1.Flexible endoscopic management of foreign body ingestion in children: A ten-year single-center retrospective study in the Philippines.

BACKGROUND AND OBJECTIVE
Foreign body (FB) ingestion is a common pediatric concern in the Philippines, but local studies on flexible endoscopic management are lacking. This study aimed to describe the clinical profile and outcomes of children referred for flexible endoscopic management and identify factors associated with poor outcomes.
METHODSThis retrospective cohort study included 145 patients aged
RESULTSMost FB ingestions (96.55%) were accidental, with coins as the most common FB (56.55%). Patients were admitted an average of 40.42 hours post-ingestion and referred for endoscopy within 9.28 hours. Flexible endoscopy was performed in 44.83% of cases, with a 98.46% success rate and an average procedure time of 32.25 minutes. Spontaneous passage occurred in 50.34% of cases. Poor outcomes were linked to age 48 hours; OR: 15.43, p = 0.0181), and prolonged procedures (>30 minutes; OR: 12, p = 0.0318). Flexible endoscopy is effective and safe for FB extraction in children. Early admission and timely intervention significantly improve outcomes, while delays and prolonged procedures increase the risk of complications.

2.ResNet-Vision Transformer based MRI-endoscopy fusion model for predicting treatment response to neoadjuvant chemoradiotherapy in locally advanced rectal cancer: A multicenter study.

BACKGROUND:
Neoadjuvant chemoradiotherapy followed by radical surgery has been a common practice for patients with locally advanced rectal cancer, but the response rate varies among patients. This study aimed to develop a ResNet-Vision Transformer based magnetic resonance imaging (MRI)-endoscopy fusion model to precisely predict treatment response and provide personalized treatment.
METHODS:
In this multicenter study, 366 eligible patients who had undergone neoadjuvant chemoradiotherapy followed by radical surgery at eight Chinese tertiary hospitals between January 2017 and June 2024 were recruited, with 2928 pretreatment colonic endoscopic images and 366 pelvic MRI images. An MRI-endoscopy fusion model was constructed based on the ResNet backbone and Transformer network using pretreatment MRI and endoscopic images. Treatment response was defined as good response or non-good response based on the tumor regression grade. The Delong test and the Hanley-McNeil test were utilized to compare prediction performance among different models and different subgroups, respectively. The predictive performance of the MRI-endoscopy fusion model was comprehensively validated in the test sets and was further compared to that of the single-modal MRI model and single-modal endoscopy model.
RESULTS:
The MRI-endoscopy fusion model demonstrated favorable prediction performance. In the internal validation set, the area under the curve (AUC) and accuracy were 0.852 (95% confidence interval [CI]: 0.744-0.940) and 0.737 (95% CI: 0.712-0.844), respectively. Moreover, the AUC and accuracy reached 0.769 (95% CI: 0.678-0.861) and 0.729 (95% CI: 0.628-0.821), respectively, in the external test set. In addition, the MRI-endoscopy fusion model outperformed the single-modal MRI model (AUC: 0.692 [95% CI: 0.609-0.783], accuracy: 0.659 [95% CI: 0.565-0.775]) and the single-modal endoscopy model (AUC: 0.720 [95% CI: 0.617-0.823], accuracy: 0.713 [95% CI: 0.612-0.809]) in the external test set.
CONCLUSION
The MRI-endoscopy fusion model based on ResNet-Vision Transformer achieved favorable performance in predicting treatment response to neoadjuvant chemoradiotherapy and holds tremendous potential for enabling personalized treatment regimens for locally advanced rectal cancer patients.

As a surgical adjunct, imaging-guided system assists surgeons in identifying vital structures and mitigating surgical risks, and they are extensively employed in operations involving the nasal cavity, sinuses, and skull base. Nevertheless, existing imaging-guided system faces several unresolved challenges. With ongoing technological advancements and the integration of novel technologies, these systems are demonstrating renewed vigor.

4.Transoral endoscopic-assisted submandibular gland resection with low-temperature plasma knife: clinical efficacy and literature review.

