1.Effect of laparoscopic surgery on incarcerated inguinal hernia in children

Objective To evaluate the effect of laparoscopic surgery on incarcerated inguinal hernia(IIH)in children.Methods A total of 81 IIH children treated at Jinhua Maternal & Child Health Hospital from September 2018 to December 2023 were selected as subjects.The children were divided into laparoscopic surgery group(n=45)and open surgery group(n=36).Comparative analysis was conducted on admission age,gender,duration of incarceration,surgical timing,intraoperative hernia contents,and postoperative complications between two groups.Results Operative duration,intraoperative blood loss,postoperative bowel function recovery time,number of occult hernias,and hospitalization duration in laparoscopic surgery group were statistically better than those in open surgery group(P<0.05).There were fewer postoperative complications in laparoscopic group compared to open surgery group,there was significant difference between two groups(P<0.05).Conclusion Compared with traditional open surgery,laparoscopic surgery for IIH in children has the advantages of less trauma,shorter operation time and hospitalization time,and less intraoperative bleeding.
2.Observation on clinical efficacy and safety of single-port laparoscopic assisted testicular fixation for pediatric inguinal cryptorchidism

Objective To discuss the application value of single-port laparoscopy assisted testicular fixation in treating pediatric inguinal cryptorchidism.Methods A total of 128 children with inguinal cryptorchidism admitted to Jinhua Maternal and Child Health Hospital from June 2024 to March 2025 were enrolled in this study.Using a randomized digital table method,the children were divided into observation group(n=64)and control group(n=64).The observation group underwent single-port laparoscopy assisted testicular fixation surgery,while control group received conventional three-port laparoscopy surgery.Intraoperative blood loss,surgical duration,hospitalization time,hospitalization costs,and incision length,differences in inflammatory markers,pain stress indicators,sex hormone levels,and complications were compared between two groups.Results Compared to control group,the surgical duration and postoperative hospitalization time were shorter in observation group,intraoperative blood loss was lower in observation group,incision length was shorter in observation group,hospitalization costs was higher in observation group,and vancouver scar scale scores was lower in observation group(P<0.05).One day postoperatively,interleukin-6,superimposed C-reactive protein,and prostaglandin E2 were increased compared to pre-treatment levels in both groups.The reduction in observation group was more significant than that in control group(P<0.05).One day postoperatively,levels of neuropeptide Y,nerve growth factor,and substance P were decreased in both groups.The increase in observation group was more pronounced than that in control group(P<0.05).Conclusion Single-port laparoscopy is used for pediatric inguinal cryptorchidism with the advantages of minimally invasive,safety and fast recovery.It can effectively reduce postoperative inflammation and pain response,and does not affect sex hormone level and testicular development,with high clinical application value.
3.Role of METTL3-mediated m6 A modification in regulating miR-1224-5p in the proliferation and migration of prostate cancer cells

Objective:To investigate the biological role of miR-1224-5p in the proliferation,migration and apoptosis of prostate cancer cells,as well as its regulatory mechanism of expression.Methods:Human prostate cancer cells(PC3,DU145,LNCaP,22Rv1)and normal prostate epithelial RWPE-1 cells were selected for this study.The expression of miR-1224-5p in prostate cancer cells was detected by qPCR.miR-1224-5p mimic,inhibitor and corresponding negative control(NC)plasmids were transfected into PC3 and DU145 cells by liposome transfection technology,respectively.The transfection efficiency was verified by qPCR.The effects of miR-1224-5p mimic and inhibitor on cell proliferation,migration and apoptosis were detected by CCK-8 assay,plate clone formation assay,scratch healing assay and flow cytometry.The potential N6-methyladenosine(m6A)modification sites in the pri-miR-1224-5p sequence were predicted using SRAMP website,and the prediction results were verified using methylated RNA immunoprecipitation(MeRIP).The expression of methyltransferase 3(METTL3),a key methyltransferase involved in m6A modification,in prostate cancer cells was detected by qPCR.CCK-8 assay and Transwell assay were used to detect the effect of METTL3 siRNA transfection on the proliferation and migration of PC3 and DU145 cells.The regulatory effect of METTL3-mediated m6A modification on the expression of miR-1224-5p was detected by qPCR and MeRIP.Results:miR-1224-5p was upregulated in prostate cancer cells(all P<0.01).Transfection with miR-1224-5p mimic promoted the proliferation,migration and inhibited the apoptosis of PC3 and DU145 cells(P<0.05 or P<0.01).Conversely,miR-1224-5p inhibitor suppressed the proliferation,migration and induced apoptosis of PC3 and DU145 cells(P<0.05 or P<0.01).pri-miR-1224-5p contained m6A modification sites.METTL3 was highly expressed in prostate cancer cells(all P<0.01).Transfection of METTL3 siRNA inhibited the proliferation and migration of PC3 and DU145 cells(all P<0.01).METTL3-mediated m6A modification regulated the expression of miR-1224-5p.Conclusion:miR-1224-5p is upregulated in prostate cancer cells through METTL3-mediated m6A modification.Down-regulation of miR-1224-5p can inhibit the proliferation,migration and induce apoptosis of prostate cancer cells.

