1.Application effects and safety of continuous negative pressure drainage device in postoperative drainage of congenital microtia reconstruction of auricle

Objective:To analyze the application effects and safety of continuous negative pressure drainage devices in postoperative drainage of congenital microtia reconstruction of auricle.Methods:A retrospective study was conducted, including 1 211 patients with congenital microtia treated at the Plastic Surgery Hospital of Chinese Academy of Medical Sciences and Peking Union Medical College from January 2018 to August 2021. All patients underwent auricular reconstruction surgery and were divided into two groups based on the type of negative pressure drainage device used. The study group consisted of 624 patients [315 males and 309 females, aged 6-24 (15.1±2.2) years], while the control group consisted of 587 patients [286 males and 301 females, aged 5-23 (14.7±2.3) years]. The study group used a novel negative pressure drainage ball, while the control group used a conventional negative pressure syringe for drainage. The preoperative and postoperative 1, 7 days scores of the modified Yale preoperative anxiety scale (mYPAS) and the quality of life scores postoperative 3 days (including functional activity, self-image, psychological health, satisfaction and pain) were compared between the two groups. All patients were followed up at 4, 8, 12, and 16 weeks postoperatively to compare the surgical scar area between the two groups. The incidence of adverse reactions within 12 weeks after surgery in two groups were recorded.Results:There was no significant difference in preoperative mYPAS scores between the two groups ( P=0.897). The mYPAS scores at 1 and 7 days after surgery were lower than the preoperative scores in both groups (all P<0.05), and the study group had significantly lower mYPAS scores at all time points than those of the control group (all P<0.001). The study group had higher scores in functional activity, self-image, psychological health, and satisfaction than those of the control group (all P<0.001), and a lower pain score than that of the control group ( P<0.001). Follow-up at 4, 8, 12, and 16 weeks showed that the surgical scar area was significantly smaller in the study group at all time points than in the control group (all P<0.001). The incidence of adverse reactions within 12 weeks after surgery was 9.0% (56/624) in the study group, which was lower than the 19.4% (114/587) in the control group ( P<0.001). Conclusion:The use of continuous negative pressure drainage devices in congenital microtia reconstruction of auricle is more effective and can reduce the incidence of postoperative adverse reactions.
2.Effects of understory environmental factors on understory planting of medicinal plants.

Understory planting of medicinal plants is a new planting mode that connects Chinese herbal medicine(CHM) with forest resources.The complex and variable understory environmental factors will inevitably affect the yield and quality of understory CHM.This research summarized the research progress on understory planting of medicinal plants based on forest types and environmental factors within the forest from the perspectives of understory light, air temperature and humidity, soil characteristics, and the interaction between crops within the forest.The results showed that the complex and variable light, temperature and humidity, and soil factors(such as fertility, acidity and alkalinity, and microorganisms) under the forest could affect the yield and quality of medicinal plants to varying degrees through physiological activities such as photosynthesis and respiration, resulting in a significant increase or decrease in yield and quality compared to open field cultivation.In addition, the competition or mutual benefit between different crops within the forest could lead to differences in the yield and quality of understory medicinal plants compared to open field cultivation.A reasonable combination of planting could achieve resource sharing and complementary advantages.Therefore, conducting systematic research on the effects of understory environmental factors on the yield and content of medicinal plants with different growth and development characteristics can provide theoretical guidance and technical references for formulating comprehensive strategies for understory planting of medicinal plants, such as selecting suitable medicinal plant varieties, optimizing planting density, and conducting reasonable forest management, thus contributing to the sustainable development and ecological protection of CHM.