Objective:To evaluate the feasibility and clinical efficacy of transoral endoscopic-assisted submandibular gland resection with low-temperature plasma knife technology, aiming to establish a scarless surgical approach to meet the patients aesthetic demands. Methods:A retrospective analysis was conducted on 5 consecutive patients with benign submandibular gland pathologies treated by a single surgical team between January 2021 and December 2023. All procedures employed a transoral mucosal incision in the floor of mouth, with 0-degree high-definition endoscope assistance and low-temperature plasma knife for precise dissection and hemostasis. Close postoperative follow-up was performed. A systematic literature review compared surgical approaches regarding critical anatomical landmarks, complication profiles, and scar formation. The clinical efficacy of this technique was summarized and analyzed. Results:Successful transoral endoscopic plasma knife resections were performed for benign submandibular conditions(including neoplasms, chronic sialadenitis, and sialolithiasis). All operations were completed without conversion to open approach. No permanent lingual nerve or marginal mandibular nerve injuries occurred. Transient lingual hypoesthesia recovered within 2 weeks. During 6-12 months follow-up, there was no recurrence and absence of visible cervical scarring, with 100% patient satisfaction regarding cosmesis. Conclusion:Transoral endoscopic plasma knife resection of the submandibular gland demonstrates procedural safety and technical feasibility. This approach offers significant advantages in minimally invasive access, superior aesthetic outcomes, and accelerated recovery, representing a viable novel alternative for benign submandibular gland disease management.

5.Curative effect analysis of tympanoplasty with auricular cartilage combined with eustachian tube balloon dilation in the treatment of adhesive otitis media by endotoscope.

Objective:This study was aimed to evaluate the clinical effectiveness of tympanoplasty using auricular cartilage combined with balloon eustachian tuboplasty for the treatment of adhesive otitis media(adhesive otitis media, AdOM) under endoscopic. Methods:A retrospective analysis was conducted on 60 patients with unilateral adhesive otitis media who visited Department of Otolaryngology Head and Neck Surgery, the First Affiliated Hospital of Xiamen University between January 2017 and February 2022. All patients were divided into three groups: ①conservative treatment group;②simple tympanoplasty group; ③tympanoplasty combined with balloon dilation group(BET group). All patients were regularly assessed for the improvement of tympanic membrane morphology, hearing, and Eustachian tube function, as well as complications, after treatment. Results:There was no significant improvement in eardrum morphology, hearing, or eustachian tube function in the conservative treatment group(P>0.05); both the simple tympanoplasty group and the BET group showed significant improvements in eardrum morphology and hearing after surgery(P<0.01); In terms of Eustachian tube function improvement, the BET group showed significantly greater improvements in Eustachian tube manometry(TMM) and Eustachian Tube Dysfunction Questionnaire(ETDQ-7) scores compared to the tympanoplasty alone group(P<0.01). Conclusion:Tympanoplasty using auricular cartilage combined with balloon eustachian tuboplasty shows good clinical outcomes in the treatment of adhesive otitis media, significantly ameliorating patients' subjective symptoms such as tinnitus and ear congestion after surgery, thereby improving the patient's quality of life.

6.Single-Center clinical application and analysis of Robot-Assisted endoscopic salivary gland surgery via a postauricular approach.

