1.Qualitative study on the reasons of medical treatment delay in patients with sudden hearing loss

Objective:To explore the reasons for medical treatment delay in patients with sudden hearing loss.Methods:This was a qualitative research. A purposive sampling method was used to select patients with sudden hearing loss who visited the Otolaryngology and Head and Neck Surgery Department at the First Hospital of Shanxi Medical University from January to March 2024. Data were collected through face-to-face, semi-structured interviews and analyzed using Colaizzi's 7-step method.Results:A total of 23 participants were interviewed, aged 19 to 72 years; six were male, and 17 were female. A total of four main themes and nine sub-themes were identified: insufficient disease awareness (lack of symptom related knowledge, rational attribution of symptoms, lack of symptom management ability) , physical and psychological perception differences, and negative emotional experiences (conflict with existing roles, negative emotional experiences) , socio-economic factors (insufficient social support, economic burden) , and limitations of medical services and resources (limitations in the diagnostic and treatment capabilities of primary healthcare institutions, insufficient medical resources) .Conclusions:The reasons for medical treatment delay in patients with sudden hearing loss are diverse. Healthcare providers can implement targeted interventions to help patients establish correct medical awareness, reducing the delay in seeking treatment.

The clinical data of a patient with mitochondrial diabetes mellitus complicated with hypopituitarism were analyzed, the patient′s mitochondrial gene was detected by microarray capture high-throughput sequencing, and the related domestic and foreign literature was reviewed and analyzed. The results showed that the patient had m. 3243 A>G variant on MT-TL1 gene and the clinical features were consistent with mitochondrial diabetes mellitus and hypopituitarism.
5.Influencing factors of quality of life of patients with osteoradionecrosis of the jaws

Objective To investigate the level and influencing factors of quality of life in patients with osteoradionecrosis of the jaws (ORNJ) in order to provide theory basis for nursing intervention. Methods Ninety-five patients hospitalized from January 2015 to Febury 2017 and participated in the investigation by a convenient sampling method. The functional assessment of cancer therapy-head and neck (FACT-H&N), social support rating scale (SSRS) and general information questionnaire were used to assess the quality of life, social support and basic information in ORNJ patients in our hospital respectively. Results The score of quality of life was (79.99 ±15.72). The index value of common module of quality of life was higher than that of additional concern. The average scores of each dimension of common module from low to high were as follows: functional well-being, social/family well-being, physical well-being and emotional well-being. The multiple linear regression analysis showed that the influencing factors of quality of life were social support, severe mouth opening difficulty (grade Ⅲ and Ⅳ), the course of primary disease (3 ~5 years, 5 ~10 years), which explained 35.6%variation in the quality of life. Conclusions The quality of life of ORNJ patients is generally lower than that of other head and neck cancer patients. The social support, severe mouth opening difficulty and course of primary disease can affect the patients' quality of life.The smaller the mouth opening (≤1 cm), the longer the course of primary disease (3~5 years) and the worse the quality of life is. Thus, nursing staff should pay attention to the quality of life of ORNJ patients and give prospective nursing intervention to improve the quality of life according to the characteristics of the development of the disease.
6.Expression and clinical significance of Gli1 in peripheral blood and cancer tissues of gastric cancer patients

Objective To explore the relationship and clinical significance of Gli1 expression in peripheral blood and cancer tissues of gastric.Method 95 gastric cancer patients were recruited as surgery group,and 40 recurrent gastric cancer patients were recruited as recurrent group.50 normal subjects were recruited as control group.Paraffin specimens of gastric cancer tissues and gastric mucosa adjacent to cancer were collected to test expression of Gli1 protein.ELISA was applied to test Gli1 in peripheral blood.Results Levels of Gli1 in peripheral blood of preoperative surgery group were higher than in control group [(9.21 ± 1.37) ng/ml vs.(0.79 ±0.18) ng/ml,t =43.193,P <0.001].Post-up Gli1 decreased significantly to the level of control group (t =0.722,P =0.472).Levels of Gli1 in peripheral blood of recurrent group were higher than that in preoperativesurgery group (t =14.575,P < 0.001).Gli1 protein in cancer tissues was higher than that in adjacent gastric mucosa (t =33.471,P <0.001),and Gli1 protein in cancer tissues of surgery group was higher than that in cancer tissues of recurrent group (t =8.808,P <0.001).Conclusion In postoperative gastric cancer patients increased Gli1 level in peripheral blood and cancer tissues predicts recurrence.
7.Protective effect of human umbilical cord-derived mesenchymal stem cells against severe acute pancreatitis in rats

