1.Epileptic encephalopathy with movement disorder and arthrogryposis: a case with SCN1A gene variant

The clinical characteristics of a neonatal patient with movement disorder and arthrogryposis (NDEEMA) caused by gain-of-function mutation of the SCN1A gene were reported in this article. The 1-year-and-9-month-old boy started seizures since 2 hours after birth. He had funnel-breast, dislocation of the hip, and bipedal varus. Genetic testing showed SCN1A gene de novo missense mutation c.706A>G(p.Ile236Val), causing overall gain-of-function effect. The frequency of seizures decreased significantly after treatment of oxcarbazepine.
2.Correlation between cerebral perfusion and cognitive function in patients with minor stroke or transient ischemic attack caused by severe intracranial arterial stenosis or occlusion

Objective This study aimed to investigate the correlation of cerebral perfusion and cognitive function status in patients with minor stroke(MS)or transient ischemic attack(TIA)complicated by severe intracranial arterial stenosis or occlusion(hereafter referred to as ICAS-MSTIA).Methods Retrospectively enrol consecutive ICAS-MSTIA patients admitted to the Department of Neurology,the First Affiliated Hospital of Xi'an Jiaotong University,from June 2023 to May 2024.In the meantime,healthy controls were openly recruited.The ICAS-MSTIA patients were divided into two groups based on the side of intracranial large artery stenosis or occlusion:the left intracranial large artery involvement group and the right intracranial large artery involvement group.All patients with intracranial large artery stenosis or occlusion underwent MR scanning within 2 weeks after the first episode of TIA or MS,while there was no specific time requirement for MR examination in the healthy control group.On the day of MR scanning,the Montreal cognitive assessment(MoCA)scale was used to evaluate the participants'global cognitive function and performance in various cognitive domains,including visuospatial/executive function,naming,attention,language,abstraction,delayed recall,and orientation.General information of all participants was collected,including age,sex,educational level,body mass index,and history of smoking and alcohol consumption.Clinical data were collected from both left and right intracranial large artery involvement groups,including cerebrovascular risk factors(such as,diabetes mellitus,hypertension,and hyperlipidemia),National Institutes of Health stroke scale(NIHSS)score at admission,responsible stenotic or occluded arteries(internal carotid artery,middle cerebral artery),degree of stenosis in the responsible vessel(severe stenosis[stenosis rate 70%-99%],occlusion[stenosis rate100%])and non-responsible vessel(no stenosis[0],mild stenosis[stenosis rate>0-49%]),collateral circulation compensation(American Society of Interventional and Therapeutic Neuroradiology/Society of Interventional Radiology[ASTIN/SIR]collateral circulation classification),and responsible events(TIA,MS).General data and MoCA scale scores were compared across the three groups,while clinical data were compared between the left and right intracranial large artery involvement groups.Statistical parametric mapping 12(SPM 12)was used to perform voxel-wise independent samples t-tests on cerebral blood flow(CBF)differences among the left ICAS-MSTIA group,right ICAS-MSTIA group,and healthy control group,with cluster-level family-wise error(FWE)correction applied for adjustment.Multiple linear regression analysis was conducted to evaluate the relationship between global CBF values and total MoCA scores in ICAS-MSTIA patients with left or right intracranial large artery involvement.Results A total of 33 ICAS-MSTIA patients and 33 healthy controls were enrolled in the study.Among the ICAS-MSTIA patients,21 had left intracranial large artery involvement and 12 had right involvement.(1)Among the three groups,statistically significant differences were observed in the proportions of individuals with reported smoking history(P=0.024)and alcohol consumption history(P=0.011).The left intracranial large artery involvement group had a higher NIHSS score(0[0,2]vs.0[0,0],P=0.044)and a higher proportion of patients with internal carotid artery involvement(13/21 cases vs.2/12 cases,P=0.027)compared with the right side group.No statistically significant differences were observed in other general or clinical data across the three groups or between the two non-control groups(all P>0.05).(2)Statistically significant differences were found across the three groups in the MoCA scale total score and scores of visuospatial/executive function,attention,language,abstraction,delayed recall,and orientation cognitive domains(all P<0.05),while no significant difference was noted in the naming score(P=0.063).The left intracranial large artery involvement group had lower total MoCA score and lower scores in visuospatial/executive function,attention,language,abstraction,delayed recall,and orientation in comparison to the healthy control group(all P<0.016 7).The right intracranial large artery involvement group had significantly lower scores in language,abstraction,and orientation domains than the healthy control group(all P<0.016 7).Additionally,the left side group had a lower attention domain score than the right side group(P<0.016 7).No other statistically significant differences were found in pairwise comparisons(all P>0.016 7).(3)Patients in both the left and right intracranial large artery involvement groups exhibited a significant decrease in CBF in extensive regions on the affected side,including the temporal lobe,dorsolateral prefrontal cortex,and occipital lobe.Furthermore,after correction,in the left involvement group CBF was higher in the contralateral lingual gyrus,cuneus,and calcarine sulcus compared with the healthy control group(P<0.05).While in the right involvement group,no regions had increased CBF compared to the healthy control group.(4)Multiple linear regression showed positive correlation between CBF in ipsilateral precentral gyrus and superior temporal gyrus,and the total MoCA score in patients with left intracranial large artery involvement(FWE-corrected,P<0.05).In contrast,there was no correlation between CBF and total MoCA score in patients with right intracranial large artery involvement.Conclusions ICAS-MSTIA patients exhibited various degrees of impairment in cerebral perfusion and cognitive function.A significant positive correlation is observed between these two impairments in patients with left intracranial large artery involvement.

