1.Characteristics and outcomes of out-of-hospital cardiac arrest among students under school supervision in Japan: a descriptive epidemiological study (2008-2021).

BACKGROUND:
A comprehensive understanding of the epidemiology of pediatric out-of-hospital cardiac arrest (OHCA) occurring under school supervision is lacking. We aimed to comprehensively describe the characteristics and outcomes of OHCA among students in elementary schools, junior high schools, high schools, and technical colleges in Japan.
METHODS:
OHCA data from 2008-2021 were obtained from the SPIRITS study, which provides a nationwide database of OHCAs occurring under school supervision across Japan. We included cases in which resuscitation was attempted by emergency medical service personnel or bystanders. The cases were classified into three groups based on their etiology: cardiac, non-cardiac, and traumatic origin. The primary outcome was one-month survival with favorable neurological outcomes, defined as a Glasgow-Pittsburgh cerebral performance category of 1 or 2. The demographic characteristics, event details, and outcomes were compared across the three groups by using χ2 tests for categorical variables and one-way analyses of variance for continuous variables.
RESULTS:
During the 14-year study period, 602 OHCA cases were confirmed, with 430 (71.4%) classified as cardiac, 91 (15.1%) as non-cardiac, and 81 (13.5%) as traumatic origin. Non-cardiac and traumatic cases were less likely to be witnessed at the time of arrest (46.2% and 42.0%, respectively) than cardiac cases (82.6%; p < 0.001). Initiation of cardiopulmonary resuscitation by bystanders was less common in non-cardiac and traumatic cases (62.6% and 42.0%, respectively) than that in cardiac cases (82.8%; p < 0.001). The delivery of defibrillation using public-access automated external defibrillators was also significantly less frequent in non-cardiac (3.3%) and traumatic cases (6.2%) than that in cardiac cases (59.8%; p < 0.001). Ventricular fibrillation (VF) as the first documented rhythm was observed in 77.9% of cardiac cases but was much less common in non-cardiac (5.5%) and traumatic cases (8.6%; p < 0.001). One-month survival with favorable neurological outcomes was significantly lower in non-cardiac (6.6%) and traumatic cases (0%) than that in cardiac cases (50.2%; p < 0.001).
CONCLUSIONS
OHCAs of cardiac origin were more frequently associated with VF and had relatively good prognoses. In contrast, OHCAs of non-cardiac and traumatic origins consistently resulted in poor outcomes, highlighting the critical importance of prevention strategies to reduce the occurrence of these incidents.

2.Epidemiological studies on the health impact of air pollution in Japan: their contribution to the improvement of ambient air quality.

In Japan, during the high economic growth period (1950-1960s), air pollution due to sulfur dioxide (SO2) and dust derived from large-scale factories and power plants was apparent in many industrial districts, and it caused serious health problems such as the so-called "Yokkaichi Asthma." Many epidemiological studies have revealed the relationship between air pollution and respiratory diseases, and have provided scientific evidence for the regulatory control of air pollution. The concentration of SO2 has markedly decreased since the 1970s, and its adverse health effects have improved. In contrast, increased automobile traffic has caused considerable traffic-related air pollution, including nitrogen oxides (NOx) and particulate matter (PM). Epidemiological studies in Chiba and Tokyo revealed that the prevalence and incidence of asthma were significantly higher among individuals living in roadside areas than among those living in other areas. Large-scale epidemiological studies conducted in urban districts have revealed an association between traffic-related air pollution and the onset of asthma in schoolchildren and persistence of asthmatic symptoms in preschool children. Thereafter, the concentrations of NOx and PM gradually decreased due to the control measures based on the Automobile NOx/PM Law enforced in 2001. Thus, epidemiological studies have contributed to a reduction in air pollution caused by automobile exhaust emissions. Recently, the adverse health effects of ambient fine PM (PM2.5) and ozone (O3) at ground level have become an international concern. Our epidemiological studies showed that short-term exposure to considerably low concentrations of PM2.5 and O3 was associated with a decrease in pulmonary function among asthmatic children and increased airway inflammation in healthy adolescents. The effects of exposure to PM2.5 during pregnancy and early childhood on children's development have also been reported. These air pollutants consist of not only emissions from primary sources but also secondary formations in the atmosphere. They are affected by climate change and spread worldwide. Air quality control measures and climate change adaptation and mitigation strategies are synergistic, and will have co-benefits on human health. Therefore, global efforts are required to protect populations from the health risks posed by these air pollutants.

3.A 4-year retrospective epidemilogical study of common dermatoses among pediatric from a tertiary referral center in Pangasinan.

