1.Establishment of the reference intervals of four biochemical parameters for liver function among adults in Changchun, China based on the indirect method

Objective To establish the reference intervals (RIs) of the four biochemical parameters for liver function [aspartate aminotransferase (AST), alanine aminotransferase (ALT), gamma-glutamyl transpeptidase (GGT), and alkaline phosphatase (ALP)] in adults by using the indirect method based on the data of subjects undergoing physical examination in laboratory information system (LIS). Methods With the help of the LIS, related data were collected from the healthy adults who underwent physical examination in Physical Examination Center of The First Hospital of Jilin University from October 2019 to October 2020. The Kolmogorov-Smirnov test was used to check the normal distribution of the original data; data with skewed distribution were transformed into data with approximate normal distribution using the BOX-COX method; the Turkey method was used to remove outliers; the Mann-Whitney U test or the Z -test was used for comparison between groups; the non-parametric method was used to calculate RIs. Results A total of 27 218 subjects were included after the removal of outliers. All four parameters showed age and sex differences. The RIs of AST were 14-39 U/L for male individuals aged 20-79 years, 12-32 U/L for female individuals aged 20-49 years, and 14-39 U/L for female individuals aged 50-79 years; the RIs of ALT were 10-71 U/L for male individuals aged 20-54 years, 10-49 U/L for male individuals aged 55-79 years, 7-43 U/L for female individuals aged 20-49 years, and 9-49 U/L for female individuals aged 50-79 years; the RIs of GGT were 11-70 U/L for male individuals aged 20-64 years, 10-64 U/L for male individuals aged 65-79 years, 6-45 U/L for female individuals aged 20-49 years, and 7-54 U/L for female individuals aged 50-79 years; the RIs of ALP were 38-96 U/L for male individuals aged 20-79 years, 33-89 U/L for female individuals aged 20-49 years, and 40-106 U/L for female individuals aged 50-79 years. The RIs of AST, ALT, and GGT established in this study were similar to those in the industry standards of China (relative deviation < reference change value), and the RIs of all four biochemical parameters were verified by applicability. Conclusion The RIs of the four biochemical parameters for liver function established by the indirect method are relatively consistent with those established by the direct method, which holds promise for application in clinical laboratory.
2.Thirty-year follow-up observation of the impact of iodine fortification strategy on intellectual development of children

Objective:To follow up and observe the intellectual development of school children aged 8 to 10 in Baicheng County, Xinjiang Uygur Autonomous Region, which was a severe iodine deficiency disorders region in history, before and after effective control of the disease, in order to evaluate the impact of iodine supplementation on protection of children's intellectual development and provide a theoretical basis for scientific supplements of iodine.Methods:From 1989 to 2018, Combined Raven's Test for Rural in China (CRT-RC) was used to observe the intellectual development status of 660 Uyghur school children aged 8 to 10 in Baicheng County in 1989, 2002, 2006, 2012 and 2018, respectively. Children's intelligence quotient (IQ) was calculated using CRT-RC's 1987 normal sample of rural children in the same age group; the data of average iodized salt coverage rate (C-IS) and childhood goiter rate (GR) from multiple local surveys and the median urinary iodine (MUIC) of children were collected, combined with the "Criteria for Elimination of Iodine Deficiency Disorders" (GB 16006-2008) and the United Nations International Children's Emergency Fund (UNICEF) recommended standards, the status of iodine deficiency during children's growth (IDG) was divided into complete exposure to iodine deficiency, no exposure, and semi-exposed. The Flynn effect (FE) gain was calculated using norm samples of children aged 8 to 10 in 1987, 1996 and 2006 of CRT-RC, and the differences in children's intellectual development after FE correction before and after IDG reached the standard were compared.Results:The IQ of children were (81.67 ± 14.13), (83.26 ± 14.05), (89.68 ± 13.58), (98.50 ± 14.33) and (103.23 ± 15.25) points in Baicheng County in 1989, 2002, 2006, 2012 and 2018, respectively, the difference between different years was statistically significant ( F = 58.357, P < 0.01). The three indicators of C-IS, GR, and MUIC didn't meet the standards during the IDG evaluation period in the 1989, 2002, and 2006 groups, which were the complete exposure to iodine deficiency; in the 2012 group, only the MUIC met the standard, which was semi-exposed; in the 2018 group, three indicators all met the standard, which was no exposure. The FE gains of 1987 with 1996, 1996 with 2006 were 0.96 points/year and 0.74 points/year, respectively; after FE correction, the actual gains of IQ of 2002 and 2006 compared with 1989, 2012 and 2018 compared with 2006 were - 9.57, - 6.11, 4.38, and 4.67 points, respectively. Conclusions:In iodine deficiency areas, intermittent iodine supplementation (1989 - 2009) for children exposed to iodine deficiency during growth still cannot effectively protect children's intellectual development; continuous and effective iodine supplementation (2010 - 2018) with iodized salt as the core and covering children's growth period has obvious positive effects on protection of children's normal intellectual development. In the future, we will continue to observe the influence of IDG full-cycle suitable iodine nutrition on children's intellectual development.
3.Diagnosis and treatment of transient osteoporosis of the hip.

