1.Medical resource consumption of healthcare-associated infection based on disease diagnosis-related grouping payment model

Objective To analyze the medical resource consumption of healthcare-associated infection(HAI)in patients in different groups of disease diagnosis-related grouping(DRG)based on the DRG payment model,provide reference for optimizing prevention and control of HAI as well as resource management.Methods Medical records and DRG-related indicators of discharged patients from a municipal hospital in Sichuan Province from January 1 to December 31,2024 were analyzed retrospectively.Medical resource consumption of patients in HAI group and non-HAI group was compared.Differences in average length of hospital stay and average expense per hospitalization be-tween two groups of patients were analyzed using stratified analysis.Results In 2024,HAI incidence of discharged patients in DRG management in this hospital was 1.57%.There were statistically significant differences in age,gender,admission and discharge ways between the HAI group and the non-HAI group(all P<0.05).The main HAI sites were lower respiratory tract,surgical site,urinary tract,and blood.The time consumption index(1.63 vs 0.85),average length of hospital stay(21.00 vs 5.00 days),expense consumption index(1.53 vs 0.92),ave-rage expense per hospitalization(44 700 vs 7 300),and multiple expense in HAI group were all higher than those in non-HAI group(all P<0.05).The consumption of medical resources for bloodstream infection was relatively higher.Patients with HAI were mostly concentrated in the groups related to acute leukemia with major complications or co-morbidities(MCC),intracranial or craniotomy surgery with MCC,tracheotomy with mechanical ventilation for 96 hours,as well as gastric,esophageal,and duodenal surgery.The average length of hospital stay and average ex-pense per hospitalization of patients in HAI group were both higher than those in the non-HAI group,differences were statistically significant(both P<0.05).Conclusion HAI significantly increase the consumption of medical resources.Based on DRG analysis,key disease groups for infection prevention and control can be further identified,and the consumption of medical resources can be more accurately and precisely evaluated,thereby optimizing the allocation of medical resources and improving hospital operational efficiency.
2.Study on Key Quality Control Indicators and Antioxidant Activity of Typical Propolis Samples from domestic and Abroad

OBJECTIVE To analyze the ethanol extracts,total phenols,total flavonoids contents and HPLC fingerprints of typi-cal propolis samples from 6 foreign countries and 5 domestic regions,optimize the extraction process and evaluate the antioxidant activi-ty,so as to provide data support for improving the quality control system of propolis.METHODS The optimization of the propolis ex-traction process utilized flavonoid content as an indicator.Three flavonoid detection methods-namely,the aluminum trichloride meth-od,aluminum nitrate method,and polyamide method-were compared.The content of ethanol extract,total phenol content,and the scavenging ability of DPPH free radicals for each sample were determined.Further analysis was conducted using HPLC fingerprint pro-filing.RESULTS The propolis extract with the highest flavonoid content was obtained using 80%ethanol as the extraction solvent,operating at 50℃,with a stirring time of 3 h,ultrasonic power of 180 W,and ultrasonic time of 15 min.The aluminum trichloride method was proved to be the most effective for determining total flavonoids in propolis.While the ethanol extract,total flavonoids,and total phenols of propolis from Xinjiang,China were relatively low,their antioxidant activity exhibited superiority.HPLC analysis re-vealed,Brazilian red propolis lacked of chrysin,galangin,caffeic acid phenethyl ester and Brazilian green propolis lacked ferulic acid,apigenin,p-coumaric acid,chrysin,and pinocembrin.In contrast,the content of these four compounds in other samples varied,with the antioxidant capacity of the extracts not precisely corresponding to the compound content.CONCLUSION Propolis exhibits a complex chemical composition with significant variations among varieties.Key quality control indexes must be comprehensively consid-ered,encompassing physicochemical characteristics and biological activity.Establishing a multi-angle assessment system with a mate-rial basis-functional linkage is essential.This approach facilitates the realization of high quality and cost-effectiveness,thereby promo-ting the healthy development of the industry.

5.Study on contract renewal willingness and influencing factors of contracted residents to family doctor service in Shandong Province

Objective:To understand the willingness of contracted residents to renew the family doctor contract service in Shandong Province, and to explore its influencing factors.Methods:From July to August 2020, 1 500 contracted residents in 3cities of Shandong Province were investigated by questionnaire survey.Descriptive statistical analysis, Mann-Whitney U test and binary logistic regression model were used to analyze the contracted residents′ cognition, utilization, satisfaction evaluation and renewal intention of family doctor contract service. Results:1 445 valid questionnaires were obtained, of which 682(47.2%)were willing to renew their contracts.The results of binary logistic regression analysis showed that marital status, educational level, time to see a doctor in contracted institutions, optimism about the development prospect of contracted service policy, whether the proportion of medical insurance reimbursement increased after signing the contract, whether follow-up work was carried out on time, satisfaction with family doctor service attitude and satisfaction with the effect of disease treatment were factors affecting the willingness of contracted residents to renew the contract.Conclusions:The contracted residents in Shandong Province have a high willingness to renew their contracts. On the basis of consolidating and improving the policy cognition and confidence of the contracted residents, we should actively optimize and improve the contracted service quality, ensure the sense of service access of contracted residents, and continuously and effectively realize the comprehensive promotion of the contracted services of family doctors.
6.Preliminary study of the changes in blood system in pyrrolizidine alkaloid-related liver damage

