1.Influencing Factors of Depression in Patients with Postoperative Ovarian Cancer

Objective To explore the prevalence of depressive symptoms in postoperative patients with ovarian cancer and to analyze its influencing factors from multiple dimensions, including clinical characteristics, psychological factors, and laboratory indicators. Methods A cross-sectional study was conducted, which enrolled 235 postoperative patients with ovarian cancer. Depressive status was assessed using the patient health questionnaire, and the demographic, pathological, and medical record data of the patients were collected using the generalized anxiety disorder scale, Pittsburgh sleep quality index, European organization for research and treatment of cancer quality of life questionnaire core 30, and ECOG performance status score. Peripheral blood tumor marker (CA125), routine blood test, lymphocyte subsets, and serum cytokine levels were measured. Univariate and multivariate binary logistic regression analysis were used for statistical analysis. Results The prevalence of depression in postoperative patients with ovarian cancer was 39.15% (92/235). Univariate analysis showed that ECOG score ≥ 2 points, pain, anxiety, poor sleep quality, low quality of life, low life satisfaction, tumor recurrence, six or more cycles of chemotherapy, as well as higher levels of CA125, NLR, and NAR, and lower hemoglobin levels were significantly associated with depression (all P<0.05). Multivariate binary Logistic regression analysis showed that anxiety (OR=1.975, 95%CI: 1.231-3.170), sleep efficiency (OR=4.181, 95%CI: 1.211-14.43), sleep latency (OR=34.806, 95%CI: 4.258-284.542), ECOG performance status score, cognitive function (OR=0.918, 95%CI: 0.868-0.97), and life satisfaction were independent risk factors for depression (all P<0.05). Laboratory indicators were not independent influencing factors in the multivariate Logistic regression model. Conclusion Depression in postoperative patients with ovarian cancer is influenced by physiological, psychological, and social factors. Clinical management should focus on patients with anxiety, sleep disorders, poor physical condition, and low life satisfaction, and a comprehensive prevention and treatment strategy centered on psychological intervention and taking into account symptom management and social support should be implemented.

Professor Zou Yunxiang proposed the"kidney essence theory"in 1955,which believes that the kidney,as an important excretory organ in the human body,participates in the body's metabolism,and the basis for producing this effect is the essence of the kidney.Subsequently,the Zou nephrology team established the core medical technique of"tonifying the kidney element"based on this foundation,constructed a system of syndrome differentiation and treatment for chronic kidney disease,proposed the traditional Chinese medicine names,causes,and mechanisms of chronic kidney disease,as well as four major methods for diagnosing and treating chronic kidney disease,and developed representative drugs representing the core medical technique of"tonifying the kidney element"-Huang-zhi Yishen Capsules and Shenwu Yishen Tablets.In addition,the Zou nephrology team has extensively applied and promoted the core medical technique of"tonifying the kidney element".
3.Clinical Efficacy of Suyin Jiedu Prescription in the Treatment of Stage 3-5 Chronic Kidney Disease of Kidney Deficiency and Turbid Toxin Type and Its Effect on Serum Klotho

OBJECTIVE To explore the clinical efficacy of Suyin Jiedu Prescription in the treatment of Stage 3-5 non-dialysis chronic kidney disease of kidney deficiency and turbid toxin type and its effect on serum Klotho and inflammation levels.METHODS A total of 102 patients with Stage 3-5 non-dialysis chronic kidney disease of kidney deficiency and turbid toxin type who met the inclu-sion criteria were randomly divided into a treatment group and a control group,51 cases in each group;a total of 84 cases were comple-ted,including 44 cases in the control group and 40 cases in the treatment group.The control group was given conventional Western medicine basic treatment,and the treatment group was given Suyin Jiedu Prescription on the basis of the control group treatment.The treatment course of both groups was 16 weeks.The levels of renal function indexes[hemoglobin(Hb),serum albumin(Alb),urea ni-trogen(BUN),serum creatinine(Scr),estimated glomerular filtration rate(eGFR),24 h urine protein(UTP)]and TCM syndrome scores were observed in the two groups before and after treatment.The levels of serum Klotho,tumor necrosis factor α(TNF-α)and interleukin 18(IL-18)were detected by ELISA.The clinical efficacy between the two groups after treatment was compared.RE-SULTS After treatment,BUN and Scr levels of the two groups of patients were significantly decreased(P<0.05),eGFR was signifi-cantly increased(P<0.05,P<0.01),the treatment group was better than the control group(P<0.05);the TCM syndrome scores of the two groups were significantly decreased(P<0.05,P<0.01),and the treatment group was better than the control group(P<0.05,P<0.01);the serum Klotho level in the treatment group was significantly increased(P<0.05),the IL-18 level was significantly de-creased(P<0.01),and the serum Klotho,TNF-α,and IL-18 levels in the treatment group were better than those in the control group(P<0.05,P<0.01).The total clinical effective rate in the treatment group was higher than that in the control group(P<0.01).CONCLUSION Suyin Jiedu Prescription can effectively improve renal function and relieve clinical symptoms in patients with Stage 3-5 non-dialysis chronic kidney disease of kidney deficiency and turbid toxin type,which may be related to increasing Klotho level and reducing inflammation in the body.

