Objective To obtain the data of radiological diagnosis and treatment resource distribution at medical institutions of different levels and in various cities, understand the status of resource allocation, provide policy-making basis and suggestions for optimizing the allocation of radiological diagnosis and treatment resources within the province, and offer data and references for related research. Methods A basic situation questionnaire survey was conducted on radiological diagnosis and treatment institutions in Hunan Province. Data were reviewed, analyzed, and statistically processed using Excel software to understand the allocation situation of radiological diagnosis and treatment resources in Hunan Province. Results As of 2022, there were 3031 radiological diagnosis and treatment institutions, 19770 radiation workers, and 6617 pieces of radiological diagnostic and treatment equipment. There were 80 radiation treatment institutions, 49 nuclear medicine institutions, 178 interventional radiology institutions, and 3028 X-ray imaging diagnosis institutions. There were 1077 radiation workers in radiation treatment, 461 radiation workers in nuclear medicine, 3369 radiation workers in interventional radiology, and 14,863 radiation workers in X-ray imaging diagnosis. There were 135 pieces of radiation treatment equipment, 56 pieces of nuclear medicine equipment, 262 pieces of interventional radiology equipment, and 6164 pieces of X-ray imaging diagnosis equipment. Conclusion Primary and lower grade medical institutions in Hunan Province account for a relatively high proportion of the total number of medical institutions, whereas radiological diagnostic and treatment resources are predominantly concentrated in tertiary medical institutions, demonstrating a unbalanced allocation of radiological diagnosis and treatment resources in various cities. In terms of the overall allocation of radiological diagnosis and treatment resources, interventional radiology is superior to the national average, nuclear medicine and X-ray imaging diagnosis are comparable to the national averages, and radiation treatment is lower than the national average. The total GDP is not the decisive factor in determining the development of local medical levels. It is suggested that relevant authorities should improve the rationality of the planning for the allocation of radiological diagnosis and treatment resources, promote the allocation of radiological diagnosis and treatment resources to the grassroots and underdeveloped areas through various measures, and thus achieve the goal of balanced resource allocation.

OBJECTIVE To investigate the protective effect and mechanism of saikosaponin C(SSC)on acute liver injury(ALI)in mice induced by carbon tetrachloride(CCl4)based on serum metabolomics.METHODS Forty mice were divided into blank group(water),model group(water),positive control drug group(Biphenyl diester drop pills,150 mg/kg),and SSC low-and high-dose groups(2.5,10 mg/kg)using the random number table method,with 8 mice in each group.They were given water/relevant drugs,once a day,for 7 consecutive days.One hour after the last administration,all mice were intraperitoneally injected with 0.2％CCl4 olive oil to induce ALI model,except for the blank group.After 17 hours of the modeling,the liver index of mice was calculated.The levels of aspartate aminotransferase(AST),alanine aminotransferase(ALT),lactate dehydrogenase(LDH),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),and IL-1β in serum of mice were detected.The histopathological changes of liver tissue were observed.Meanwhile,the serum metabolomics of mice were analyzed by liquid chromatography-mass spectrometry.RESULTS Compared with the blank group,the levels of liver index,ALT,AST,LDH,TNF-α,IL-6,and IL-1β in the model group were significantly increased(P＜0.01).Hepatocytes were edema,vacuolar degeneration,more necrosis,and a large number of inflammatory cells were infiltrated.Compared with the model group,liver index and serum index levels of mice were significantly decreased(P＜0.05 or P＜0.01),accompanied by marked improvement in histopathological damage to the liver tissue.The metabolomics results showed that compared with the model group,there were 63 up-regulated and 256 down-regulated differential metabolites in the serum of mice in the SSC high-dose group,including prostaglandin B2,20-hydroxy-leukotriene B4,5-hydroxy-L-tryptophan,7α-hydroxycholesterol,etc.;these metabolites were primarily involved in metabolic pathways such as arachidonic acid metabolism,5-hydroxytryptamine synapse,primary bile acid biosynthesis.CONCLUSIONS SSC exerts a protective effect against CCl4-induced ALI by down-regulating the level of key metabolites such as prostaglandin B2 and 20-hydroxy-leukotriene B4,and then ruducing metabolic pathways such as arachidonic acid metabolism,5-hydroxytryptamine synapse,and primary bile acid biosynthesis.

Objective: To explore the feasibility of synthetic magnetic resonance imaging (syMRI) combined with clinicopathological characteristics for the pre-treatment prediction of chemoradiotherapy (CRT) response in advanced nasopharyngeal carcinoma (ANPC). Materials and Methods: Patients with ANPC treated with CRT between September 2020 and June 2022 were retrospectively enrolled and categorized into response group (RG, n = 95) and non RGs (NRG, n = 32) based on the Response Evaluation Criteria in Solid Tumors (RECIST) 1.1. The quantitative parameters from pre-treatment syMRI (longitudinal [T1] and transverse [T2] relaxation times and proton density [PD]), diffusion-weighted imaging (apparent diffusion coefficient [ADC]), and clinicopathological characteristics were compared between RG and NRG. Logistic regression analysis was applied to identify parameters independently associated with CRT response and to construct a multivariable model. The areas under the receiveroperating characteristic curve (AUC) for various diagnostic approaches were compared using the DeLong test. Results: The T1, T2, and PD values in the NRG were significantly lower than those in the RG (all P < 0.05), whereas no significant difference was observed in the ADC values between these two groups. Clinicopathological characteristics (Epstein–Barr virus [EBV]-DNA level, lymph node extranodal extension, clinical stage, and Ki-67 expression) exhibited significant differences between the two groups. Logistic regression analysis showed that T1, PD, EBV-DNA level, clinical stage, and Ki-67 expression had significant independent relationships with CRT response (all P < 0.05). The multivariable model incorporating these five variables yielded AUC, sensitivity, and specificity values of 0.974, 93.8% (30/32), and 91.6% (87/95), respectively. Conclusion SyMRI may be used for the pretreatment prediction of CRT response in ANPC. The multivariable model incorporating syMRI quantitative parameters and clinicopathological characteristics, which were independently associated with CRT response, may be a new tool for the pretreatment prediction of CRT response.

