On the occasion of the 110th Anniversary of the establishment of the Chinese Medical Association, the third conference of the Fourth Editorial Board of Chinese Journal of Digestive Surgery, and the Second Elite Group of Chinese Journal of Digestive Surgery, was successfully held in Kunming on July 4, 2025. This conference systematically summarizes the development experience of the journal over the past 20 years from three aspects: the role of ecological construction of thought in the discipline construction of digestive surgery, the display of the latest academic achievements in the field of digestive surgery, the development difficulties, and breakthrough paths of the discipline, and strategically plans the path of discipline construction in the new era.
2.Analysis of the Efficacy of PD-1 Inhibitor Pembrolizumab Combined with Chemotherapy in the Treatment of Cervical Cancer

Objective:To analyze the efficacy of the combination therapy of pembrolizumab,a programmed death receptor-1(PD-1)inhibitor,and chemotherapy in the treatment of cervical cancer.Methods:A total of 60 patients with cervical cancer who visited our hospital from January 2020 to January 2023 were selected and divided into two groups:30 patients were treated with first-line chemotherapy,and 30 patients were treated with pembrolizumab combined with first-line chemotherapy.Compare the recent efficacy of two groups,including immune related indicators[CD4+/CD8+ratio,interleukin-6(IL-6),interferon-γ(IFN-γ)],serum tumor markers[Squamous Cell Carcinoma Antigen(SCC Ag),Carbohydrate Antigen 125(CA125)],incidence of adverse reactions,and survival status[Progression Free Survival(PFS),Overall Survival(OS)]before and after treatment.Results:The objective response rate(ORR)of the combination group was 73.33%,while that of the chemotherapy group was 40.00%.The combination group was higher than the chemotherapy group(P<0.05);There was no significant difference in the levels of CD4+/CD8+,IL-6,and IFN-γ between the two groups before treatment(P>0.05).After treatment,the levels of CD4+/CD8+,IL-6,and IFN-γ decreased in the combination group,which was lower than those in the chemotherapy group(P<0.05);There was no significant difference in the levels of SCC Ag and CA125 between the two groups before treatment(P>0.05).After treatment,the levels of SCC Ag and CA125 in both groups decreased,with the combination group lower than the chemotherapy group(P<0.05);There was no significant difference in the incidence of hematological toxicity(anemia,thrombocytopenia),immune related toxicity(hypothyroidism,immune pneumonia,elevated transaminase),skin/mucosal reactions(rash,oral mucosal inflammation),and other adverse reactions(diarrhea,fatigue)between the two groups(P>0.05).The incidence of grade 3 or above neutropenia was higher in the chemotherapy group than in the combination group(P<0.05);The PFS and OS time of the combined group were higher than those of the chemotherapy group(P<0.05).Conclusion:The combination therapy of PD-1 inhibitor pembrolizumab and chemotherapy is significantly effective in treating cervical cancer,which can improve the immune environment,reduce serum marker levels,and has high safety.At the same time,through long-term follow-up,it was found that the combination therapy can further improve patients'long-term progression free survival and overall survival.
3.Analysis of curative effect of liver transplantation in patients with polycystic liver disease

Objective:To evaluate the treatment outcome of liver transplantation for patients with polycystic liver disease (PLD).Methods:Clinical data of 28 PLD patients admitted to the Affiliated Hospital of Qingdao University from May 2014 to November 2023 were retrospectively analyzed, including 10 males and 18 females, aged (50.4±6.6) years. Patients were divided into liver transplantation group ( n=15) and non-liver transplantation group ( n=13). In the liver transplantation group, we analyzed seve-ral critical parameters including methods of liver transplantation, intra-abdominal fluid volume, intraoperative blood loss, intraoperative red blood cell transfusion requirements, and postoperative complications. The prognosis of the two groups were also compared. Results:Among the 28 patients with PLD, 15 underwent liver transplantation, including 11 classic in situ liver transplantations, one modified back-to-back liver transplantation, and three liver-kidney combined transplantations. The 15 patients had 2 000 (300, 4 000) ml of abdominal fluid, 1 000 (600, 2 000) ml of intraoperative blood loss, and 8.0 (6.0, 17.0) U of red blood cells transfused during the operation. Postoperative complications occurred in eight cases, with four of which were managemed successfully, and the other four died. The 1-, 5-, and 10-year survival rates of after liver transplantation were 80.0%, 80.0%, and 73.3%, respectively. The 1-, 5-, and 10-year survival rates of patients with PLD without liver transplantation were 69.2%, 46.2%, and 38.5%, respectively. The difference between the two groups was statistically significant ( χ2=3.91, P=0.048). Conclusion:Liver transplantation is a treatment option for patients with PLD, with a better long-term survival compared to patients without liver transplantation.

7.Effects of Yiqi Juanbi Formula on chondrocyte pyroptosis in collagen-induced arthritic rats via NF-κB/NLRP3/Caspase-1 signaling pathway

