1.Effects of a high soy protein diet on azoxymethane (AOM)/dextran sulfate sodium (DSS)-induced colorectal cancer development in mice

Purpose:
The incidence of colorectal cancer (CRC) continues to rise, and the dietary protein intake has been suggested as a potential risk factor. Although the carcinogenic potential of animal-derived proteins has been well documented, the role of soy protein in colorectal carcinogenesis is controversial. This study examined the effects of a high soy protein diet on colitis-associated CRC in mice.
Methods:
Six-week-old BALB/c mice were assigned to 3 groups: 20% soy protein (20S, control), 20% soy protein with carcinogenesis (20SC), and 40% soy protein with carcinogenesis (40SC). Colorectal carcinogenesis was induced with azoxymethane and dextran sulfate sodium, and mice were fed experimental diets for 10 weeks. Disease activity, survival, colon pathology, tumor multiplicity, and inflammatory markers were assessed.
Results:
The 40SC group tended to have higher disease activity index scores than the 20S and 20SC groups (significant in parametric analysis) and exhibited reduced survival (log-rank p = 0.004). Cox regression analysis showed that the dietary protein level was independently associated with survival (hazard ratio, 0.232, p = 0.010). The colon weight-to-length ratio and mucosal thickness were significantly higher in the 40SC group than in the 20S and 20SC groups, but, the fecal lipocalin-2, ammonia, and plasma tumor necrosis factor-α and nitric oxide levels were not significantly different between 40SC and 20SC groups. Although the overall effect of diet on tumor multiplicity was not significant in the negative binomial model, a direct comparison between the 20SC (41.3 ± 4.1) and 40SC (58.7 ± 5.8) groups showed a significant difference in tumor counts.
Conclusion
High dietary soy protein intake was associated with aggravated colitis severity and reduced survival in this experimental model, and showed a potential link with increased tumor development under carcinogenic conditions. These results suggest that excessive soy protein intake may modulate inflammation-associated colorectal carcinogenesis, but additional research is needed to determine its significance in human populations.
2.Reduced Dose and Selective Neck Irradiation Policy in Treating Oropharynx Cancer: Excellent Oncological Outcomes with Very Low Risk of Locoregional Failure

Purpose:
This study aimed to evaluate the risk of locoregional failure after reduced dose selective neck irradiation (RD-SNI) in patients with oropharyngeal cancer (OPC).
Materials and Methods:
Between 2008 and 2022, 342 OPC patients underwent definitive radiation therapy (with or without concurrent systemic therapy). The doses of 67.2-68.4 Gy to gross tumor volume (GTV), 56-60 Gy to high-risk clinical target volumes (CTV-HR), and 32-36 Gy to low-risk clinical target volumes (CTV-LR) were irradiated. The same target delineation and dosing policy were applied to all patients regardless of human papillomavirus (HPV) status. Oncological outcomes including failure patterns were also investigated.
Results:
With a median follow-up of 60.3 months (range, 1.4 to 196.6 months), the 3- and 5-year locoregional control, distant metastasis-free survival, disease-free survival, and overall survival rates were 91.6%/90.7%, 83.7%/80.7%, 78.7%/74.8%, and 91.0%/85.8%, respectively. The HPV-positive patients exhibited significantly better outcomes. Treatment failure occurred in 61 patients (17.8%); 37 (10.8%) had distant metastasis, 22 (6.4%) had local failure, and eight (2.3%) had regional failure. GTV failure was significantly more common in HPV-negative patients (p=0.003). Among the 27 patients with locoregional failure, either GTV and/or CTV-HR failure occurred in 22 (81.5%), with CTV-LR failure in one (3.7%), and out-target regional (OTR) failure in five (18.5%). Only five failures (1.5%) could be attributed to the current RD-SNI policy: one CTV-LR failure (0.3%) reflecting the RD policy and four OTR failures (1.2%) reflecting the SNI policy.
Conclusion
Excellent oncological outcomes were achieved with the current RD-SNI policy.
3.Antidiabetic effects of water extracts of mulberry (Morus alba L.) twig by inhibition of disaccharidase activity in streptozotocin-induced diabetic mice

