Purpose: Early childhood dietary behaviors vary according to environmental and regional context, and evidence in rural preschool populations remains limited. Considering their persistence into adulthood and their association with health outcomes, understanding the dietary patterns among rural preschool children is important. This study examined the nutritional quotient scores and dietary behaviors according to the weight status among preschool-aged children in rural Korea. Methods: This cross-sectional study analyzed the secondary data from 233 preschool children aged 3–6 years, living in Changnyeong-gun, South Korea. The dietary behaviors were assessed using the Nutrition Quotient for Preschoolers (NQ-P), which comprises balance, moderation, and environmental domains. The anthropometric and body composition measurements were obtained using bioelectrical impedance analysis, and the weight status was defined using the age- and sex-specific body mass index percentiles. Results: The mean total NQ-P scores were comparable between non-obese children and obese children (56.0 and 56.3, respectively), with no significant differences observed in the total or domain-specific scores. The most common NQ-P grade was low among the non-obese children and medium-to-low among children with obesity, with no significant between-group differences in grade distribution. On the other hand, underweight and normal-weight children exhibited lower fruit and legume intake and were more likely to move around during meals. Conclusion The fruit and legume intake patterns and mealtime behaviors differed according to the weight status. These findings highlight the importance of nutrition and health promotion strategies tailored to the weight status and rural environment of preschool children.Nevertheless, further prospective studies will be needed to clarify the longitudinal relationships between regional factors, dietary behaviors, and growth patterns in early childhood.

Objectives: This study examined dental care utilization among older adults who use long-term care (LTC) insurance and identified sociodemographic and LTC-related factors associated with dental care utilization. Methods: This cross-sectional study analyzed the 2019 data from the National Health Insurance Service-National Elderly Cohort Database. We examined the data of 50,606 beneficiaries aged ≥60 years. Stepwise multiple logistic regression was performed to identify the impact of sociodemographic and LTC-specific factors on dental service utilization. Results: The dental care utilization rate was 29.4%. Higher utilization was significantly associated with male sex, younger age, urban residence, higher income, and disability. Notably, LTC characteristics were strong predictors of dental service utilization: home-based care users were 2.26 times more likely to use dental services than facility-based users (OR=2.26, 95% CI: 2.11-2.43).Utilization significantly decreased with higher LTC severity (grade 1; OR=0.22). Dental utilization was higher when a spouse was the primary caregiver but lower when care was provided by paid caregivers. Conclusions Dental care utilization among LTC recipients is constrained by structural factors, including benefit types and caregiving environments. Policy efforts should focus on institutionalizing home-visit dental services and enhancing caregivers’ oral health competencies within the LTC system.

Objectives: Clinical nutrition treatment is the central part of diabetes management, such as prevention, treatment, and self-management of diabetes, and personalized clinical nutrition treatment, which enables improvement in patients with type 2 diabetes mellitus (T2DM). Our study aimed to contribute to the improvement of appropriate nutrition management in personalized treatment for obese and non-obese diabetes patients. Methods: T2DM patients were recruited as participants, and 36 final participants were assigned to the lean diabetes mellitus group (LDM; body mass index [BMI] < 25 kg/m2) and the obese diabetes mellitus group (ODM; BMI ≥ 25 kg/m2). We assessed the dietary intakes, body composition, dietary habits, the Korean version of obesity-related quality of life, and biochemical indices. Results: According to the phenotype’s comparison, the ODM group had a high prevalence of T2DM complications and hypertension, had a dietary habit of less than 10 minutes of mealtime duration and preferred fast food intake, and had a low obesity-related quality of life. However, the LDM group had a high choice of Korean dishes at the time of eating out and a high intake of vitamin C, and iodine because of the intake of vegetables and seaweeds. Conclusion We observed differences in diet, nutrient intake, and clinical characteristics according to the phenotype of T2DM patients. In particular, obese diabetes patients have an increased risk of cardiovascular diseases, bad dietary habits, and low obesity-related quality of life. Therefore, personalized nutrition treatment is needed in consideration of the risk of cardiovascular disease and dietary habits for patients in the ODM group, as well as determining the energy requirements of Korean patients with T2DM.

