1.Opioid-Sparing Effect of Celiac Plexus Neurolysis in Palliative Care Patients with Upper Abdominal Cancer: A Single-Center Retrospective Case Series of Thirteen Patients

Purpose:
Whether celiac plexus neurolysis (CPN) retains opioid-sparing efficacy in palliative care patients receiving modern extended-release opioids remains uncertain after a recent randomized trial that reported no benefit. We describe fluoroscopy-guided CPN in such patients.
Methods:
Thirteen consecutive palliative care patients with unresectable or metastatic upper abdominal cancer (seven pancreatic, five hepatobiliary, one gastric) on strong opioid therapy underwent posterior fluoroscopy-guided CPN with absolute ethanol after a diagnostic lidocaine block. Daily oral morphine equivalent (OME) and Numeric Rating Scale (NRS) pain scores were recorded at baseline and 1, 2, and 4 weeks. Pain response was defined a priori as ≥30% NRS reduction.
Results:
Median age was 67 years, baseline median OME 86.5 mg/day (range, 15~215), and median NRS 6. Median OME decreased to 72, 60, and 57.5 mg/day at 1, 2, and 4 weeks (median reductions 46.5%, 26.7%, 43.5%;all P≤0.002), and median NRS improved to 3, 4, and 4 (all P≤0.001). A pain responsewas achieved in 8/13 (61.5%) at 1 week, 7/13 (53.8%) at 2 weeks, and 5/12 (41.7%) at 4weeks. Opioid-related adverse events declined numerically (nausea, 7/13→3/12; constipation, 4→2; somnolence, 2→0; all McNemar P>0.10). No procedural complication occurred(0/13).
Conclusion
CPN was followed by reduction in opioid use and pain scores, withresolution of opioid-related symptoms and no complications. Although limited by the smallsample size, these findings suggest a potential role for CPN in palliative care patients receiving extended-release opioids and warrant prospective investigation.
2.Changes in pulse wave transit time variability after interscalene brachial plexus block placement

Background:
The pulse wave transit time (PWTT) increases with decreased vascular tone resulting from sympathetic blockade caused by regional anesthesia. It oscillates, exhibiting variability due to the interaction between the autonomic nervous and cardiovascular systems. We hypothesized that interscalene brachial plexus block (ISBPB) placement increases the PWTT and reduces the low-frequency power of PWTT variability (LF).
Methods:
Fifty-six patients receiving an ISBPB were analyzed. The PWTT was defined as the difference in milliseconds (ms) between the R peak of the electrocardiogram and the peak of the second-derivative photoplethysmographic waveform. The LF was calculated by integrating from 0.04 to 0.15 Hz on the power spectrum obtained from fast Fourier transform. The two variables were collected during the 5 min before the end of acclimatization (baseline), between 5 and 10 min after block needle insertion, and between 15 and 20 min after block needle insertion.
Results:
The PWTT increased significantly (P < 0.001) from baseline (mean [SD]: 155.3 [16.7] ms) to 5–10 min post-needle insertion (166.9 [15.4] ms) (mean difference [MD]: 11.6, 95% CI [9.2–14.0], P < 0.001) and 15–20 min post-needle insertion (165.6 [16.1] ms) (MD: 10.3, 95% CI [7.3–13.2], P < 0.001). The natural log-transformed LF (lnLF) decreased significantly (P < 0.01) from baseline (1.539 [0.560] ln[ms2/Hz]) to 5–10 min post-needle insertion (1.341 [0.617] ln[ms2/Hz]) (MD: –0.198, 95% CI [–0.356 to –0.040], P < 0.01) and 15–20 min post-needle insertion (1.396 [0.548] ln[ms2/Hz]) (MD: –0.144, 95% CI [–0.274 to –0.013], P = 0.03).
Conclusions
The post-ISBPB decrease in lnLF and increase in PWTT may be attributable to ISBPB-induced sympathectomy.

8.Quantitative evaluation of upper extremity strength recovery after cervical epidural block: a retrospective study

Background:
We aimed to evaluate the clinical efficacy of cervical epidural block (CEB) in enhancing upper extremity (UE) muscle strength in patients with cervical disc herniation (CDH).
Methods:
Ten patients with CDH underwent a single CEB session. Follow-up assessments were conducted weekly for 2 weeks through outpatient visits. Handgrip strength on the affected side (AHGS) and the interlateral difference in handgrip strength (DHGS) between the affected and unaffected sides were measured using a dynamometer before, immediately after, 1 and 2 weeks after CEB. Neck pain and radiating UE pain were assessed using a visual analog scale (VAS) at baseline and 2 weeks posttreatment.
Results:
The median patient age was 46.4 years (range, 35–78 years). The affected disc levels were C5–6 in five patients, C6–7 in three, and C5–6–7 in two. The left side was affected in six patients and the right in four. The mean VAS score for neck pain decreased from 7.2 to 2.9, whereas that for radiating UE pain decreased from 6.3 to 2.8 after treatment. Both the AHGS and DHGS improved significantly compared to the baseline values at all posttreatment times. However, no significant differences were observed among the posttreatment time points. These findings remained consistent even after adjusting for age and sex.
Conclusion
CEB may offer therapeutic benefits by alleviating pain and improving UE muscle weakness in patients with CDH. However, further large-scale prospective studies are required to validate these preliminary results and determine the long-term efficacy of CEB in managing CDH.

Methods:
This study explores a comprehensive guideline for systematically choosing appropriate statistical analysis methods, with a particular focus on the statistical hypothesis testing stage and categorization of variables. By providing a detailed examination of these aspects, this study aims to provide researchers with a solid foundation for informed methodological decision making. Moving beyond theoretical considerations, this study delves into the practical realm by examining the null and alternative hypotheses tailored to specific statistical methods of analysis. The dynamic relationship between these hypotheses and statistical methods is thoroughly explored, and a carefully crafted flowchart for selecting the statistical analysis method is proposed.
Results:
Based on the flowchart, we examined whether exemplary research papers appropriately used statistical methods that align with the variables chosen and hypotheses built for the research. This iterative process ensures the adaptability and relevance of this flowchart across diverse research contexts, contributing to both theoretical insights and tangible tools for methodological decision-making.
Conclusions
This study emphasizes the importance of a scientific and rational approach for the selection of statistical analysis methods. By providing comprehensive guidelines, insights into the null and alternative hypotheses, and a practical flowchart, this study aims to empower researchers and enhance the overall quality and reliability of scientific studies.