Purpose:
The prevalence of diabetes mellitus (DM) continues to increase worldwide, and blood glucose control may reduce mortality from diabetic complications and healthcare costs. Mulberry twig (MT) has been used as a herbal medicine in Asia, and its antidiabetic efficacy has recently been reported, but research in this area is still limited. This study examined the antidiabetic effects of water extracts of MT in diabetic animals.
Methods:
Six weeks old male ICR mice were divided randomly into three groups; normal control (NC, n = 10), DM control (DC, n = 10), and MT (n = 10). Streptozotocin (STZ, 50 mg/ kg/day) was injected intraperitoneally into mice in the DC and MT groups for 5 consecutive days. After 10 days of the last STZ injection, the mice in the MT group were administered orally with MT water extracts (5 g/kg body weight) for 16 days.
Results:
The MT water extracts ameliorated the swelling of the liver in the diabetic mice and reduced the elevated levels of fasting blood and plasma glucose, total cholesterol (T-CHO), low density lipoprotein-CHO, and the ratio of high density lipotrotein (HDL)-CHO/T-CHO. The liver triglyceride (TG) and glycogen contents were also significantly lower in the MT group mice than in the DC group. The small intestine disaccharidase activity was analyzed to understand the therapeutic effects and the mechanism of MT water extracts in diabetic animals. MT group mice showed reduced lactase and sucrase activity in the proximal part of the small intestine. On the other hand, body weight, plasma insulin, TG, HDL-CHO, and hepatic T-CHO levels were similar in the DC and MT groups.
Conclusion
These results suggest that MT water extracts have antidiabetic effects and can be developed as a functional source to reduce the postprandial blood glucose or to prevent DM incidence.
4.Differential effects of various dietary proteins on dextran sulfate sodiuminduced colitis in mice

BACKGROUND/OBJECTIVES:
Chronic colitis is a risk factor for colorectal cancer (CRC) development in both animals and humans. Previously, we reported that a diet rich in protein (with casein as the protein source) significantly increased the risk of mouse CRC development in a dose-dependent manner. In this study, we investigated the effects of different protein sources on the risk of colitis development.MATERIALS/METHODS: Balb/c mice were divided into 7 experimental groups: 20% casein (20C), 20C-dextran sulfate sodium (DSS), 40% casein-DSS (40CD), 40% whey protein-DSS (40WD), 40% soy protein-DSS (40SD), 40% white meat-DSS (40WMD), and 40% red meatDSS (40RMD). Mice were fed an experimental diet for 4 wk and received 3% DSS in their drinking water for 6 days during the 4th wk of the experimental period.
RESULTS:
Compared to other groups, the 40CD group showed the most aggravated colitis with increased disease activity and inflammatory markers. In the 40RMD group, interleukin (IL)-6 levels were the highest among all the groups. The 40SD group showed conflicting effects, for example, elevated mortality and disease activity but decreased nitric oxide (NO) levels. The 40WD group showed attenuated colitis with increased IL-10 levels and decreased NO levels. The 40WMD group showed conflicting effects, including decreased NO levels and elevated fecal lipocalin-2 and IL-6 levels.
CONCLUSIONS
These results suggest that, at levels of 40% in the diet, casein and red meat exacerbate colitis, whereas whey protein mitigates it the most effectively.

To determine the role of diabetes mellitus (DM) in the coronavirus disease 2019 (COVID-19), we explored the clinical characteristics of patients with DM and compared risk factors such as age, glycemic control, and medications to those without DM. This was a retrospective cohort study of 117 confirmed patients with COVID-19 which conducted at a tertiary hospital in Daegu, South Korea. The primary outcome was defined as the severe and critical outcome (SCO), of which the composite outcomes of acute respiratory distress syndrome, septic shock, intensive care unit care, and 28-day mortality. We analyzed what clinical features and glycemic control-related factors affect the prognosis of COVID-19 in the DM group. After exclusion, 110 participants were finally included. DM patients ( The COVID-19 patients with DM had higher severity and resulted in SCO. Intensive and aggressive monitoring of COVID-19 clinical outcomes in DM group, especially in elderly patients is warranted.
6.Increase in dietary protein content exacerbates colonic inflammation and tumorigenesis in azoxymethane-induced mouse colon carcinogenesis.

BACKGROUND/OBJECTIVE: The incidence of colorectal cancer (CRC) has been attributed to higher intake of fat and protein. However, reports on the relationship between protein intake and CRC are inconsistent, possibly due to the complexity of diet composition. In this study, we addressed a question whether alteration of protein intake is independently associated with colonic inflammation and colon carcinogenesis. MATERIALS/METHODS: Balb/c mice were randomly divided into 4 experimental groups: 20% protein (control, 20P, 20% casein/kg diet), 10% protein (10P, 10% casein/kg diet), 30% protein (30P, 30% casein/kg diet), and 50% protein (50P, 50% casein/kg diet) diet groups and were subjected to azoxymethane-dextran sodium sulfate induced colon carcinogenesis. RESULTS: As the protein content of the diet increased, clinical signs of colitis including loss of body weight, rectal bleeding, change in stool consistency, and shortening of the colon were worsened. This was associated with a significant decrease in the survival rate of the mice, an increase in proinflammatory protein expression in the colon, and an increase in mucosal cell proliferation. Further, colon tumor multiplicity was dramatically increased in the 30P (318%) and 50P (438%) groups compared with the control (20P) group. CONCLUSIONS: These results suggest that a high protein diet stimulates colon tumor formation by increasing colonic inflammation and proliferation.