Objective:To preliminary explore the changes in blood system in pyrrolizidine alkaloids (PAs)-related liver damage.Methods:General situation, liver function, biochemical blood test, routine blood test, coagulation function markers, etc., of 77 cases with drug-induced liver damage admitted to the Zhongshan Hospital Affiliated to Fudan University from 2012 to 2019 were retrospectively analyzed. Patients’ were divided into PA group, other traditional Chinese medicine group and Western medicine group according to their medication history. Simultaneously, the changes in liver function were observed in the established mice model of monocrotaline-induced liver damage. Liver tissues HE staining and blood routine indexes were observed.Results:24 cases received PA, 24 cases received other traditional Chinese medicine, and 29 cases received western medicine. Alanine aminotransferase was lower in PA group than the other two groups ( P < 0.05), and the total bilirubin and direct bilirubin were significantly lower than the other traditional Chinese medicine group ( P < 0.05). The peripheral platelet count of the PA group was (84.11 ± 26.91) ×10 9/L, which was significantly lower than the lower limit of normal, and had statistically significant difference with other traditional Chinese medicine and western medicine group ( P < 0.01). Thrombocytocrit, mean platelet volume and platelet indices of PA group were statistically different from the other two groups ( P < 0.05). The D-dimer level in patients with PA group was (2.62 ± 1.93) mg/L, which was higher than the upper limit of normal, and significantly higher than the D-dimer level of the other two groups of patients ( P < 0.01). Meanwhile, prothrombin time was longer in PA group than that of the other two groups ( P < 0.01), and platelets count were decreased significantly in the mouse model of monocrotaline-induced liver damage after alanine aminotransferase and aspartate aminotransferase elevation ( P < 0.01). Conclusion:PA-related liver damage has lower peripheral platelet counts, and the peripheral platelet counts of these patients are lower than other types of drug-induced liver damage. In addition, increased D-dimer in patients with PA-related liver damage indicate a potential risk of thrombosis.
7.Cost-effectiveness analysis of antimicrobial susceptibility testing-guided therapy and empirical quadruple therapy in the treatment of Helicobacter pylori infection

Objective:To investigate the cost-effectiveness of Helicobacter pylori ( H. pylori) antimicrobial susceptibility testing-guided therapy versus an effective empiric modified bismuth quadruple therapy for first-line H. pylori treatment. Methods:This cost-effectiveness analysis was based on the data from a multicenter prospective randomized controlled clinical trial (NCT02935010), which included 382 patients with H. pylori infection. Among them, 286 cases were in the antimicrobial susceptibility testing group, which were diagnosed by biopsy under endoscopy. And according to the results of antimicrobial susceptibility testing of H. pylori, patients were treated with triple or quadruple therapy of eradicating H. pylori for 14 days. A total of 96 patients were in empirical therapy group, which were diagnosed by biopsy under endoscopy (96 cases, endoscopy empirical group), who were treated with 14-day empiric bismuth quadruple therapy. At the same time, based on the patients of empirical group, a simulated 13C-urea breath test (UBT) empirical group (UBT empirical group) was set up (96 cases, H. pylori infection was first diagnosed by 13C-UBT examination) and in the cost accounting the expenses related to endoscopy, biopsy and drug sensitivity test were removed and the expense of 13C-UBT was added. The information of patients of endoscopy empirical group was kept. Taking the eradication rate of H. pylori as an effectiveness indicator, the cost, cost-effective ratio (CER), incremental cost-effective ratio (ICER) among antimicrobial susceptibility testing group, endoscopy empirical group and UBT empirical group were compared by intention-to-treat (ITT) analysis and per-protocol (PP) analysis. Sensitivity analysis was conducted based on the results of ITT analysis to evaluate the reliability of the research results. Results:The results of ITT analysis indicated that the cost of antimicrobial susceptibility testing group, endoscopy empirical group and UBT empirical group were (1 747.41±149.30), 1 032.71 and 657.71 Yuan, respectively. The cost of antimicrobial susceptibility testing group was (714.70±149.30) and (1 089.70±149.30) Yuan higher than that of the endoscopy empirical therapy group and UBT empirical therapy group, respectively. The CER of antimicrobial susceptibility testing group, endoscopy empirical group and UBT empirical group were (19.08±1.49), 12.09 and 7.70 Yuan/%, respectively. ICER of antimicrobial susceptibility testing group was (115.27±1.49) and (175.76±1.49) Yuan /% in comparison with the endoscopy empirical group and the UBT empirical therapy group. Compared with that of empirical group, the ICER of antimicrobial susceptibility testing group was more likely to be affected by H. pylori eradication rate and the cost of antimicrobial susceptibility testing. When the H. pylori eradication rate decreased in the empirical group, the ICER of the antimicrobial susceptibility testing group also decreased; when the H. pylori eradication rate increased in antimicrobial susceptibility testing group, the ICER decreased, and vice versa. When the cost of antimicrobial susceptibility test was upregulated or downregulated by 20%, for every 1% increase in the eradication rate of H. pylori in comparison with that of the endoscopy empirical group, the cost of the antimicrobial susceptibility testing group increased by 99.15 and 131.40 Yuan; in comparison with that of the UBT empirical group, the cost of the antimicrobial susceptibility testing group increased by 159.63 and 191.89 Yuan. Conclusions:For patients diagnosed with H. pylori infection by 13C-UBT and without indications for endoscopy examination, the empirical quadruple therapy is the first choice. For patients intended to do endoscopy to confirm H. pylori infection, with additional affordability of 115.27 Yuan for every 1% increase in the eradication rate of H. pylori, antimicrobial susceptibility testing can be conducted and personalized. H. pylori eradication therapy can be carried out based on the results of antimicrobial susceptibility test. Reduced cost of the antimicrobial susceptibility test will be more conductive to the promotion of personalized treatment after antimicrobial susceptibility test.

