Purpose: While MRI-based tumor regression grade (mrTRG) has shown promise in evaluating pathologic response to concurrent chemoradiotherapy (CCRT) in rectal cancer, its ability to predict pathologic complete response remains limited.This study aimed to enhance mrTRG’s diagnostic performance in predicting ypT0–1N0 status, a key factor in considering non-radical management after CCRT for locally advanced rectal cancer (LARC). Methods: This retrospective study included 430 patients with LARC who underwent radical resection following CCRT at a single referral hospital between April 2018 and September 2024. Multivariable logistic regression was used to identify predictive factors associated with achieving ypT0–1N0 status. The diagnostic performances of mrTRG1–2 alone and in combination with other factors were assessed by comparing sensitivity, specificity, positive-predictive value (PPV), negative-predictive value, and area under the curve (AUC). Results: Ninety-three patients (21.6%) achieved ypT0–1N0. In the multivariable analysis, fungating type, cT1–2, and mrTRG1–2 were independent predictors for ypT0–1N0. Integrating mrTRG with gross tumor type yielded the highest AUC of 0.689 among the combined models. For predicting ypT0–1N0, the combination of mrTRG and gross tumor type improved PPV (79.2% vs. 41.5% for mrTRG alone) while also demonstrating enhanced sensitivity compared with ycT0–1N0, the conventional MRI-based predictor (40.9% vs. 22.6%). Conclusion This study demonstrated that combining mrTRG and gross tumor type improved the PPV of mrTRG in predicting ypT0–1N0 after CCRT in LARC. Further studies are warranted to validate the role of gross tumor type in refining predictive systems for selecting candidates for non-radical treatment.

Mycophenolate mofetil (MMF) is usually prescribed with a reduced fixed dose in Asian kidney transplant recipients (KTRs). However, the clinical efficacy and safety of the fixed dose have not yet been investigated via therapeutic drug monitoring. We evaluated whether reduced fixed-dose MMF is an optimal dosing strategy to achieve the therapeutic target of mycophenolic acid (MPA) exposure in Korean KTRs. Methods: This open-label, prospective study enrolled 50 de novo KTRs prescribed with tacrolimus, corticosteroid, and fixed-dose MMF (1.0–1.5 g/day). The trough level (C0) and area under the curve (AUC0–12 hr) of MPA were measured at 1 and 24 weeks after kidney transplantation (KT). The relationship of body weight (BW)-adjusted MMF dose with MPA C0 and MPA AUC0–12 hr was assessed using linear regression analysis. Results: The initial fixed dose of MMF of 1.44 ± 0.16 g/day was adjusted in 24 patients (48.0%) and then reduced to a mean dose of 1.19 ± 0.31 g/day at 24 weeks after KT. Most patients (≥84.0%) attained the minimum required MPA C0 of 1.0 μg/mL and MPA AUC0–12 hr of 30 μg × hr/mL at 1 and 24 weeks. The BW-adjusted MMF dose demonstrated significant positive correlations with MPA C0 and MPA AUC0– 12 hr at 1 and 24 weeks after KT. Moreover, 14 patients (28.0%) reported MPA-related adverse events that were predictable based on MPA AUC0–12 hr (cutoff level, 46.4 μg × hr/mL). Conclusion: The current reduced fixed-dose MMF strategy can help achieve the therapeutic target of MPA exposure in tacrolimus- treated Korean KTRs during the early posttransplant period.

Objectives: The purpose of this study was to explore the relationships among obstructive sleep apnea (OSA), dyspnea, and health-related quality of life (HRQOL), as well as the factorsinfluencing HRQOL. Methods: A total of 129 lung cancer survivors (mean age, 53.4 years; 77 men and 52 women; mean time since diagnosis, 1.6 years; and cancer stage [1/2/3/4/relapse], 43/31/19/34/2, respectively) completed a questionnaire that included demographic and clinical information, as well as questions about the severity of sleep apnea, dyspnea, and HRQOL. The severity of OSA, dyspnea, and HRQOL were assessed using the Berlin questionnaire, the Dyspnea-10 item (FACIT-Dyspnea), and the European Organization for Research and Treatment of Cancer QLQ-C30, respectively. Results: The severity of OSA and dyspnea exhibited negative correlations with HRQOL (p < 0.05).Multiple regression analysis revealed that several factors significantly impacted the HRQOLof lung cancer survivors. These included the extent of dyspnea (β = –0.369, p < 0.01), weight loss (β = 0.192, p < 0.01), OSA score (β = −0.215, p < 0.01), stage 2 cancer (β = −0.181, p < 0.01), and poor perceived health status (β = −0.179, p < 0.05). Conclusion These findings suggest that breathing difficulties, including OSA and dyspnea, contribute to decreased HRQOL. This study offers valuable insights for researchers and clinicians, aiding in the development of effective strategies to manage these issues in daily life.

Background: Diabetic nephropathy (DN) is a progressive complication among patients with diabetes and the most common cause of end-stage kidney disease. Neutrophil extracellular traps (NETs) are known to play a role in kidney disease, thus this study aimed to determine their role in the development of diabetic kidney disease using diabetic murine models. Results: Protein and histological analyses revealed that db/db mice and streptozotocin DN models expressed no significant NET-related proteins, myeloperoxidase, citrullinated histone H3 (citH3), neutrophil elastase, and lymphocyte antigen 6 complex locus G6D (Ly6G). However, the inflamed individuals in the DN model showed that citH3 and Ly6G were highly deposited in the renal system based on immunohistochemistry images. In vitro, NET treatment did not induce apoptosis in glomerular endothelial and renal tubular epithelial cells. NET inhibition by DNase administration demonstrated no significant changes in cell apoptosis. Conclusions NET-related proteins were only expressed in the DN model with tubulointerstitial inflammation. Our study revealed that NETs are only induced in mice with hyperglycemia-induced inflammation.

This report aimed to provide a summary of the 2023 survey results on the external quality assessment (EQA) scheme for the general transfusion medicine test and general transfusion antibody test programs in Korea. Proficiency testing materials were prepared at the Asan Medical Center for biannual distribution to participating laboratories. The accuracy rates and number of participating laboratories were as follows: ABO typing, 99.8%–100.0% (n=940, n=940); RhD typing, 99.8%–100.0% (n=924, n=918); crossmatching, 95.6%–99.4% (n=802, n=825); unexpected antibody screening, 99.4–100.0% (n=358, n=358); direct antiglobulin test (DAT) using a polyspecific reagent, 99.3%–100.0% (n=296, n=292); DAT using anti-immuno globulin G monospecific reagent, 100.0% (n=76, n=76); and DAT using antiC3d monospecific reagent, 97.3%–100.0% (n=73, n=73). The 2023 EQA scheme for transfusion medicine program has improved and maintained the standards of the participating laboratories.
