Body weight abnormalities, including overweight, obesity, and underweight, have become a dual public health challenge in Chinese adults: overweight and obesity lead to a variety of chronic complications, while underweight increases the risks of malnutrition, sarcopenia, and organ dysfunction. To systematically address these issues, multidisciplinary experts in endocrinology, sports science, nutrition, and psychiatry from various regions have held multiple weight management seminars. Based on the latest epidemiological data and clinical evidence, they expanded the guideline to include assessment and intervention strategies for underweight, in addition to the core content of obesity management. This guideline outlines the etiological mechanisms, evaluation methods, and multidimensional management strategies for overweight and obesity, covering key areas such as diagnosis and assessment, medical nutrition therapy, exercise prescription, pharmacological intervention, and psychological support. It is intended to provide a scientific and standardized approach to weight management across the adult population, aiming to curb the rising prevalence of obesity, mitigate complications associated with abnormal body weight, and improve nutritional status and overall quality of life.

Sustained inflammatory responses are closely related to various severe diseases, and inhibiting the excessive activation of inflammasomes and pyroptosis has significant implications for clinical treatment.Natural products have garnered considerable concern for the treatment of inflammation.Huanglian-Wumei decoction(HLWMD)is a classic prescription used for treating inflammatory diseases, but the necessity of their combination and the exact underlying anti-inflammatory mechanism have not yet been elucidated.Inspired by the supramolecular self-assembly strategy and natural drug compatibility theory,we successfully obtained berberine(BBR)-chlorogenic acid(CGA)supramolecular(BCS),which is an herbal pair from HLWMD.Using a series of characterization methods,we confirmed the self-assembly mechanism of BCS.BBR and CGA were self-assembled and stacked into amphiphilic spherical supra-molecules in a 2:1 molar ratio, driven by electrostatic interactions, hydrophobic interactions, and π-πstacking;the hydrophilic fragments of CGA were outside, and the hydrophobic fragments of BBR were inside. This stacking pattern significantly improved the anti-inflammatory performance of BCS compared with that of single free molecules.Compared with free molecules,BCS significantly attenuated the release of multiple inflammatory mediators and lipopolysaccharide(LPS)-induced pyroptosis. Its anti-inflammatory mechanism is closely related to the inhibition of intracellular nuclear factor-kappaB(NF-κB)p65 phosphorylation and the noncanonical pyroptosis signalling pathway mediated by caspase-11.

Aim To clarify the mechanism by which Qishen Yixin Granules improved microcirculation vas-cular endothelial dysfunction(VED)in mice, through activating the Nrf2/HO-1 signaling pathway to regulate oxidative stress. Methods C57 mice were randomly divided into six groups:blank group, model group,pos-itive drug group, and low-,medium-,and high-dose groups of Qishen Yixin Granules. The VED model was established by long-term infusion of Ang Ⅱ combined with a high-fat diet. Each treatment group received the corresponding drug intervention. After four weeks of drug intervention, cardiac function was assessed by echocardiography.Carstairs staining was used to ob-serve the formation of microthrombi in myocardial tis-sue.The micro vascular ischemia was evaluated by Hei-denhain staining. The ultrastructure of endothelial cells was observed by electron microscopy. The levels of EMPs,ROS,NO,ET-1,TF,TM,VWF,and TXA2 in serum were measured by ELISA.The expression levels of MDA,SOD,and GSH-Px in mouse heart tissue were determined by chemical methods.Cardiac microvascu-lar density and the expression of Nrf2,Keap1,and HO-1 proteins were detected by Immunohistochemical stai-ning.The protein expressions of Keap1,cytoplasmic Nrf2,nuclear Nrf2,and HO-1 in myocardial tissue were detected by Western blot. Results Qishen Yixin Granules could effectively improve the cardiac function of mice, alleviate the damage of endothelial cells and endothelial function. They could up-regulate serum NO levels and the activities of antioxidant enzymes SOD and GSH-Px,while down-regulating the expression of ROS and vascular inflammatory injury factors such as ET-1,VWF,TXA2,TF,TM,and EMPs.Qishen Yixin Granules also increased the positive counts of CD34,Nrf2,and HO-1,as well as microvessel density.Fur-thermore,they inhibited the expression of MDA,Keap1,and cytoplasmic Nrf2 protein in myocardial tis-sue,while increasing the expression of nuclear proteins HO-1 and Nrf2.Conclusions Qishen Yixin Granules may inhibit oxidative stress and inflammatory response by regulating the Nrf2/HO-1 signaling pathway, thereby improving vascular endothelial damage and cardiac function in VED mice.

Objective:To explore the effects of multi-dimensional nursing based on empowerment theory in patients undergoing thoracoscopic and laparoscopic esophagectomy. Methods:A convenience sampling method was used to select 110 esophageal cancer patients who were admitted to Henan Provincial People's Hospital from August 2022 to April 2024. Patients were randomly divided into two groups using a random number table: the control group and the observation group, with 55 patients in each group. The control group received routine care, while the observation group received multi-dimensional nursing based on empowerment theory in addition to routine care. The two groups were compared in terms of stress responses (heart rate, systolic blood pressure, diastolic blood pressure) , self-care ability, nutritional status (hemoglobin, albumin) , and nursing satisfaction. Results:After the intervention, the observation group had lower heart rate, systolic blood pressure, and diastolic blood pressure compared to the control group, with statistically significant differences ( P<0.01) . The self-care ability score in the control group was lower than that in the observation group, with a statistically significant difference ( P<0.01) . The observation group also had higher hemoglobin and albumin levels compared to the control group, with statistically significant differences ( P<0.01) . The nursing satisfaction rate in the observation group was higher than that in the control group, with a statistically significant difference ( P<0.05) . Conclusions:Multi-dimensional nursing based on empowerment theory can effectively reduce stress responses, improve self-care ability, enhance nutritional status, and increase nursing satisfaction in esophageal cancer patients.

Objective To investigate the feasibility of non-ECG-gated scan using wide-body detector in left atrial computed tomo-graphy angiography(CTA)and to assess its value in evaluating left atrial appendage closure(LAAC)outcomes. Methods Patients who underwent LAAC and completed left atrial CTA were prospectively selected.Participants were randomly divided into groups A and B.In group A,the heart rate was measured using a finger clip oximeter before the examination, and the heart rate was set as the simula-ted heart rate. The simulated signal was generated by the CT equipment to complete the examination. In group B,the conventional ECG-gated scan was adopted. The residual leakage of left atrial appendage occluder, image quality, patient comfort and radiation dose of the two groups were compared, and statistical analysis was performed for these indicators. Results Both groups completed the evaluation of the endothelialization degree of the left atrial appendage occluder, and there was no significant difference in the objective image quality evaluation or subjective image score between the two groups(P>0.05).The total scores of the physiological dimension,psy-chological dimension, social cultural dimension and environmental dimension of patients in group A were higher than those in group B(P<0.001),and the volume CT dose index(CTDIvol)and dose length product(DLP)decreased by 25%and 25.36%respectively,with statistically significant differences(P<0.001).Conclusion During the left atrial CTA examination, the wide-body detector with non-ECG-gated scan mode can provide good image quality for clinical use and observe the embolization of the left atrial appendage after LAAC surgery. At the same time,it can effectively reduce the radiation dose for patients and improve patient comfort dur-ing the examination, showing high clinical feasibility and appli-cation value.