7.Interventional effects of ginsenoside in pyroptosis and its potential for the treatment of diabetic nephropathy

Diabetic nephropathy(DN)is a common complication of diabetes and the leading cause of end-stage renal disease.Pyroptosis is a mode of programmed cell death that plays an important role in the development of DN.Pyroptosis of podocytes,glomerular endothelial cells and tubular epithelial cells leads to renal injury and dysfunction,accelerating the progression of DN.In recent years,studies have shown that a variety of ginsenosides have the potential to inhibit pyroptosis-related pathways,providing new ideas for DN treatment.Among them,ginsenoside compound K,ginsenoside Rg5,ginsenoside Rg1,ginsenoside Rh2 and ginsenoside Rb1 exert a protective effect against DN by regulating nucleotide-binding domain-rich repeat and pyrin domain-containing protein 3 inflammatory vesicle pathway,such as caspase-1,nuclear factor-KB,gasdermin D-N,interleukin-1β(IL-1β)and IL-18,thereby inhibiting inflammation and cellular pyroptosis.However,most current studies have focused on animal models,and further clinical trials are needed to verify the efficacy and safety of ginsenosides in humans.In addition,future studies need to explore the differences in response to ginsenosides in different individuals and the fine-tuning mechanism of ginsenosides against the cellular pyroptosis pathway to optimize therapeutic efficacy and reduce potential side effects.
8.Loratadine in combination with desloratadine for the treatment of chronic spontaneous urticaria in children:a clinical observation

Objective To evaluate clinical efficacy and safety of loratadine combined with desloratadine in the treatment of chronic spontaneous urticaria(CSU)in children. Methods A total of 177 children with CSU were enrolled into this study, and randomly and equally divided into 3 groups:combination group treated with an age?based dose of desloratadine tablet every morning and a weight?based dose of loratadine tablet before sleep every night for consecutive 28 days, loratadine group treated with a half tablet of placebo(starch tablet)every morning and oral loratadine tablet before sleep every night for consecutive 28 days, and desloratadine group treated with a half tablet of placebo (starch tablet) every morning and oral desloratadine tablet before sleep every night for 28 consecutive days. Possible adverse reactions were observed and recorded after the start of treatment, and therapeutic effects were evaluated at the end of treatment. Results A total of 166 patients completed the trial, including 55 in the combination group, 56 in the loratadine group and 55 in the desloratadine group. After 28?day treatment, the total response rate was significantly higher in the combination group(90.9%, 50/55)than in the loratadine group (71.4%[40/56],χ2=6.865, P<0.05)and desloratadine group(74.5%[41/55],χ2=5.153, P<0.05). No significant difference in the incidence of adverse reactions was observed among the combination group (10.9%[6/55]), loratadine group (8.9%[5/56]) and desloratadine group (9.1%[5/55], P > 0.05). Conclusion Combination of loratadine and desloratadine was superior to loratadine or desloratadine alone in the treatment of childhood CSU, and there was no significant difference in the incidence of adverse reactions among the 3 treatment groups.

10.Introduction of Professor Zhou Enchao’s Experience of the Application of Herbal Pairs in the Treatment of Chronic Renal Failure

Objective]To summarize and promote Professor Zhou Enchao’s experience in the treatment of chronic renal failure especially the usage of herbal pairs.[Methods] Illustrate Professor’s medication experience in the treatment of chronic renal failure and summarize the herbal pairs that he commonly used by the methods of learning from his daily treatment in inpatient and outpatient department, arranging medical cases records of chronic renal failure, reading relative books and articles, recognition from the study of ancient classics, modern phamocological research and Professor’s own academic opinion etc. [Results]In Professor’s treatment of chronic renal failure, besides protecting the Zheng Qi(anti-pathogenic Qi), he emphasizes the methods of detoxification and promotion of the discharging of toxin which is in feature of the treatment by herbal pairs. In his experience, there are coincidence, supplement and prevention in the function between the pairs of herbs when using them together. The commonly used herbal pairs include the pair of Su Ye and Huang Lian with the function of resolving dampness and detoxificaion, the pair of Ban Zhilian and BaiHua She She Cao with the function of clearing away heat and detoxification, the pair of Tu Fu Ling and Liu YueXue with the function of reducing and discharging the turbid fluid and toxin, the pair of TuBie Chong and Tao Ren with the function of breaking the hard blood stasis and detoxification, the pair of Mu Li and Da Huang with the function of resolving the phlegm and activating and discharging blood stasis, the pair of Hei Da Dou and Wan Can Sha with the function of detoxifying the wind-toxin, the pair of Yin Chen and Shi Xiao San with the function of resolving damping, clearing away heat. [Conclusion] Pro. Zhou Enchao's clinical experience of chronic renal failure from toxin and using drug pairs is effective and worth promotion of application.
