Background: Total ankle arthroplasty (TAA) is increasingly used as an alternative to ankle arthrodesis for patients with rheumatoid arthritis (RA), although its outcomes remain controversial. Using propensity score matching, this study compared clinical and radiographic outcomes of TAA for patients with RA and osteoarthritis (OA). Methods: Eighteen patients with RA were selected from 21 eligible cases and matched—using propensity score matching based on 8 baseline variables—to 36 patients with OA from a pool of 386. All patients underwent mobile-bearing TAA and were followed up for a minimum of 2 years. Clinical outcomes were assessed using the Ankle Osteoarthritis Scale, American Orthopaedic Foot and Ankle Society ankle–hindfoot score, Short Form-36 Physical Component Summary, and a visual analog scale for pain. Radiographic outcomes and postoperative complications were also evaluated. Results: At a mean follow-up of 7 years, both groups demonstrated improvement in all clinical outcome measures, with no significant differences between the RA and OA groups (p > 0.05). Radiographic outcomes, including postoperative tibiotalar angle, talar tilt angle, as well as the incidence values for periprosthetic osteolysis, implant subsidence, and aseptic loosening, were similar between groups. However, periprosthetic medial malleolar fractures occurred only in the RA group (3 cases, 16.7%; p = 0.033), and 2 cases of incision wound dehiscence were also observed in the RA group. No deep infections were reported in either group. Overall, reoperation rates did not differ significantly between groups. Conclusions Patients with RA who underwent TAA achieved clinical and radiographic outcomes comparable to those with OA.However, periprosthetic fractures and wound complications were more common in the RA group. Further studies with larger, matched cohorts and longer follow-up are needed to confirm these findings.

Objectives: Large language models (LLMs) demonstrate remarkable potential in healthcare communication. However, whether they can process complex, high-volume medical information, such as stroke-related content, remains insufficiently validated. This study aimed to evaluate the natural language processing capabilities of LLMs in handling such content and to develop an evaluation instrument. Methods: A survey compared educational materials generated by two LLMs (ChatGPT 4.0 and Claude 3) with neurologist-authored content on stroke. The materials were based on two clinical scenarios representing distinct stroke etiologies: cardioembolism and large-artery atherosclerosis. They were evaluated in terms of accuracy, legality, ethics, comprehensiveness, and information delivery. Scores for comprehensiveness and information delivery were compared according to participants’ agreement with the use of LLMs in healthcare. Results: ChatGPT received the highest scores across all domains, except for legality in Scenario 2. In Scenario 1, the ranking for accuracy and summarization of clinical information was, from highest to lowest, ChatGPT, Claude, and the neurologist (η2 = 0.140, p < 0.001; η2 = 0.175, p < 0.001). The same hierarchy was observed in Scenario 2 for accuracy (η2 = 0.077, p < 0.001) and summarization (η2 = 0.194, p < 0.001). Participants who agreed with the use of LLMs in healthcare assigned higher scores for the comprehensiveness (Scenario 1, p = 0.005; Scenario 2, p = 0.007) and information delivery (Scenario 1, p = 0.003; Scenario 2, p = 0.026) of ChatGPT-generated materials than participants who did not agree. Conclusions LLMs demonstrated adequate capability to convey complex content, such as stroke-related information, in an accessible and understandable manner for non-experts.