Objectives: Korea is experiencing rapid population aging, with older adults forming a large proportion of rural communities. Aging leads to physiological and functional declines, resulting in lower physical activity, poor diet quality, and higher risk of chronic diseases. Although the Nutrition Quotient for the Elderly (NQ-E) is a validated tool to assess dietary quality, few studies have applied it to rural populations. This study aimed to compare nutrient intake and NQ-E scores by age and sex and examine their associations with lifestyle factors. Methods: This study investigated the relationship between nutrient intake and NQ-E scores among older adults in rural Korean community, considering age, sex, and lifestyle factors. A cross-sectional study was conducted with 79 community-dwelling older adults (24 male and 55 female; mean age: 76.3 years) residing in Geochang-gun, Gyeongsangnam-do, Korea. Participants were recruited from community centers and health posts between June 2024 and December 2024. Data collection included general characteristics, 24-hour dietary recalls, and NQ-E questionnaires. Results: Female aged > 75 years had significantly lower intakes of energy, protein, fat, vitamin E, riboflavin, folate, and zinc than their male counterparts (P < 0.05). The mean NQ-E score was 55.01, which was lower than the national average reported for urban older adults (57.6). Participants with higher NQ-E grades had significantly higher intakes of dietary fiber, vitamin A, thiamine, riboflavin, niacin, potassium, and magnesium, and regular physical activity and dietary supplement use were positively associated with higher NQ-E grades (P < 0.01). Conclusion These findings suggest that older female in rural communities are particularly vulnerable to inadequate nutrient intake and lower dietary quality, and that the NQ-E is a useful screening tool for identifying nutritional risk in this population. Community-based nutrition interventions promoting physical activity, supplement use, and dietary diversity are warranted to improve dietary quality and support healthy aging.

Background/Aims: Metabolic dysfunction-associated steatotic liver disease (MASLD) is a significant risk factor for cardiovascular disease. This study aimed to evaluate differences in carotid atherosclerosis and coronary artery disease severity according to MASLD subtypes using general health screening data from multiple tertiary hospitals. Methods: A total of 3,466 subjects who underwent health check-ups including abdominal ultrasound, carotid ultrasound, and cardiac computed tomography were analyzed. Participants were categorized into non-SLD, MASLD, metabolic dysfunction-associated alcohol-relat-ed liver disease (MetALD), and alcohol-related liver disease. Carotid plaque prevalence and coronary artery stenosis severity were compared across groups and stratified by fibrosis stage using the Fibrosis-4 index (FIB-4). Results: Patients with MASLD and MetALD showed a significantly higher prevalence of carotid plaques compared with the non-SLD group. The severity of coronary artery stenosis was also greater in MASLD and MetALD, particularly among those with advanced fibrosis (FIB-4 ≥2.67). Conclusions MASLD and MetALD are associated with increased burden of systemic atherosclerosis and coronary artery disease, especially in patients with advanced fibrosis. These findings highlight the importance of cardiovascular risk assessment in individuals with MASLD and its subtypes.

