The statement"Blood is the material carrier of spirit and qi"indicates that the generation of blood relies on the coordinated governance of spirit and qi,while simultaneously serving as their material foundation.Based on the physiological connection among"blood-spirit-qi",this article explores the pathogenesis of premature ovarian insufficiency(POI)accompanied by anxiety and depression. It proposes that essence and blood deficiency, premature exhaustion of Tian Gui(reproductive essence),and failure of blood to nourish the spirit form a pathological chain:"blood deficiency-spirit disturbance-POI with anxiety and depression".Blood stasis obstructing the uterine chamber, disorder of the chong and ren meridians, and failure of blood to carry the spirit give rise to the transformation:"blood stasis-spirit depression-POI with anxiety and depression".Liver qi stagnation and disruption of qi and blood further exacerbate the imbalance between blood and spirit, aggravating the disease progression.Based on this pathological analysis, the general treatment principle of"regulating blood, harmonizing spirit, and rectifying qi"is established. For patients with blood deficiency syndrome, treatment should focus on tonifying essence and blood, replenishing reproductive essence, and nourishing the spirit, selecting Guanyuan(CV4),Zusanli(ST36),and Sanyinjiao(SP6)as main acupoints. For patients with blood stasis syndrome, treatment should aim to regulate the chong and ren meridians, promote blood circulation, and calm the spirit, selecting Qichong(ST30),Zhongji(CV3),and Xuehai(SP10)as main acupoints.Simultaneously, the method of regulating the liver should be applied throughout the entire treatment process to soothe the liver, regulate qi,relieve depression, and calm the spirit, selecting Baihui(GV20),Taichong(LR3),and Ganshu(BL18)as main acupoints.Appropriate acupuncture techniques and methods should be chosen according to the patient's constitution and condition, providing new therapeutic ideas and approaches for clinical practice.

Objective:To explore the therapeutic characteristics of population with gout achieving treat-to-target (T2T) indicators through real-world research and evaluate their safety. Methods:A total of 3 287 patients diagnosed with gout by rheumatologists in 21 first-class tertiary hospitals in 10 provinces, municipalities, and autonomous regions in China from January 2015 to December 2021 were included in this polycentric cross-sectional study. The database included patients′ general information, disease characteristics, and clinical application of traditional Chinese and Western medicine treatment measures. SPSS and Excel software were used for data analysis. Frequency analysis, cluster analysis, and factor analysis were used to summarize the characteristics and rules of treatment measures for patients with gout who achieved the target after treatment. The occurrence of adverse events (AE) was recorded during treatment. Results:After treatment, 691 visits (7%) achieved the serum urate (SUA) target, and the most frequent use of urate-lowering therapy (ULT) was febuxostat, followed by benzbromarone. The most common treatment options were following: GroupⅠ: traditional Chinese medicine (TCM) decoction-TCM external treatment-physical exercise-proprietary Chinese medicine; GroupⅡ: ferulic acid-nonsteroidal anti-inflammatory drugs (NSAIDs); Group Ⅲ: allopurinol-sodium bicarbonate-benzbromarone; Group Ⅳ: glucocorticoid-colchicine; Group Ⅴ: febuxostat. A total of 5 898 visits (60%) chieved manifestations of joint pain VAS scores target, and the most frequently used drug to control joint symptoms was NSAIDs. The frequency of use of drugs to control joint symptoms were 2 118 times (usage rate reached 35.9%), while the frequency of ULT were 2 504 times (usage rate reached 42.5%), which was higher than the joint symptom control drug. The most common treatment options were following: Group Ⅰ: proprietary Chinese medicine-TCM decoction-TCM external treatment-physical exercise; Group Ⅱ: NSAIDs-colchicine hormones; Group Ⅲ: allopurinol, Group Ⅳ: benzbromarone; Group Ⅴ: febuxostat. A total of 59 adverse events occurred during treatment. Conclusion:The proportions of gout patients who reach target serum urate level & good control of joint symptoms are both very low, and ULT and anti-inflammatory prescription patterns are very different from international guidelines, so it is necessary to strengthen the standardized management of gout patients. At the same time, life intervention measures account for a certain proportion of the treatment plans for the T2T population, and further exploration is needed.

Cholesterol is an important precursor of many endogenous molecules. Disruption of cholesterol homeostasis can cause many pathological changes, leading to liver and cardiovascular diseases. CYP1A is widely involved in cholesterol metabolic network, but its exact function has not been fully elucidated. Here, we aim to explore how CYP1A regulates cholesterol homeostasis. Our data showed that CYP1A1/2 knockout (KO) rats presented cholesterol deposition in blood and liver. The serum levels of low-density lipoprotein cholesterol, high-density lipoprotein cholesterol and total cholesterol were significantly increased in KO rats. Further studies found that the lipogenesis pathway (LXRα-SREBP1-SCD1) of KO rats was activated, and the key protein of cholesterol ester hydrolysis (CES1) was inhibited. Importantly, lansoprazole can significantly alleviate rat hepatic lipid deposition in hypercholesterolemia models by inducing CYP1A. Our findings reveal the role of CYP1A as a potential regulator of cholesterol homeostasis and provide a new perspective for the treatment of hypercholesterolemia.

Chinese Journal of Medical Aesthetics and Cosmetology 2022;28(6):471-474
Objective:To observe the effect of continuous shaping and compression in the comprehensive treatment of auricle keloids. Methods:From June 2018 to April 2020, 40 patients with auricular keloids (53 ears) admitted to the Plastic Surgery Department of Henan Provincial People's Hospital were divided into surgical injection group (20 cases, 27 ears) and surgical injection compression group (20 cases, 26 ears) according to the treatment method. The patients were followed up for more than 18 months continuously; the clinical efficacy, satisfaction with auricle morphology, and the occurrence of infection and recurrence were observed in the two groups. Results:The effective rate of surgical injection compression group (24 ears, 92.31%) was higher than that of surgical injection group (16 ears, 59.26%), and the difference was statistically significant (χ 2=5.121, P<0.05). The Vancouver scar scale (VSS) scores of the two groups at 6 months and 12 months after treatment were significantly lower than that before treatment, and the VSS score of the surgical injection compression group was lower than that of the surgical injection group, the difference was also statistically significant ( P<0.05). The satisfactory rate of the surgical injection compression group (22 ears, 84.62%) was higher than that of the surgical injection group (13 ears, 48.15%), and the difference was statistically significant (χ 2=4.048, P<0.05). The recurrence rate of the surgical injection compression group was significantly lower than that of the surgical injection group (χ 2=5.779, P<0.05); there was no significant difference in the infection rate between the two groups of patients after treatment (χ 2=0.001, P>0.05). Conclusions:Surgical resection of auricle keloids combined with local injection of corticosteroids and local shaping and compression can significantly improve the clinical efficacy, maintain the shape of the auricle, and the therapeutic effect is satisfactory.
