP.

Dopamine(DA)is an essential neurotransmitter.Research on its dynamic concentration fluctuations, particularly within the extracellular microenvironment,is crucial for elucidating neural signaling mechanisms and pathogenesis of related disorders.Among DA detection methods, fluorescence probe-based imaging offers advantages such as high sensitivity, high specificity, and non-invasiveness.However,the application of these probes for in situ DA monitoring has been limited by the challenges including probe internalization and difficulties in labeling across cells. Here, leveraging the programmability and high stability of DNA nanostructures,a DNA prism-based probe for imaging DA release at the single-cell level was designed and constructed. This probe utilized surface-modified cholesterol for efficient membrane anchoring and a DA aptamer-based"turn-on"sensing module to detect DA directly on the cell membrane.Using this probe, rapid DA release triggered by high K+stimulation was observed, with the released DA concentration increasing over time and peaking at 8 min post-stimulation.More notably, exploiting the probe's ability to simultaneously anchor to two cells, thereby forming cell clusters, revealed that the distribution of DA within the intercellular space was significantly higher than that in the cell body regions. This probe not only provided a method for DA imaging on the cell membrane but also laid a theoretical foundation for developing broader neurotransmitter detection platforms, holding significant scientific merit and application potential.

This article systematically analyzes the historical evolution of the name, origin, quality evaluation, harvesting, processing and other aspects of Picrorhizae Rhizoma by referring to the medical books, prescription books, and other documents of the past dynasties, combined with relevant modern research materials, in order to provide a basis for the development and utilization of famous classical formulas containing this medicinal herb. The research results indicate that Picrorhizae Rhizoma was first recorded in New Revised Materia Medica from the Tang dynasty. Throughout history, Huhuanglian has been used as its official name, and there are also aliases such as Gehu Luze, Jiahuanglian and Hulian. The main source of past dynasties is the the rhizomes of Picrorhiza kurrooa and P. scrophulariiflora. In ancient times, Picrorhizae Rhizoma was mainly imported by foreign traders via Guangzhou and other regions, and also produced in China, mainly in Xizang. In ancient times, it was harvested and dried in early August of the lunar calendar, while in modern times, it is mostly harvested from July to September, with the best quality being those with thick and crispy rhizomes without impurities, and bitter taste. Throughout history, Picrorhizae Rhizoma was collected, washed, sliced, and dried before being used as a raw material for medicine, it has a bitter and cold taste, mainly used to treat bone steaming, hot flashes, infantile chancre fever, and dysentery. There is no significant difference in taste and efficacy between ancient and modern times. Based on the research results, it is recommended that the rhizomes of P. scrophulariiflora in the 2020 edition of Chinese Pharmacopoeia, or the rhizomes of P. kurrooa, can be used in famous classical formulas containing this medicinal herb, which can be processed according to the processing requirements marked by the original formula. For those without clear processing requirements, the dried raw products are used as medicine.