AIM To investigate the effects of Yiqi Juanbi Formula on chondrocyte pyroptosis in rat models of collagen-induced arthritis(CIA).METHODS Fifty rats were subcutaneously injected at the tail base with an emulsion containing equal volumes of bovine type Ⅱ collagen and incomplete Freund's adjuvant(IFA)to establish the CIA models.These rats were then randomly assigned to the model group,the methotrexate group(0.35 mg/kg),and the low-dose,medium-dose,and high-dose Yiqi Juanbi Formula groups(9.4,18.7,37.4 g/kg),in contrast to the ten intact rats serving in the normal control group.Following four weeks of intragastric administration,the rats had their general conditions observed;their joint swelling and arthritis indices measured;their ankle joint pathology assessed by HE staining;their serum levels of IL-1β,IL-18 and TNF-ɑ detected by ELISA;their mRNA expressions of NLRP3,Caspase-1,GSDMD,IL-1β,IL-18 and TNF-ɑ in ankle cartilage quantified by RT-qPCR;their protein expressions of NF-κB,NLRP3 and Caspase-1 in ankle cartilage analyzed by Western blot;and their NLRP3 and GSDMD positive expressions in ankle cartilage examined by immunohistochemistry.RESULTS Compared to the control group,the model group showed significantly increased joint swelling and arthritis indices(P<0.01);elevated serum levels of IL-1 β,IL-18 and TNF-ɑ(P<0.01);pathological changes including cartilage surface defects,reduced cell count,altered cellular morphology,irregular cell arrangement,and significant inflammatory cell infiltration in synovial tissue;upregulated mRNA expressions of NF-κB,NLRP3,Caspase-1,GSDMD,IL-1β,IL-18 and TNF-ɑ(P<0.01)and increased protein expressions of NF-κB,NLRP3 and Caspase-1(P<0.01)in ankle cartilage;enhanced positive expressions of NLRP3 and GSDMD in ankle cartilage(P<0.01).Compared to the model group,the groups intervened with methotrexate or medium-or high-dose Yiqi Juanbi Formula exhibited reduced joint swelling and arthritis indices(P<0.01);alleviated pathological damage in ankle joints;decreased serum levels of IL-1β,IL-18 and TNF-ɑ(P<0.01);downregulated mRNA expressions of NF-κB,NLRP3,Caspase-1,GSDMD,IL-1β,IL-18 and TNF-ɑ(P<0.05,P<0.01),and reduced protein expressions of NF-κB,NLRP3 and Caspase-1(P<0.05,P<0.01)in ankle cartilage;and diminished positive expressions of NLRP3 and GSDMD in ankle cartilage(P<0.01).CONCLUSION Yiqi Juanbi Formula alleviates inflammation in CIA rats,potentially by inhibiting the activation of the NF-κB/NLRP3/Caspase-1 signaling pathway,thereby suppressing chondrocyte pyroptosis.
8.Mediating effect of sleep quality on the relationship between serum vitamin D3 level and depressive symptoms in community-dwelling elderly people with mild cognitive impairment

Objective:To explore the mediating effect of sleep quality on the relationship between serum vitamin D3 level and depressive symptoms in community-dwelling elderly people with mild cognitive impairment(MCI).Methods:A total of 480 elderly people with MCI meeting inclusion criteria in a community health service center from Jan 2023 to Dec 2023 were selected as the research objects.The general information questionnaire,mini-mental state examination(MMSE),activity of daily living scale(ADL),Center for Epidemiological Studies Depression Scale(CES-D)and Pittsburgh sleep quality index(PSQI)were used to investigate the subjects.Serum vitamin D3 level was detected within 1 week.Spearson correlation analysis was used to explore the correlation between sleep quality,serum vitamin D3 level and depressive symptoms.The mediating effect model was tested by Model 4 in the Process plug-in.Results:Among the 480 MCI elderly,the average score of depression was(12.28±4.75),the average serum vitamin D3 level was(25.81±7.09)ng/ml,and the average score of sleep quality was(7.17±2.71).Depression severity was positively correlated with sleep quality score(r=0.294,P<0.01),and negatively correlated with serum vitamin D3 level(r=-0.237,P<0.01).Serum vitamin D3 level was negatively correlated with sleep quality score(r=-0.225,P<0.01).Bootstrap analysis showed that the mediating effect of sleep quality between serum vitamin D3 level and depression level was-0.064(95%CI:-0.115--0.023),accounting for 26.57%of the total effect.Conclusion:Sleep quality partially mediates the relationship between serum vitamin D3 level and depressive symptoms in the elderly with MCI in the community.

10.Clinical and genetic characteristics of congenital adrenal hyperplasia: a retrospective analysis.

OBJECTIVES:
To study the clinical and genetic characteristics of children with congenital adrenal hyperplasia (CAH).
METHODS:
Clinical data, laboratory findings, and genetic test results of 63 children diagnosed with CAH at Henan Children's Hospital from January 2017 to December 2024 were retrospectively reviewed.
RESULTS:
Of the 63 patients, the mean age at the first visit was (21 ± 14) days; 29 (46%) were of male sex and 34 (54%) were of female sex. The predominant clinical manifestations were poor weight gain or weight loss (92%, 58/63), poor feeding (84%, 53/63), skin hyperpigmentation (83%, 52/63), and female external genital anomalies (100%, 34/34). Laboratory abnormalities included hyponatremia (87%, 55/63), hyperkalemia (68%, 43/63), metabolic acidosis (68%, 43/63), and markedly elevated 17-hydroxyprogesterone (92%, 58/63), testosterone (89%, 56/63), and adrenocorticotropic hormone (81%, 51/63). Among 49 patients who underwent genetic testing, CYP21A2 variants were identified in 90% (44/49), with c.293-13A/C>G (33%, 30/91) and large deletions/gene conversions (29%, 26/91) being the most frequent; STAR (8%, 4/49) and HSD3B2 (2%, 1/49) variants were also detected. Following hormone replacement therapy, electrolyte disturbances were corrected in 57 cases, with significant reductions in 17-hydroxyprogesterone, adrenocorticotropic hormone, and testosterone levels (P<0.001).
CONCLUSIONS
CAH presenting in neonates or young infants is characterized by electrolyte imbalance, external genital anomalies, and abnormal hormone levels. Genetic testing enables definitive subtype classification; in CYP21A2-related CAH, c.293-13A/C>G is a hotspot variant. These findings underscore the clinical value of genetic testing for early diagnosis and genetic counseling in CAH. Citation:Chinese Journal of Contemporary Pediatrics, 2025, 27(11): 1367-1372.
