1.Effect of luteinising hormone levels on the outcome of in vitro fertilization and embryo transfer in an infertile population with normal ovarian reserve

ObjectiveTo investigate the effects of luteinizing hormone (LH) levels on in vitro fertilization and embryo transfer (IVF-ET) outcomes in an infertile population with normal ovarian reserve (NOR) undergoing controlled ovarian hyperstimulation (COH) with gonadotropin-releasing hormone antagonist protocol, and to explore the reasonable level intervals of LH during the follicular phase. MethodsA retrospective analysis was conducted on 911 NOR infertility patients undergoing IVF-ET treated with gonadotropin-releasing hormone antagonist protocol. Patients were grouped based on baseline LH (bLH) levels, LH levels on the day of antagonist addition, and LH levels on the day of human chorionic gonadotropin (hCG) injection. According to bLH levels, patients were divided into Group A2 (n=391) and Group B2 (n=391). Based on LH levels on the day of antagonist addition, patients were divided into Group A3 (n=600) and Group B3 (n=311). According to LH levels on the day of hCG injection, patients were classified into Group A4 (n=225), Group B4 (n=227), Group C4 (n=231), and Group D4 (n=228). The clinical characteristics, COH outcomes, and pregnancy outcomes of patients in each group were compared. Results① The bLH level could be used as a predictor of assisted reproduction outcomes in an infertile population with NOR, and going for assisted reproduction at bLH≥4.51 IU/L was more favorable for the development of pregnancy outcomes; ② Addition of antagonist at LH≥5 IU/L on the day of antagonist addition improved pregnancy outcome ;③ On the day of hCG injection, there was no statistically significant difference in pregnancy outcomes among the four groups; ④ Analyses using generalized linear models revealed that basal follicle-stimulating hormone (bFSH), basal estradiol (bE2), Gn days, Gn volume and 2PN affected the number of good quality embryos. ConclusionIn an infertile population with NOR using gonadotropin-releasing hormone antagonist, bLH and gonadotropin-releasing hormone antagonist addition day LH levels may influence IVF outcomes. LH levels should be controlled at 5 IU/L and above on antagonist addition days, and bLH levels should be controlled at 4.51 IU/L and above.
2.Application of thermal insulation blanket combined with self-heating patch in patients undergoing robotic surgery for thyroid cancer

Objective To explore the application effect of a thermal insulation blanket combined with a self-heating patch in patients undergoing robotic surgery for thyroid cancer.Methods A total of 130 patients with thyroid cancer who underwent robotic surgery were selected.Using the random sequence method,they were divided into control group[n=65,received conventional thermal insula-tion+a thermal insulation blanket]and observation group[n=65,received conventional thermal in-sulation+a thermal insulation blanket+a self-heating patch].The core body temperature,stress re-sponse before and at the end of surgery,the incidence of intraoperative shivering and hypothermia,as well as the awakening time,rewarming time,stay time in the post-anesthesia care unit(PACU),and thermal comfort were compared between the two groups.Results Compared with the control group,the observation group had higher core body temperature at 2 hours after anesthesia(T2),4 hours after anesthesia(T3),at the end of surgery(T4),upon entering the PACU(T5),and upon leaving the PACU(T6).During the T1 to T6 period,the control group showed a downward trend in body temper-ature,while the observation group remained relatively stable,with statistically significant differences(P<0.05).Compared with the control group,the observation group had lower stress levels at the end of surgery,a lower incidence of intraoperative shivering and hypothermia,with statistically signif-icant differences(P<0.05).Compared with the control group,the observation group had shorter awakening and rewarming time,higher thermal comfort scores,and a shorter stay time in the PACU,with statistically significant differences(P<0.05).Conclusion The combination of thermal insu-lation blanket and self-heating patch can improve the intraoperative core body temperature and ther-mal comfort of patients undergoing robotic surgery for thyroid cancer,reduce the incidence of intrao-perative shivering and hypothermia,alleviate stress responses,and shorten the awakening time,re-warming time,and stay time in the PACU.
3.Expression,Diagnostic Value,and Correlation Analysis of PCT,WBC,and CRP with CPIS Score in ICU Respiratory Infections