Background: and Purpose To compare the epidemiological and clinical features of the rare patients with cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL) with age- and sex-matched controls in a nationwide cohort from South Korea. Methods: This observational cohort study analyzed newly diagnosed CADASIL patients aged at least 20 years and matched controls using data from the National Health Information Database for 2004–2022. The cumulative incidence of CADASIL was assessed by age and sex, and compared between regions. Neurologic and systemic diseases were compared between the CADASIL and control groups. Results: The study analyzed 816 CADASIL patients and 816 age- and sex-matched controls aged 56.8±15.2 years (mean±standard deviation), among whom 48.3% were male. The cumulative incidence of CADASIL was 1.86 per 100,000 people (95% confidence interval [CI]=1.85– 1.87 per 100,000), and peaked at 60–69 years of age. In terms of regional distribution, the incidence was highest for Jeju, at 39.67 per 100,000 (95% CI 37.84–41.49 per 100,000). Neurologic diseases were more frequent in CADASIL patients, including Alzheimer’s disease (33.1% vs.20.0%), vascular dementia (84.9% vs. 5.0%), epilepsy (34.6% vs. 15.9%), stroke (70.7% vs. 27.6%), parkinsonism (18.9% vs. 11.0%), and depression (60.8% vs. 44.9%). Systemic diseases such as diabetes mellitus (78.9% vs. 68.9%) were also more common in CADASIL patients, while cancer (27.9% vs. 38.7%) and myocardial infarction (10.0% vs. 13.6%) were less common than in controls. The onset ages of all diseases were lower in CADASIL patients. Conclusions This study has provided a precise nationwide estimate of the CADASIL incidence and its regional distribution in South Korea. CADASIL patients showed higher incidence rates and earlier onsets of diverse clinical manifestations.

Purpose: To propose improvements for promoting eyeball donation and managing donated corneas, this study analyzed the current system in Korea and reviewed relevant Korean laws, international standards, and foreign practices. Methods: To understand the current situation in Korea, annual reports published by the Korean Network for Organ Sharing and existing Korean laws were examined. For the international context, references were made to the National Organ Transplant Act, 21 CFR Part 1271 (Code of Federal Regulations Title 21 Part 1271), Current Good Tissue Practice guidelines of the Food and Drug Administration, and the Medical Standards of the Eye Bank Association of America in the United States. Opinions on promoting eyeball donation, improving the monitoring system for donated corneas and revising laws related to cornea management were gathered. The perspectives of 31 experts affiliated with the Korea Cornea Society were collected through a survey. Results: Currently, there are no laws or regulations that can be appropriately applied to the cornea which has properties of both organs and tissue. Additionally, there is no law regulating imported corneas. Therefore, there is a need to legislate or revise the current law; all experts who conducted the survey agreed on this. Furthermore, the current system faces limitations in the efficient procurement, stable supply, and management of donor corneas, as well as in donation promotion. To address these issues, the establishment of independent legislation for managing donated corneas and a National Central Eye Bank was proposed. This central body would oversee continuous personnel training, education, and monitoring, along with ensuring stable procurement, processing, and supply of corneas within a structured management system. Sixty-eight percent of the surveyed experts agreed with this proposal. Conclusions To establish a safe and efficient Korean corneal supply and demand system, it is imperative to enact cornea-specific laws, including the establishment of a National Central Eye Bank.

Background: As nationwide government-led whole-genome sequencing (WGS) projects progress, optimizing the clinical integration of large-scale WGS results is crucial. We explored how the initial analysis from Korea’s First WGS Pilot Study for Rare Diseases was applied in clinical practice, and then we reanalyzed the data comprehensively at Samsung Medical Center (SMC) Seoul, Korea. Methods: A prospective cohort study designed to collect WGS data under a Korean national initiative was conducted from August 2020 to December 2021. We focused on patients with rare diseases recruited from 16 university hospitals. The participants included 5,000 individuals (2,200 probands and 2,800 family members). The initial WGS data and diagnostic reference reports (from 682 probands and 484 family members), generated based on the First Korean WGS Pilot Study for Rare Diseases, were subsequently reanalyzed by SMC. Results: The initial analysis of the First Korean WGS Pilot Study data revealed a diagnostic rate of 17%. Upon receiving these results, the SMC conducted two rounds of reanalysis, increasing the diagnostic rate from 15% in the first analysis, to 18% in the second, and finally to 24% in the third (P = 1.6 × 10 −5 ). Key factors in improving the genetic diagnosis included increased detection of novel (likely) pathogenic variants (P = 1.0 × 10 −4 ), improved diagnostic rates with larger family recruitment (P = 0.004), and refined clinical information for more precise genotype–phenotype correlation analysis (40%). Conclusions Although national WGS projects lay a foundation for rare disease diagnosis, hospital-level reanalysis and multidisciplinary collaborations are crucial for optimizing diagnostic outcomes.