5.PRELIMINARY STUDY ON EGG RECOGNITION MODEL OF AEDES ALBOPICTUS BASED ON DEEP LEARNING

Objective In this study,a pictorial database of Aedes albopictus eggs was built to establish a model for the automatic recognition and counting of eggs of this mosquito species.Methods In total,449 images of Ae.albopictus eggs from field strains in three districts of Shanghai and laboratory strains were collected.The eggs were manually calibrated using the Python environment labeling library,and the faster region-based convolution neural network(Faster R-CNN)was used to train the model with the tile overlap method.Results The results of model validation evaluation using accuracy,recall,and F-measure showed that after 15 training sessions,the loss gradually decreased with increasing training frequency and ultimately decreased to 0.000119.The mean accuracy(mAP)increased from 0.968 to 0.980 with increasing training frequency.The final model had an accuracy of up to 0.90,a recall rate of 0.97,and an F-measure of 0.93.Conclusion The established model achieved its function of assisting in egg identification and counting.With further optimization of the model and refining of its classification and recognition capabilities,it will serve as a simple and efficient auxiliary monitoring tool.
6.IMPORTED MALARIA CASES FOLLOWING LOCAL MALARIA ELIMINATION IN MINHANG DISTRICT,SHANGHAI

Objective The aim of the paper is to analyze the characteristics of imported malaria cases in Minhang District after local malaria elimination,and to improve prevention and control measures.Methods As local malaria was announced eliminated in Shanghai,in 2015,the malaria cases report information in Minhang District from 2016 to 2023 were enrolled to analyze the epidemic characteristics,infection sources,and treatment of malaria with descriptive epidemiological methods.Results A total of 47 cases were reported in Minhang District from 2016 to 2023,all of which were imported cases confirmed by the laboratory,mainly falciparum malaria.The overall proportion of falciparum malaria after 2020 was significantly higher than that from 2016 to 2019,with a statistically significant difference.There was no obvious seasonality in the onset of malaria cases,with a peak in February(six cases,12.77%)and a low in June(one case,2.13%).Most cases were young to middle-aged males and employed workers.All cases originated in Africa,mainly West Africa(35 cases,65.96%).Therewere 35 cases(74.47%)that occurred within two weeks of arrival and 89.36%of cases preferred tertiary medical facilities for medical services.The initial correct diagnosis rate was 78.72%.The median interval from onset to diagnosis was one day.The median interval from onset to entry was three days for falciparum,74 days for ovale,and two days for vivax malaria.Conclusions Malaria surveillance capacity in Minhang District remained excellent after local malaria elimination.However,imported malaria cases still pose a threat to public health,and multi-sectoral collaboration and health administration should be strengthened to improve clinical malaria screening capacity,diagnostic techniques,and medical management in the future.
7.Interventional Effect and Metabolomics Analysis of Linderae Radix on Fibrosis of Rats with Intrauterine Adhesions