Diarrhea-predominant irritable bowel syndrome(IBS-D)is a clinically common functional gastrointestinal disease,the"SCFAs-intestinal barrier"pathway plays an important role in the pathogenesis of IBS-D.Traditional Chinese medicine monomers/compounds or Chinese medicine compound can treat IBS-D by regulating the"SCFAs-intestinal barrier"through multiple pathways and multiple targets.This article takes the relationship between SCFAs and the four major intestinal barriers,as well as the mediating effect of IBS-D,as the starting point to systematically review and sort out the relevant literature on the targeted regulation of"SCFAs-intestinal barrier"in the treatment of IBS-D by Traditional Chinese medicine monomers/compounds and Chinese medicine compound;explores the theoretical basis of IBS-D caused by"SCFAs-intestinal barrier"from the perspective of"Large intes-tine dominating fluid",in order to provid ideas for Traditional Chinese medicine to establish a precision treatment system with Chinese medicine characteristics.

Objective To examine the changing antimicrobial resistance profile of Enterobacter spp.isolates in 53 hospitals across China from 2015 t0 2021.Methods The clinical isolates of Enterobacter spp.were collected from 53 hospitals across China during 2015-2021 and tested for antimicrobial susceptibility using Kirby-Bauer method or automated testing systems according to the CHINET unified protocol.The results were interpreted according to the breakpoints issued by the Clinical & Laboratory Standards Institute(CLSI)in 2021(M100 31st edition)and analyzed with WHONET 5.6 software.Results A total of 37 966 Enterobacter strains were isolated from 2015 to 2021.The proportion of Enterobacter isolates among all clinical isolates showed a fluctuating trend over the 7-year period,overall 2.5%in all clinical isolates amd 5.7%in Enterobacterale strains.The most frequently isolated Enterobacter species was Enterobacter cloacae,accounting for 93.7%(35 571/37 966).The strains were mainly isolated from respiratory specimens(44.4±4.6)%,followed by secretions/pus(16.4±2.3)%and urine(16.0±0.9)%.The strains from respiratory samples decreased slightly,while those from sterile body fluids increased over the 7-year period.The Enterobacter strains were mainly isolated from inpatients(92.9%),and only(7.1±0.8)%of the strains were isolated from outpatients and emergency patients.The patients in surgical wards contributed the highest number of isolates(24.4±2.9)%compared to the inpatients in any other departement.Overall,≤ 7.9%of the E.cloacae strains were resistant to amikacin,tigecycline,polymyxin B,imipenem or meropenem,while ≤5.6%of the Enterobacter asburiae strains were resistant to these antimicrobial agents.E.asburiae showed higher resistance rate to polymyxin B than E.cloacae(19.7%vs 3.9%).Overall,≤8.1%of the Enterobacter gergoviae strains were resistant to tigecycline,amikacin,meropenem,or imipenem,while 10.5%of these strains were resistant to polycolistin B.The overall prevalence of carbapenem-resistant Enterobacter was 10.0%over the 7-year period,but showing an upward trend.The resistance profiles of Enterobacter isolates varied with the department from which they were isolated and whether the patient is an adult or a child.The prevalence of carbapenem-resistant E.cloacae was the highest in the E.cloacae isolates from ICU patients.Conclusions The results of the CHINET Antimicrobial Resistance Surveillance Program indicate that the proportion of Enterobacter strains in all clinical isolates fluctuates slightly over the 7-year period from 2015 to 2021.The Enterobacter strains showed increasing resistance to multiple antimicrobial drugs,especially carbapenems over the 7-year period.

