1.Antimicrobial Effect of Psoralea corylifolia Extract on Streptococcus mutans, Streptococcus sobrinus, Candida albicans

Background:
The increasing prevalence of dental caries and oral candidiasis has led to growing interest in natural antimicrobialagents with fewer adverse effects than conventional antiseptics. Psoralea corylifolia, a traditional medicinal plant, has been reported to possess antimicrobial properties; however, its effects on major oral pathogens remain unclear.
Methods:
The antimicrobial activity of P. corylifolia ethanol extract against Streptococcus mutans, Streptococcus sobrinus, and Candida albicans was evaluated using the paper disk diffusion method, optical density (optical density at 600 nm) measurements at 6, 12, 24, 48, and 72 hours, and colony-forming unit (CFU) assays. Data were analyzed using one-way analysis of variance followed by Tukey’s post-hoc test, with p<0.05 considered statistically significant.
Results:
The extract demonstrated significant concentration-dependent antimicrobial activity against all tested microorganisms(p<0.05). At 4 mg/ml, the maximum inhibition zones were 7.5±0.5 mm for S. mutans, 2.0±0.0 mm for S. sobrinus and 2.0±0.5 mm for C. albicans. Optical density and CFU analyses further confirmed significant reductions in microbial growth compared to that in the control group, with up to 94∼96% reduction in viable cell counts at 0.5 mg/ml.
Conclusion
P. corylifolia extract exhibits measurable antimicrobial effects against major oral pathogens in vitro. Although its inhibition zones were smaller than those of chlorhexidine (13.3±0.6 mm for S. mutans), the extract demonstrated consistent antimicrobial activity across all experimental methods. These findings suggest that the extract has the potential to be used as a complementary natural antimicrobial agent for preventive oral care. Further studies are required to evaluate its cytotoxicity, mechanism of action, and clinical applicability.

We evaluated a multicenter retrospective cohort of incorporated patients with cirrhosis between 2013 and 2019. We conducted comparisons of the area under the receiver operating characteristic curve (AUROC) of the MELD3.0 and other models for predicting 3-month mortality. Additionally, we assessed the risk of cirrhosis-related complications according to the MELD3.0 score.
Results:
A total of 3,314 patients were included. The mean age was 55.9±11.3 years, and 70.2% of the patients were male. Within the initial 3 months, 220 patients (6.6%) died, and the MELD3.0had the best predictive performance among the tested models, with an AUROC of 0.851, outperforming the Child-Pugh classification, ALBI grade, MELD, and MELDNa. A high MELD3.0score was associated with an increased risk of mortality. Compared with that of the group with a MELD3.0 score <10 points, the adjusted hazard ratio of the group with a score of 10–20 pointswas 2.176, and that for the group with a score of ≥20 points was 4.892. Each 1-point increase inthe MELD3.0 score increased the risk of cirrhosis-related complications by 1.033-fold. The risk of hepatorenal syndrome showed the highest increase, with an adjusted hazard ratio of 1.149, followed by hepatic encephalopathy and ascites.
Conclusions
The MELD3.0 demonstrated robust prognostic performance in predicting mortality in patients with cirrhosis. Moreover, the MELD3.0 score was linked to cirrhosis-related complications, particularly those involving kidney function, such as hepatorenal syndrome and ascites.

9.Comparative analysis of the incidence and mortality of COVID-19 in Korean end-stage kidney disease patients: hemodialysis, peritoneal dialysis, and transplantation

Patients with end-stage kidney disease (ESKD) are more susceptible to viral epidemics and are known to have higher incidence and death rates of coronavirus disease 2019 (COVID-19) compared to the general population. We determined COVID-19 incidence and mortality among chronic hemodialysis (HD), peritoneal dialysis (PD), and kidney transplantation (KT) patients in Korea. Methods: We conducted a retrospective cohort study and data regarding Korean ESKD adults (aged ≥18 years) were obtained from the National Health Insurance Service of Korea from October 2020 to December 2021. We examined and compared the incidence of COVID-19–related infections and deaths among the patients receiving HD, PD, and KT. Results: Of all ESKD patients, 85,018 (68.1%) were on HD, 8,399 (6.7%) on PD, and 31,343 (25.1%) on KT. The COVID-19 incidence was 1.3% for HD, 1.2% for PD, and 1.5% for KT. COVID-19 mortality was 16.3% for HD, 12.2% for PD, and 4.7% for KT. PD patients had a lower incidence of infection compared to HD patients (odds ratio [OR], 0.76; 95% confidence interval [CI], 0.607–0.93), but KT patients had a significantly higher risk of infection (OR, 1.28; 95% CI, 1.13–1.44). Compared with HD, the risk of COVID-19–related death was not different for PD patients but was significantly lower for KT patients (hazard ratio, 0.55; 95% CI, 0.35–0.88). Conclusion: COVID-19 incidence was lower in PD patients than in HD patients, but mortality was not different between them. KT was associated with a higher risk of COVID-19 infection but lower mortality compared to HD.
10.Inhibitory Effects of Ethanol Extracts of Ginkgo biloba Leaves on Physiological Activities of Streptococcus mutans in Different Carbohydrate Conditions

Background:
Dental caries is closely associated with physiological activities of Streptococcus mutans, including growth, acid production, calcium ion (Ca2+ ) release, and biofilm formation. Although the antimicrobial properties of ethanol extracts of Ginkgo biloba leaves (GLE) against various cariogenic and periodontal pathogens have been reported, their effects on physiological responses of S. mutans under different carbohydrate environments remain unclear. This study aimed to investigate the influence of GLE on the physiological activities of S. mutans in brain heart infusion broth, and in media supplemented with 1% sucrose or 1% xylitol. Based on these findings, we aimed to evaluate the potential of GLE as a natural functional material for inhibiting S. mutans–associated virulence and preventing dental caries.
Methods:
To evaluate the antimicrobial activity of GLE against S. mutans under various media conditions, a disk diffusion test was performed. Effects of GLE on physiological activities of S. mutans were assessed by measuring bacterial growth and conducting assay for biofilm formation, Ca2+ release, acidogenicity, and buffering capacity.
Results:
GLE exhibited significant, dose-dependent antimicrobial activity against S. mutans, particularly in sucrose-containing media. Increasing GLE concentrations significantly inhibited bacterial growth, biofilm formation, and Ca2+ release. Notably, GLE at concentrations of ≥5 mg/ml effectively suppressed multiple physiological activities of S. mutans. In addition, GLE reduced bacterial acid production regardless of concentration and enhanced the buffering capacity of the culture media.
Conclusion
GLE has the potential to serve as a safe and effective natural functional material with physiological activity inhibitionand anticariogenic potential GLE inhibits physiological activities of S. mutans, including biofilm formation and Ca2+ release, particularly in sucrose-containing media, and effectively neutralizes S. mutans-induced the acidic conditions by, thereby contributing to its anticariogenic effects.
