1.Clinicopathological features and ATRX application in hepatic angiosarcoma

Objective:To investigate the clinicopathological characteristics and the application of ATRX(alpha-thalassemia mental retardation X-linked)in hepatic angiosarcoma.Methods:We retrospectively analyzed the pathological and clinical data of seven patients diagnosed with hepatic angiosarcoma at the Department of Pathology, Beijing Hospital, between 2014 and 2023.ATRX immunohistochemical staining was conducted.Results:Among the patients, six were identified incidentally during physical examinations, while only one patient presented with abdominal pain.Six patients had multiple nodules, whereas one had a solitary tumor.The morphological features were diverse; high-grade angiosarcoma exhibited liver plate atrophy and hepatic sinusoids lined with atypical cells, while low-differentiated angiosarcoma displayed a higher degree of atypia and should be differentiated from other sarcomas.Tumor cells expressed vascular endothelial markers, including CD31(platelet endothelial cell adhesion molecule-1), CD34(hematopoietic progenitor cell antigen CD34), and ERG(Ets-related gene).Additionally, loss of ATRX expression was observed in four patients.Conclusions:Hepatic angiosarcoma is characterized by a lack of specific clinical manifestations and imaging findings.A comprehensive analysis of clinical symptoms, morphological features, and immunohistochemical staining—particularly for vascular markers and ATRX—is essential for an accurate pathological diagnosis.
2.Value of serum 25-hydroxyvitamin D,heart-type fatty acid-binding protein and N-terminal pro-brain natriuretic peptide in assessing early myocardial injury in patients with acute exacerbation of chronic obstructive pulmonary disease

Objective To investigate the evaluation value of serum 25-hydroxyvitamin D[25-(OH)D],heart-type fatty acid-binding protein(H-FABP),and N-terminal pro-brain natriuret-ic peptide(NT-ProBNP)in early myocardial injury in patients with acute exacerbation of chronic ob-structive pulmonary disease(AECOPD).Methods A total of 120 patients with AECOPD(AECOPD group)were enrolled in this study.Based on the presence of early myocardial injury,they were divided into injury group(n=68)and non-injury group(n=52).Additionally,40 healthy individuals undergoing physical examinations during the same period were included as control group.The differences in serum 25-(OH)D,H-FABP,and NT-ProBNP levels were compared,and the correlations between these markers and clinical data were analyzed.Binary logistic regression analysis was used to explore the relationships between these markers and the occurrence of early myocardial injury.Receiver op-erating characteristic(ROC)curve analysis was employed to assess the diagnostic value of these markers for early myocardial injury in AECOPD patients.Results The forced expiratory volume in the first second(FEV1),forced vital capacity(FVC),the ratio of FEV1 to FVC(FEV1/FVC),and arterial partial pressure of oxygen[pa(O2)]levels in the AECOPD group were lower than those in the control group,while the arterial partial pressure of carbon dioxide[p a(CO2)]level was high-er,with statistically significant differences(P<0.05).The serum 25-(OH)D levels in the AECOPD group and the injury group were lower than those in the control group and the non-injury group,re-spectively,while the H-FABP and NT-ProBNP levels were higher,with statistically significant differences(P<0.05).In AECOPD patients,serum 25-(OH)D was positively correlated with FEV1,FVC,FEV1/FVC,and pa(O2),and negatively correlated with pa(CO2)(P<0.05).In contrast,H-FABP and NT-ProBNP were negatively correlated with FEV1,FVC,FEV1/FVC,and pa(O2),and positively correlated with pa(CO2)(P<0.05).Binary Logistic regression analysis revealed that 25-(OH)D,H-FABP,and NT-ProBNP were related influencing factors for early myo-cardial injury in AECOPD patients(P<0.05).ROC curve analysis showed that the areas under the curve(AUCs)for evaluating myocardial injury status based on 25-(OH)D,H-FABP,and NT-ProBNP values were 0.814,0.959,and 0.837,respectively.The AUC of their combination was 0.983,with a sensitivity of 97.06%and a specificity of 80.77%.Conclusion During early myocardial injury in AECOPD patients,there is low expression of serum 25-(OH)D and high ex-pression of H-FABP and NT-ProBNP.These three markers are correlated with early myocardial inju-ry and can serve as reference indicators for clinical diagnosis.

For over a decade, Chinese cytopathologist has participated in the compilation of international reporting systems and subsequently promoted them widely in China; The clinical diagnostic level has significantly increased and has been widely recognized by clinical doctors in the industry; Dozens of clinical research papers have been published in international SCIE journals, among which the unique arrangement of cancer cell escape patterns was first discovered by Chinese cytology experts; Molecular detection methods are widely used in cytological specimens, and the differentiation between transient and persistent cervical HPV infection has achieved ideal results in the laboratory; The urgent need for standardized standards in clinical diagnosis has been resolved through expert consensus; Artificial intelligence assisted cervical cytology diagnosis has been applied in the screening of cervical cancer in a few hospitals and local areas.

Over the last ten years, China has made significant breakthroughs in the construction of a standardized diagnosis and treatment system and technological innovation in thyroid tumor cytology. In terms of diagnostic techniques, a standardized process combining manual smears and liquid-based cytology has been established. The combination of hematoxylin-eosin staining and cell block technology has significantly improved diagnostic accuracy, while immunocytochemical staining plays an important role in the differential diagnosis of difficult cases. The molecular pathology detection system has achieved leapfrog development, evolving from single-gene BRAF detection to a multi-gene panel covering BRAF, TERT and RAS genes. The application of next-generation sequencing and other new technologies has significantly improved preoperative diagnostic efficacy. The publication of the "Expert consensus on the cytopathological diagnosis of thyroid-fine needle aspiration (version 2023)" has further promoted the standardization of thyroid cytology diagnosis in China. Meanwhile, Chinese scholars have made breakthroughs in the field of medical artificial intelligence. A self-developed deep learning model was published in The Lancet Digital Health, achieving a diagnostic accuracy of 96.8% in differentiating benign and malignant thyroid nodules. A series of high-impact research results published in journals such as the Chinese Journal of Pathology marks China′s entry into the international forefront in this field.