Objective To analyze the expression and diagnostic value of procalcitonin(PCT),white blood cell count(WBC),and C-reactive protein(CRP)in respiratory infections in the intensive care unit(ICU),and their correlation with the clinical pulmonary infection score(CPIS).Methods A total of 105 patients with respiratory infections admitted to the ICU from March 2019 to June 2024 were selected as the observation group,while 117 patients with no respiratory infection in the ICU were selected as the control group.PCT levels in both groups were measured using the Zybio-Q7 immunoquantitative analyzer,WBC levels were measured using XT-4000i white blood cell analyzer,and CRP levels were measured using the immunoturbidimetric method.The relationship between the expression of PCT,WBC and CRP and the clinical characteristics and CPIs score of respiratory infection in ICU were analyzed.Results The proportion of patients in the observation group with ICU stay>15 days and mechanical ventilation>7 days was significantly higher than that in the control group(P<0.05).Compared to the control group,the levels of PCT,WBC,CRP and CPIS score in the observation group were elevated,with statistically significant differences(P<0.05).Multivariate logistic regression analysis showed that ICU stay>15 days(OR=4.087),PCT(OR=6.543),WBC(OR=2.652),and CRP(OR=8.964)were risk factors for the occurrence of respiratory infections in the ICU.Pearson correlation analysis indicated a positive correlation between PCT and CPIS scores(r=0.925,P=0.001),a positive correlation between WBC and CPIS scores(r=0.739,P=0.001);and a positive correlation between CRP and CPIS scores(r=0.948,P=0.001).The ROC curve demonstrated that the combined diagnostic value of the three markers for ICU respiratory infections was higher than that of PCT,WBC,or CRP alone(P=0.002).Conclusion The combination of PCT,WBC,and CRP has high diagnostic value for ICU respiratory infections and is positively correlated with CPIs scores.Therefore,the above indicators can provide a reliable basis for the early diagnosis of respiratory infection in ICU.
4.Effect of An Enhanced Recovery After Surgery(ERAS)Nursing Protocol on Perioperative Outcomes in Patients Undergoing Thrombectomy for Acute Pulmonary Embolism

Objective To develop a perioperative nursing protocol based on the Enhanced Recovery After Surgery(ERAS)philosophy for patients undergoing percutaneous pulmonary thrombectomy for acute pulmonary embolism(PE),and to evaluate its clinical effectiveness.Methods A convenience sampling method was employed.Thirty patients admitted to the Third Department of General Surgery at Yan'an Hospital of Kunming from May 2023 to February 2024 were enrolled as the control group and received routine perioperative care.Another 30 patients admitted from March 2024 to December 2024 were enrolled as the experimental group and were managed according to the newly developed ERAS-based nursing protocol.The differences in length of hospital stay,quality of life(QoL),complication rates,and 30-day unplanned readmission rates between the two groups were compared.Results A significant difference was observed in the total score of the Pulmonary Embolism Quality of Life(PEmbQoL)questionnaire between the experimental and control groups(t=-15.83,P<0.01).The score for the experimental group was 60.34 points lower than that of the control group,substantially exceeding the minimal clinically important difference(MCID)of 15 points.Analysis of specific dimensions showed that the experimental group had significant improvements(P<0.01)in all six areas:symptom frequency(t=-8.188),activity limitation(t=-8.722),anxiety(t=-10.31),social functioning(t=-9.118),treatment satisfaction(t=-8.202),and overall impact(t=-9.1).The length of hospital stay was significantly shorter in the experimental group(7.57±0.96 days)than in the control group(10.17±2.44 days)(t=-5.456,P<0.01).No postoperative complications occurred in the experimental group(0/30),whereas the incidence rate in the control group was 16.7%(5/30),a statistically significant difference(χ2=5.455,P<0.05).Furthermore,there were no 30-day readmissions in the experimental group(0/30),compared to a rate of 13.3%(4/30)in the control group;this difference was also statistically significant(χ2=4.286,P<0.05).Conclusion This study provides preliminary validation for the effectiveness of a multidisciplinary nursing protocol based on the ERAS philosophy for patients undergoing percutaneous pulmonary thrombectomy.
5.Advances in pathogenesis of type 1 diabetes mellitus in children induced by COVID-19

Diabetes is a disorder of the metabolism of sugar,fat and protein caused by the absolute or relative lack of insulin secretion.Type 1 diabetes mellitus(T1DM)is the most important type of childhood diabetes,and its etiological mechanism is complex.Genetic susceptibility and environmental factors are the main causes of its pathogenesis,and respiratory virus infection may be one of the environmental factors leading to T1DM.A large number of epidemiological studies have shown that during the novel coronavirus infection(COVID-19)pandemic,new cases of T1DM and diabetic ketoacidosis in children have increased significantly,suggesting that COVID-19 may be a predispose factor for T1DM in children.Based on the literature at home and abroad,this paper discusses the overview of childhood diabetes,epidemiological studies on COVID-19 and T1DM,and the pathogenesis of T1DM,so as to provide new strategies and ideas for the prevention,management and treatment of T1DM in children induced by COVID-19.
6.Effect of evening primrose oil on aortic endothelial injury in rats with polycystic ovary syndrome based on network pharmacology