Background/Aims: Quick sequential organ failure assessment (qSOFA) is believed to identify patients at risk of poor outcomes in those with suspected infection. We aimed to evaluate the ability of modified qSOFA (m-qSOFA) to identify high-risk patients among those with acutely deteriorated chronic liver disease (CLD), especially those with acute-onchronic liver failure (ACLF). Methods: We used data from both the Korean Acute-on-Chronic Liver Failure (KACLiF) and the Asian Pacific Association for the Study of the Liver ACLF Research Consortium (AARC) cohorts. qSOFA was modified by replacing the Glasgow Coma Scale with hepatic encephalopathy, and an m-qSOFA ≥2 was considered high. Results: Patients with high m-qSOFA had a significantly lower 1-month transplant-free survival (TFS) in both cohorts and higher organ failure development in KACLiF than those with low m-qSOFA (Ps<0.05). Subgroup analysis by ACLF showed that patients with high m-qSOFA had lower TFS than those with low m-qSOFA. m-qSOFA was an independent prognostic factor (hazard ratios, HR=2.604, 95% confidence interval, CI 1.353–5.013, P=0.004 in KACLiF and HR=1.904, 95% CI 1.484– 2.442, P<0.001 in AARC). The patients with low m-qSOFA at baseline but high m-qSOFA on day 7 had a significantly lower 1-month TFS than those with high m-qSOFA at baseline but low m-qSOFA on day 7 (52.6% vs. 89.4%, P<0.001 in KACLiF and 26.9% vs. 61.5%, P<0.001 in AARC). Conclusions Baseline and dynamic changes in m-qSOFA may identify patients with a high risk of developing organ failure and short-term mortality among CLD patients with acute deterioration.

Background/Aims: Besifovir dipivoxil maleate (BSV) and tenofovir alafenamide fumarate (TAF) have been recently approved in Korea as the initial antiviral agents for chronic hepatitis B (CHB).However, the real-world outcome data for these drugs remain limited. Therefore, we conducted a noninferiority analysis using real-world data to compare the clinical outcomes of the two nucleotide analogs in treatment-naïve patients with CHB. Methods: We retrospectively investigated a cohort of patients with CHB who received BSV or TAF as first-line antiviral agents. The endpoints were virological response (VR) and liver-related clinical outcomes. Results: A total of 537 patients, consisting of 202 and 335 patients administered BSV and TAF, respectively, were followed up for 42 months. No significant difference was observed between the VRs of the patients from the two groups. The rates of biochemical response, virologic breakthrough, and incidence rates of hepatocellular carcinoma did not differ between the groups. However, the hepatitis B e antigen seroclearance rate was higher and the renal function declined less in the BSV group. Multivariable analysis indicated older age, alcohol abuse, cirrhosis and ascites, and lower serum HBV DNA level to be independently associated with increased hepatocellular carcinoma risk. The 1:1 propensity score-matched analysis with 400 patients showed VR rates of 85.0% and 88.7% in the BSV and TAF group patients, respectively, at 2 years. The absolute value of the 95% confidence interval for the difference (–0.04 to 0.12) satisfied the a priori limit of a noninferiority of 0.15. Conclusions BSV is noninferior to TAF in terms of VR, and their clinical outcomes are comparable to CHB.

Background: Developmental trajectories of clinical skills in training physicians vary among tasks and show interindividual differences. This study examined the predictors of medical internship performance and residency entrance and found subtypes of performance trajectory in training physicians. Methods: This retrospective cohort study involved 888 training physicians who completed a medical internship between 2015 and 2019. After the internship, 627 physicians applied for residency training between 2016 and 2020. Finally, 160 of them completed their first-year residency in internal medicine, surgery, pediatrics, and psychiatry departments between 2016 and 2020. Pearson’s correlation coefficients of internship performance and first year-residency performance (n = 160) were calculated. Latent profile analysis identified performance trajectory subtypes according to medical school grade point average (GPA), internship performance, English proficiency, and residency selection procedures. Multivariate logistic regression models of residency acceptance (n = 627) and performance in the top 30%/lower 10% in the first year of residency were also constructed. Results: Medical internship performance showed a significant positive correlation with the medical school GPA (r = 0.194) and the written score for the medical licensing examination (r = 0.125). Higher scores in the interview (adjusted odds ratio [aOR], 2.57) and written examination (aOR, 1.45) of residency selection procedures and higher medical internship performance (aOR, 1.19) were associated with a higher chance of residency acceptance. The latent profile analyses identified three training physician subgroups: average performance, consistently high performance (top 30%), and adaptation to changes (lowest 10%). Higher scores in the interview for residency selection (aOR, 1.35) and lower scores for medical internship performance (aOR, 0.79) were associated with a higher chance of performing in the top 30% or lowest 10% in the first year of residency, respectively. Conclusion Performance in the interview and medical internship predicted being among the top 30% and lowest 10% of performers in the first year of residency training, respectively.Individualized educational programs to enhance the prospect of trainees becoming highfunctioning physicians are needed.

