1.Effect of hyperbaric oxygen combined with drugs on nerve damage caused by nitrous oxide poisoning

Objective:To investigate the clinical effect of drugs combined with hyperbaric oxygen(HBO)on nervous system damage caused by nitrous oxide(N 2O)poisoning. Methods:The clinical data of 12 patients with nerve damage caused by N 2O poisoning admitted to the Department of Neurology,Hyperbaric Oxygen of a certain PLA hospital from February 2020 to April 2021 were reviewed. The changes of neuroelectrophysiological and neurological functions of patients before and after treatment were analyzed using electroneurogram detection,Berg balance scale,and Holden’s functional ambulation categories(FAC)as observation indicators. Results:After drug treatment combined with HBO,the motor conduction velocity,sensory conduction velocity,motor latency,motor evoked potential,and sensory evoked potential of electroneurogram of 12 patients were significantly improved,and the differences were statistically significant( P < 0.05).Berg balance scale scores were significantly improved compared with those before treatment,and the differences were statistically significant( P < 0.05).FAC scale was improved compared with before treatment,but the difference was not statistically significant( P > 0.05). Conclusion:HBO combined with folic acid,vitamin B12 and other neurotrophic drugs has a certain clinical effect in the treatment of nerve damage caused by N 2O poisoning,which is worthy of further study and improvement.

3.Efficacy analysis of endovascular therapy combined with hyperbaric oxygen intervention in acute anterior circulation macrovascular occlusive stroke

Objective:To explore efficacy and safety of endovascular therapy combined with hyperbaric oxygen (HBO) intervention in acute anterior circulation macrovascular occlusion stroke.Methods:A total of 150 patients with acute anterior circulation macrovascular occlusive stroke admitted in No.902 Hospital of the CPLA Joint Logistic Support Force were selected and randomly divided into endovascular treatment group and combined treatment group (endovascular+ HBO). Endovascular therapy was adopted in the treatment of all patients. Modified thrombolytic flow grading (mTICI) was used to evaluate vascular recanalization after operation, and the incidences of complications of hemorrhage transformation and vascular re-occlusion were recorded. National institutes of health stroke scale (NIHSS) was used to evaluate the neurological functions on admission, at 24 hours after the operation, and one course after HBO therapy. The prognosis was evaluated by observing the changes of blood homocysteine (HCY) and high-sensitivity C-reactive protein (hs-CRP) levels before and after treatment. The patients had been followed up by the modified Rankin Scale (mRS) for 90 days. At the same time, 75 patients with acute anterior circulation macrovascular occlusive stroke without taking endovascular treatment were selected as the control group.Results:Among 150 patients, 126 cases had a grade of mTICI≥2b, while 24 cases had a grade of mTICI<2b. Postoperative vascular re-occlusion occurred in 3 cases, and symptomatic intracranial hematoma occurred in 6 cases. The NIHSS score at 24 hours after the operation was significantly lower than that on admission, and the NIHSS score was further improved after HBO treatment. Ninety days after the operation, mRS showed 98 cases of good prognoses, while 52 cases of poor prognoses. The levels of HCY and hs-CRP were significantly lower than those before the treatment.Conclusion:Endovascular therapy for acute anterior circulation macrovascular occlusive stroke is safe and effective. Postoperative HBO therapy can significantly improve the prognosis and the quality of life of patients.

5.Effects of multi-mode revascularization combined with hyperbaric oxygen in the treatment of acute internal carotid artery occlusion

Objective To investigate the effects of multi-mode revascularization combined with hyperbaric oxygen (HBO) in the treatment of acute internal carotid artery occlusion.Methods One mid-aged female patient was admitted into hospital 1 hour after sudden onset of unconsciousness.Past medical history included rheumatic heart disease and atrial fibrillation.Digital subtraction angiography (DSA) showed occlusion at the distal end of internal carotid artery.With the knowledge and consent of the dependants,endovascular intervention treatment was performed and HBO therapy was supplemented after surgery.Results Due to the location,size and complexity of shapes of thrombi,re-canalization was not successful after trying in sequence stent thrombectomy,balloon catheter disruption of thrombi and contact arterial thrombolysis.Finally,the occluded vessel was re-canalized successfully by the catheter aspiration technique,and the prognosis of the patient improved significantly.Conclusions This case suggests that the combined use of multi-mode endovascular interventional technology is an effective method for re-canalizing occluded vessels.Supplemented HBO therapy after surgery is beneficial to the improvement of prognosis.