Objective:Traditional salivary gland surgery involves incisions in the visible facial and cervical regions, leaving postoperative scars that affect cosmesis. This study aims to investigate the clinical efficacy, safety, and application value of robot-assisted endoscopic resection of benign submandibular and parotid gland lesions via a postauricular approach, while clarifying its advantageous differences compared with endoscopic surgery. Methods:Clinical data of 23 patients who underwent robot-assisted endoscopic surgery via a postauricular approach(11 parotid gland cases and 12 submandibular gland cases) from January 2017 to February 2025 were retrospectively analyzed. Meanwhile, A matched control group of patients who received postauricular endoscopic surgery during the same period was selected as the control group in a 1∶1 ratio(11 parotid gland cases and 12 submandibular gland cases). Indicators such as operation time, intraoperative blood loss, complications, and postoperative aesthetic satisfaction scores(Numeric Satisfaction Scale, NSS) were collected and compared between the two groups. The inclusion criteria were limited to benign lesions of the parotid superficial lobe(diameter ≤5 cm, without deep lobe involvement) and benign submandibular gland lesions(diameter ≤4 cm, without extension through the mylohyoid muscle). Results:All robot-assisted surgeries were successfully completed without conversion to open surgery. In the robot group, there were 7 male patients(mean age 39.5 years) and 16 female patients(mean age 35.9 years). For parotid gland surgeries, the mean operation time was (114.00±38.35) minutes. For submandibular gland surgeries, the mean operation time was(140.00±30.75) minutes.Temporary facial paralysis occurred in 0 of patients after robotic submandibular gland surgery (vs.8% in the endoscopic group) and 18% after robotic parotid gland surgery (vs.27 % in the endoscopic group),all of which resolved within 1 month, with no occurrence of salivary fistula or infection. Patients had high aesthetic satisfaction(NSS scores: 8.90±1.20 for parotid surgeries and 9.00±0.70 for submandibular surgeries). No tumor recurrence was observed during the 8-77 month follow-up period. Conclusion:Robot-assisted endoscopic salivary gland surgery via a postauricular approach is safe and feasible. With three-dimensional high-definition visualization and precise mechanical manipulation, it outperforms traditional endoscopic surgery in reducing blood loss, lowering the risk of nerve injury, and achieving long-term cosmetic outcomes. It is particularly suitable for young patients and cases with benign lesions of the parotid superficial lobe or submandibular gland that have high aesthetic demands. However, this surgical approach is not suitable for deep parotid lobe tumors, and its long-term efficacy requires verification through large-sample studies.

7.Application of a new super-micro flap in endoscopic tympanoplasty.

Objective:To introduce a new design of super-micro flap for endoscopic ear surgery, and to evaluate the application effect of super-micro flap in endoscopic tympanoplasty. Methods:Between January, 2023 and March, 2024, 58 patients(64 ears) with tympanosclerosis underwent tympanoplasty with super-micro flap. Continuous irrigating mode endoscopic ear surgery(CIM-EES) was used to complete type Ⅱ or Ⅲ tympanoplasty with the tragus cartilage with followed up for 12 to 24 months. The operation time, postoperative efficacy and complications were statistically analyzed. Results:Of the 64 ears, 63 ears had primary healing of the tympanic membrane, and 1 ear had cartilage necrosis due to multi-drug resistant bacteria infection. The second operation was performed one year later, and the success rate of operation was 98.40%. The average operation time was (48.40±8.86) minutes. The average hearing threshold of 0.5 kHz to 4.0 kHz before operation was (59.63±10.62) dB HL, and the average air conduction threshold of 0.5 kHz to 4.0 kHz one year after operation was(38.79±10.91) dB HL, which was significantly improved compared with that before operation(P<0.01). Bone conduction threshold also improved significantly (24.49±8.55) dB HL vs(21.88±7.58) dB HL(P<0.01). No outer tympanic membrane healing and ear canal scar stenosis occurred. Conclusion:The design of super-micro flap can effectively solve the interference of flap floating during continuous irrigating mode in endoscopic ear surgery, relieve the difficulty of flap reposition, simplify the operation process, help to shorten the operation time, and reduce the possibility of circular scar stenosis of conventional free flap, which provides a new flap design option for endoscopic ear surgery.

8.Endoscopic staged surgery for stage Ⅲ external auditory canal cholesteatoma: an efficacy analysis.

