1.Research progress of modalities and initiation timing of renal replacement therapy for acute kidney injury

Acute kidney injury (AKI) usually requires renal replacement therapy (RRT). Common RRT modes include peritoneal dialysis, continuous renal replacement therapy, intermittent hemodialysis and continuous low-efficiency hemodialysis. In clinical practice, there is still controversy over how to select the appropriate RRT mode for AKI patients and when initiating RRT is more beneficial for AKI patients. This article summarizes previous studies on the impact of RRT mode and initiation timing on the prognosis of AKI patients, with the aim of providing assistance for clinical decision-making.
2.Effects of Moving Pattern on Dipping Thickness Distributions in Polymer Heart Valve

Objective To explore the impact of different moving patterns during the dip-coating process on thickness distributions of polymer heart valves.Methods Based on the volume of fluid(VOF)multiphase flow model,the Eulerian wall-film(EWF)model,and dynamic mesh technology,the dip-coating manufacturing process of polymer heart valves were numerically simulated.The effects of vertical,horizontal,and circular moving patterns on flow characteristics of the surface impregnation liquid and liquid film distributions under self-rotation conditions of the models were mainly studied.Subsequently,seven identical test points were set on each valve leaflet to collect thickness data,and the coefficient of variation(CV)was calculated to evaluate the uniformity of the liquid film thickness.Given that the vertical and horizontal patterns had fewer moving planes,limiting the optimization space,the circular pattern(45°)with richer moving planes was selected as the basis for optimization,and comparative analysis of numerical simulation was conducted.Results In the vertical pattern,the peak CV was 0.461 3;in the horizontal pattern,the CV was 0.060 8;and in the circular pattern,the CV at 30°,45° and 60° were 0.457 5,0.272 8,and 0.255 6,respectively.After optimization,the CV for the circular pattern(45°)decreased to 0.052 5,representing an 80.7%reduction compared to the pre-optimization value.Conclusions The moving patterns significantly affect the uniformity of dip-coating thickness distributions.The horizontal pattern demonstrates the best uniformity,while the vertical pattern shows the poorest uniformity.The CV for the circular pattern decreases as the angle increases,with its uniformity between that of the vertical and horizontal patterns.Optimization of moving pattern parameters based on simulation results has improved the uniformity of thickness distributions.

4.Efficacy of PD-1/PD-L1 inhibitors in first-line treatment of extensive-stage small cell lung cancer: a network meta-analysis