Objective:
To investigate the association between the preterm birth and low birth weight and parental thalassemia.
Methods:
Pregnant women and their husbands receiving prenatal examination in local hospitals or maternal and child health centers in Jingxi and Debao in Guangxi from January to December 2017 were selected as study subjects. A total of 758 pregnant women with pregnancy outcomes and their husbands, who were both or alone diagnosed with thalassemia through thalassemia gene detection, were selected as case group and 758 pregnant women with pregnancy outcomes and their husbands, who were negative in thalassemia gene detection and hemoglobin electrophoresis test were selected as control groups. The case group were further divided into mother group, father group and both mother and farther group. Clinical and pregnancy outcome data of the study subjects were collected for the analysis on the association between parental thalassaemia and preterm birth or low birth weight by the independent sample
9.Clinical application of laparoscopic Roux-en-Y gastric bypass in patients with type 2 diabetes mellitus

Objective To evaluate the clinical effects of the laparoscopic Roux-en-Y gastric bypass (LRYGB) on patients with type 2 diabetes mellitus (T2DM).Methods From Dec.2013 to Dec.2015,83 cases with T2DM received LRYGB were enrolled to this study.They were divided into low body mass index group (LBMI,25≤BMI<28),intermediate body mass index group (IBMI,28≤BMI<32) and high body mass index group (HBMI,BMI≥33).Meanwhile,the clinical effects and side-effects were evaluated among three groups.The factors of adverse effects were evaluated by Cox multivariate analysis.Results Only one patient in HBMI was conversed to open surgery,all the others received LRYGB.The surgery time,intraoperative blood loss and complica tion were positively related to BMI,and the difference was statistically significant between the 3 groups(P<0.05).The main side-effect of LRYGB was gastrointestinal reaction.The level of fasting blood-glucose,postprandial 2 hours blood sugar,HbA1c and cholesterol total in three groups were lower than baseline (P<0.05),however,there was no significant difference among the three groups (P>0.05).The rate of clinical effects in three groups was 89.5%,89.3% and 91.7%.There was no significant difference among the three groups(P>0.05);In Cox multivariate analysis,cholesterol total and BMI were independent prognostic indicator for LRGB adverse.Conclusions LRYGB is a safe and effective method for patients with T2DM,which can effectively improve glucose and lipid metabolism.The increased BMI can increase the incidence of LRYGB complications.Total cholesterol is an independent prognostic indicator for the adverse of LRYGB.
10.Fetal Lateral Ventricular Dilatation Complicated with Malformation Diagnosed by Ultrasound and Its Relationship with Chromosome Abnormalities

Purpose To investigate the relationship between fetal lateral ventricle dilatation complicated with malformation and chromosome abnormalities,so as to provide reference for clinical consultation.Materials and Methods The ultrasound images,karyotype analysis and high resolution microarray comparative genomic hybridization (aCGH) results of 150 fetuses with lateral ventricular dilatation diagnosed by ultrasound were analyzed retrospectively.Results Among 150 lateral ventricular dilatation fetuses,81 cases were isolated lateral ventricle dilatation,30 cases were found complicated with fetal ultrasound soft index,22 cases with other CNS malformations and 17 cases with other malformations.Karyotype analysis of the above 4 groups showed 13 cases of abnormal karyotypes and 15 cases ofaCGH abnormalities.There was statistical significant difference (P<0.05) in each group of abnormal chromosome and aCGH detection rate,in which the softer marker's group had a significantly higher rate than the isolated ventriculomegaly's group in abnormal chromosome and aCGH (P<0.05).There was no statistical significant difference in any other groups (P>0.05).Conclusion Fetal systems should be carefully examined when prenatal ultrasound reveals lateral ventricular dilatation.The probability of abnormal chromosome increases significantly when complicated with fetal ultrasound soft index or other structural abnormalities.Chromosomal abnormalities need to be excluded and regular ultrasound follow-up is necessary in fetuses with isolated lateral ventricles.