Objective To explore the effect of evening primrose oil(EPO)on aortic endothelial damage in rats with polycystic ovary syndrome(PCOS),using network pharmacology and in vivo experiments.Methods The potential targets of EPO for improving aortic endothelial injury in PCOS rats were predicted by network pharmacology,and the selected core targets and renin-angiotensin signaling(RAS)pathway were verified by experiments.Fifty-eight female SD rats were divided randomly into a blank group(n=10)and a modeling group(n=48).Rats in the blank group were fed a normal diet and rats in the modeling group received a high-fat diet for 8 weeks.The PCOS model was prepared at week 6 by administration of letrozole(1 mg/(kg·d))for 21 days.Blood was taken from the tail vein after modeling and serum was collected to detect hormone levels.The model rats were then divided randomly into four groups and treated with the corresponding drugs for 6 weeks.Blood,blood vessels,and ovaries were then collected.Tissue morphology was examined by hematoxylin and eosin staining and serum levels of luteinizing hormone(LH),testosterone(T),follicle-stimulating hormone(FSH),endothelin(ET-1),and tumor necrosis factor(TNF-α)were detected by enzyme-linked immunosorbent assay(ELISA).Serum levels of nitric oxide(NO)were determined by spectrophotometry.Protein expression levels of core targets and RAS pathway-related factors were assessed by western blotting and immunohistochemistry.Results Twenty-five intersection targets of EPO and PCOS were identified by network pharmacological analysis.Kyoto encyclopedia of genes and genomes analysis showed that EPO improved vascular injury in PCOS rats via multiple pathways,including RAS.Serum levels of ET-1,FSH,LH,and T measured by ELISA were significantly decreased after EPO treatment,compared with the model group(P<0.01).EPO significantly decreased the expression levels of Ang Ⅰ,VEGF-B,AT2R,ET-1,and TNF-α proteins in the aorta(P<0.01)and significantly increased expression levels of Ang Ⅱ,CD31,and endothelial NO synthase proteins(P<0.01).Conclusions EPO may ameliorate vascular endothelial injury in PCOS model rats by inhibiting the RAS signaling pathway and by overactivation of the ACE/Ang Ⅱ/AT1 axis.
7.Effects of Lycium barbarum glycopeptide on testes of senescent rats induced by D-galactose

To investigate the protective effect of Lycium barbarum glycopeptide(LbGp)on testicu-lar injury induced by D-galactose(D-gal)in aging rats,male SD rats were randomly divided into 6 groups:the blank control group(Control),aging model group(Model),positive control group(β-nicotinamide mononucleotide,NMN),low dose LbGp group(LLbGp),medium dose LbGp group(MLbGp)and high dose LbGp group(HLbGp).The testicular mass of rats was counted,the morphological changes of testicular tissue were observed by hematoxylin-eosin(HE)staining,the testicular senescence was detected by β-galactosidase(SA-β-gal)staining,and the levels of testos-terone(T),luteinizing hormone(LH)and follicle stimulating hormone(FSH)in serum were de-tected.The levels of oxidative factors such as glutathione peroxidase(GSH-Px),superoxide dis-mutase(SOD),malondialdehyde(MDA),catalase(CAT)and inflammatory factors such as tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β)and interleukin-6(IL-6)in rat tissues were measured.TUNEL staining was used to detect the apoptosis of testicular cells,epididymal sperm quality,and the expression of Keap1,Nrf2 and Nqo1 mRNA were analyzed.The results showed that compared with the Model group,the testicular coefficient of Lycium barbarum glycopeptide rats in MLbGp and HLbGp groups increased(P<0.01),the level of T in serum and sperm quality increased(P<0.05),the structural degeneration and aging of testicular tissue decreased(P<0.01),the level of antioxidant factors increased,and the levels of inflammatory factors and apopto-sis decreased(P<0.05).The expression of Keap1 decreased significantly(P<0.01),while the ex-pression of Nrf2 and Nqo1 mRNA increased(P<0.05).The above results indicate that Lycium barbarum glycopeptide can improve D-gal-induced testicular senescence,attenuate oxidative stress,reduce inflammatory response,decrease apoptosis,and exert a protective effect on testicular injury in rats due to senescence through the Keap1-Nrf2 signaling pathway.
8.3D Pulse Image Detection and Pulse Pattern Recognition Based on Subtle Motion Magnification Technology.