Objective:To investigate the effect of different programmed death receptor 1 (PD-1)/programmed death receptor ligand 1 (PD-L1) inhibitors combined with chemotherapy in the first-line treatment of extensive-stage small cell lung cancer (ES-SCLC).Methods:All randomized controlled trials (RCT) of PD-1/PD-L1 inhibitors for the first-line treatment of ES-SCLC from the establishment of the database to March 2025 were searched in CNKI database, Wanfang database, VIP Chinese Science and Technology Journal database, China Biomedical Literature Service System, PubMed database, Cochrane Library database and Embase database. Literatures were screened according to inclusion and exclusion criteria, and data were extracted. A network meta-analysis was performed using R 4.3.1 software to analyze the survival and safety of patients with ES-SCLC treated with PD-1/PD-L1 inhibitors combined with chemotherapy and chemotherapy±placebo first-line treatment.Results:A total of 8 RCT involving 3 832 patients with ES-SCLC were included in this analysis. PD-1/PD-L1 inhibitors included pembrolizumab, serplulimab, toripalimab, tislelizumab, adebrelimab, durvalumab, atezolizumab and benmelstobart, and benmelstobart combined with anti-vascular targeted drug anlotinib; the chemotherapy regimen was etoposide combined with platinum-based drugs. Except for durvalumab combined with chemotherapy compared with chemotherapy alone, the control group of the rest was chemotherapy combined with placebo. The included study data were complete and the risk of bias was small, and there was no closed loop in the outcome measures, so the consistency model was uniformly used for analysis. The results of network meta-analysis showed that compared with the control group, PD-1/PD-L1 inhibitors combined with chemotherapy benefited the progression-free survival (PFS) of patients (all P < 0.05), among which benmelstobart and anlotinib combined with chemotherapy had the best improvement in PFS ( HR = 0.32, 95% CI: 0.25-0.40, P < 0.05), and this regimen could benefit the PFS of patients more than other PD-1/PD-L1 inhibitors combined with chemotherapy and serplulimab combined with chemotherapy could benefit the PFS of patients more than pembrolizumab, toripalimab, adebrelimab, durvalumab, and atezolizumab combined with chemotherapy (all P < 0.05). The results of network meta-analysis showed that compared with the control group, all PD-1/PD-L1 inhibitors combined with chemotherapy benefited the overall survival (OS) of patients (all P < 0.05), among which benmelstobart and anlotinib combined with chemotherapy had the best improvement in OS ( HR = 0.61, 95% CI: 0.47-0.79, P < 0.05), but there was no statistically significant difference in OS benefit between each PD-1/PD-L1 inhibitor combined with chemotherapy (all P > 0.05). Ranking of the efficacy of all interventions, the P-scores for PFS and OS were the highest for benmelstobart and anlotinib combined with chemotherapy, which were 0.99 and 0.89, respectively. The results of the network meta-analysis showed that, except for the higher risk of ≥ grade 3 adverse reactions in benmelstobart and anlotinib combined with chemotherapy group compared to the control group ( OR = 2.01, 95% CI: 1.09-3.73, P < 0.05), there was no statistically significant difference in the risk of ≥ grade 3 adverse reactions between the other PD-1/PD-L1 inhibitors combined with chemotherapy group and the control group, or between all PD-1/PD-L1 inhibitors combined with chemotherapy groups (all P > 0.05). Among all PD-1/PD-L1 inhibitors combined with chemotherapy regimens, the tislelizumab combined with chemotherapy regimen had the lowest incidence of ≥grade 3 adverse reactions, with a P-score of 0.70. Conclusions:For the first-line treatment of ES-SCLC patients, the combination of PD-1/PD-L1 inhibitors and chemotherapy regimen has better survival benefits than chemotherapy regimen. Among them, compared with other PD-1/PD-L1 inhibitors, benmelstobart and anlotinib may benefit patients' survival more, but its related adverse reactions should be noted.

7.Nutlin-3a regulates mouse adipose function by inhibition of CIDEC expression

Objective To investigate the effect of Nutlin-3a,a mouse double minute 2 homolog(MDM2)inhibitor,on lipid metabolism of mouse adipose.Methods High-fat diet-induced obesity(DIO)C57BL/6J mice were randomly divided into a control group injected with DMSO and an experimental group injected with Nutlin-3a.Then we conducted glucose tolerance(GTT)and insulin tolerance(ITT)tests.The epididymal white adipose tissue(eWAT),inguinal white adipose tissue(iWAT)and brown adipose tissue(BAT)of animals were isolated and microscopy of WATs with hematoxylin-eosin(HE)staining was performed to observe the morphological changes of adipocytes.The expression of lipid metabolism related gene cell death-inducing DFF45-like effector C(CIDEC)in eWAT were detected by qPCR and Western blot.Results Compared with the control group,Nutlin-3a was found to promote the body weight(P<0.001),but no effect on glucose tolerance and insulin sensitivity in DIO mice.Nutlin-3a treatment decreased the size of adipocytes and fat deposition in adipose tissue and downregulated the mRNA and protein levels of CIDEC in eWAT.Conclusions Nutlin-3a inhibits the formation of lipid droplets by downregulating expression of CIDEC in white adipose tissue.
8.Differential study on intra-abdominal pressure measurement in severe patients with extracorporeal membrane oxygenation combined with prone position integrated treatment at different positions

