1.Quality of care among patients with acute heart failure at the emergency room and adherence of physicians at the University of the Philippines – Philippine General Hospital to the division of cardiovascular medicine – heart failure pathway:A retrospective cohort study.

OBJECTIVES
Clinical pathways (CPs) ensure adherence to heart failure (HF) management guidelines. To optimize quality care in a low resource setting, an evidence-based care pathway for the management of acute HF was implemented at the emergency department (ED) of the Philippine General Hospital (PGH), the designated national tertiary hospital and referral center. This study aimed to describe the characteristics of adults with acute HF admitted at the ED and evaluate the quality of care they received, measured using physician adherence to the hospital’s acute heart failure CP.
METHODSThis was a retrospective, descriptive cohort study. We reviewed the inpatient charts of all adult patients with acute HF admitted to the ED of the PGH and referred to the Division of Cardiovascular Medicine between December 1, 2022 and May 31, 2023. Quality of care was assessed based on adherence to quality indicators adapted from routine and conditional order sets detailed in the pathway. Descriptive statistics was utilized to describe patient characteristics, quality of care, and outcomes.
RESULTSTwo hundred thirty-six (236) patients were included, with a mean age of 51.8 years. Majority were male (53.4%); hypertension (61.4%) and ischemic heart disease (53.8%) were the most common comorbidities, and infection the most common precipitant of decompensation (60.6%). There were optimal adherence rates to routine orders, which included referrals to Internal Medicine and Cardiology, baseline vital signs monitoring, fluid intake and output monitoring, chest radiograph, complete blood count, blood urea nitrogen, sodium, potassium, prothrombin time, partial thromboplastin time, arterial blood gas, urinalysis, and N-terminal pro b-type natriuretic peptide. Conditional orders, such as oxygen support, focused echocardiography, thyroid - stimulating hormone, and the use of vasopressors, diuretics, and venous thromboembolism prophylactic agents, were optimally performed when warranted. However, we noted suboptimal adherence to certain resource-intensive conditional orders, such as hourly monitoring of urine output (61.4%), hooking to cardiac monitor (53.8%), and performance of 12-lead ECG within 10 minutes (56.8%). Further, only 43.9% of patients were referred to the intensive care unit. Troponin I, calcium, magnesium, and albumin were ordered in excess.
CONCLUSIONOverall adherence rate of physicians to the hospital’s Acute Heart Failure Pathway was satisfactory. Work is needed to improve adherence to hourly urine output monitoring, consistent hooking to cardiac monitor, and timely performance of 12-lead ECG – an effort that begins with expanding in-hospital diagnostic equipment and human resource supply. We recommend continuous pathway implementation with periodic evaluation and stakeholder feedback to further improve quality of care.

2.Analysis and Research on Comprehensive Evaluation Method of Operation Performance of Hospital Valuable Equipment.

OBJECTIVE:
To study the effective method of comprehensive evaluation and analysis of hospital valuable medical equipment performance.
METHODS:
The operation performance of 6 valuable equipment was evaluated by cost-benefit method, comprehensive index method and public evaluation method.
RESULTS:
Utilize equipment information management methods for data collection and evaluation, and construct an assessment data model based on evaluation indicators from three aspects: equipment operation status, profitability status, and scientific research contribution.
CONCLUSIONS
Through the performance analysis of different types of valuable medical equipment, a more real and comprehensive quantitative analysis is carried out, which plays a key role in the reasonable purchase, efficient operation and avoiding idling.

In order to solve above problems, the Ninth People's Hospital Affiliated to Shanghai Jiao Tong University of Medicine aims to use the Internet of Things to build a medical equipment management information platform, realize the whole hospital area, whole subject and whole process management of medical materials, and achieve the objectives of business linkage, information connectivity and data sharing between management departments and clinical departments.

4.Large-scale Hospital Material Supply Chain Practice under Closed-loop Management.

In the context of public health emergencies, a Hospital used the existing SPD supply chain model as a basis, research and practice proceeded simultaneously and formed a set of "three-group three-port" emergency plan by itself. The program played a positive role and effectiveness in this emergency incident, assisting the hospital to obtain a valuable experience in closed-loop management of emergency supplies. This article elaborated on how the hospital can supply materials in case of emergency medical supplies shortage after emergencies by focusing on the three groups of closed-loop working group, inventory management group, and material procurement group, and the three ports of material storage port, logistics receiving and dispatching port, and closed-loop releasing port. In the case of emergency medical supplies being in short supply after emergencies, how can hospitals ensure adequate and balanced supply of supplies; barrier-free demand information; command and dispatch without chaos and reasonable deployment; materials receiving and dispatching are efficient and distributed in an orderly manner.

5.Research on Maintenance Management of Equipment of Magnetic Resonance Imaging Based on RAM.

OBJECTIVE:
To optimize the maintenance quality management of MRI equipment and ensure the quality and safety of its clinical use.
METHODS:
The data of failure time and repair time of a MRI equipment in three years were collected by magnetic resonance repair report system, and then the reliability, availability and maintainability(RAM) were studied and analyzed.
RESULTS:
The results of reliability analysis showed that the communication module was the key subsystem of the MRI equipment. The results of usability analysis showed that RF module was a key subsystem of MRI equipment. Maintainability results showed that the proportion of the MRI equipment not fully utilized due to maintenance-related problems was 2.58%. In order to improve the availability of MRI equipment, the maintenance time of MRI equipment should be shortened.
CONCLUSIONS
RAM-based analysis of MRI equipment can help hospital equipment managers to carry out the work of operation optimization, maintenance strategy formulation and safety management of MRI equipment.

6.Medical Consumable Management System Research under the New Situation.

7.Reliability Analysis of Magnetic Resonance Imaging Equipment Based on Non-homogeneous Poisson Process.

In order to evaluate the quality of different brands of magnetic resonance imaging system, optimize the quality of equipment repair and maintenance, and ensure the quality and safety of the equipment in clinical use. Meanwhile build the reliability evaluation model of magnetic resonance imaging equipment based on the large failure data collected by magnetic resonance repair system. In this paper, using the fault data of a certain brand of magnetic resonance in the past two years, applying the statistical judgment criterion, selecting the appropriate statistical distribution model, and finally fitting the equipment reliability evaluation model based on the non-homogeneous Poisson process, and then obtaining the magnetic resonance imaging equipment. Reliability evaluation indicators such as cumulative fault strength function, magnetic resonance imaging equipment fault density function, and mean time between failures. According to the calculated reliability index, on the one hand, it can evaluate the system reliability quality of a certain brand of magnetic resonance imaging equipment, and then evaluate the quality of different brands of magnetic resonance imaging equipment, on the other hand, it can help hospital equipment managers to carry out operational optimization, maintenance strategy development and safety management of magnetic resonance imaging equipment.

8.Construction of Quality Control System for Medical Equipment under the Model of County Medical Service Community.

9.Design and Development of Medical Equipment Lifecycle Management System Based on Structured Information Flow Architecture.

This paper introduces the design and development of a light-weighted medical equipment management system in hospital based on structured information flow architecture, to collect and analyze data in medical equipment lifecycle management processes. It applies co-operative work flows both online and off-line to achieve high quality data collection, and to increase the efficiency for clinical personals and clinical engineers. This system manages to apply the UDI system, implements the intelligent access to medical equipments, uses data analysis to provide intelligent services. It will boost the quality control management and information construction synchronously.