Objective:To explore the clinical characteristics of patients with primary ureteral sarcomatoid carcinoma, thereby enhancing the understanding of the clinical context surrounding this disease.Methods:We report a case of primary ureteral sarcomatoid carcinoma in an elderly patient diagnosed through postoperative pathology at the Fourth Medical Center of the General Hospital of the People's Liberation Army.Additionally, a literature review was conducted to analyze articles related to primary ureteral sarcomatoid carcinoma published both domestically and internationally up to July 2024.This review summarizes and analyzes information regarding the clinical manifestations, diagnosis, treatment, and prognosis associated with this condition.Results:A total of 20 articles were reviewed, encompassing 24 cases of primary ureteral sarcomatoid carcinoma.Among the patients, 17 were older than 60 years, and when combined with the cases presented in this review, a total of 18 cases involved patients over the age of 60.Of these, 66.7%(12/18)were male, with a median age at diagnosis of 65 years.The reported cases often exhibited no clinical manifestations in the early stages; however, as the disease progressed, common urinary symptoms emerged, including frequency, urgency, pain, or painless hematuria, occurring throughout the ureteral course.The lesions were predominantly located in the left ureter(17 cases), which frequently exhibited dilation with effusion.Pathologic examination revealed the presence of microscopic spindle cells.Surgical treatment was administered to 20 patients, of whom 9 did not experience recurrence or metastasis during the follow-up period, while 7 cases resulted in mortality.Conclusions:Primary ureteral sarcomatoid carcinoma is a rare malignant tumor of the ureter.Its clinical manifestations are generally regarded as non-specific, making pathological examination the gold standard for diagnosis.Immunohistochemical and imaging studies can assist in the diagnostic process.The current preferred treatment modality is surgical resection, while first-line therapies that combine chemotherapy with immunotherapy and targeted therapy have been shown to enhance tumor control.

Objective To analyze the development of nuclear medicine services in Hunan Province and to assess internal radiation doses among the nuclear medicine workers (NMWs) involved in iodine-131 radionuclide therapy. Methods Based on a survey of nuclear medicine institutions in Hunan Province, a total of 61 NMWs from seven hospitals providing iodine-131 therapy for thyroid cancer were selected as the study subjects by convenience sampling method. Thyroidal iodine-131 activity was measured using a portable gamma spectrometer to estimate internal dose and total annual effective dose. Results A total of 47 nuclear medicine institutions were reported in Hunan Province by 2023, most of which were public and tertiary hospitals, accounting for 38. Iodine-131 therapy was performed in 30 institutions, including nine for thyroid cancer. A total of nine participants had detectable thyroidal iodine-131 activity among 61 workers involved in iodine-131 thyroid cancer treatment, with the detection rate of 14.8%. Their internal radiation annual committed effective doses ranged from 0.100 to 1.584 mSv, with a mean of 0.499 mSv and median of 0.426 mSv. Except for one cleaner, the remaining eight physicians and nurses had the total annual effective doses ranging from 0.311 to 3.007 mSv, with a mean of 1.305 mSv, all below the annual dose limit of 20.000 mSv among radiation workers specified in national standard. Conclusion Internal exposure to iodine-131 among the NMWs should not be neglected. Standardized procedures and strengthened internal dose monitoring are recommended.

Objective:To explore the current status of clinical trial registration for TCM treatment of heart failure.Methods:The clinical trials of TCM in the treatment of heart failure registered by China Clinical Trial Registry (ChiCTR) from the establishment of the database to June 30, 2024 were retrieved. Excel 2019 was used to input data and register time, registration area and institution, registration center and sample size, funding source, intervention measures, outcome indicators, random methods and blind methods were used for frequency analysis.Results:A total of 71 clinical trials of TCM in the treatment of heart failure in China were included, involving 13 provincial-level administrative regions. There were 39 clinical trial institutions participating in the registration, with Shanghai (16 items), Beijing (15 items), and Tianjin (12 items) ranking at the top in terms of registration numbers; the funding mainly relied on local finances, followed by national finances and hospital grants; in the registered projects, interventional studies accounted for the majority, mainly grouped through random number table methods, and only a small number of trials used blinding methods, with single-center participation being the norm; in terms of intervention measures, mainly Chinese patent medicines were used, while TCM decoctions were used less frequently. Outcome indicators were mainly efficacy indicators such as biochemical results and echocardiography, lacking monitoring of safety indicators.Conclusions:The number of clinical trial registrations for TCM in the treatment of heart failure in China is relatively small, but it shows a trend of increasing year by year. In terms of intervention measures, the treatment methods are also continuously enriching. However, there are still issues with clinical trials of TCM for the prevention and treatment of heart failure, such as insufficient awareness of registration among researchers, unbalanced regional and institutional distribution, and the need for improvement in the use of randomization and blinding methods.