BACKGROUND
Pediatric dermatoses are multifactorial and diverse. Environmental factors play a significant role in skin disease. This study described the prevalence of common dermatoses among pediatric population seen at a tertiary referral center in Pangasinan.
METHODSThis was a 4-year retrospective, single center, chart review study in a tertiary referral hospital in Pangasinan. The researchers reviewed both electronic and medical charts of all pediatric outpatient and inpatient referrals with dermatoses from January 1, 2019 to December 31, 2022 by utilizing Proportion sampling technique.
RESULTSThis study reviewed a total of 501 (Males, 49.5% and Females, 50.5%) outpatient and inpatient pediatric patients who consulted due to skin related concerns. The majority of the patients were school-age children and adolescents followed by newborns, infants and toddlers respectively. Scabies infestation (n=76, 15.2%), Acne vulgaris (n=53, 10.6%) and Atopic dermatitis (n=48, 9.6%) were the top 3 common dermatoses among pediatric age groups. Scabies was the top diagnosis for both male (n=37, 14.9%) and female (n=39, 15.4%). Acne vulgaris (n=50, 31.6%) was the most common diagnosis in the adolescent age group.
CONCLUSIONThere was wide variation in the most common dermatosis according to age, sex, and year. Poor sanitation, overcrowding and low socioeconomic status are important factors for the increased prevalence of parasitic infestations. Acne vulgaris was seen almost exclusively in adolescents. This study served as a preliminary epidemiological report on pediatric dermatoses in Region 1 Medical Center that may pave the way for well-controlled and better designed researches in the future.

5.How to conduct and write a cohort study.

Cohort studies is an epidemiologic study that follows a group of individuals who share a common characteristic at the start of the study to observe the emergence of outcomes. Cohort studies are classified based on the population characteristics from where they were drawn, the way the data collection occurred or if its open or closed. This allows the computation of the absolute risk or the incidence of an outcome. There are several advantages in conducting a cohort study, such as clarity of temporal relationship of the exposure and outcome, permits the computation of incidence, permits multiple effects of a single exposure, and avoids selection bias on admission. While there are advantages, there are also disadvantages in doing this study, such as it requires long follow-up, need of large sample size, maybe costly, and may make it difficult to argue causation due to the presence of confounding. The statistical test that can be used to analyze the results will depend on the type of variable used. Statistical test such as T-test, Chi square test, and Regression can be used. Writing the final report follows the STROBE guidelines.

6.Progress on development and application of the diet-related inflammatory index in cancer epidemiological studies.

Cancer is a major public health problem worldwide, causing an more serious burden of disease. Inflammation is considered a predisposing factor for cancer with close relationship with its incidence. In recent years, the public and epidemiologists has paid more attention to the association between nutrition and cancer and other chronic diseases in the perspective of inflammation. This paper summarizes the development and application of the diet-related inflammatory index in cancer epidemiological studies based on the literature retrieval of common diet-related inflammatory index. Firstly, we highlight the common diet-related inflammatory indices and their construction methods, such as the Dietary Inflammatory Index, a literature-derived diet-related inflammatory index, and the Empirical Dietary Inflammatory Index, an empirically derived diet-related inflammatory index, and so on. Secondly, the epidemiological research progress on the commonly used diet-related inflammatory indices is briefly introduced. Finally, the advantages and disadvantages of the two types of this inflammatory indices are also briefly described for the purpose of providing reference for nutrition epidemiological studies of cancer and other chronic diseases in China.

Isotemporal substitution model is a powerful tool to explore the real association between physical behavior and health outcomes, which has the potential of the application in large-scale cohort study. This paper systematically introduces the principle of isotemporal substitution model and its implementation method in specific analysis to provide analytical ideas for the epidemiological research related to physical behavior in China. The baseline data of Regional Ethic Cohort Study in Northwest China conducted in Shaanxi province were used to analyze the relationship between physical behavior and cardiovascular disease with single-factor model, partition model and isotemporal substitution model. The advantages and disadvantages of different models were compared, and the advantages of isotemporal substitution model in quantifying physical activity health risk were introduced. Isotemporal substitution model could qualify physical behavior and health outcomes, which has wide application value in epidemiological research.

9.Research progress of follow-up monitoring design in mega cohort.

Its quality has a direct impact on the conclusions of cohort study. Therefore, it is necessary to develop a reasonable follow-up monitoring program for a mega cohort.In this paper, we summarized the contents and methods of the follow-up monitoring program in the mega cohorts at home and abroad, which aimed to provide suggestions for the new cohort and improve the follow-up program for the existing cohort.

10.Application of phylogenetic analysis in the molecular epidemiological study of infectious diseases.

The rapid development of sequencing technology brings the explosive growth of pathogen genetic data. The combination of genomic data and phylogenetic method is being used to elaborate the origin and evolution of pathogens, the time and space distribution and parameter changes in the prevalence process, and how phenotypes like antigen, virulence, and resistance change over time. This method is also being used to predict pathogen transmission trends. In this study, we described the aim of phylogeny and the process of the phylogenetic construction method. We elaborated the advantages and disadvantages and scope of application of tree-building methods including distance-based, maximum parsimony, maximum likelihood and bayesian methods. We have reviewed the application and the estimation methods of major epidemiological parameters of phylodynamics and phylogeography in domestic and foreign studies. We concluded that the time- and location-scaled phylogenetic trees are increasingly used for outbreak investigation and routine surveillance of infectious diseases.
