Objective:To analyze the clinical characteristics, RAN-binding protein 2 ( RANBP2) gene variations, and prognosis in Chinese acute necrotizing encephalopathy (ANE) patients. Methods:A retrospective analysis of ANE cases registered in the Peking Union Medical College Hospital Encephalitis Registry System from 2022 to 2024, involving patients from Peking Union Medical College Hospital and other hospitals, was conducted. A descriptive study was performed on the clinical characteristics, treatments and prognosis, cerebrospinal fluid examination results, and imaging findings of these patients based on adjusted ANE diagnostic criteria. Whole-exome sequencing technology was used to detect gene mutations in these patients. Results:A total of 18 ANE cases were included, ranged in age from 2 to 72 [20(5, 43)] years. The male-to-female ratio was 4∶5. All patients were found with precipitating infections including COVID-19, influenza A virus and Mycoplasma pneumoniae infections. All patients presented with fever, with varying degrees of consciousness disturbance observed in 16 cases, and seizures in 10 cases. All patients underwent lumbar puncture, with normal or mildly elevated white cell counts [3(2, 13)×10 6/L] and mildly to moderately elevated protein levels [1.90(0.92, 4.65) g/L]. A total of 6 patients were found with extremely elevated interleukin-6 level [950(164, 2 000) pg/ml] in cerebrospinal fluid. Bilateral symmetric thalamic lesions were typical imaging features of ANE, while involvement of other areas such as cortical and subcortical white matter, brainstem, and cerebellum was also observed. A total of 14 patients performed genetic tests while 4 patients were identified with RANBP2 gene mutations (c.1754C>T in 3 cases, c.1966A>G in 1 case). All patients received immunotherapy, and 7 patients died at discharge while other patients presented with neurological sequelae of varying degrees. Conclusions:ANE is a rare and severe parainfectious encephalopathy that can occur in both children and adults. Clinically, it is characterized by rapidly progressing encephalopathy following systematic infection, with bilateral symmetric thalamic lesions. The detection of RANBP2 gene mutations could help make the diagnosis.

Background and purpose:Accurately locating pulmonary nodules is the key to the success of thoracoscopic surgery. This study aimed to investigate the strategy and evaluate the feasibility, safety, and clinical value of thoracoscopic surgical treatment for pulmonary nodules located beneath the interlobar pleura and close to the pulmonary hilum. Methods:The patients who underwent pulmonary nodule surgery at Liaocheng Tumor Hospital from May 2023 to November 2024 were enrolled, and the patients who did not meet the inclusion criteria were excluded. This study was approved by the Ethics Committee of Liaocheng Tumor Hospital(EC-20240112-1020)and informed consent was obtained from the patients. The research was designed as a prospective single-arm study. The patients were treated with wedge resection, which was performed following CT-guided localization, where the location needle was inserted through the interlobar pleura. The feasibility of the procedure was evaluated by analyzing the success rate of preoperative localization and perioperative complications. Results:A total of 28 patients who met the inclusion criteria were included in this study. There were 5 male and 23 female patients with an average age of(56.0±8.5)years(range 38-69 years). In all,28 patients with 28 nodules underwent thoracoscopic wedge resection, and the preoperative CT-guided localization was successfully performed in all patients, without urgent complications. The mean operation time of thoracoscopic surgery was(15.6±4.0)min,intraoperative bleeding was(20.9±14.3)mL,and postoperative drainage was(214.3±62.2)mL.No cases of postoperative air leaks or conversion to thoracotomy were observed. The average length of hospital stay was(5.4±0.9)days.The postoperative histological diagnosis revealed 3 benign lesions(pulmonary fibrosis in 2 cases, atypical adenomatous hyperplasia in 1 case)and 25 malignant lesions(adenocarcinoma in situ in 5 cases, minimally invasive adenocarcinoma in 16 cases, and invasive adenocarcinoma in 4 cases). Conclusion:Thoracoscopic wedge resection following CT-guided nodule localization through the interlobar pleura is a feasible approach for nodules located beneath the interlobar pleura and close to the pulmonary hilum. The method ensures precise tumor localization, adequate margin, and minimal loss of normal lung tissue, with a low incidence of postoperative complication, which has important guiding significance for the surgical treatment of pulmonary nodules in such special locations.

Objective To study the clinical significance ofneutrophil gelatinase-associated lipocalin (NGAL), which in serial plasma and urine samples was measured in participants with lupus nephritis (LN)and healthy persons. Methods NGAL in serial plasma and urine samples was measured by enzyme-linked immunosorbent assay (EL.ISA) in 35 patients with LN by 1997 ACR systemic lupus erythematosus (SLE)standard with varied degree of kidney damage and 30 healthy persons with matching sex and age in physical examination center. Disease activity was measured by the SLE disease activity index (SLEDAI-2K),and 35LN patients were classified in active group and (25 cases) non-active group (10 cases) according to the SLEDAI-2K. Results Urinary NGAL were significantly increased in LN patients [(78.94 ± 81.97) μg/L]compared with healthy persons[(28.50 ± 18.08) μ g/L] (P = 0.002). And urinary NGAL were significantly increased in active group [(92.90 ± 94.88) μg/L] compared with non-active group [(48.20 ± 24.77)μ g/L] (P = 0.049). NGAL in serial plasma had no statistically significant difference between active group and non-active group (P >0.05). Conclusions NGAL in urine but not in plasma represents a novel biomarker for renal disease activity in LN. The increase might be related to renal tubule pathological changes.