Objective To investigate the incidence rate,clinical characteristics,and prognosis of neonatal stroke in Shenzhen,China.Methods Led by Shenzhen Children's Hospital,the Shenzhen Neonatal Data Collaboration Network organized 21 institutions to collect 36 cases of neonatal stroke from January 2020 to December 2022.The incidence,clinical characteristics,treatment,and prognosis of neonatal stroke in Shenzhen were analyzed.Results The incidence rate of neonatal stroke in 21 hospitals from 2020 to 2022 was 1/15 137,1/6 060,and 1/7 704,respectively.Ischemic stroke accounted for 75%(27/36);boys accounted for 64%(23/36).Among the 36 neonates,31(86%)had disease onset within 3 days after birth,and 19(53%)had convulsion as the initial presentation.Cerebral MRI showed that 22 neonates(61%)had left cerebral infarction and 13(36%)had basal ganglia infarction.Magnetic resonance angiography was performed for 12 neonates,among whom 9(75%)had involvement of the middle cerebral artery.Electroencephalography was performed for 29 neonates,with sharp waves in 21 neonates(72%)and seizures in 10 neonates(34%).Symptomatic/supportive treatment varied across different hospitals.Neonatal Behavioral Neurological Assessment was performed for 12 neonates(33%,12/36),with a mean score of(32±4)points.The prognosis of 27 neonates was followed up to around 12 months of age,with 44%(12/27)of the neonates having a good prognosis.Conclusions Ischemic stroke is the main type of neonatal stroke,often with convulsions as the initial presentation,involvement of the middle cerebral artery,sharp waves on electroencephalography,and a relatively low neurodevelopment score.Symptomatic/supportive treatment is the main treatment method,and some neonates tend to have a poor prognosis.

Objective To compare the changes of the medical animal resources(MAR)in Sichuan based on the data of the 3rd Chinese Materia Medica Resource Inventory(CMMRI,1983-1986)and the 4th CMMRI(2020-2022).Methods After field investigation and identification of the photos of the animals,the data of the MAR in Sichuan found in the 4th CMMRI were analyzed and compared with the data of 3rd CMMRI.Results 745 species of MAR were found in Sichuan during the 4th CMMRI,including 212 families and 468 genera.Compared with the 108 species found in 3rd CMMRI,the number of MAR in Sichuan had greatly increased,The Aves was found to have 243 species of MAR,which is the most plenty one among the 7 classes.There were 14 families which have more than 10 species of MAR.The family,Cyprinidae had 48 species of MAR.There were 33 common Chinese medicinal herbs and 3 genuine medicinal materials including Cordyceps sinensis,musk and Venenum bufonis were found in this investigation.The new distributions of Liangshan Cordyceps and Cordyceps gunnii were found in the investigation,and the new resources of Atypus heterothecus was found in Mountain Emei.There were 140 species of key protection of wild medicinal animals,including 44 species of animals under first-class protection and 96 species of animals under second-class protection.There were 230 species of animals which had important ecological,scientific,and social values,too.Conclusion Sichuan was plenty of medical animal resources and the protection of the forest musk deer,the Cordyceps sinensis and the pangolin was need to be strengthened.
8.One-step Circulating Tumor DNA Microfluidic Sensing Based on Toehold-mediated Entropy-driven Bipedal DNA Walker and Dendritic Hybridization Chain Reaction

Circulating tumor DNA (ctDNA) in blood,present at low abundance,serves as a critical biomarker for cancer. Precise detection of ctDNA is of great significance for early diagnosis,disease monitoring,and prognosis evaluation. In this study,a microfluidic chip-based entropy-driven bipedal DNA walker combined with dendritic hybridisation chain reaction was designed as a cascade signal amplification strategy for detection of ctDNA in blood in a microfluidic chip. In the presence of the target,toehold A interacted with target and triggerd a bidirectional strand displacement reaction with the aid of fuel strand F,thereby releasing the target and the 8-17 DNAzyme active centre. Among them,the released target was recycled in a new round of entropy-driven chain replacement reaction. Meanwhile,8-17 DNAzyme active center activated by Pb2+would act on the cleavage site of the substrate hairpin at the microbead surface,exposing the capture probes on the surface of the microbead. The numerous capture probes induced a dendritic hybridization chain reaction,which resulted in fluorescent signals being concentrated on the surface of the microbeads. With the breast cancer mutant gene PIK3CAE545K as the target model,under the optimal experimental conditions,the linear range of sensor was 50-10000 fmol/L,the detection limit was 0.94 fmol/L (LOD,3σ),and the regression equation was y=31.65 lgCT+474.08 (CT is the concentration mutant gene PIK3CAE545K sequence,fmol/L). This method showed spiked recoveries between 98.8% and 106.5% when applied to detection of mutant gene PIK3CAE545K in human serum. Characterized by its sensitivity,specificity,anti-interference capability,high throughput,and one-step operation,this method was ideally suited for the rapid analysis of complex samples.
9.Quercetin alleviates inflammation in acute bronchitis rats by inhibiting HMGB1/TLR4 signaling pathway

