1.Clinical Observation on the Treatment of Non-Valvular Paroxysmal Atrial Fibrillation with Self-Prescribed Chaihu Jia Longgu Muli Decoction Derivative Based on the Pathogenesis Theory of"Mutual Fanning of Liver Wind and Heart Fire"

OBJECTIVE To explore the clinical efficacy of Chaihu Jia Longgu Muli Decoction Derivative in the treatment of non-valvu-lar paroxysmal atrial fibrillation based on the pathogenesis theory of"mutual fanning of liver wind and heart fire".METHODS A total of 80 patients with non-valvular paroxysmal atrial fibrillation admitted to Hainan Provincial Traditional Chinese Medicine Hospital from January 2021 to July 2022 were prospectively selected as the research subjects and randomly divided into an observation group and a control group with 40 cases in each group.The control group was given conventional treatment such as western medicine anticoagulation and ventricular rate con-trol.The observation group was given Chaihu Jia Longgu Muli Decoction Derivative on the basis of the control group.The treatment course of both groups was 30 d.The two groups were compared in terms of TCM syndrome scores,atrial fibrillation episodes(number of episodes,du-ration),clinical efficacy,changes in electrocardiogram parameters(P wave duration,P wave dispersion),heart rate variability indexes[standard deviation of RR interval(SDNN),root mean square difference of adjacent RR intervals(RMSSD),standard deviation of the mean value of RR interval(SDANN)],left ventricular function indexes[left atrial diameter(LAD),left ventricular ejection fraction(LVEF),ear-ly diastolic mitral annular motion velocity(EM)and left ventricular early diastolic blood flow propagation velocity(VP)]and the occurrence of adverse reactions.RESULTS After treatment,the TCM syndrome scores of the two groups of patients were significantly reduced(P<0.05,P<0.01),and the observation group was better than the control group(P<0.05,P<0.01);the number and duration of atrial fibrilla-tion in the two groups were significantly decreased(P<0.05,P<0.01),and the observation group was superior to the control group(P<0.05,P<0.01);the clinical efficacy of the observation group was better than that of the control group(P<0.05);the heart rate variability indexes,electrocardiogram parameters and left ventricular function indexes were significantly improved(P<0.05),and the observation group was supe-rior to the control group(P<0.05);there was no significant difference in the occurrence of adverse reactions between the two groups(P>0.05).CONCLUSION Based on the pathogenesis theory of"mutual fanning of liver wind and heart fire",the application of Chaihu Jia Longgu Muli Decoction Derivative for the treatment of non-valvular paroxysmal atrial fibrillation can improve clinical symptoms,reduce atrial fibrillation,enhance cardiac function,and has good clinical efficacy.

3.Characteristics of antibiotic resistance in group A Streptococcus strains isolated from children with scarlet fever

Objective To analyze the characteristics of antibiotic resistance in group A Streptococ-cus ( GAS) strains isolated from children with scarlet fever in Tianjin in order to provide reference for clinical drug administration. -ethods GAS strains were collected from 2011 to 2016. A total of 276 isolates were analyzed by antibiotic susceptibility test and emm typing. Results All of the isolates were susceptible to penicillin, cefazolin and vancomycin, while 98. 2% were susceptible to both chloramphenicol and levofloxa-cin. The resistance rates to azithromycin, erythromycin, clarithromycin, clindamycin and tetracycline were 97. 8%, 97. 1%, 94. 2%, 94. 2% and 79. 3%. The concomitant resistance to erythromycin, azithromycin, clarithromycin, clindamycin and tetracycline was 73. 2%. The resistance rates of GAS strains isolated from different years to tetracycline, clindamycin, clarithromycin, erythromycin and azithromycin were significantly different. A statistically significant difference was found between the percentages of emm12 and emm1 strains resistant to tetracycline (84. 0% vs 59. 5%, χ2=13. 820, P=0. 000). Conclusions The isolated GAS strains are sensitive toβ-lactams and highly resistant to macrolide antibiotics, clindamycin and tetracycline. Penicillin remains the preferred treatment for GAS infection and cephalosporins may be used as a substitute if the patient is allergic to penicillin.