Objective:This study aims to investigate the therapeutic efficacy of staged endoscopic surgery for Shin-Ⅲ stage external auditory canal cholesteatoma. Methods:A retrospective analysis was conducted on the clinical data of 25 patients diagnosed with Shin-Ⅲ cholesteatoma of the external auditory canal, who were admitted to the Otology Center of the First People's Hospital of Foshan City from May 2020 to October 2024. All patients initially underwent endoscopic cholesteatoma removal. Based on the outcomes of the first-stage postoperative follow-up, patients were categorized into two groups: the repair type and the non-repair type. The non-repair type was further subdivided into simple and complex types. Of the total cases, 10 patients were of the repair type, with 9 requiring no further surgical intervention. The non-repair type comprised 15 patients, of which 8 were classified as simple type and underwent either tympanoplasty type Ⅰ or external auditory canal wall reconstruction during the second stage. The remaining 7 patients, identified as complex type, received open mastoidectomy or tympanotomy in the second stage, with or without ossicular chain reconstruction. Results:All patients were monitored for a minimum of six months postoperatively. The incidence of dry ear was observed in 22 patients, corresponding to a dry ear rate of 88.0%. Four cases experienced primary complications. Conclusion:Endoscopic phased operation for managing Shin-Ⅲ stage cholesteatoma of the external auditory canal can ensure that the surgical options match the severity of the lesions, reducing unnecessary surgical trauma and achieving good efficacy.

9.Risk factors of wet ear and its impact on surgical outcomes of endoscopic type Ⅰ tympanoplasty.

Objective:To investigate the risk factors of wet ear status and its impact on the efficacy of endoscopic type Ⅰ tympanoplasty. Methods:A retrospective analysis was conducted at the Department of Otolaryngology Head and Neck Surgery, the First Affiliated Hospital of Chinese People's Liberation Army(PLA)Air Force Medical University, on 160 ears that underwent endoscopic type Ⅰ tympanoplasty; these were assigned to a dry-ear group (n= 118) and a wet-ear group (n= 42).Univariate analysis and binary logistic regression were used to identify risk factors for wet ear status. Postoperative outcomes, including tympanic meoombrane healing rate and hearing improvement across frequencies, were compared between groups. Results:①Significant intergroup differences were observed in age, residual tympanic membrane status, external auditory canal condition, mastoid pneumatization(MC0), and middle ear ventilation dysfunction(P<0.05); ②The degree of mastoid pneumatization being MC0 is an independent risk factor for wet ear(P<0.05); ③No significant difference in tympanic membrane healing rates was found(P>0.05); ④The wet ear group showed significantly higher pre-and postoperative air-conduction(AC) and bone-conduction(BC) thresholds at 2 kHz and 4 kHz compared to the dry ear group(P<0.05), though the postoperative air-bone gap(ABG) improvement was comparable. Conclusion:Poor mastoid pneumatization is a risk factor for wet ears. The wet ear state has no effect on tympanic membrane healing and air-bone conduction gap, but patients in the wet ear group may have more severe inner ear or auditory nerve pathway damage.

10.Long-term outcomes of the endoscopic transcanal transpromontorial approach for vestibular schwannoma.

Objective:To summarize the initial experience and evaluate the technical feasibility of the endoscopic transcanal transpromontorial approach(TTA) for vestibular schwannoma resection by analyzing long-term follow-up outcomes. Methods:A retrospective analysis was conducted on the perioperative and long-term follow-up data(mean follow-up time: 5 years) of patients who underwent endoscopic TTA for vestibular schwannoma resection in the Department of Otorhinolaryngology Head and Neck Surgery at Xiangya Hospital, Central South University, between January 2020 and December 2020. Long-term outcomes were systematically evaluated. Results:This study included two patients(one 41-year-old male and one 51-year-old female). According to the AAO-HNS hearing classification system, preoperative hearing was Class C in one patient and Class D in the other. Preoperative imaging confirmed Koos stageⅠ tumors in both cases. Postoperative transient facial nerve paralysis(House-Brackmann Grade Ⅲ) recovered to Grade Ⅰ within 4 months. No complications such as cerebrospinal fluid leakage, intracranial infection, or intracranial hemorrhage occurred. No tumor recurrence was observed during the 5-year follow-up period. Conclusion:The endoscopic transcanal transpromontorial approach is minimally invasive, facilitates rapid recovery, and demonstrates satisfactory technical feasibility and safety when strict patient selection criteria(Koos stageⅠtumors with non-serviceable hearing) are applied.