Objective To study the protective effects of human umbilical cord-derived mesenchymal stem cells (ucMSCs)against severe acute pancreatitis (SAP) in rats.Methods A total of 135 Sprague-Dawley male rats were randomly divided into Sham group,SAP group and SAP+ucMSCs group (45 each).SAP+ucMSCs group:Severe acute pancreatitis was induced by injecting 5% sodium taurocholate (0.1ml/100g) into the common bilio-pancreatic duct and then CM-DiI-labeled ucMSCs at 1 × 107cells/kg were injected via the tail vein.All the rats were sacrificed 12,24 and 72 hours after SAP.The 72h death rate was counted.Pathological changes in the pancrease were detected by HE staining and pathological score was graded,ucMSCs colonization was observed by fluorescence microscopy.The serum levels of amylase,lipase,TNF-α,IL-1β,IL-4 and IL-10 were determined by ELISA.Results ucMSCs colonize the injured area of pancreatic tissue,the 72h death rate was reduced,and the serum amylase and lipase were also reduced significantly.Moreover,ucMSCs significantly reduced the pathological score of the pancrea and the level of proinflammatory cytokines (TNF-α and IL-1β),but the levels of anti-inflammatory cytokines were increased (IL-4 and IL-10).Conclusion Transplantation of ucMSCs can reduce the severity of pancreatic injury and inflammation in SAP rats.
8.Expressions of Krüppel-like factor 12 in cancer tissue and serum of patients with colorectal cancer and their significances

Objective:To detect the expressions of Krüppel-like factor 12 (KLF12)protein in the cancer tissue and serum of the patients with colorectal cancer,and to investigate their clinical values.Methods:A total of 120 colorectal cancer patients were selected as case group,and 100 normal cases were regarded as control group. The cancer tissue and adjacent mucosa tissue were collected.Immunohistochemical method was used to detect the expressions of KLF12,nm23,CyclinE1,matrix metalloproteinase-2 (MMP-2),and MMP-9 proteins,and the relationships among these proteins were evaluated.ELISA assay was applied to detect the serum KLF12 levels of the patients in case group before operation and 1 month after operation the levels of serum KLF12 of the subjects in control group.Results:The positive expression rates of KLF12,CyclinE1,MMP-2 and MMP-9 proteins in cancer tissue were higher than those in adjacent mucosa tissue (χ2 =66.155,52.795,64.515,52.632;P <0.001),while the positive expression rate of nm23 protein in cancer tissue was lower than that in adjacent mucosa tissue (χ2 =13.019,P <0.001).The Spearman analysis results showed that the expression of KLF12 protein was positively correlated with the expressions of MMP-2 and MMP-9 proteins (r=0.3963,P <0.001;r=0.3264,P <0.001);the expression of KLF12 was negatively correlated with the expression of nm23 protein (r = - 0.2273,P =0.013).The level of serum KLF12 of the patients in case group before operation was higher than that in control group.The cut-off value of ROC curve of serum KLF12 protein expression level was 3.795 μg·L-1 ,and the area under the ROC curve was 0.834.The sensitivity and specificity were 60.8% and 94.0%,respectively. After operation,the serum level of KLF12 protein in the patients in case group was decreased,which was still higher than that in control group (t=2.708,P =0.007).The expression level of KLF12 protein in cancer tissue was positively correlated with the level of serum KLF12 protein (r =0.4069,P <0.001).According to degree of lymph node metastasis,and the serum level of KLF12 protein in N3 group was the highest,and the lowest serum level of KLF12 protein was found in N0 group (F =21.731,P <0.001).Conclusion:KLF12 protein might be involved in the invasion and metastasis of colorectal cancer,and the detection of serum level of KLF12 protein could be valuable for the judgement of disease and prediction of lymph node metastasis of the patients.
9.Feasibility of waist-to-height ratio to evaluate the body composition of people aged 60-69 years in Hubei minority areas

Objective To evaluate the feasibility of waist-to-height ratio (WHtR) to reflect the body composition of people aged 60-69 years in Hubei minority areas.Methods A total of 398 eligible residents between 60-69 years old were selected through a stratified sampling method.Body height,weight,waist circumference,hip circumference and skinfold thickness were measured at the interview.Bland-Altman analysis was applied to estimate the consistency among WHtR,body mass index (BMI),waist-to-hip ratio (WHR),and Suzuki-changling formula.Results After standardization,the mean of WHtR and BMI was 0.007,the mean of WHtR and WHR was 0.007,and the mean of WHtR and Suzuki-changling formula was 0.711,which revealed that there were good consistency among WHtR,BMI,WHR and Suzuki-changling formula.Conclusions WHtR may serve as an typical index to reflect the body composition of people aged 60-69 years in Hubei minority areas.
10.Application of diagnosis -related group nursing quality control in patients with parotid pleomorphic adenoma

Objective To explore the effects of applying standards of diagnosis-related group ( DRG) nursing quality to patients with parotid pleomorphic adenoma .Methods Sixty-four patients in our department with parotid pleomorphic adenoma were randomly divided into control group ( n=32 ) and experimental group ( n=32 ) . The control group received traditional nursing care , while the experimental group received departmental standards of DRG ( parotid pleomorphic adenoma ) nursing.The nursing quality and effects were compared, as well as the satisfaction of patients .Results The related knowledge understanding rates in the experimental group and control group were 93.75% and 71.88%, respectively, which showed a significant difference (χ2 =7.540,P<0.05).And patients’ satisfaction rates in the experimental group and control group were 90.63% and 81.25%, respectively, which showed a significant difference (χ2 =13.506,P<0.05). There were significant differences in the mean of hospitalization days and hospitalization expenses (t=730.00, 883.50,respectively;P<0.05).Conclusions The new quality standards and template of DRG in patients with parotid pleomorphic adenoma can make nursing work more standardized , and can improve nursing quality effectively.The hospitalization days and expenses can be significantly reduced and the understanding of health education and patients ’ satisfaction can be improved .