Purpose: Sodium is essentially required for homeostasis and physiological functions, but excessive sodium consumption increases the risk of obesity and other chronic disorders.Korean studies on the sodium-obesity relationship are limited, and thus, this study was undertaken to determine the nature of the relationship between sodium intake and obesity in Korean adults. Methods: Forty-two participants were divided into 2 groups according to body mass index (BMI, non-obese BMI < 25 kg/m2 , obese BMI ≥ 25 kg/m2 ). Dietary intakes and eating habits were analyzed using 3-day food records and a food frequency questionnaire. Anthropometric data were obtained from bioimpedance results, and fasting glucose and lipid levels were measured. Results: Mean weight, BMI, waist and hip circumferences, and body fat mass were greater in the obese group than in the non-obese group for men and women. Skeletal muscle mass and body fat mass were higher in obese women than in non-obese women. Biochemical data were no different in these two subgroups except triglycerides (TGs), which were higher in obese women. Nutrient intakes were not significantly different in obese and non-obese groups.However, obese men consumed excessive sodium, while obese women consumed slightly more than non-obese women. Obese men preferred salty foods and tended to overeat.Positive correlations were found between sodium intake and weight in men and percent body fat mass (PBFM) in women. Correlation analysis (adjusted for energy intake) of the relation between sodium intake and obesity-related factors showed sodium intake was positively correlated with PBFM and TG in women. Conclusion This anthropometric and biochemical data analysis emphasizes the need for awareness and interventions to mitigate the health risks of elevated sodium consumption.Our findings should aid future studies on the relationship between sodium and obesity and contribute to preventing and managing this metabolic condition.

Purpose: Korean patients with type 2 diabetes mellitus (T2DM) and obesity are at a high risk of developing severe non-alcoholic fatty liver disease (NAFLD). This study examined the dietary intakes and compared the risks of NAFLD-related complications in Korean patients with T2DM and obesity. Methods: Data from the Korean National Diabetes Program cohort were used to study patients with T2DM. Two hundred and sixty-five obese patients with T2DM (body mass index ≥ 25 kg/m2 ) were classified into NAFLD and non-NAFLD groups. The nutrient intake was analyzed using a 24-hour dietary recall questionnaire. Anthropometric and biochemical data were also obtained. Statistical analyses were performed to determine the significant differences between the 2 groups. Results: The serum gamma-glutamyl transpeptidase levels in obese patients with T2DM and NAFLD were significantly higher than in obese T2DM patients without NAFLD (p < 0.05). The serum glucose and lipid profiles showed no significant differences between the NAFLD and non-NAFLD groups. The carbohydrate, protein, and fat levels also did not differ significantly. The results showed that the fiber intake of the NAFLD and non-NAFLD groups was 14.11 ± 3.86 g/100 kcal and 15.70 ± 4.56 g/1,000 kcal, respectively, showing that the dietary fiber intake of the non-NAFLD group was significantly higher (p < 0.05). A correlation was observed between total fiber intake and γ-glutamyl transpeptidase in either patient group. In addition, the odds ratio of developing NAFLD was 0.29× lower when the fiber was consumed at 125% of adequate intake. Conclusions A higher dietary fiber intake may reduce the risk of NAFLD in obese patients with T2DM. The dietary intake of Korean obese patients with T2DM should include and be enriched in dietary fiber to aid in preventing and treating NAFLD.