OBJECTIVETo study diagnosis and therapeutic efficacy of transient oeteoporods of the hip (TOH).
METHODSFrom January 2005 to February 2010, 5 patients with TOH were treated with traditional methods. All the patients were male, with an average age of 38.6 years (ranged, 27 to 46 years). The clinical manifestation, physical examination and imageology characteristic was investigated. The therapeutic efficacy was evaluated by Harris hip score.
RESULTSAll the patients were followed up, the duration ranged from 12 to 36 months (averaged, 24 months). The Harris hip score before treatment were 63.1, 86.0, 74.9, 63.6 and 64.8 respectively, while after 6 months treatment, the scores improved to 90.5, 94.5, 89.7, 93.9 and 87.8 respectively. Moreover, 6 months later, the abnormal signal disappeared in MR imaging and X-ray.
CONCLUSIONTransient osteoporosis of the hip is a self-resolving condition and a self-limited disease, the expectant treatment is useful for it.

4.Effect of modified shegan mahuang decoction on cytokines in children patients with cough and variant asthma.

OBJECTIVETo study the influence of modified Shegan Mahuang Decoction (SGMH) on cytokines such as tumor necrosis factor-alpha (TNF-alpha), interleukin (IL)-10 and IL-13 in children suffered from cough and variant asthma (C&VA).
METHODSOne hundred and fifty-four children with C&VA were randomly assigned to two groups: 79 in the treatment group were medicated orally with SGMH one dose per day taking in twice; 75 in the control group were medicated with Montelukast once a day in dose of 4 mg for children aged from 2 to 5 years and 5 mg for those from 6 to 14 years, the medication for all was given 4 weeks. Serum contents of cytokines, including TNF-alpha, IL-10 and IL-13, in patients were measured before and after treatment. Besides, serum contents of these cytokines in 45 healthy children were measured for control.
RESULTSSerum levels of TNF-alpha and IL-3 in the treatment group were 2510 +/- 1500 ng/L and 60.76 +/- 23.67 ng/L, and in the control group, 2890 +/- 1410 ng/L and 61.56 +/- 20.37 ng/L, respectively, all were significantly higher than those of healthy (709 +/- 280 ng/L and 39.49 +/- 3.09 ng/L, P < 0.01); but level of IL-10 was significant lower in the two patient groups than that in control (1546 +/- 1434 ng/L and 1823 +/- 1314 ng/L vs 7123 +/- q2641 ng/L, P < 0.01). After treatment, the levels of TNF-alpha and IL-13 decreased and IL-10 increased significantly in the treatment group, and showed significant different to those in the control group respectively (960 +/- 420 ng/L, 43.67 +/- 12.37 ng/L and 6834 +/- 2216 ng/L vs 2610 +/- 1220 ng/L, 50.56 +/-19.56 ng/L and 2529 +/- 1223 ng/L, P < 0.01). Clinical efficacy between groups also showed that the total effective rate in the treatment group was significantly better (86.07% vs. 42.67%, P < 0.01).
CONCLUSIONSGMH can regulate the serum levels of TNF-alpha, IL-10 and IL-13, and shows evident clinical effect in treating children's C&VA.