ObjectiveTo investigate the pharmacological effect and metabolic mechanism of Linderae Radix on the intrauterine adhesion (IUA) rat model. MethodAn IUA rat model was induced by mechanical injury and infection. Molecular biology and pharmacology techniques were employed to evaluate the inhibitory effect of Linderae Radix extract (LAE) on fibrosis in IUA. Serum metabolomics analysis based on gas chromatography-mass spectrometry (GC-MS) was conducted to explore the metabolic regulation mechanism of LAE. ResultAnimal experiments showed that LAE significantly improved the morphology and structural damage of uterine tissue cells in the IUA rat model, promoted endometrial proliferation, vascular regeneration, and morphological recovery, inhibited the mRNA expression of transforming growth factor-β1 (TGF-β1), Smad2, and Smad3, and increased the expression of Smad7 mRNA to suppress fibrosis. Additionally, LAE significantly suppressed the levels of estrogen (E2), follicle-stimulating hormone (FSH), luteinizing hormone (LH), and tumor necrosis factor-α (TNF-α) expression (P<0.01), thereby improving the uterine microenvironment. Metabolomics analysis revealed significant metabolic abnormalities in the serum of IUA rats compared with the results in the normal group, and nine differential metabolites were identified. LAE effectively ameliorated these metabolic abnormalities, primarily by influencing six differential metabolites, including five shared metabolites among the nine identified markers: L-aspartic acid, L-pyroglutamic acid, L-serine, glucose, and L-norvaline. Pathway enrichment analysis indicated that the aminoacyl-tRNA biosynthesis pathway was the main affecting mechanism. ConclusionIn combination with the pharmacological research results, LAE effectively improved uterine damage and inhibited fibrosis in the IUA rat model. Its mechanism may involve the inhibition of the aminoacyl-tRNA biosynthesis pathway and the improvement of the microenvironment.
8.Analysis of in vitro activity and compatibility of Dunhuang Yifang Dabupi Decoction on gastric cancer based on computer-aided drug design

Based on the research system of computer-aided drug design combined with complex network analysis, the potential mechanism of Dunhuang Dabupi Decoction in preventing and treating gastric cancer (GC) is analyzed, and the scientific connotation of its prescription rules is explored through the efficacy grouping. To study the effect of Dabupi Decoction freeze-dried powder solution on the proliferation activity of gastric cancer cells through cell experiments; the TCMSP and TCMID databases were used to collect the compound components of Dabupi Decoction. Swiss Target Prediction is used to predict potential targets of compounds. DrugBank, GeneCards, TTD, and DisGeNET were used to collect potential targets for gastric cancer. Analyze protein interactions of potential targets through the STRING database. DAVID database was used for KEGG enrichment analysis; Dabupi Decoction was divided into Wenzhong group (dried ginger), Yiqi group (ginseng, licorice, Atractylodes macrocephala), nourishing Yin group (Ophiopogon japonicus, Schisandra) and Jiangni group according to its efficacy characteristics. The inverse group (inula) has 4 functional compatibility groups. Cytoscape was used to construct a network of "medicinal flavor-potential active ingredient-key target" respectively, and the network was used to analyze the scientific connotation of the compatibility of efficacy groups. The Schrödinger software was used to verify the molecular docking of the core components and the core targets. The material basis of the Dabupi Decoction to prevent and treat gastric cancer was discovered through the combination of pattern analysis and combined free energy calculation. The core drug was analyzed from the perspective of dynamics through molecular dynamics simulation. Potential targets and representative potential compounds interact with each other. Cell experiments confirmed that Dabupitang freeze-dried powder solution can down-regulate the mitochondrial membrane potential of AGS gastric cancer cells, block the cell cycle in the G0/G1 phase (
9.Adequate Animal Models of Hyperuricemia for Traditional Chinese Medicine Screening

Objective:To compare the characteristics of four commonly adopted animal models of hyperuricemia (HUA) for traditional Chinese medicine (TCM) screening, so as to choose the adequate model for screening Chinese herbs and herbal compounds capable of lowering the uric acid. Method:Fifty-four male SD rats were randomly divided into nine groups, namely the normal group, hypoxanthine (HX) + oxonic acid potassium salt (OAPS) model group, yeast extract (YE) + OAPS model group, low-dose adenine (AD) + ethambutol (EMB) model group, high-dose AD + EMB model group, and four positive drug allopurinol (Allo) groups. The modeling lasted for 14 d. The levels of serum uric acid (SUA), urinary uric acid (UUA), serum creatinine (SCr), urea nitrogen (BUN), kidney injury molecule 1 (KIM-1), and neutrophil gelatinase-associated lipocalin (NGAL) were detected on the 3rd, 7th, and 14th days. Urine was collected on the 7th and 14th days to investigate changes in urine volume, and the crystals in the residual urine were observed under a polarizing microscope.