Purpose: Endoscopic submucosal dissection (ESD) has become a standard minimally invasive treatment for selected patients with early gastric cancer (EGC). This study presents the first nationwide survey of patients with EGC treated with ESD in 2023, conducted by the Korean College of Helicobacter and Upper Gastrointestinal Research. Materials and Methods: Data were retrospectively collected from participating referral centers across Korea using a standardized case report form covering patient characteristics, tumor features, procedural details, histopathological findings, and clinical outcomes.Descriptive and comparative analyses were conducted to summarize nationwide ESD practice patterns and outcomes. Results: Data from 5,460 ESD cases from 5,250 patients across 27 institutions were analyzed. The mean age was 67.4 years, with 74.1% males. Multiple synchronous lesions were identified in 3.7%. Most lesions were located in the lower third of the stomach (64.0%), and differentiated-type adenocarcinomas accounted for 87.8%. The en bloc and complete resection rates were 99.2% and 91.4%, respectively. Curative resection was achieved in 80.5%, whereas local non-curative resection (L-NCR) and surgical non-curative resection (S-NCR) were identified in 2.8% and 16.7%, respectively. Additional surgery was performed more frequently in patients with S-NCR than in those with L-NCR (59.3% vs. 24.7%). The bleeding and perforation rates were 3.6% and 0.9%, respectively, and were mostly managed conservatively or endoscopically. The median length of hospitalization was 4.0 days. Conclusions This first nationwide survey provides a comprehensive overview of the current practice of EGC treatment using ESD in Korea, demonstrating high technical success and safety, and establishing a baseline dataset for future longitudinal research.

Purpose: This study aimed to develop and test a structural equation model of respiratory infection prevention behaviors among elementary school teachers based on the Information–Motivation–Behavioral Skills model. Methods: This study included 250 teachers who were currently working in elementary schools and had experience as homeroom teachers. Data were collected from December 18 to December 31, 2023 using self-reported questionnaires. The proposed model included school organizational culture as the exogenous variable and respiratory infection prevention knowledge, respiratory infection prevention attitude, social support, self-efficacy, and respiratory infection prevention behavior as endogenous variables. Data were analyzed using IBM SPSS for Windows ver. 24.0 and Smart PLS ver. 4.1. Results: Of the 19 hypothesized paths in the research model, five were statistically significant. School organizational culture had a significant direct effect on respiratory infection prevention knowledge (β=.20, p=.041), social support had a significant direct effect on self-efficacy (β=.33, p=.001), and self-efficacy had a significant direct effect on respiratory infection prevention behaviors (β=.22, p=.021). Respiratory infection prevention attitude (direct: β=.23, p=.029; total: β=.26, p=.008) and school organizational culture (direct: β=.22, p=.017; total: β=.30, p=.002) had significant direct and total effects on respiratory infection prevention behaviors. Conclusion To enhance respiratory infection prevention behaviors among elementary school teachers, interventions should focus on strengthening positive attitudes toward infection prevention and improving self-efficacy. Organizational-level strategies, including establishing supportive school organizational cultures, promoting peer support among teachers, and sharing infection prevention practices within schools, are also needed to facilitate and sustain preventive behaviors.

Menopause is a normative midlife transition characterized by profound endocrine remodeling and a high burden of symptoms and functional change. Accumulating epidemiologic and translational evidence links menopausal transition with shifts in cardiometabolic risk, musculoskeletal health, sleep, mood, and perceived cognitive function. However, most data support association rather than definitive causal acceleration of aging independent of chronological time. This narrative review proposes a menopause-centered healthspan framework that integrates biological, neurocognitive, and psychosocial domains and translates evidence into pragmatic clinical pathway tools. To inform topic selection and prioritize higher-level evidence, including guidelines, randomized trials, meta-analyses, and large cohort studies, a structured evidence scan utilizing PubMed and targeted citation tracking was performed. The evidence scan identified 256 citations. After duplicate removal, titles/abstracts of 162 unique records were reviewed, of which 102 citations informed the final narrative synthesis. The results emphasize that menopausal hormone therapy remains the most effective treatment for vasomotor symptoms and an evidence-based option for reducing fracture risk in appropriate candidates. However, it is not indicated for the primary prevention of cardiovascular disease or dementia. Absolute risks and benefits vary by age, time since menopause, and formulation/route. The neurocognitive section distinguishes common, often transient midlife cognitive complaints from long-term neurodegenerative outcomes, highlighting that menopause is not established as an independent dementia risk factor in the general population. Overall, the framework aims to support individualized, equity-informed care during menopausal transition.