6.Prognostic impact of adjuvant therapy on patients with esophageal squamous cell carcinoma receiving neoadjuvant therapy plus esophagectomy: A systematic review and meta-analysis

Objective To investigate whether adjuvant therapy can bring survival benefits to patients with esophageal squamous cell carcinoma (ESCC) who have received neoadjuvant therapy plus esophagectomy. Methods Studies were identified by searching databases including PubMed, EMbase, Web of Science, The Cochrane Library and CNKI from inception to November 2022 to collect studies which conformed to the objective of this study. Clinical outcomes including overall survival (OS) and recurrence-free survival (RFS) were extracted from eligible studies after screening. RevMan 5.4 and Stata 14.0 were used to perform the meta-analysis. Results A total of 9 studies were selected including 1 340 patients. Compared with the neoadjuvant therapy plus surgery (NS) group, the neoadjuvant therapy plus surgery+adjuvant therapy (NS+A) group had no significant benefit in the OS [HR=0.88, 95%CI (0.75, 1.02), P=0.09], but had remarkable benefit in the RFS [HR=0.75, 95%CI (0.58, 0.97), P=0.03]. Subgroup analysis by nodal status showed that adjuvant therapy could improve the RFS of patients with node-positive disease. Prolonged OS was observed in the patients with both positive and negative nodes but not in the patients with only positive nodes. In terms of the subgroup analysis by prescription, it revealed that triple agents exhibited advantages in improving RFS but not OS. However, dual agents did not bring additional survival benefits to the NS+A group compared with the NS group. Subgroup analysis by adjuvant therapy indicated that neither postoperative chemoradiotherapy nor chemotherapy improved OS, whereas postoperative chemoradiation elongated RFS. Conclusion Adjuvant therapy can improve the prognosis of patients with ESCC after neoadjuvant therapy followed by esophagectomy.
7.Recent advances on the early clinical diagnosis and surgical indications for neonatal necrotizing enterocolitis

Necrotizing enterocolitis (NEC) is one of the most frequent and severe gastrointestinal diseases among neonates,especially preterm newborns.Its incidence is inversely associated with birth weight and gestational age.NEC is characterized by widespread or localized necrosis of the small intestine and colon,and intestinal perforation,septic shock and other complications can occur in severe cases.The overall survival rate of NEC has been improved in recent years.However,there is no uniform standards for early diagnosis and surgical intervention.Several biomarkers have been proposed for the early diagnosis of NEC and for the prediction of disease progression and severity.This review summarized the progress on early diagnosis and surgical indications of NEC.