Objective To determine whether the different pathological types of astrocytic tumours can be differentiated by diffusion tensor MR imaging. Methods In this study, diffusion tensor MR imaging was performed preoperatively in 67 patients with different pathological types of astrocytic tumours. Furthermore , T1WI, T2WI, FLAIR and T1WI enhancement scanning by GD-DTPA were performed preoperatively in all patients. The Mean diffusivity (ADC) values in the tumor parenchyma and normal white matter from different pathological types of astrocytic tumours were measured and analyzed. The ADC with the tumor parenchyma and normal white matter from the tumours were statistically analyzed by SPSS 19.0. The results were expressed as ± s and P < 0.05 was regarded as statistically significant differences. Results There were many differences with ADC (< 10-9 mm2/s) in the tumor parenchyma between fibrillary astrocytoma (1.48 ± 0.12、0.72 ± 0.05) from fat cell astrocytoma (1.31 ± 0.07, 0.69 ± 0.03), There were also many differences with ADC ( < 10-9 mm2/s) in the tumor parenchyma between fibrillary and fat cell astrocytoma from anaplastic astrocytoma (1.06 ± 0.11, 0.71 ± 0.04) and giant cell glioblastoma (0.98 ± 0.09, 0.73 ± 0.04). There were not differences with ADC in the tumor parenchyma between anaplastic astrocytoma from giant cell gliobastona. Conclusion Measurement of tumor parenchyma′s ADC values can identify different pathological types of astrocytic tumours , but not between anaplastic astrocytoma and giant cell glioblastoma. DTI is essential to preoperative evalution of astrocytic tumours.
6.Characterization of the tunicamycin gene cluster unveiling unique steps involved in its biosynthesis.

Tunicamycin, a potent reversible translocase I inhibitor, is produced by several Actinomycetes species. The tunicamycin structure is highly unusual, and contains an 11-carbon dialdose sugar and an α, β-1″,11'-glycosidic linkage. Here we report the identification of a gene cluster essential for tunicamycin biosynthesis by high-throughput heterologous expression (HHE) strategy combined with a bioassay. Introduction of the genes into heterologous non-producing Streptomyces hosts results in production of tunicamycin by these strains, demonstrating the role of the genes for the biosynthesis of tunicamycins. Gene disruption experiments coupled with bioinformatic analysis revealed that the tunicamycin gene cluster is minimally composed of 12 genes (tunA-tunL). Amongst these is a putative radical SAM enzyme (Tun B) with a potentially unique role in biosynthetic carbon-carbon bond formation. Hence, a seven-step novel pathway is proposed for tunicamycin biosynthesis. Moreover, two gene clusters for the potential biosynthesis of tunicamycin-like antibiotics were also identified in Streptomyces clavuligerus ATCC 27064 and Actinosynnema mirums DSM 43827. These data provide clarification of the novel mechanisms for tunicamycin biosynthesis, and for the generation of new-designer tunicamycin analogs with selective/enhanced bioactivity via combinatorial biosynthesis strategies.

7.Study on coronary artery imaging with dual source computed tomography in senile people

Objective To explore the image quality of coronary artery imaging with dual source computed tomography (DSCT) and analyze the coronary artery lesions. Methods Two hundred cases with clinically suspected coronary heart disease, who underwent plain and enhanced DSCT coronary artery imaging without oral Betaloc preparation were retrospectively analyzed. The patients were divided into two groups: the senile group of 126 cases with the average age of (71.3±7.7) years old and the control group of 74 cases with the average age of (50.4±6.2) years old. The clinical characteristics, the average heart rate and breath-holding time for plain and enhanced scan, the average calcium score, the image quality evaluation and the coronary stenosis were compared between the two groups. Results Compared with the control group, the incidences of chest pain or discomfort, hypertension, diabetes mellitus, hypercholesterolemia and old myocardial infarction were significantly higher in the senile group. The average calcium score and the numbers of <50% and ≥50% stenosis were also significantly higher in the senile group than those in the control group (364.9±442.6)points vs. (44.8±106.9)points, 159 vs. 45 branches, 212 vs. 32 branches, respectively, all P<0.01). The average heart rate and breath-holding time of plain and enhanced scan had no significant differences compared with the control group. The proportion of the coronary segments with grade 1 ,grade 2 and grade 3 image quality also had no significant difference compared with the control group (96.5%,2.3% and 1.2% vs. 97.1%, 2.0% and 0.9%, P>0.05). Conclusions DSCT coronary artery imaging in senile patients is noninvasive, safe and simple, and excellent coronary artery image can be obtained without oral Betaloc preparation. It can be used as a routine examination in the clinical diagnosis of coronary heart disease.

Objective To explore the scanning technique and image quality of coronary artery imaging with dual-source CT without oral Betaloc preparation in the patients with high heart rate.Methods 412 cases were undergone coronary imaging with dual-source CT (including plain and enhanced scans) ,among them,there were 30 cases with heart rate more than 100 bpm.Multi-planar reconstruction(MPR),maximum intensity projection(MIP) and volume rendering (VR) were performed using contrast-enhanced images.The image quality was classified into 3 grades, and coronary segments named according to AHA standard were evaluated.Results The average heart rate during enhanced scan in the 30 cases was (115.6?11.8)(101~139)bpm,the average breath hold time was (5.7?1.2) s.The best reconstruction phase was in the systolic phase. Altogether 424 coronary segments were evaluated, among them 93.9%(398/424)belonged to the first grade,5.0%(21/424)belonged to the second grade,and 1.2%(5/424) belonged to the third grade. Conclusion Without oral administration of Betaloc preparation, good coronary artery images can be obtained in the patients with high heart rate by dual-source CT.