To investigate the effects of Quercetin(QUE)on inflammatory response and high mobility group protein B1/Toll like receptor 4(HMGB1/TLR4)signaling pathway in acute bronchitis(AB)rats,an AB rat model was constructed,and the successfully modeled rats were assigned into AB group,quercetin low and high dose groups(QUE-L,QUE-H groups),Quercetin high dose+HMGB1 recombinant protein group(QUE-H+r-HMGB1 group),with remaining normal healthy rats as control group.Wright's staining was applied to detect the number of inflammatory cells in the sediment of bronchoalveolar lavage fluid(BALF);ELISA was applied to detect the levels of inflammatory factors in BALF;flow cytometry was used to detect the levels of CD4+T,CD8+T and CD4+T/CD8+T in BALF;HE staining was applied to observe the pathological morphology of bronchial tissue and lung tissue;Western blot was applied to detect the expression of proteins related to the HMGB1/TLR4 signaling pathway.Compared with the control group,the AB group had increased secretion in the bronchial lumen,narrowed lumen,obvious infiltration of inflammatory cells in the wall,shedding and degeneration of bronchial mucosal epithelial cells,obvious thickening of smooth muscle layer,thickening of alveolar wall,widening of alveolar septum,and obvious infiltration of inflammatory cells,increased numbers of NEU,EOS,LYM,and elevated levels of TNF-α,IL-6,CD4+T,CD4+T/CD8+T,and increased expression of HMGB1,TLR4,p-NF-κB p65/NF-κB p65,while lower levels of IL-10 and CD8+T(P<0.05).QUE(both low and high doses)could alleviate these changes mentioned above in AB rats(P<0.05).As compared with QUE-H group,QUE-H+r-HMGB1 group demonstrated more severe pathological damage of bronchial and lung tissues,increased numbers of NEU,EOS and LYM,higher levels of TNF-α,IL-6,CD4+T and CD4+T/CD8+T,and higher expression of HMGB1,TLR4 and p-NF-κB p65/NF-κB p65 increased,while lower levels of IL-10 and CD8+T(P<0.05).Taken together,quercetin can alleviate the inflammatory response in AB rats,and its mechanism of action is related to the inhibition of the HMGB/TLR4 signaling pathway.
10.Research progress in the mechanism of microRNA regulation in diarrhea-predominant irritable bowel syndrome and traditional Chinese medicine interventions

The incidence of diarrhea-predominant irritable bowel syndrome(IBS-D)remains high,with microRNA(miRNA)-mediated intestinal barrier dysfunction,visceral hypersensitivity,low-grade inflammation,and dysbiosis playing significant roles in its pathogenesis.Traditional Chinese medicines can treat IBS-D by directly or indirectly targeting miRNAs to regulate multiple pathways and targets in a coordinated manner.This article systematically reviews the involvement of miRNAs in the pathogenesis of IBS-D and the progress of traditional Chinese medicine interventions.It explores the relationship between traditional genetics,epigenetics,and the Chinese medicine concepts of'kidney'and'spleen'in terms of congenital and acquired interactions from a molecular biology perspective.This review thus provides a reference for exploring the micro-material basis of the'Zang-Xiang'theory of traditional Chinese medicine,and offers new ideas and method for the effective treatment of IaBS-D using traditional Chinese medicine.
