Objective:To perform RHD gene detection on a blood sample with serological weak D phenotype. Methods:A specimen received by the People's Hospital of Zhijin County was serologically identified by the microcolumn gel method and saline method.RHD gene detection was conducted by the PCR-SSP method, and the full sequence determination of the 10 exons amplified was performed. The sequencing results were compared with the ISBT database to determine the genotype.Bioinformatics tool was used to predict the functional damage of mutant proteins, and Alphafold-3 was used for tertiary structural modeling of wild-type and mutant RhD proteins, and the structures of the two proteins were compared and analyzed to explore the reasons why mutations lead to weak serological manifestations. Results:The patient's genotype was identified as RHD*DV.5/RHD*01N.01 heterozygote, with the complete deletion of RHD genes on one chromosome, unable to express the D antigen. On the other chromosome,a G>A mutation occurred at the 697th base of the 5th exon, resulting in a partial D phenotype. This mutation causes internal hydrogen bond changes at the 233 position of RhD protein, resulting in a change in the conformation of the protein, affecting binding to the corresponding antibody. Conclusion:The patient is a heterozygous mutant individual with RHD*DV.5/RHD*01N.01,exhibiting a partial D phenotype serologically. This variation is extremely rare and has been scarcely reported globally.

Objective:Observation of the therapeutic effect of Yugu Ju detergent combined with fusidic acid cream on the repair of wounds infected with Staphylococcus aureus,as well as its impact on serum inflammatory markers TNF-α,IL-6,IL-1β,oxidative stress markers MDA,ROS,SOD and Nrf2 levels.Methods:96 patients with skin defects and SA U infection caused by trauma admitted to the hospital from June 2021 to December 2023 were randomly divided into a control group and an observation group. Both groups were treated with debridement, while the control group was treated with external application of fusidic acid cream. The treatment group received external washing with Yugu Ju detergent in addition to the control group. One course of treatment lasted for 7 days, with three consecutive courses of treatment.Observe the wound healing rate, bacterial clearance rate, and changes in TNF-α,IL-6,IL-1β,SOD,MDA,ROS,and Nrf2 before and after treatment to evaluate clinical efficacy and safety. Results:The total clinical effective rate of the observation group after treatment was 93.75%(45/48),while that of the control group was 75%(36/48).The observation group was significantly better than the control group(P<0.05);The wound healing rate and bacterial clearance rate of the observation group at each time point after treatment were higher than those of the control group(P<0.05);After treatment, the Nrf2 and SOD values in the observation group were higher than those in the control group, while the TNF-α,IL-6,IL-1β,MDA,and ROS values were lower than those in the control group(P<0.05).No adverse reactions occurred during the treatment of both groups. Conclusion:The combination of Yugu Ju detergent and Fusidic acid cream can inhibit SA U and promote the healing of infectious wounds, which may be related to the activation of Nrf2 expression to inhibit oxidative stress and inflammatory response. It is safe and effective, and worthy of promotion and application.

Objective:To construct an indicator system for a nursing science and technology innovation curriculum guided by innovation competence, in order to provide a reference for cultivating innovation ability in nursing students. Methods:The overall research period was from March to December 2024. A nursing innovation curriculum indicator framework was initially developed through literature analysis and brainstorming. From October to December 2024, 19 experts from nine hospitals or universities across five provinces and cities were selected via purposive sampling to participate in two rounds of Delphi consultation. Revisions were made based on expert feedback. Results:Both rounds of expert consultation achieved a 100% response rate. The authority coefficient of the experts was 0.92. The final indicator system included four curriculum elements: course content, course objectives, teaching methods, and assessment, encompassing 14 first-level indicators and 40 second-level indicators. Conclusions:The innovation-oriented indicator system for nursing science and technology education demonstrates good scientific validity and reliability. It offers a foundational framework for advancing innovation-focused nursing education and curriculum design.

Objective To explore the pharmacokinetic(PK)characteristics of desloratadine tablets and reference drugs in healthy subjects, and evaluate their bioequivalence and safety. Methods The random,open,two-period,cross-over pharmacokinetic study method was adopted, each subject received a single oral dose of desloratadine tablets test drug(T)or reference drug(R)for 5 mg.The concentrations of desloratadine and 3-hydroxy desloratadine in plasma were determined by liquid chromatography-tandem mass spectrometry(LC-MS/MS);and the PK parameters were calculated by WinNonlin 8.1 software to evaluate the bioequivalence. Results The main PK parameters of T and R of desloratadine were as follows:the fasting condition Cmax were respectively(3 809.82±1 016.54)and(3 642.36±777.07)pg·mL-1;AUC0-120h were respectively(5.75 ×104±5.03 ×104)and(5.51 × 104±4.00 × 104)pg·h·mL-1;AUC0-∞ were respectively(6.85× 104±1.03× 104)and(6.37 × 104±7.92 × 104)pg·h·mL-1.The fed condition Cmax were respectively(4 398.98±1 191.22)and(4 744.4±1 511.97)pg·mL-1;AUC0-120h were respectively(5.25 × 104±1.82 × 104)and(5.55 × 104±1.98 × 104)pg·h·mL-1;AUC0-∞ were respectively(5.37 × 104±1.86 × 104)and(5.68 × 104±2.04 × 104)pg·h·mL-1.The 90%confidence interval of Cmax,AUC0-t and AUC0-∞ of desloratadine were all within 80.00%~125.00%.Conclusion There was no significant difference in the main PK parameters between T tablets and R under fasting or high-fat postprandial conditions, and desloratadine tablets were bioequivalent, safe and well tolerated.

Rheumatoid arthritis(RA)is a chronic autoimmune disease of unknown etiology that can cause joint destruction and deformity. As a small molecule cytokine, the chemokine C-X-C motif chemokine ligand 12(CXCL12)regulates the pathogenesis of rheumatoid arthritis by binding to the specific receptor CXC chemokine receptor 4(CXCR4).Therefore,based on the bio-logical characteristics of CXCL12 and CXCR4,this paper intro-duces the pathogenesis of CXCL12/CXCR4 in RA and summari-zes the progress in RA-related research, with the aim of providing clinical value for understanding the pathogenesis of RA and de-veloping novel therapeutic targets.