Objective:To compare and analyze the differences in the measurement of intra-abdominal pressure in different positions of critically ill patients treated with extracorporeal membrane oxygenation (ECMO) combined with prone position integration, with a view to finding a more optimal intra-abdominal pressure monitoring strategy, which can provide a theoretical basis for clinical diagnosis and treatment.Methods:Forty critically ill patients who underwent ECMO combined with prone position integrated treatment in the department of Intensive Care Medicine of the First Affiliated Hospital of Guangxi Medical University from January 2020 to June 2023 were selected by convenience sampling method using an own-control trial. The differences in intra-abdominal pressure between supine position with head elevated at 0°, 15°and 30°and prone position with head high and foot low slopes at 0°, 15°and 30°were compared and analyzed. Heart rate, respiration, mean arterial pressure and oxygen saturation were also compared in patients in different positions.Results:There were 29 males and 11 females in 40 patients with the age of (62.58 ± 17.99) years.The intra-abdominal pressure in supine position with head elevated at 30° was (12.45 ± 3.43) mmHg(1 mmHg=0.133 kPa), which was higher than that of 0° and 15° of (9.38 ± 2.52) and (10.70 ± 2.95) mmHg, and the differences were statistically significant ( t=4.56, 2.45, both P<0.05);the difference in intra-abdominal pressure between 0° and 15° was not statistically significant ( P>0.05); the intra-abdominal pressure in prone position with head-high-foot-low slope of 30° was (12.92 ± 4.19) mmHg, which was higher than that of 0°and 15°of (9.67 ± 2.80), and (11.01 ± 3.10) mmHg, and the differences were statistically significant ( t=4.08, 2.32, both P<0.05); the difference in intra-abdominal pressure between 0° and 15° was not statistically significant ( P>0.05).The differences in intra-abdominal pressure between groups of supine bed head elevation 0°, 15°, 30°and prone position with head high and foot low slopes 0°, 15°, 30°were not statistically significant (all P>0.05). The differences in heart rate, respiration, mean arterial pressure and oxygen saturation in the supine position with head elevated at 0°, 15°and 30° were not statistically significant when compared within groups (all P>0.05); the differences in heart rate, respiration, mean arterial pressure and oxygen saturation in the prone position with head elevated with feet and feet on low slopes at 0°, 15°and 30°were not statistically significant when compared within groups (all P>0.05); and the differences in supine position with head elevated at 0°, 15°, 30°and prone head-height-foot-low slope 0°, 15°, 30°of heart rate, respiration, mean arterial pressure were not statistically significant (all P>0.05); supine bed head elevation 0°, 15°, 30°and prone head-height-foot-low slope 0°, 15°, 30°of oxygen saturation between the groups, the differences were statistically significant ( Z=6.85, 6.82, 6.68, all P<0.05). Conclusions:Intra-abdominal pressure can be measured in the 15° prone position in critically ill patients treated with ECMO combined with prone position integration; the different positions have little effect on vital signs, but the prone position significantly improves oxygen saturation.
9.Etiological and genomic analysis of Neisseria meningitidis serogroup Y in Fujian Province

