2.Application of thermal insulation blanket combined with self-heating patch in patients undergoing robotic surgery for thyroid cancer

Objective To explore the application effect of a thermal insulation blanket combined with a self-heating patch in patients undergoing robotic surgery for thyroid cancer.Methods A total of 130 patients with thyroid cancer who underwent robotic surgery were selected.Using the random sequence method,they were divided into control group[n=65,received conventional thermal insula-tion+a thermal insulation blanket]and observation group[n=65,received conventional thermal in-sulation+a thermal insulation blanket+a self-heating patch].The core body temperature,stress re-sponse before and at the end of surgery,the incidence of intraoperative shivering and hypothermia,as well as the awakening time,rewarming time,stay time in the post-anesthesia care unit(PACU),and thermal comfort were compared between the two groups.Results Compared with the control group,the observation group had higher core body temperature at 2 hours after anesthesia(T2),4 hours after anesthesia(T3),at the end of surgery(T4),upon entering the PACU(T5),and upon leaving the PACU(T6).During the T1 to T6 period,the control group showed a downward trend in body temper-ature,while the observation group remained relatively stable,with statistically significant differences(P<0.05).Compared with the control group,the observation group had lower stress levels at the end of surgery,a lower incidence of intraoperative shivering and hypothermia,with statistically signif-icant differences(P<0.05).Compared with the control group,the observation group had shorter awakening and rewarming time,higher thermal comfort scores,and a shorter stay time in the PACU,with statistically significant differences(P<0.05).Conclusion The combination of thermal insu-lation blanket and self-heating patch can improve the intraoperative core body temperature and ther-mal comfort of patients undergoing robotic surgery for thyroid cancer,reduce the incidence of intrao-perative shivering and hypothermia,alleviate stress responses,and shorten the awakening time,re-warming time,and stay time in the PACU.

Objective To investigate the use of parenteral nutrition preparations in Jiangsu Province,and to provide reference for the standardized management of parenteral nutrition preparations.Methods 720 cases using parenteral nutrition preparations from January 2017 to June 2017 in the department of general surgery of 12 hospitals in Jiangsu province were selected.The rate of nutritional risk screening,the indications of parenteral nutrition,the way of infusion,the rationality and economy of the prescriptions were retrospectively evaluated.The calorie,amino acid content,non-protein calorie/nitrogen ratio,glycolipid ratio and cation concentration of the patients received total parenteral nutrition were calculated.Results The total costs of parenteral nutrition preparations of 720 cases were 1.614 1 millions,and 346 cases did not have the indications for parenteral nutrition.The results of prescription comment showed that only 16 patients were screened for nutritional risk by Nutritional Risk Screening 2002 tool at admission.544 cases were intravenous dripped with amino acid and fat emulsion from peripheral vein.In the 176 total parenteral nutrition prescriptions,there were 39 non-protein calorie/nitrogen ratio cases,15 glycolipid ratio cases,69 cation concentration cases,61 calorie cases and 32 amino acid content cases failing to comply with the recommendation of the guidelines.Only 31 total parenteral nutrition prescriptions were completely reasonable.Conclusion The costs of parenteral nutrition preparations used in hospitals of Jiangsu are high but the rate of rationality is low.Nutrition support team should be established to regulate the use of parenteral nutrition preparations and save medical resources.
5.Comparison of adverse reaction and nursing satisfaction in patients with chronic hepatitis B at different time points of first injection of interferon

Objective To observe adverse reaction and nursing satisfaction in patients with chronic hepatitis B at different time points of first injection of interferon.Methods Ninety patients were chosen and randomly divided into the observation group and the control group,each with 45 cases.The observation group received the first injection at 8:00 pm,and the control group received the first injection at 8:00 am The time duration of fever,the fever associated with significant discomfort symptoms and nursing satisfaction were compared in two groups.Results The time duration of fever was (5.2 ± 1.1) h in the observation group,and was (7.9 ± 1.4) h in the control group,and the difference was statistically significant (t =7.389,P < 0.05).The peak temperature was (37.9 ± 0.2) ℃ in the observation group,and was (38.4 ± 0.4) ℃ in the control group,and the difference was statistically significant (t =5.276,P < 0.05).The incidence rates of headache and dizziness,muscle pain and dyspnea in the observation group were lower than those in the control group,and the nursing satisfaction in the observation group was higher than that in the control group,and the differences were statistically significant (P < 0.05).Conclusions The first injection of interferon at 8:00 pm can effectively relieve the symptoms of fever,alleviate the fever associated with discomfort symptoms,and improve the nursing satisfaction.