In the past ten years, the research and application of microbiome has continued to increase. The microbiome has gradually become the research focus in the fields of life science, environmental science, and medicine. Meanwhile, many countries and organizations around the world are launching their own microbiome projects and conducting a multi-faceted layout, striving to gain a strategic position in this promising field. In addition, whether it is scientific research or industrial applications, there has been a climax of research and a wave of investment and financing, accordingly, products and services related to the microbiome are constantly emerging. However, due to the rapid development of microbiome sequencing and analysis related technologies and methods, the research and application from various countries have not yet unified on the standards of technology, programs, and data. Domestic industry participants also have insufficient understanding of the microbiome. New methods, technologies, and theories have not yet been fully accepted and used. In addition, some of the existing standards and guidelines are too general with poor practicality. This not only causes obstacles in the integration of scientific research data and waste of resources, but also gives related companies unfair competition opportunity. More importantly, China still lacks national standards related to the microbiome, and the national microbiome project is still in the process of preparation. In this context, the experts and practitioners of the microbiome worked together and developed the consensus of experts. It can not only guide domestic scientific research and industrial institutions to regulate the production, learning and research of the microbiome, the application can also provide reference technical basis for the relevant national functional departments, protect the scale and standardized corporate company's interests, strengthen industry self-discipline, avoid unregulated enterprises from disrupting the market, and ultimately promote the benign development of microbiome-related industries.

9.Transumbilical single-site laparoscopic surgery for congenital duodenal obstruction in neonates.

OBJECTIVETo evaluate the efficacy and safety of transumbilical single-site laparoscopic surgery for congenital duodenal obstruction (CDO) in neonates.
METHODSA retrospective analysis of clinical data of 15 patients with CDO undergoing transumbilical single-site laparoscopic treatment during November 2017 and January 2018 (single-site group), and 20 patients with CDO undergoing conventional three-hole laparoscopic treatment during August 2017 and October 2017 (three-hole group) was performed. All patients were from the Children's Hospital, Zhejiang University School of Medicine. The operation time, time of initial feeding, time of adequate feeding, length of hospital stay after operation and postoperative complications were compared between two groups.
RESULTSThe operations were completed in all patients. No patient converted to laparotomy, and no massive hemorrhage was observed during operation. The operation time of single-site group was (90±10) min for patients with duodenal diamond-shaped anastomosis and (81±15) min for patients with Ladd operation, while those of three-hole group were (85±9) min and (72±11) min, respectively. Postoperative initial feeding time of single-site group was (5.0±1.0) d, and that of the three-hole group was (4.8±0.8) d. The adequate feeding time was (9.0±1.2) d in the single-site group, and (9.3±0.8) d in the three-hole group. The length of hospital stay after operation was (11.2±2.5) d in the single-site group, and (11.5±2.8) d in the three-hole group. There was no significant difference in operation time, postoperative initial feeding time, adequate feeding time and length of hospital stay after operation between two groups (all >0.05).
CONCLUSIONSTransumbilical single-site laparoscopic surgery for CDO in neonates is safe and effective, and the postoperative abdominal scar is more hidden.
10.Efficacy of mirror therapy for stroke patients with hemiplegia

Objective To evaluate the efficacy of mirror therapy for stroke patients with hemiplegia.Methods Eighty stroke patients with hemiplegia were randomly divided into study group and control group with 40 cases in each group.Conventional rehabilitation training was given to patients in both groups;patients in study group were given additional mirror therapy for 24 weeks.The Fugl-Meyer Assessment (FMA) scale was used to assess the motor function,Functional Independence Measurement (FIM) scale to assess the activity of daily living,Modified Barthel Index to assess the ability of daily living,Brunnstrom Stage Scale to evaluate the states of the upper extremities,and the modified Ashworth Scale to evaluate muscle spasticity before and after 24-week treatment.Results Mter 24 weeks of treatment,there were significant differences in FMA score [(44.08 + 2.44) vs.(40.53 +2.11),t =7.07,P<0.05,FIM score [(71.08 + 5.37) vs.(64.18 + 4.47),t =7.20,P < 0.05],modified Barthel index score [(59.75+7.84) vs.(54.25 +8.88),t =3.03,P<0.05 and Brunnstrom stage (Z=-3.000,P< 0.05) between the study group and the control group.However,there was no significant difference in modified Ashworth score between the two groups of patients (Z =-1.732,P > 0.05).Conclusion Mirror therapy can improve upper limb and hand function of the stroke patients with hemiplegia and enhance their ability of daily life activities;however,it has no significant effect to improve the limb spasm.