Objective:To analyze the etiological characteristics of epidemic cerebrospinal meningitis caused by serogroup Y Neisseria meningitidis ( Nm) in Fujian Province. Methods:The strain identification, serogroup identification, antimicrobial susceptibility test, molecular typing, average nucleotide identity (ANI), core genome multilocus sequence typing (cgMLST), and single nucleotide polymorphism (SNP) analysis were performed on eight Nm strains isolated from cases of epidemic cerebrospinal meningitis, close contacts, or healthy population. Results:All eight isolates from Fujian Province in 2024 were serogroup Y Nm strains. The antimicrobial susceptibility test for 12 antibiotics showed that all isolates from Fujian Province in 2024 were resistant to trimethoprim-sulfamethoxazole, and NMFJ202406 and NMFJ202407 isolates were resistant to nalidixic acid and intermediate to ciprofloxacin, while the others were sensitive to antibiotics such as penicillin, cefotaxime, and ceftriaxone. All isolates were ST-1655 sequence type (ST), except for NMFJ202406 isolate which were ST-2039, and all isolates belonged to clonal complex 23 (CC23). The finetyping antigen profile of all isolates were P1.5-1, 10-1: F4-1. ANI and phylogenetic analysis indicated that the isolates in Fujian Province were mainly divided into two clusters, namely NMFJ202401 and its close contact isolates (NMFJ202402-NMFJ202405), as well as NMFJ202406, NMFJ202407, and NMFJ202408 isolates. Compared to the isolates from Guangdong, Shanghai, and Hebei Provinces, the strains isolated in Fujian Province were more closely related to the strains isolated in Canada (NML2019-167, 2019), Japan (NIID501, 2009), and the United Kingdom (CA41956, 2015), respectively. Conclusions:This is the first report of epidemic cerebrospinal meningitis caused by serogroup Y Nm isolates in Fujian Province, which might be transmitted from different sources abroad and have spread in many areas. In the future, the wider usage of tetravalent meningococcal vaccine ACYW135 is needed, and the epidemiological and etiological surveillance of serogroup Y epidemic cerebrospinal meningitis should be strengthened.
10.A prospective cohort study of CEUS predictive value in diagnosing ITBLs after liver transplantation

Objective:To investigate value of contrast-enhanced ultrasound(CEUS)in predicting ischemic-type biliary lesions(ITBLs)in patients with thickened hilar bile duct wall at early stage after liver transplantation.Methods:A total of 45 patients,who underwent liver transplantation at the Liver Transplantation Center of Beijing Friendship Hospital Affiliated to Capital Medical University from June 25,2020 to December 28,2022,and occurred hilar bile duct wall thickening at early stage after surgery,were prospectively included.CEUS was performed on biliary tract when the thickened hilar bile duct wall was first detected by routine ultrasound,and the enhanced mode of duct wall at each phase was recorded.Subsequently,according to the results of cholangiography,these patients were divided into ITBLs group(15 cases)and non-ITBLS group(30 cases).The enhanced degree of each phase of CEUS of two groups was qualitatively analyzed and compared,and the diagnostic efficacy of CEUS for ITBLs after liver transplantation was evaluated.Results:There were no significant differences in source of liver donor,biliary anastomosis,autoimmune liver disease,hepatic artery occlusion(HAO),rejection,cytomegalovirus infection and cholangitis between the two groups(P>0.05).The compared results of the enhanced mode of CEUS at arterial phase between the two groups indicated that 25 patients(83.3%)were hyper-enhancement,and 5 patients(16.7%)were iso-enhancement,and 0 patient was hypo-enhancement or non-enhancement in non-ITBLS group.The compared results also indicated that 3 patients(20.0%)were hyper-enhancement,and 4 patients(26.7%)were iso-enhancement,and 8 patients(53.3%)were hypo-enhancement or non-enhancement in ITBLs group.The difference of above results between the two groups was statistically significant(x2=22.946,P<0.000).There was no significant difference between the two groups in the enhanced mode at the late phase(P>0.05).The accuracy,sensitivity,specificity,positively predictive value and negatively predictive value of the prompted hypo-enhancement or non-enhancement at arterial phase of CEUS on biliary tract were respectively 84.4%,53.3%,100%,100%and 84.4%in diagnosing ITBLs.For 8 patients who were diagnosed as ITBLs by CEUS,the diagnostic time of CEUS for ITBLs was 1 to 6 months[3.0(1-5)months]ahead of that of cholangiography.Conclusion:CEUS can more accurately predict ITBLs before the biliary tract occurs significant morphological change,which can significantly advance the diagnostic time for ITBLs.