Objective: Juvenile idiopathic arthritis (JIA) is the most common chronic arthritis in children, with an unknown cause and prolonged disease course. Biologic disease-modifying anti-rheumatic drugs (bDMARDs) have improved outcomes in severe or refractory cases, but challenges remain due to disease heterogeneity and long-term management needs. Understanding treatment patterns of novel therapies is essential for optimizing care. This study aimed to investigate bDMARD treatment patterns in JIA patients using a real-world database. Methods: A retrospective analysis was conducted using Health Insurance Review and Assessment (HIRA) national claims data from 2007 to 2019. JIA patients were classified into bDMARDs and non-bDMARDs groups, with treatment patterns compared. Results: Among 1,728 JIA patients, 31.3% (n=541) received bDMARDs, with 15.5% (n=84) discontinuing treatment. The median time from diagnosis to first bDMARD was 36.7 (9.7~58.1) months, with a median treatment duration of 28.1 (11.5~54.4) months and follow-up of 24.3 (7.7~43.7) months post-discontinuation. In the non-bDMARDs group (68.7%, n=1,187), 68.7% (n=815) used conventional synthetic DMARDs (csDMARDs), with a 46.6% (n=380) discontinuation rate. Methotrexate, sulfasalazine, and hydroxychloroquine were the most prescribed csDMARDs. Etanercept (69.7%) was the most common first bDMARD, followed by adalimumab (19.0%) and tocilizumab (7.6%), with 14.8% switching to a second biologic. Discontinuation rates of csDMARDs were higher in the non-bDMARDs group (p<0.001). Conclusion This is the first comprehensive study profiling the treatment patterns of Korean patients with JIA using populationbased claims data. The findings enhance understanding of real-world treatment trends, aiding clinical decision-making.

Objective: The prognostic value of coronary artery calcium (CAC) volume and density was derived from an automated artificial intelligence (AI)-based analysis of non-electrocardiogram-gated chest CT. Materials and Methods: In this retrospective study, 7,552 asymptomatic adults who underwent chest CT as part of a national health screening program between 2007 and 2014 at two tertiary hospitals were examined for eligibility, of whom 1,109 with detectable CAC were analyzed. CAC density was derived by back-calculation from the Agatston score and CAC volume, both of which were obtained using AI software on chest CT. Differences in the probability of being free from major adverse cardiovascular events (MACE) across the four combined CAC volume-density groups were assessed using Kaplan–Meier curves and restricted mean survival time (RMST). Multivariable Cox proportional hazards models were used to assess the association between CAC volume and density and MACE. Results: Among the 1,109 participants with nonzero CAC (median age, 60.3 years; 87% men), 207 experienced MACE during a median follow-up of 7.7 years. Ten-year RMSTs were 9.45 years in the low-volume-high-density group, 9.07 years in the lowvolume-low-density group, 8.03 years in the high-volume-high-density group, and 7.68 years in the high-volume-low-density group. Differences in time to MACE were predominantly driven by CAC volume, with no significant density-related differences within the volume strata. CAC density demonstrated a significant, independent, inverse association after adjusting for CAC volume and clinical covariates (hazard ratio [HR] per increase by standard deviation [SD], 0.786; 95% confidence interval [CI], 0.659–0.936; P = 0.007). CAC volume also remained independently associated with an increased risk of MACE (HR per increase by SD, 2.608; 95% CI, 2.016–3.374; P < 0.001). Conclusion CAC density derived from chest CT using automated AI quantification was independently and inversely associated with MACE, providing additional prognostic value when added to CAC volume.