8.Analysis of influencing factors and impact path of benefit finding in patients with cervical cancer and their spouses

Objective This study aims to analyze the factors influencing benefit finding among cervical cancer patients and their spouses,as well as the interconnections between these factors.The goal is to provide a foundation for developing targeted clinical interventions.Methods Using the convenience sampling method,cervical cancer patients and spouses of 245 pairs who attended or were hospitalized in a tertiary-level hospital in Taiyuan City from October 2022 to July 2023 were selected as study subjects.Data were collected using a general information questionnaire,the Distress Disclosure Index,the Connor-Davidson Resilience Scale,and the Benefit Finding Scale.Univariate analysis,Pearson correlation analysis,and multiple linear regression were employed to scrutinize the data,leading to the establishment of Actor-Partner Interdependence Model.Results Benefit finding scores for cervical cancer patients and their spouses were(65.31±7.94)and(69.87±9.63),respectively.Multiple linear regression revealed that the educational level of patients and their spouses,whether or not they received chemotherapy or radiotherapy,self-disclosure and psychological resilience were the factors that affected patients'benefit finding.Spouse's education level,occupation,self-disclosure,psychological resilience and patients'self-disclosure and psychological resilience were the influencing factors of spouse's benefit finding.The Actor-Partner Interdependence Model analysis indicated that the self-disclosure and psychological resilience of cervical cancer patients positively predicted their own benefit finding and that of their spouses(path coefficients were 0.415,0.501,0.216,and 0.168,respectively,all P<0.05).However,spouses'self-disclosure and psychological resilience could only positively predict their own benefit finding(path coefficients were 0.188 and 0.254,respectively,all P<0.05).Conclusion Benefit finding among cervical cancer patients and their spouses is moderate and influenced by various factors.Both self-disclosure and psychological resilience of cervical cancer patients and their spouses have positive subjective effects on their own benefit finding.Healthcare professionals should encourage both parties to engage in healthy interactions about the disease,take steps to increase the level of psychological resilience of both,and ultimately tap into a higher level of benefit finding.
9.Practical application of the Paris system for reporting urinary cytology

Objective:To validate the diagnostic performance of the Paris system for reporting urinary cytology (TPS).Methods:A total of 7 046 urine cytology samples from 3 402 patients collected in the Department of Pathology, Beijing Hospital, China from January 2020 to January 2022 were analyzed. 488 patients had a biopsy or resection specimen during the follow-up period of 6 months. The sensitivity, specificity, risk of malignancy (ROM) and risk of high-grade malignancy (ROHM) of the TPS were evaluated using histological diagnosis as the golden standard.Results:Among the 7 046 samples, high-grade urothelial carcinoma (HGUC) accounted for 5.7% (399/7 046), suspicious for high-grade urothelial carcinoma (SHGUC) for 3.2% (227/7 046), atypical urothelial cells (AUC) for 8.4% (593/7 046), and negative for high-grade urothelial carcinoma (NHGUC) for 72.9% (5 139/7 046) including low-grade urothelial neoplasm (LGUN) for 0.8% (59/7 046) and insufficient samples for 9.8% (688/7 046). 488 patients had a bladder biopsy or resection in the follow-up of six months, including 314 males and 174 females, aged 27 to 92 years (average, 66 years). The ROHM of TPS was 94.7% in HGUC, 83.3% in SHGUC, 41.3% in AUC and 18.8% in NHGUC. The sensitivity and specificity of urine cytology were 70.1% (169/241) and 97.0% (162/167), respectively. The negative predictive value of NHGUC was 69.2% (162/234).Conclusions:The study has shown that TPS classification has high sensitivity and specificity, high ROHM for HGUC and SHGUC, and high negative predictive value for NHGUC. The application of TPS reporting system can better interpret the clinical significance of cytology samples, improve the accuracy of urine cytopathology and ensure continuous diagnostic consistency.
10.Scientific, transparent and applicable rankings of Chinese pathological guidelines and consensus published in the medical journals in 2022

The STAR tool was used to evaluate and analyze the science, transparency, and applicability of Chinese pathology guidelines and consensus published in medical journals in 2022. There were a total of 18 pathology guidelines and consensuses published in 2022, including 1 guideline and 17 consensuses. The results showed that the guideline score was 21.83 points, lower than the overall guideline average (43.4 points). Consensus ratings scored an average of 27.87 points, on par with the overall consensus level (28.3 points). Areas that scored above the overall level were "conflict of interest" and "working groups", while areas that scored below the overall level were "proposals", "funding", "evidence", "consensus approaches" and "accessibility". To sum up, the formulation of pathology guidelines and consensuses in 2022 is not standardized, and the evidence retrieval process, evidence evaluation methods and grading criteria for recommendations on clinical issues are not provided in the formulation process; the process and method for reaching consensus are not provided, the plan is lacking, and registration is not carried out. It is therefore suggested that guidelines/consensus makers in the field of pathology should attach importance to evidence-based medical evidence, strictly follow guideline formulation methods and processes, further improve the scientific, applicable and transparent guidelines/consensuses in the field, and better provide support for clinicians and patients.