To address the problem of large reconstruction errors in 3D pulse signals caused by excessively small out-of-plane displacement of the contact membrane in the existing traditional Chinese medicine fingertip tactile binocular vision detection technology, this study proposes a 3D pulse image detection method based on subtle motion magnification technology and explores its application in pulse pattern recognition. Firstly, a 3D pulse image detection system based on binocular vision to obtain pulse image signals is developed as experimental data. Then, the phase motion video magnification algorithm is used to amplify the original signals, and the amplified signals are reconstructed in three dimensions to obtain 3D pulse signals. On this basis, nine features are extracted from the 3D pulse signals and features selection is performed using a two-sample Kolmogorov-Smirnov test. Finally, machine learning algorithms such as decision trees and random forests are used to identify the five types of pulse conditions: deep pulse, intermittent pulse, flooding pulse, slippery pulse, and rapid pulse. The experimental results show that compared to the methods without subtle motion magnification technology, the proposed method significantly improves waveform clarity, amplitude stability, and periodic regularity. Meanwhile, the average accuracy in pulse pattern recognition reaches 96.29%±0.26%.

9.Dihydromyricetin mitigates abdominal aortic aneurysm via transcriptional and post-transcriptional regulation of heme oxygenase-1 in vascular smooth muscle cells.

Abdominal aortic aneurysm (AAA) is a deadly condition of the aorta, carrying a significant risk of death upon rupture. Currently, there is a dearth of efficacious pharmaceutical interventions to impede the advancement of AAA and avert it from rupturing. Here, we investigated dihydromyricetin (DHM), one of the predominant bioactive flavonoids in Ampelopsis grossedentata (A. grossedentata), as a potential agent for inhibiting AAA. DHM effectively blocked the formation of AAA in angiotensin II-infused apolipoprotein E-deficient (ApoE-/-) mice. A combination of network pharmacology and whole transcriptome sequencing analysis revealed that DHM's anti-AAA action is linked to heme oxygenase (HO)-1 (Hmox-1 for the rodent gene) and hypoxia-inducible factor (HIF)-1α in vascular smooth muscle cells (VSMCs). Remarkably, DHM caused a robust rise (∼10-fold) of HO-1 protein expression in VSMCs, thereby suppressing VSMC inflammation and oxidative stress and preserving the VSMC contractile phenotype. Intriguingly, the therapeutic effect of DHM on AAA was largely abrogated by VSMC-specific Hmox1 knockdown in mice. Mechanistically, on one hand, DHM increased the transcription of Hmox-1 by triggering the nuclear translocation and activation of HIF-1α, but not nuclear factor erythroid 2-related factor 2 (NRF2). On the other hand, molecular docking, combined with cellular thermal shift assay (CETSA), isothermal titration calorimetry (ITC), drug affinity responsive target stability (DARTS), co-immunoprecipitation (Co-IP), and site mutant experiments revealed that DHM bonded to HO-1 at Lys243 and prevented its degradation, thereby resulting in considerable HO-1 buildup. In summary, our findings suggest that naturally derived DHM has the capacity to markedly enhance HO-1 expression in VSMCs, which may hold promise as a therapeutic strategy for AAA.

Ursodeoxycholic acid (UDCA) is a naturally occurring, low-toxicity, and hydrophilic bile acid (BA) in the human body that is converted by intestinal flora using primary BA. Solute carrier family 7 member 11 (SLC7A11) functions to uptake extracellular cystine in exchange for glutamate, and is highly expressed in a variety of human cancers. Retroperitoneal liposarcoma (RLPS) refers to liposarcoma originating from the retroperitoneal area. Lipidomics analysis revealed that UDCA was one of the most significantly downregulated metabolites in sera of RLPS patients compared with healthy subjects. The augmentation of UDCA concentration (≥25 μg/mL) demonstrated a suppressive effect on the proliferation of liposarcoma cells. [15N2]-cystine and [13C5]-glutamine isotope tracing revealed that UDCA impairs cystine uptake and glutathione (GSH) synthesis. Mechanistically, UDCA binds to the cystine transporter SLC7A11 to inhibit cystine uptake and impair GSH de novo synthesis, leading to reactive oxygen species (ROS) accumulation and mitochondrial oxidative damage. Furthermore, UDCA can promote the anti-cancer effects of ferroptosis inducers (Erastin, RSL3), the murine double minute 2 (MDM2) inhibitors (Nutlin 3a, RG7112), cyclin dependent kinase 4 (CDK4) inhibitor (Abemaciclib), and glutaminase inhibitor (CB839). Together, UDCA functions as a cystine exchange factor that binds to SLC7A11 for antitumor activity, and SLC7A11 is not only a new transporter for BA but also a clinically applicable target for UDCA. More importantly, in combination with other antitumor chemotherapy or physiotherapy treatments, UDCA may provide effective and promising treatment strategies for RLPS or other types of tumors in a ROS-dependent manner.
